AND PREVENTION OF COLORECTAL CANCERS. The present invention relates to the prevention and treatment of adenomas and cancers of the colon and rectum. These tumors are the leading cause of cancer death in non-smokers. The treatments offered to date are based on surgical excision of tumors, with adjuvant chemotherapy, using for example 5-Fluoro-Uracile. These have many drawbacks, among which we can cite more particularly a high failure rate (40% five-year survival), a high cost for society and high suffering for the patient. It is also envisaged to prevent digestive cancers by chronic administration of aspirin, or an equivalent NSAID. However, this type of treatment still has significant side effects and remains relatively ineffective. The purpose of the present invention is precisely to provide new, easy-to-use means for effectively preventing and / or curing adenomas and cancers of the colon and rectum. This object is achieved by the use of polyethylene glycol or a derivative thereof as an active agent for the preparation of a medicament intended for the treatment and / or prevention of colon cancers and / or rectum. Polyethylene glycol (PEG) is a well known polymer, used in particular as an adjuvant in pharmaceutical compositions, corresponding to the following formula (I): H- (O- CH2-CH2) n-OH in which n is around 200 for high molecular weight PEGs, for example PEG 8000. High molecular weight PEGs, that is to say greater than about 3000, are known for their laxative effects. PEG is considered to attract and retain water in the intestine by an osmotic effect (Davis, G.R., SantaAna, C.A., et al., 1980, Gastroenterology, 79: 35-
Thus PEG is used in humans in various preparations either as a preparative enema before colonoscopy (4 liters), or as a laxative (20g / d). As examples of such preparations, there may be mentioned:

Le macrogol est un polymère sur lequel sont retenues les molécules d'eau par liaisons macrogol-based osmotic (PEG with molecular weight 3350), in the form of a sachet containing 2.95 g of PEG and an excipient consisting of NaCl, anhydrous Na2SO4, Na2CO3, apple flavor, aspartame. The efficacy of 20 g of PEG 4000, without excipient or electrolytes, to combat chronic constipation was also shown in a recent randomized double-blind trial (Hudziak, H.,

The inventors have now demonstrated the unexpected ability of PEG and its derivatives, advantageously of high molecular weight, of the order of about 3000 to 9000 daltons, to prevent or cure colorectal cancers. In the context of its use as an adjuvant, it is known to use PEG as a vehicle for various molecules, in particular anticancer chemotherapy, such as for example doxorubicin, but PEG has never been proposed to date to prevent or

L'homme du métier est à même de tester les capacités de prévention ou de guérison des cancers colorectaux des dérivés du PEG à partir des tests rapportés According to the invention, the term “polyethylene glycol derivative” means the compounds of formula I in which one or more of the reaction chemical groups are modified or substituted while retaining the capacities for preventing or curing cancer. As indicated above, the invention relates more particularly to PEGs of high molecular weight, of the order of approximately 3000 to 9000 Daltons, PEG being probably not absorbed, the invention relates more particularly to any pharmaceutical composition in in which the PEG or a derivative thereof is associated with a pharmaceutical vehicle in a form allowing administration by the oral route or in a suppository or alternatively in enema or in any other

The compositions of the invention are dosed so as to allow administration of approximately 0.4 to 0.8 g / day and preferably of the order of 0.6 g / day of PEG per

Thus, the pharmaceutical compositions of the invention are dosed so as to allow administration of 10 to 80 g of PEG per day. The invention relates to a "food"; this designation includes in particular a food composition (nutraceutical) or a functional food (food), comprising PEG or a derivative thereof of high molecular weight, preferably of the order of 3000 to 9000 daltons, and most preferably of the order of 8000 daltons to be used in the prevention of cancers of the

Such a functional food (food) has a double advantage, on the one hand its role in the prevention of cancers of the rectum and / or colon in accordance with the present invention, and on the other hand a laxative effect in accordance with the known properties of PEG . Thus, the dosage of PEG in said functional food (food) makes it possible to promote either the laxative effect at low doses of PEG, or the effect of preventing cancers at higher doses as indicated above with regard to the compositions.

Les résultats présentés ci-après ne concernent que le groupe The experiment which made it possible to show the anti-cancer effect of PEG in rats comprised 8 groups of 20 rats initiated by azoxymethane (AOM). Indeed in order to study the effect of hydration of the stools on carcinogenesis, certain groups received a salty diet (3 to 6%, NaCl), the other a liquid diet (leading to water consumption 150 and 200% of the controls), or with a restriction of the water supply (9o% of the control), another finally treated with a diuretic (furosemide). The results presented below only concern the group

L'eau a été donnée dans des biberons en plastique Fischer 344 rats, 4 weeks old females (Iffa Credo, L'Arbresles 69), were kept for 5 days on arrival, then they received an intraperitoneal injection of azoxymethane (20 mg / kg bodyweight, in NaCl 9 g / l, Sigma, St Quentin 38). The rats were distributed, 7 days later, at random by 2 rats per cage of the metal hanging cage type in a stainless steel grid, in an air-conditioned pet store (22 C 2 C) with a 12-hour day and night cycle. The rats were then given ad libitum, one of the regimes provided for in the protocol, in hemispherical porcelain ramekins renewed weekly. Water was given in plastic bottles
à raison d'un biberon par cage.at the rate of one bottle per cage.
Vingt rats témoins ont reçu un régime semi-purifié en poudre de composition standardisée (20 % protéines et 5 % lipides, UAR, Villemoisson 91) (American Institute of Nutrition, 1977, Report of the American Institute of Nutrition Ad Hoc Committee on standards for nutritional studies, J. Nutr. Les régimes ont été donnés une semaine après l'injection du cancérigène, et pendant 100 Twenty control rats received a semi-purified powdered diet of standardized composition (20% protein and 5% fat, UAR, Villemoisson 91) (American Institute of Nutrition, 1977, Report of the American Institute of Nutrition Ad Hoc Committee on standards for nutritional studies, J. Nutr. 107: 1340-1345), and ten treated rats received the same diet to which PEG 8000 was added (ref. 194839 [25322- 68-31] from ICN, Orsay 91). The quantity of PEG was adjusted each week according to the weight of the rats so as to provide them with 3 g / kg / day (approximately 5% of the dry weight of the food). Diets were given one week after the injection of the carcinogen, and for 100
jours, jusqu'au sacrifice des animaux. days, until the animals are sacrificed.
37: 147-151)( Bruce, W.R., Archer, M.C., Corpet. D. E.. Medline, A., Minkin, S., Stamp, D., Yin, Y. and Zhang, X.M., 1993, The rats were weighed before injection, and weekly until sacrificed. The energy intake and daily consumption of water and food for each rat were also recorded 1 day per week (Tuesday). After 100 days of experimental diet, the rats of each group were killed by CO2 asphyxiation, opened, and their colon was removed, emptied by injection of Krebs' Ringer buffer, opened and fixed flat between two sheets of paper- filter (coded) in buffered formalin (10%). After at least 12 hours, the colon were stained with methylene blue (15 minutes, 0.2% in Krebs' Ringer). The microadenomas (aberrant crypt foci, ACF) were counted (number and size) under the microscope (x40) by a single observer for the whole of the experiment, in double blind (Bird, R. P, 1987, Cancer Lett. 37: 147-151) (Bruce, WR, Archer, MC, Corpet. DE. Medline, A., Minkin, S., Stamp, D., Yin, Y. and Zhang, XM, 1993,

The study of the relationship between the dose of PEG and the protective effect made it possible to observe a very clear relationship between the dose of PEG administered and the protective effect. L'effet du PEG à court terme montre que l'administration de 5% de PEG pendant 30 jours, via l'aliment ou l'eau de boisson, diminue fortement la We can see for these two parameters of carcinogenesis the relationship between the dose of PEG and the protective effect is very clear, even if the dose of 5% is only really effective. This study finally shows that PEG is effective in males, but slightly less than in females. The effect of PEG in the short term shows that the administration of 5% PEG for 30 days, via food or drinking water, greatly reduces the

In the following discussion, the protective power of PEG is compared with that of the other molecules supposed to allow a "chemoprevention" of colorectal carcinogenesis. PEG is by far the most effective product.

Plus la valeur est Table 4 below shows the efficacy of different agents on colon carcinogenesis in rats.
