"Composition pharmaceutique anorexigène comprenant du 5-hydroxytryptophane
La présente invention concerne une composition pharmaceutique anorexigène comprenant du 5-hydroxytryptophane (5-HTP) comme principe actif unique.
On a déjà effectué antérieurement des études expérimentales sur les rats, par lesquelles on a démontré l'action potentialisante du 5-HTP sur des produits pharmaceutiques anorexigènes connus, tels que l'amphétamine et la fenfluramine (J. Pharm. Pharmac.
1975, 27,31-37). Toutefois, ces études ont été menées sans aucune intention de mettre en valeur les effets thérapeutiques du 5-HTP en soi mais pour démontrer le rôle du système sérotoninergique dans la modulation du comportement alimentaire chez le rat. A cette fin, on a provoqué sur les animaux des lésions des structures hypothalamiques impliquées dans la régulation du comportement alimentaire et on a par exemple constaté que le 5-HTP potentialise l'anorexie provoquée par la fenfluramine seulement chez les rats comportant une lésion à l'hypothalamus.
Il est évident que les résultats de ces expériences ne peuvent pas faire l'objet d'une extrapolation pour l'homme. En fait, elles n'offrent aucune suggestion quant à une éventuelle action potentialisante du 5-HTP sur des anorexigènes pour l'homme, et elles ne peuvent encore moins faire deviner l'action anorexigène du 5-HTP seul ni de ce fait son utilisation en thérapeutique humaine, en l'absence d'anorexigènes avérés, tels que l'amphétamine et la fenfluramine.
Il est évident d'autre part qu'il serait intéressant de disposer, en thérapeutique humaine, d'un agent anorexigène qui ne présente pas les effets indésirables bien connus propres à l'amphétamine et à la fenfluramine ou à leurs analogues, effets qui varient de la nausée et de l'insomnie à l'hypertension, aux arythmies cardiaques et même, dans les cas de surdosage, à la mort par arrêt cardiaque.

On a maintenant trouve grâce à des études cliniques sur l'homme que le 5-HTP est en soi un puissant agent anorexigène.
L'avantage est évident de pouvoir utiliser le 5-HTP tout seul pour provoquer un état d'anorexie dans un but thérapeutique chez l'homme, au lieu des anorexigènes mentionnés précédemment. Le 5-HTP est une substance naturelle ; il est en fait le précurseur direct de la sérotonine. Sa toxicité est extrêmement basse. Des études effectuées sur le rat et la souris ont démontré que la valeur DL50 est négligeable par rapport aux doses thérapeutiques : DL50 per os chez la souris : 2500 mg/kg ; DL50 i. p. : 1400 mg/kg.
Concernant le 5-HTP, on connaît déjà un usage thérapeutique précédent en psychiatrie à titre de co-adjuvant dans la dépression psychogène, endogène et involutive et en neurologie, par exemple dans l'épilepsie, en association avec des anticonvulsifs, et dans la maladie de Parkinson, en association avec le L-Dopa.
Les effets thérapeutiques observés sur ces patients ont confirmé que le 5-HTP traverse la barrière hémato-encéphalique, ce qui est d'une importance fondamentale pour l'effet pharmacologique central suivant. Ces études démontrent en outre la capacité optimale de tolérance du produit.
Il n'existe cependant aucune corrélation entre l'usage thérapeutique connu et l'usage comme anorexigène.
Etude clinique
On a procédé à une étude clinique en double-aveugle avec inversion vis-à-vis d'un placébo. On a admis pour l'étude, 19 patients (âge compris entre 26 et 52 ans, âge moyen de 41 ans), hyperphagiques, affectés d'obésité du second degré, en excluant les patients qui présentaient une intolérance aux hydrates de carbone, des dyslipidémies et des altérations du métabolisme de l'acide urique. Après avoir donné leur accord et après randomisation, tous les patients ont été soumis, en deux périodes consécutives de cinq semaines chacune, à un traitement avec du 5-hydroxytryptophane par voie orale (8 mg/kg/ jour subdivisés en trois administrations) et un placébo. A la fin de la première période, les patients qui avaient reçu le produit pharmaceutique ont continué avec le placébo et vice versa.

Durant les 10 semaines d'étude, les patients n'ont été soumis à aucune restriction alimentaire. A une fréquence hebdomadaire, on a procédé à la mesure du poids corporel et à l'estimation de l'anorexie. Sur un échantillon de récolte urinaire de 24 heures, on a procédé à la détermination de l'acide 5-hydroxy-3-indoleacétique (5-HIAA), catabolite terminal du métabolisme de la sérotonine, dans le but d'estimer, aux fins de l'étude, la compliance des patients.
La détermination du 5-HIAA urinaire a été réalisée avec la méthode colorimétrique de H. Udenfried et H. Weissabach sur un échantillon de 4 ml d'urine (J. Biol. Chem. 216,449, 1955). Lors de chaque contrôle hebdomadaire, on a demandé aux patients s'ils avaient observé une réduction subjective de l'appétit.
Pour avoir une définition la plus objective de l'existence d'une anorexie, les patients ont été invités à répondre à un questionnaire visant à mettre en évidence la présence de symptômes, tels que des altérations du goût (AG) et de l'odorat (AO), l'aversion pour la viande (AC) les nausées et/ou les vomissements (N/V) et le sens précoce de la satiété (SPS) qui interviennent dans l'acceptation de la nourriture et qui peuvent être mis en relation avec une altération des centres neurorégulateurs du comportement alimentaire. Les patients qui présentaient un ou plusieurs des symptômes cités ci-dessus ont été considérés comme anorexiques.
Deux fois par semaine, les patients ont été soumis à une auto-évalutation de la sensation de la faim et de la satiété grâce au test de Silverstone. Il a ainsi été possible d'évaluer subjectivement et objectivement l'anorexie des patients. Résultats
Au cours de l'étude, la prévalence des symptômes responsables de l'anorexie a été nettement supérieure durant la période de traitement avec le produit pharmaceutique par rapport à la période de traitement avec le placébo (p < . 0,01) (si on considère le tableau IV).

Chez les patients, durant l'absorption du 5-HTP, l'aversion pour la viande a existé dans 57 % des cas, l'altération du goût dans 27 % et le sens précoce de la satiété dans 32 %. Les mêmes symptômes se sont présentés avec une moindre fréquence durant l'absorption du placébo ; à titre d'exemple, le sens précoce de la satiété dans 21 % des cas (voir le tableau IV). Durant 10 semaines d'étude, on a noté l'apparition d'un seul effet secondaire : la diarrhée, dont la prédominance dans le cas du traitement avec le produit pharmaceutique a été supérieure en pourcentage par rapport à la période de traitement avec le placébo (voir le Tableau IV).
L'auto- évaluation du sens de satiété, effectuée avec l'échelle de Silverstone, a mis en évidence une augmentation significative de la prédominance de ce symptôme durant la période de traitement avec le 5-HTP par rapport à la période de traitement avec le placébo (p < 0,001).
Poids corporel : la réduction moyenne du poids corporel des 19 patients durant la période de traitement avec le 5-HTP a été plus importante que la réduction moyenne du poids corporel des mêmes patients durant l'absorption du placébo (p < 0,02) (voir Tableau II).
Acide 5-hydroxy-3-indoleacétique : les taux urinaires de l'acide 5-HIAA chez les patients traités avec le placébo ne sont pas différents des valeurs de base (voir Tableau I). Au contraire, on a constaté que la concentration urinaire de l'acide 5-HIAA est augmentée de façon significative durant l'absorption du produit pharmaceutique aussi bien en ce qui concerne les valeurs observées avant l'étude que les valeurs obtenues durant l'absorption du placébo (p < 0, 001) (voir Tableau I).

Pour l'usage thérapeutique suivant la présente invention, le 5-HTP peut être formulé suivant les mêmes formes pharmaceutiques que celles prévues pour l'utilisation déjà connue en psychiatrie et en neurologie.

Experimental studies in rats have already been carried out in which the potentiating action of 5-HTP has been demonstrated on known anorectic pharmaceuticals, such as amphetamine and fenfluramine (J. Pharm. Pharmac.

The advantage is obvious of being able to use 5-HTP alone to cause a state of anorexia for a therapeutic purpose in humans, instead of the anorectics mentioned above. 5-HTP is a natural substance; it is in fact the direct precursor of serotonin. Its toxicity is extremely low. Studies carried out on rats and mice have shown that the LD50 value is negligible compared to the therapeutic doses: LD50 per os in mice: 2500 mg / kg; LD50 i. p. : 1400 mg / kg.
