Abstract

The pincers have two legs with jaws (4,5) articulated around a common axis and having closure stops. The two jaws have formations (10-13) to receive the plate at the level of one of its eyes and to lock the plate to hold it in a fixed position w.r.t. the pincers. The jaws have a circular passage (16) coaxial to the geometrical axis of an eye held in the jaws to allow passage and guiding of a drill. The passage can be formed by the contact of two semi-circular sections.

Description

The present invention relates to ancillary forceps to facilitate the placement of maxillofacial osteosynthesis plates.

In maxillofacial surgery, osteosynthesis plates are used, intended to remain implanted temporarily or permanently and generally in the form of straight or angled blades, with or without branches, and provided with holes or eyelets arranged to receive screw heads, and preferably having, for this purpose, spherical seats or surfaces. These plates have eyelets which are generally separated by blade portions called bridges.

These blades must be resistant, malleable in order to be deformed at the convenience of the surgeon, and well supported by the patient. The blades are therefore generally made of high purity titanium.

As for the screws, for reasons of breaking strength, they are made of titanium alloy.

In the event of maxillary or facial fractures, the surgeon uses a suitable approach and frees up a suitable surface of the cortical bone on either side of the fracture line. After having made a rigorous reduction of the fracture, he keeps the plate applied against the fracture and proceeds to drill the screw passages using the eyelets of the plate as guides. I1 then proceeds to screw the titanium screws to immobilize the plate definitively.

Generally, after having chosen an appropriate osteosynthesis plate, the surgeon models it or deforms it to give it the conformation adapted to the bone surfaces present. He must then proceed to drill the pilot holes into which the screws to hold the plate will be screwed, generally at least two screws for each side of the fracture. For this, he manually maintains the plate in the position which must be his and proceeds to drill the pilot holes through the eyelets of the plate. This operation is particularly delicate because the surgeon must not only avoid moving the plate but also prevent the drill from attacking the internal edge of the eyelet and detaching small particles of titanium which can cause metallosis.

The present invention proposes to remedy these drawbacks and to provide ancillary forceps which allow the surgeon to carry out all of these operations while minimizing the risks listed above.

The subject of the invention is an ancillary clamp for maxillofacial osteosynthesis plates, having two branches ending in respective jaws, articulated around a common axis and having usual locking means of the notch type, characterized in that that the two jaws have plate receiving means capable of receiving a plate at one of its eyelets and of tightening the plate to maintain it in a fixed position relative to the clamp, said jaws being arranged to present a passage circular coaxial with the geometrical axis of the eyelet held in the pliers to allow a drill to pass and guide, and preferably, followed by a widening in the form of a cup making it possible to receive the head of the screw, so that it it can be screwed through said clamp to a position very close to the final position.

In addition, a drill guide can thus be ensured, avoiding substantial contact with the edge of the plate eyelet.

Preferably, each of the jaws of the clamp has, perpendicular to the plane of spacing and approximation of the jaws, a relief extending outside said plane, and provided with a groove of circular appearance to receive a corresponding circular edge of the plate at the level of the eyelet, the jaw being provided with a substantially semi-spherical notch coaxial with the geometric axis of said circular groove, the notches and grooves of the two jaws being symmetrical with respect to the contact plane of the two jaws when the clamp is closed.

Advantageously, the jaws can be bent at their end, depending on the anatomical uses sought.

Other advantages and characteristics of the invention will appear on reading the following description, given by way of nonlimiting example, and referring to the appended drawing in which

FIG. 3 represents a detail view from above of the terminal end of the jaws of the clamp,
- Figure 4 shows a sectional view of the terminal end of the jaws of the clamp.

The ancillary forceps according to the invention have, in a conventional manner, two branches 1 and 2 articulated around a common axis 3 and defining two jaws 4 and 5. Clamping means of the notch type, usual in surgical forceps have been shown in 6. When the surgeon closes the clamp, the notches come into engagement and the clamp is blocked. Additional tightening, authorized by the elasticity of the clamps, makes it possible to unlock the notches, in a well known manner.

The jaws 4.5 have end pieces 6.7, which follow the jaws after an elbow of an angle chosen according to anatomical considerations, for example 450
The two end pieces are symmetrical with respect to the plane of symmetry of the clamp passing through the axis 3.

Each end piece has a semi-spherical notch 8, 9 whose union forms a spherical seat when the two jaws have been brought together. The notch 8, 9 communicates with the other face of the clamp by a semi-circular passage of small thickness 14, 15, the meeting of which forms a circular passage 16 of low height, which will be coaxial with the eyelet of the plate to to pose. The diameter of the passage thus produced is very slightly greater than the diameter of the drill used to make the pilot holes.

In addition, each of the two end pieces 6,7 has, perpendicular to the common plane of the two end pieces, a relief 10,11 whose internal face, perpendicular to the end piece, has a groove 12,13, the height of which is slightly greater than the height of an osteosynthesis plate, and the depth of which varies circularly, as can be seen in FIG. 3, the geometric circle thus defined having the same axis transverse to the plane of the endpiece as the axis of the passage and seat formed by the meeting of notches 8, 9, 14 and 15.

The radius of this circle corresponds substantially to the radius of the circular part or coping of the plate at the level of an eyelet.

When the surgeon, having practiced the approach first, proceeded to the reduction of the fracture and the rouinage discovering the cortical bone on both sides of the line of fracture at the place where the plate must be posed, and after having, if necessary, shaped the plate, he grasps the plate, by one of its end eyelets, in the forceps, so that the eyelet is caught between the two jaws formed by the reliefs 10 and 11, the plate being received and clamped in the grooves 12,13. Once, the notch 6 blocked, the osteosynthesis plate is strictly secured to the ancillary forceps which it extends. In this state, the hole in the eyelet is coaxial with the substantially cylindrical passage 16 formed by the union of the two semicircular notches 14, 15.

The surgeon can then, with the forceps, present the osteosynthesis plate in the desired location and apply it against the bone. While holding the plate in position, it can then proceed to drill the pilot hole, through passage 16 then the hole of the eyelet, which serve as a guide to the forest and prevent it from deflecting and thus risking to cut metal particles in the eyelet. The hardness of the metal forming the tip is preferably equal to or greater than the hardness of the drill, so as to avoid cutting of particles.

The surgeon, after drilling the pilot hole and removing the drill, can then insert the screw and start to screw the plate by the eyelet held in the pliers until the spherical head of the screw is received in the seat in a spherical bowl 8.9.

After removing the forceps, the surgeon finishes screwing the screw into the eyelet, which quickly immobilizes the plate in the desired position. He can then proceed to drill the other holes through the plate now immobilized. Of course, if necessary, he can also use another identical clamp but possibly bent otherwise, to hold another end of the plate.

The clamp according to the invention can be produced by machining two branches. However, it is also possible to use a standard flat-jaw pliers, possibly shortening the jaws of this pliers and welding, at the level of said jaws, the two ends 6,7.

For example, the end caps 6,7 may have, on one of the faces of the clamp, two extensions in the form of semi-cylindrical walls 17,18 extending transversely to the clamp. The reliefs 10.11 are formed there, and the grooves 12.13 by machining. The notches 8, 9 and passages 14, 15 are machined on the other side of the clamp.

The invention can, of course, be the subject of numerous variants. Thus, for example, instead of having reliefs extending on one side of the clamp, it is possible, using thicker jaws, to make recesses directly in the clamp to enable it to receive and tighten the end of an osteosynthesis plate, notches being formed to form, when joined, the guide passage, and it is understood that any variant falls within the scope of the invention as soon as it allows to give the pliers the triple function of plate presentation making it possible to tighten and maintain the plate, to guide the drill, and to hold the plate for pre-tightening.
