Abstract
Description
Système d'occlusion vasculaire. Vascular occlusion system.
L'invention concerne un système d'occlusion vasculaire. The invention relates to a vascular occlusion system.
To suppress the blood supply to a tumor or to treat an aneurysm by limiting or suppressing any pressure in the blood, the vessel concerned is occluded or the aneurysm is occluded.
To effect this occlusion, it is known to use a solidifiable liquid or gel.
It is also known to position an inflated balloon at the desired location.
However, for many years, flexible and elastic threads have been used which are wound and / or form loops.
These threads may even include hairs in order to increase the desired effects of thrombosis.
To put them in place, a catheter and a pusher are generally used, a technique recalled in particular in US Pat. No. 4,994,069.
There are also known devices for positioning spiral prostheses in an organic channel.
For example, WO-A-92/14408 describes the means of positioning a prosthesis, which is initially stretched in a catether, by an internal thread, then taking up a spiral shape fixing said prosthesis definitively at the desired location.
The inventor has imagined a complete occlusion system comprising, in a known manner, at least one flexible occlusion wire intended to be introduced and pushed into a delivery catheter until being expelled from it by its distal end for reach the desired location of the vessel to be treated.
Advantageously, at least one of the occlusion threads forms, at rest, the windings, while it can be temporarily straightened and that it is tubular and open at at least one of its ends, unlike the aforementioned US patent.
A tubular occlusion wire is, for example, itself formed from a helically coiled tungsten filament whose surface condition is treated to be roughened, while it is heat treated to make it easily breakable. The roughness of the surface in particular facilitates the effects of thrombosis.
The fact that the tubular occlusion wire is open makes it possible to provide a straightening mandrel in the form of an at least semi-rigid wire with a diameter smaller than that of the tubular occlusion wire and intended to be introduced into the latter for straighten it and introduce it into the catheter.
Preferably in this case, the system comprises at least one tubular plunger intended to be crossed by the straightening mandrel during the straightening operation in order to then exercise its function as a plunger.
However, if this technique will make it possible to carry out most of the occlusion, in order to finish occluding a vessel or an aneurysm after the introduction of one or more occlusion wires, it is useful to be able to remove the last wire after having evaluated the necessary length, in order to cut the correct length.
Until now, a known technique consists in soldering said wire at the end of a pusher and then, in order to definitively put this last wire in place, destroying said weld by a weak direct current.
Cette technique est particulièrement longue. This technique is particularly long.
This is why the inventor has imagined a system which is remarkable in that it comprises at least one pusher provided at its active end with a nozzle in which is provided a housing opening outwards, intended to receive a means retainer of conjugate shape and fixed to one end of the occlusion wire, while the outside diameter of said tip is close to the inside diameter of the delivery catheter so that the occlusion wire cannot detach from the pusher as long as the tip is in the catheter, while outside of said catheter, the retaining means can escape from the housing of the tip.
Advantageously, the housing of the end piece of the pusher has the shape of a cylinder whose longitudinal axis is transverse with respect to the pusher, while said cylinder is open at its two ends and has a slot which opens in the cylinder and outward to pass a fixing rod fixed to the occlusion wire and provided with the retaining means.
Preferably in this case, the means for retaining the occluding wire is a ball secured to the fixing rod and whose outside diameter is slightly less than the inside diameter of the cylindrical housing of the end piece of the pusher.
Advantageously also, the system comprises a tubular introducer intended to facilitate the introduction of the occlusion wire with its pusher into the catheter by the proximal end thereof.

sion de la figure 7.The invention will be clearly understood and other particularities will appear on reading the description which follows and which refers to the appended drawings in which:
- Figure 1 shows schematically the establishment of several son

Figure 7.
Figure 1 shows schematically an aneurysm 1, on a blood vessel 2.
To treat aneurysm 1, the technique consists of filling it as well as possible by means of a plurality of occlusion wires 3, 3 ′ (FIGS. 2 to 10) forming by interlacing and convolutions, a stopper referenced 4 on the FIG. 1, said wires being introduced by means of a catheter 5.
Each occlusion wire 3, 3 'is a tubular wire itself constituted by a helically wound filament as shown in the drawings.
The filament used is advantageously made of a non-ferromagnetic material and in particular tungsten heat treated to make it easily breakable and treated at the surface so that it is rough.
Each occlusion wire constitutes a sort of spring which retains or resumes elastically and has a substantially helical-shaped shape.
The process for placing the first occlusion wires is illustrated in Figures 1 to 6.
As shown in Figure 2, is introduced into the tubular wire 3, a straightening mandrel 6 in the form of a wire of a diameter smaller than that of said wire 3, in order to straighten the latter.
The straightening mandrel 6 advantageously passes through a tubular pusher 7 (FIG. 2).
The tubular pusher 7 will thus allow the wire 3 to be pushed after the introduction of the latter into the delivery catheter 5, as illustrated in FIG. 3.
As shown in Figure 4, the straightening mandrel 6 is then withdrawn (possibly temporarily holding the tubular plunger 7).
The wire 3 is then ejected from the catheter 5, by the distal end of the latter, for example by injection of a contrast liquid by means of a syringe 8, as illustrated in FIG. 5 and / or also by means of another pusher shown diagrammatically at 9 in FIG. 6.
In this way, the wires 3 become entangled, resuming their shape as shown in particular in FIG. 1.
To complete the filling of the aneurysm shown diagrammatically at 1 in FIG. 1, a last particular wire is used, referenced 3 ′ in FIGS. 7 to 10, or more of these wires. The wire 3 'is here equivalent in nature to the wire 3 previously mentioned but it could be otherwise, or even not tubular.
The wire 3 'is introduced into the catheter 5, by means of a tubular introducer 10 (FIG. 8) and a pusher 11 (FIGS. 7 to 10), the tubular introducer 10 being able moreover to serve as a means of storing said 3 'wire.
The pusher 11 has at its end, a tip 12 which is engaged with the occlusion wire 3 '.
As shown more particularly in Figure 7, the nozzle 12 has a cylindrical housing 13 transverse to the pusher 11 and open at its two ends. The housing 13 of the endpiece 12 is intended to receive a ball 14 integral with a rod 15 fixed to the corresponding end of the wire 3 ', which rod 15 can pass through a diametrical slot 16 open in the housing 13 and towards the outside.
The outside diameter of the nozzle 12 is slightly smaller than the inside diameter of the catheter 5.
It is thus possible to push the wire 3 'to the distal end of the catheter (end visible in Figures 1, 9 and 10).
The length necessary for this last element is evaluated by radioscopy (thanks to the opacity of the wires to X-rays) and, thanks to the pusher 11, it is possible to completely remove said wire 3 ', to cut it to the right length and to reintroduce.
To allow the wire 3 'to escape definitively into the aneurysm, it suffices, as shown in FIG. 10, to take out the said wire 3' through the distal end of the catheter so that the ball 14 of the occlusion wire 3 ' leaves the housing 13 through one of its open ends.
Au cas où le fil d'occlusion 3' ne sortirait pas automatiquement du logement, il suffit de tourner légèrement le guide 11 comme le montre la flèche R. In case the occlusion wire 3 'does not automatically come out of the housing, it suffices to slightly turn the guide 11 as shown by the arrow R.
On peut ensuite retirer le poussoir 11. The pusher 11 can then be removed.
Other means and variants can also be imagined.
Thus, for example, FIGS. 9 and 10 show rings such as 17 and 17 ′ on the catheter 5, and 18 on the pusher 11.
The rings 17, 17 'and 18 are, for example, made of gold in order to assess by radioscopy the position of the wire 3' relative to the end of the catheter 5.
