Abstract
Description
Dispositif orthopédique ancillaire.Ancillary orthopedic device.
The present invention relates to an orthopedic device allowing the repair of bone fractures, in particular by the implementation of surgical methods known as nailing, or even locked nailing.
The invention relates more particularly to an ancillary orthopedic device for implantation, locking or removal in situ in an oblong organ, such as a fractured bone, of an implant, such as an intramedullary nail. The nailing or locked nailing operating methods are techniques well known to date and use implants of variable design. These surgical techniques, for the repair of fractured bones, consist in the introduction into the medullary cavity of the bone of an oblong element called an implant and possibly of one or more reinforcing elements which make it possible to improve the quality of stabilization of the fractured bone.
In the prior art, there are many intramedullary nails, that is to say generally metallic, long objects, which are introduced into the medullary cavity through the ends of the long bone which should be stabilize, this establishment posing some problems not yet resolved to date. In particular, it is often difficult to obtain stiffening of the fractured bone by means of an implant allowing it to withstand not only compression or tensile forces, but also rotation. To obtain this result, it is necessary to block the implant not only longitudinally but also in rotation around its axis. Consequently, it is often necessary to resist the rotation of the bone ends to place transverse screws which pass diametrically through the bone as well as one or more pairs of aligned holes provided in particular near the distal end of the nail.
Such meticulous work requires the implantation of a nail and its immobilization in a precise, fast and easy manner.
Today, nails are implanted using an ancillary device equipped with an anvil screw on which the nail or threaded implant is generally locked by screwing. This method of attachment is not very fast and does not always have the desired precision. In addition, this method of attachment is suitable in the case of hollow nails, more particularly used in human surgery; on the other hand, in the case of veterinary surgery, it requires special machining of the nail which is expensive. In addition, by this method, it is impossible to remove, once the bone has been repaired, the implant from 1 bone.
Such methods of fixing the implant to the ancillary device are in particular described in patents FR-A2,647,006 and FR-A-2,587,196.
The aim of the present invention is therefore to overcome this drawback by proposing an ancillary orthopedic device for implantation, locking or removal in situ in an oblong organ such as a fractured bone, of an implant such as an intramedullary nail, which allows in particular, on the one hand an easy locking of the implant in the bone, on the other hand its removal once the fracture has been repaired.
To this end, the invention relates to an ancillary orthopedic device for implantation, locking or removal in situ in an oblong organ, such as a fractured bone, of an implant, such as an intramedullary nail, capable of '' be introduced and immobilized in the bone by so-called nailing surgery methods, possibly locked nailing, device of the type comprising an ancillary body consisting of at least one hollow element disposed coaxial with the implant and being capable of receiving by insertion at one of its ends an anvil screw and at its other end the implant, the anvil screw securing the implant and the ancillary, characterized in that the implant has, in the vicinity of its end proximal, a projecting element comprising a recess engaging with an internal shoulder of the through bore formed in the hollow branch of the body of the ancillary, this recess being held in engagement with the shoulder only by the penetration of the projecting element of said implant inside an axial bore of the anvil screw, mounted integral with sliding, in at least a portion of the bore of the hollow branch of the body of the ancillary, so that any relative movement between the implant and the ancillary body is prevented.
According to a preferred embodiment of the invention, the projecting element of the implant assumes the shape of a stud whose base or its vicinity has a recess formed by a notch, one of the edges of this notch coming taken with the internal shoulder formed in the through bore of the hollow element of the ancillary body.
According to another preferred embodiment of the invention, the internal shoulder formed in the hollow element of the ancillary body consists of a radial finger preferably arranged in the vicinity of the end of the hollow element of the ancillary body receiving the implant, this finger partially closing the through bore of the latter.

L'autre branche de l'équerre constitutive du corps d'ancillaire 6 constitue une poignée de manoeuvre 7.According to the invention, the ancillary orthopedic device for implantation, locking or removal in situ of an implant, generally an intramedullary nail placed in a long bone by operating methods known as nailing or nailing locking , is used in particular in veterinary surgery. This device comprises in known manner an ancillary body 6 for example, in the general form of a square, one of the hollow branches of the square being arranged coaxial with the implant 1 constituted here by an intramedullary nail. This hollow branch is capable of receiving by insertion at one of its ends an anvil screw 8 and at its other end the implant 1. The other branch of the bracket constituting the ancillary body 6 constitutes an operating handle 7.
The implant, characteristic of the invention, has, as shown in Figures 1 to 3, near its proximal end, on the one hand a projecting element, such as a stud 5, capable of penetrating inside an axial bore 11 of the anvil screw 8 mounted to slide in at least one portion of the bore of the hollow branch of the body 6 of the ancillary, on the other hand means of axial immobilization of the implant 1 in the hollow branch of the body of the ancillary device constituted by a notch 13 or by a recess formed at the base of said stud, at least one of the edges of this notch engaging an internal shoulder 12 preferably formed in the vicinity of the end of the ancillary body and partially closing off the through bore of the latter, this edge of the notch being held in engagement by the insertion of the stud 5 in the axial bore 11, preferably blind, of the anvil screw 8, thus preventing any movement r elatif between the implant and the ancillary body 6. Thus, unlike existing devices, the anvil screw is secured to the ancillary body, the nail is engaged with the ancillary body, the entire assembly being performed when the anvil screw is positioned definitively in the ancillary body without positive direct locking action of the anvil screw on the nail 1.
In addition to the aforementioned characteristics, the intramedullary nail takes the form of a solid rod with a distal end generally at a point shown in 4 in FIG. 1 to penetrate the bone and with a proximal end used to drive the nail into the medullary cavity of said bone. , by action by means of a tool, such as a hammer, on the anvil screw 8. This nail 1 has transverse holes 2 and 3, for receiving fixing members such as screws (not shown) ensuring the immobilization of the implant in the bone. Generally, the nail 1 has a transverse hole 3 at the proximal end and a pair of transverse holes 2 at the distal end. To allow the fixing of fixing members in these transverse bores, it is necessary to complete the ancillary body by adding a sight 9 to it.
The aim of this viewfinder is to identify the orifices 2 and 3 of the implant without being obliged to resort to radioscopy techniques. This viewfinder 9 is mounted integral with the second branch 7 of the ancillary body 6 forming an operating handle. This viewfinder, which constitutes a drilling guide, makes it possible to position the fixing members transversely to the implant in correspondence with the transverse holes 2, 3 of the latter. To allow the viewfinder to be fixed to the operating handle, the second branch 7 of the ancillary body has a longitudinal slide inside which the viewfinder moves longitudinally, affecting the shape of a slide movable along the axis longitudinal of said slide. This slide comprises on the one hand holes 14 and 15 for the passage of the fixing members, on the other hand a clamping member 10 constituted in the example represented by a nut or a knurled screw, ensuring the immobilization of the viewfinder 9 inside said slide (not shown in the figures).
We will not describe below the technique used to coincide and identify the orifices 2 and 3 of the implant by means of said viewfinder. These techniques are well known to those versed in this art. On the other hand, it is possible to provide that the transverse holes forming the drilling guide 14 and 15 of the viewfinder 9 are spaced by a distance D greater than the distance separating the distal and proximal holes of the implant 1 so as to allow a tightening and compression compression of fractured bone fragments.
Cette opération s'effectue très rapidement par dévissage de la vis enclume 8 qui permet un enlèvement très rapide du corps d'ancillaire 6.For the implementation of an operational surgical method known as for example locked nailing, the procedure is as follows. We start by performing a preoperative radiological measurement of the bone to be operated on in order to define the diameter and length of the nail to be inserted. Once the fracture has been reduced if necessary, the nail is inserted inside the medullary cavity of the bone. The nailing is finished after having correctly secured the nail to the ancillary. This attachment of the nail to the ancillary takes place in the following manner. As shown in FIGS. 1 and 2, the anvil screw 8 comprising a thread 16 is introduced by sliding inside the hollow branch of the ancillary body 6 and comes by rotation progressively engaged with a thread 17 of the hollow branch of the ancillary body 6. At the same time, the nail 1 is inserted through the other end of the hollow branch of the ancillary body 6 and is arranged so that the recess or notch 13 formed at the base of the member protruding 5 of the nail 1 engages the internal shoulder 12 of the hollow branch of the ancillary body 6. In fact, the edges of the notch 13 come to grip with or without play the flat faces of the internal shoulder or radial finger 12 of the hollow branch of the ancillary body 6. The screwing of the anvil screw 8 is progressively continued inside the hollow branch of the ancillary body until the protruding member 5 of the nail 1 enters an axial bore blind it of the screw in anvil 8 thus ensuring the axial immobilization and centering of the nail 1 in the hollow branch of the anvil body 6. When the anvil screw 8 is completely introduced inside the hollow branch of the ancillary body 6, its face equipped with the blind axial bore 11 can come to bear against the internal face of the internal shoulder 12 of the hollow branch of the ancillary body 6. Thanks to this method of fixing, any relative movement of the nail 1 relative to the body of ancillary 6 is prevented. It should be noted that due to the design of the internal radial shoulder 12 of the hollow branch of the ancillary body 6, on the one hand it is necessary to introduce the nail 1 at an angle inside the hollow branch of the ancillary body 6, on the other hand, one obtains immobilization in rotation of the nail 1 relative to the hollow branch of the ancillary body 6. In fact, as shown in FIG. 2, the internal shoulder 12 of the hollow branch of the ancillary body 6 is constituted by a semicircle at least partially closing the through bore of the hollow branch of the ancillary body 6, the rectilinear part of this semicircle coming to bear on the bottom plane of the notch 13 of the nail 1 preventing any rotational movement of the latter. Obviously, to avoid this rotational movement, one could also have adapted the section of the projecting member 5 so that this section is not of revolution so as to cause immobilization in automatic rotation of the nail 1 during its introduction in the axial bore preferably blind it of the anvil screw 8 also different from a form of revolution. Once the nailing is done and the nail is properly secured to the ancillary, a few blows with a mallet are applied to the anvil, allowing the trocar tip of the end of the nail to come to rest safely on the internal cortex of the bone. Thereafter, the viewfinder adapted to the type of nail used is placed on the ancillary body. Thanks to its clamping device and the slide inside which it can slide freely, the viewfinder is brought as close as possible to the bone to be locked in order to eliminate any risk of parallax when aiming. When the three locking holes corresponding to the nail used have been identified on the viewfinder, a wicking is carried out through the piercing barrels or piercing orifices, then a wicking of the external and internal cortices of the bone is carried out by means of d 'a forest. Subsequently, the fixing members, generally constituted by locking screws, are anchored in the holes 2 and 3 of the implant provided for this purpose. Once the locking has been carried out, as may be assumed in FIG. 3, the ancillary device is detached from the implant. This operation is carried out very quickly by unscrewing the anvil screw 8 which allows very rapid removal of the ancillary body 6.
Subsequently, two cases are possible. Either the stud 5 for centering and for radial or transverse immobilization of the nail causes discomfort and is cut. Otherwise, that is to say in the case where it is preserved, it can allow possible removal of the implant after consolidation. In this case, after resumption of the incision and removal of the locking screws, the implant is easily removed by pulling on the ancillary after having previously consolidated the latter to the stud 5 of the nail 1. This has an important advantage not known to date.
This type of device applies to a large number of bones, in particular all of the long bones for which central medullary nailing applies.
