Abstract

Vis à ligament destinée à la fixation dans les tunnels osseux de transplants ligamentaires souples (allogreffe, autogreffe ou synthétique). La vis est cylindrique, elle est vissée dans l'axe du même tunnel osseux que celui où est ancré le néoligament, et ne comporte aucune protubérance. Elle est caractérisée par: - un filetage à grand pas comportant un bord arrondi (2) - un noyau dont la partie céphalique présente une section progressivement croissante (5) assurant un blocage du transplant contre l'orifice cortical du tunnel osseux, - des encoches (6) disposées à sa tête pour permettre un blocage secondaire par repousse osseuse - un canal axial pour le passage d'une broche - une empreinte hexagonale (7).

Description

La presente invention porte sur une vis particulière utilisée dans la chirurgie osseuse et articulaire, pour fixer des transplants ligamentaires de tous types qui remplacent ou suppléent des ligaments défectueux.
Clest une vis è Ligament.
Elle permet de fixer solidement tous ligaments a l'os directement dans un tunnel osseux.
Cette vis spéciale est placée directement dans le me me tunnel osseux. Elle se place dans itaxe du tunnel, d'od la qualification retenue d'axiale.
La solidité de la fixation du ligament dans le tunnel au moyen de cette vis procède des deux mecanismes Suivants
- application du ligament contre les parois du tunnel
par la vis
- blocage du ligament conte Itentrée du tunnel
qui est particulièrement dur. entant formé d'os
cortical.
Pour ce faire, la vis présente les caracteristiques suivantes: - Elle est atraumatique pour éviter de léser le transplant:
* son filet est mousse (arrondi à grand rayon) pour
appuyer par une grande surface, d'une part sur les
parois spongieuses du tunnel, et d'autre part sur
la paroi spongieuse elle même
* le pas de son filet est long pour limiter le frotte
ment de son bord mousse sur les fibres lors de la
mise en place.
- Elle a un noyau large pour occuper un grand volume
du tunnel et ainsi appliquer le transplant sur l'os
spongieux des parois du tunnel. Ce moyen de fixation
mécanique primaire permet la repousse osseuse entre
les fibres du transplant jusqu'd obtention d'une fi
xation biologique secondaire.
- Le noyau large s'évase en tronc de c3ne au niveau de la tête de la vis. Comme cette tête vient forcé
ment se positionner au niveau de l'orifice cortical du
tunnel les fibres du transplant sont bloquées de
façon ferme a cet endroit.
Ce système de blocage - interne a la vis - permet
de garder une vis cylindrique sur toute sa longueurs
pouvant être vissée complètement dans le tunnel, ne
faisant aucune saillie et laissant ainsi toute liberté
au jeu musculaire et tendineux.
- La tête de la vis comporte des encoches en nombres
et tailles adaptes réparties sur toute a circonfé
rence, dans lesquelles l'os peut repousser.
Ceci permet d'obtenir une fixation biologique qui
prendra le relai de la fixation mécanique par blocage
au niveau de la tête.
- Elle possSde une extrémité distale en forme de cone
tronqué pour favoriser l'insertion et une perforation
axiale qui admet une broche-guide mousseS
Ainsi la vis est guidée par la broche qui traverse
le tunnel dtun orifice cortical 9 l'autre, en s' appu-
yant sur eux. De ce fait la vis ne peut dévier dans
le spongieux.
Le guidage, en cas de serrage non conforme, permet
d'enlever la vis et de la remplacer par la vis de
diamètre idéal.
- La tête de la vis comporte une empreinte hexagonale
permettant l'utilisation d'un des tournevis standard
de salle d'opération.
La vis à ligament permet de fixer des transplants.

sur l'individu dans le manie temps opé
ratoire
* allogreffe; tendon, ligament, aponévrose, prélevés
sur un autre individu. Ces tissus ont
été traités industriellement et stockés
déshydratés et stériles
* synthétique: fibres de tous types, tressées, tricotEes
ou juxtaposées.
Elle pourrait être aussi utilisée dans les cas de:
* xénogreffe
* fibres collagènes synthétiques
Plus généralement, cette vis permet de fixer tous les transplants constitués de fibres souples dans des tunnels
Osseux.
Un grand éventail de tailles permet d'utiliser cette vis dans toutes les grandes artjculattons où elle répondra à des besoins spécifiques.
Ces vis seront réalisées en métal ou alliage biocompatibles ainsi qu'en tous matériaux bocompatible-s résorbables.
Grâce aux caractéristiques décrites c-de;ssus et comme le montre la figure 2 de la planche ci jointes la vis a ligament présente une somme de particuliratés qui lui sont propres et qui en font son originalité et son intérêt.
1. La fixation du transplant, a distance de l'articulation,
laissant une bonne longueur de transplant en contact
avec l'os spongieux sur toute sa circonférence représente
l'avantage majeur:
- il permet le maintien de la precision du positionnement
du transplant pendant toute la durée de la réhabitation
tissulaire
- il permet la repousse osseuse sur toute la périphérie
du transplant à son site d'insertion.
2. L'ancrage direct du transplant dans le tunnel permet
une incision cutanée plus courte:
- le temps opératoire est raccourci car il n'est plus
nécessaire de faire, à coté du tunnel, un autre forage
taraudage pour visser une rondelle, ou de dégager
un espace pour agrafer le transplant - l'avantage esthétique est certaiti.
3. La vis, de par sa forme cylindrique et sa position
axiale dans le tunnel, fait bouchon et empêche toute
fuite de synovie â partir de l'articula-tian,

    l'introduction et la mise en place.
