Abstract

La présente invention concerne une prothèse articulaire, notamment quoique non exclusivement, une prothèse de hanche à collerette externe. Elle est constituée d'une tige (1), comportant une logette (2) permettant de loger la cale d'appui externe (3). La cale comporte en son extrémité une vis (4). Cette prothèse présente de bonnes caractéristiques mécaniques car elles ont un appui symétrique bien réparti sur le fémur. La présence de la cale d'appui externe diminue les pics de contraintes au niveau de la partie distale de la prothèse.

Description

PROTHESE DE HANCHE A COLLERETTE EXTERNE
La présente invention concerne une prothèse articulaire notamment, quoique non exclusivement, une prothèse de hanche. Le principe de conception et de fabrication d'une telle prothèse de hanche s ' applique à toute autre prothèse articulaire.
Le but de cette invention est de palier le mauvais appui des prothèses sur l'os. En effet, les problèmes qui surviennent avec le temps (descellement, lyse, fracture, ...) sont surtout dûs au mauvais appui de la prothèse sur l'os.
Les prothèses que nous pouvons trouver actuellement sur le marché sont peu performantes d'un point de vue mécanique:
Les prothèses posées sans ciment réhabilitables par l'os spongieux sont dangereuses en cas de reprise (fracture, hémorragie,...).
Les prothèses scellées avec beaucoup de ciment sont souvent descellées au bout d ' un certain temps.
Les prothèses posées avec très peu de ciment engendrent des déséquilibres mécaniques: elles n'ont qu un appui interne.
Les prothèses autoblocantes s'enfoncent progressivement dans le canal médullaire avant la stabilisation définitive au prix d' un raccourcissement.
Pour palier les inconvénients décrits précédemment, la prothèse est conçue selon les caractéristiques suivantes: 1) La prothèse présente une collerette sur la face externe s'appuyant sur la corticale externe fémorale.
2) La cale d'appui externe s'emboite dans une logette externe de la prothèse et s'y fixe par une vis ou par tout autre moyen approprié.
La logette est située dans une zone ne compromettant pas la résistance de la prothèse, en particulier au niveau du grand trochanter.
3) Ces prothèses ont montré la bonne conception mécanique en présentant un appui symétrique bien réparti sur le fémur. Ces prothèses permettent une arthroplastie avec ciment et avec une fixation sur la face externe.
4) Les prothèses pourront être fabriquées dans tous matériaux biocompatibles, par exemple alliage à base de titane, inox, chrome-cobalt, ou tout autre matériel présentant des caractéristiques favorables à son implantation.
L'invention va être maintenant décrite plus en détail en référence aux dessins annexés sur lesquels:
La figure 1: représente la prothèse de hanche.
Le dispositif selon l'invention comprend: 1) Une tige (1) qui peut-être de différentes tailles ou de différentes formes adaptées à la morphologie de l'os à opérer. Cette tige comporte ou ne comporte pas, selon les cas, une collerette interne (5) venant prendre appui sur le col.
Elle comporte une logette (2) ou une fente ou tout autre dispositif permettant de loger la cale d'appui externe (3).
2) Une cale d'appui externe (3) de différentes formes. Tout système susceptible de fixer la pièce à l'os sur sa partie externe pourra être implantée. Un mode préférentiel de cette cale serait un cylindre.
La cale comporte en son extrémité une vis (7).
Un calcul de contraintes nous a permis d'apprécier les caractéristiques mécaniques de ce type de prothèse.
Ces études montrent que: 1) La cale d'appui externe est essentiellement soumise à des contraintes de traction.
2) La présence de cette cale d'appui externe crée un accrochage de la prothèse dans la masse osseuse et contribue au meilleur transfert des charges de traction de la prothèse vers l'os, éliminant ainsi les problèmes dûs à un éventuel liseret sur la face externe.
3) En absence de charges musculaires, la cale d'appui externe empêche la formation du liseret. En cas de sollicitations par des forces musculaires, la cale joue un rôle encore plus important.
4) Le fait que la cale soit horizontale n'a pas d'effet sensible sur l ' état des contraintes. Nous pourrons donc incliner la cale d'appui externe si nous le désirons.
5) L'étude mécanique montre que la cale d'appui externe diminue le niveau des pics de contraintes dans le région distale et donne une meilleure distribution des contraintes dans cette région.
6) La collerette interne favorise la transfert des charges de compression sur l'os cortical et diminue les charges de cisaillement sur la face interne.
7) La collerette externe favorise le transfert des charges de traction.
Elle favorise aussi les contraintes de compression dans des cas de charges particuliers tels que la montée ou descente d1 escaliers.
Comme il va de soi, l'invention ne se limite pas à la seule forme d'exécution décrite ci-dessus à titre d ' exemple non limitatif. Elle en embrasse, au contraire, toutes les variantes de réalisation comportant des moyens équivalents.
