Abstract
Description
The present invention relates to a surgical instrument, known as a trocar, and which is commonly used in human or veterinary surgery, to take samples of living tissue and in particular bone samples.
The usual trocar is in the form of a metal sampling tube, one end edge of which is cut in the form of teeth, while at the other end there is a handle and a handle. The surgeon grasps the handle in the palm of his hand, and he pushes the toothed end of the trocar by manually giving it an alternating movement of rotation around the longitudinal geometric axis of the tube. Thus, the tube is inserted into the tissues, where it is filled with a carrot cut by its circulmary end teeth. This core constitutes the sample which is generally taken for analysis.
The trocar being inserted in an alternating rotational movement, the toothed ring located at its tubular end has teeth whose profile is necessarily symmetrical. The movement being performed manually, this results, for the surgeon, both physical fatigue, and above all a lack of precision in the conduct of the operation.
The object of the present invention is to avoid these drawbacks, by producing a mechanized trocar, that is to say one whose rotational movement is produced mechanically, so that the surgeon is only concerned with the precise guidance of the end of sampling.
A surgical trocar according to the invention comprises a tubular drill bit with an end provided with an annular toothing, while means are provided for dragging it in rotation, and it is characterized in that the drive means consist of a connector. connected to the first end of a hose, the second end of which is actuated by a motor assembly which rotates it always in the same direction, in a continuous movement, while in addition, the teeth of the drill bit are cut according to a profile which can be asymmetrical on each tooth.
According to another characteristic of the invention, a gun comprising a handle and an "AMRCHE - ARRET" control button is interposed between the hose and the drill bit.
According to another characteristic of the invention, the front of the gun is connected to the drill bit by a detachable adapter
According to another characteristic of the invention, the rear of the gun is connected to the hose by a removable quick coupler.
According to another characteristic of the invention, the gun comprises, over its entire length, a rotating shaft, the front and rear of which are provided with means for quick attachment to the drill bit adapter and to the hose coupler.
Thanks to these last provisions, the drill bit, the gun, and the adapter constitute a set of elements which can be easily disassembled and sterilized at each intervention.
According to another characteristic of the invention, the motor assembly is in the form of a portable control unit which contains in particular
- an electric motor, connected to the second end
flexible
- a power-on button, to activate the mo
tor
- a, speed variation button, to allow the
surgeon adjusting the rotation speed of the hose
and drill bit.
According to another characteristic of the invention, the profile of the trocar teeth is a profile with different inclinations on the two sides of the tooth, which makes it possible to obtain both
- the most effective size, given the nature of the
tissue to be removed
- the most efficient disposal for waste and parSi-
cules produced by the size during the insertion of the
rotary trocar
The appended drawing, given by way of nonlimiting example, will allow a better understanding of the characteristics of the invention, and the advantages which it is likely to provide.

Figure 4 is a longitudinal section corresponding to Figure 3 after assembly.
Figure 5 shows the pistol hung in the rest and transport position.
Figure 6 illustrates the drill bit and its drive adapter.
Figures 7 to 10 are longitudinal sections illustrating the successive phases of use to perform a biopsy.
The mechanized trocar illustrated in the drawings includes
- a drill bit 1 formed by a tubular body 2 and a cu
rear fixing set 3
- a guide gun 4
- a flexible 5 whose fixed outer sheath 6 contains
a rotating soul 7
- a portable control unit 8
The tubular body 2 of the drill bit 1 ends, at the front, by an annular toothing 9, each tooth of which has an asymmetrical profile defined by two flanks, namely
- an attack flank 10, located at the front relative to
in the direction of rotation (arrow 11)
- a rear clearance side 12
The inclination of the front flank 10 relative to the longitudinal geometric axis 13 is smaller than that of the rear flank 12
Furthermore, helical grooves 14 are preferably hollowed out on the cylindrical external face of the body 2, in order to collect and store the size waste detached by the toothing 9 during the insertion into the organic tissues 15 (FIGS. 7 to 10).
The base 3 is provided with lugs 16 so that it can be fixed both by a bayonet assembly in the front of a rotary adapter 17, the rear of which is connected to the front 18 of a rotating shaft 19. This one turns freely in the body of a pistol 4 which the practitioner grasps by a handle 20
The rotating rear 21 of the shaft 19 receives the first end of the rotating core 7 which drives it. A quick connector 22 allows the first end of the hose 5 to be connected to the rear of the gun 4, with in particular
- connection of the fixed sheath 6 on the udder body
tolet 4
- connection of the rotating core 7 on the shaft 19 of the
pistol 4
- electrical connection of incorporated conductors 23
to sheath 6 with conductors not referenced from
pistol 4, connected to a control button 24 or
trigger "ON - OFF" of the handle 20
The second end of the hose 5 is connected to the housing 8 which contains an electric motor, the energization of which is actuated by a button 25.
In addition, a speed variator button 26 makes it possible to choose the speed of rotation of drill bit 1.
A support leg 27, surmounted by a hook 28, is preferably incorporated into the housing 8 to receive the gun 4 in the rest position. In particular, one can provide, for this purpose, on the body of the gun 4, a handle 29, capable of fitting on the hook 28 (Figure 5).
The operation is as follows
In the rest position (FIG. 5), the entire device constitutes an easily transportable device.
After each use, the removable nature of the constituents (and in particular of the members 1, 7, 4) facilitates sterilization.
Finally, during the intervention itself (Figures 7 to 10), we see that the drill bit 1 is driven in a continuous rotational movement, always in the same direction, while the surgeon is only concerned with guiding the progressive insertion. of body 2 (Figures 7 and 8). During this phase, the particles or cutting waste accumulate in the storage grooves 14 (FIG. 8), without polluting the surrounding tissues 15. This gyidage is carried out conveniently by the poi gonze 20 of the gun 4, which the practitioner holds well in hand
When the practitioner removes the trocar 1 with the biopsy sample 30 which it contains (FIGS. 9 and 10), the hole 31 left in the tissues 15 is perfectly clean and not flared, as could happen with known manual trocards .
