Fundamentos
El descubrimiento del código genético hace cuatro décadas definió los principios mediante los cuales los genes codifican las proteínas (1). Sin embargo ha permanecido desconocido el código proteico que es el lenguaje biológico por el cual las proteínas actúan e interactúan (2,3). La interacción de hormonas y otros ligandos con sus respectivos receptores, de las enzimas con los sustratos proteicos, de las proteínas adhesivas con integrinas, y de los anticuerpos con los antígenos, así como otras acciones e interacciones de proteínas están determinadas por los mismos principios de estructura/función y el mismo lenguaje biológico. Para un número muy limitado de proteínas el interior hidrófobo de la proteína forma el centro activo que también determina su función. En la amplia mayoría de las proteínas, sin embargo, la actividad biológica está mediada principalmente por sus estructuras superficiales específicas que a su vez median sus funciones específicas. Se descubren los principios de estructura/función que determinan estas funciones proteicas (1) y se hace referencia en lo sucesivo a los mismos como el código proteico.
Sumario de la invención
Al igual que el lenguaje humano el código proteico consiste en letras, palabras y frases. Las letras (aminoácidos) y las frases (proteínas tridimensionales completas) ya son conocidas. El nuevo descubrimiento son las palabras o los verbos proteicos. Estos verbos proteicos están representados por oligopéptidos de señal que están localizados en la superficie de la proteína y están representados por la máxima hidrofilicidad de la proteína. Estos oligopéptidos de señal están enriquecidos en aminoácidos cargados en una disposición versátil con aminoácidos espaciadores neutros. El carácter de señal específica de estos oligopéptidos está determinado por una combinación característica de conformación y carga dentro de la misma secuencia de señal.
Al igual que en el lenguaje humano es necesaria la frase completa (proteína tridimensional completa) para determinar la acción específica y completa de cualquier proteína dada. En el lenguaje humano eliminar o cambiar el verbo de una frase hace ininteligible a la frase completa. Similarmente, el bloqueo de los verbos del código proteico (los oligopéptidos de señal) puede ser usado terapéuticamente para bloquear la acción o interacción indeseada de una proteína completa.
El descubrimiento del código proteico proporciona el conocimiento racional para descifrar el código de comunicación de las enfermedades. Las enfermedades infecciosas, el cáncer, las enfermedades cardiovasculares y otras enfermedades humanas se desarrollan por medio de una o más proteínas que median la patogenicidad. El bloqueo de los oligopéptidos de señal de estas proteínas permite la intercepción terapéutica específica de una comunicación patológica, y bloquea de este modo la propagación de la enfermedad.
Por el trabajo de Lukey y Stevens, de 1985 en The Journal of Biological Chemistry 260:10271-10277 se conoce el cDNA de longitud completa para la 3-hidroxi-3-metilglutamil-coenzima A-reductasa que es la glicoproteína unida a la membrana que regula la síntesis de colesterol.
Los análogos sintéticos a las secuencias de oligopéptidos de señal se usan terapéuticamente de diversos modos. Primeramente, los análogos sintéticos a secuencias de oligopéptidos de señal pueden usarse como inhibidores competitivos de comunicación patológica. En segundo lugar los análogos sintéticos a los oligopéptidos de señal pueden usarse como vacunas. Este segundo enfoque terapéutico hace uso del hecho de que los oligopéptidos de señal en la superficie de la proteína son idénticos a los epítopos determinantes de la antigenicidad de esta proteína. Por tanto los anticuerpos son interceptores de la comunicación metabólica. Por unión al oligopéptido de señal los anticuerpos y otros mediadores de la respuesta inmune reducen o bloquean su interacción metabólica. Si se usan como vacunas análogos sintéticos a los oligopéptidos de señal es necesario convertirlos en antigénicos a estos péptidos para permitir su discriminación como "no propios". Los análogos sintéticos a los oligopéptidos de señal pueden hacerse inmunógenos acoplándolos a haptenos y por otros métodos convencionales.
La invención se basa en métodos en los cuales las secuencias de oligopéptidos pueden hacerse inmunógenas. Este método se basa en el descubrimiento de los principios estructurales primarios que determinan la inmunogenicidad. La discriminación entre lo propio y lo extraño entre especies y entre individuos está basada principalmente en sustituciones de residuos de aminoácidos u otras variaciones de residuos dentro de las secuencias de oligopéptido de señal de una proteína. Haciendo uso de este descubrimiento pueden producirse rápidamente oligopéptidos de señal eficaces del siguiente modo: los oligopéptidos de señal de una proteína dada en una especie de animales son los determinantes antigénicos de esta proteína en otra especie de animales. Para bloquear la acción de una proteína mediadora de la patogenicidad en el tratamiento de una enfermedad humana las vacunas de oligopéptidos de señal sintéticos se diseñan copiando las correspondientes secuencias de señal de aminoácidos de otra especie. Un oligopéptido de señal del glucagón para el tratamiento de pacientes diabéticos estaría basado en las secuencias de señal del glucagón de especies de conejos, ovejas, ratones u otras especies. Una valoración de la eficacia terapéutica es posible. Cuanto mayor es la distancia genética y evolutiva a los seres humanos de la especial animal seleccionada mayor es la antigenicidad y, consecuentemente, mayor es la eficacia terapéutica como vacuna.

Diferenciación: La sustitución, deleción u otras variaciones de residuos de aminoácidos dentro de la secuencia de señal de una proteína facilitan la diferenciación entre lo propio y lo extraño. Las secuencias de señal son los epítopos antigénicos de una proteína y son responsables de las potenciales respuestas inmunes cuando se exponen a otros organismos. Intercepción: La terapia de intercepción de péptidos (abreviadamente PIT por las iniciales de su expresión inglesa Direct Peptide Intercepción Therapy) directa puede emplearse para interceptar la comunicación no deseada o patológica y con ello bloquear la enfermedad. En la PIT directa los análogos sintéticos a los oligopéptidos de señal pueden ser usados para inhibir competitivamente la interacción de proteínas (Figura 1C). La PIT indirecta hace uso de oligopéptidos sintéticos que han sido convertidos en inmunógenos. Estas vacunas estimulan la producción de anticuerpos que bloquean las rutas de comunicación patológicas no sólo en la prevención y tratamiento de enfermedades infecciosas, sino también en la terapia de enfermedades neoplásicas, trastornos metabólicos y otras enfermedades
(Figura 1D).

I. Principios determinantes del código proteico
1.
Universalidad. El código proteico es universalmente válido. Las proteínas son vehículos de información metabólica en los organismos vivos. El código proteico es el lenguaje biológico para la información mediada por proteínas durante la salud y la enfermedad. El código proteico es el principio de comunicación subyacente para la interacción de antígenos con anticuerpos, enzimas con sustratos, receptores con ligandos, moléculas de adhesión con integrinas y otras formas de comunicación proteica.

Compatibilidad. El código proteico es compatible con otros elementos de comunicación biológica. El código proteico y el código genético están estrechamente conectados. El código genético determina la estructura de las proteínas y, por tanto, su función. El código proteico determina la relación entre estructura y función dentro de una secuencia de proteína y, por tanto, la acción biológica específica. Las acciones específicas y eficientes de una proteína son responsables de la ventaja de un gen y, por tanto, de la supervivencia de la información genética que codifica la proteína.

Legibilidad. El código proteico está compuesto de letras, frases y palabras. Las letras del código proteico están representadas por los residuos de aminoácidos de una proteína. La frase del código proteico está representada por la estructura tridimensional completa de una proteína que finalmente determina su acción. La señal decisiva de comunicación de proteínas está mediada por las palabras del código proteico. Estas palabras del código proteico están representadas por uno o más oligopéptidos de señal dentro de una secuencia de proteína. En los verbos del lenguaje humano los verbos son las palabras de acción dentro de una frase. Los oligopéptidos de señal son los verbos del código proteico y determinan la acción de la proteína. La interferencia con estos péptidos de señal puede conducir a una modificación sustancial o a la pérdida del mensaje biológico de una proteína.
4.
Accesibilidad. El código proteico ha de ser accesible. La transferencia de información proteica y la interacción de proteínas son dependientes de la accesibilidad de la secuencia de señal. Los oligopéptidos de señal están representados generalmente por las regiones de máxima hidrofilicidad en la superficie de la molécula de proteína. Los determinantes críticos para estos máximos de hidrofilicidad son un enriquecimiento de los residuos de aminoácidos catiónicos arginina y lisina y/o los residuos de aminoácidos aniónicos glutamato y aspartato.

Variabilidad. El código proteico requiere una reserva suficiente de variaciones estructurales que codifican un gran número de señales diversas. El código proteico se basa en un número casi infinito de estructuras oligopeptídicas de señal posibles. Una disposición específica de residuos de aminoácidos cargados en combinación con un número variable de residuos no cargados dentro de tripéptidos, tetrapéptidos, pentapéptidos, hexapéptidos o polipéptidos mayores de señal proporcionan el número necesario de combinaciones posibles de residuos para una enorme reserva de señales.

Un nuevo tipo de señales está representado por oligopéptidos que obtienen su conformación característica mediante una disposición específica de residuos de aminoácidos cargados dentro de esta secuencia oligopeptídica. Estos residuos con carga opuesta pueden atraerse entre sí modulando un plegamiento característico de esta secuencia de señal. En la secuencia de señal RGD (tripéptido constituido por arginina-glicina-ácido aspártico) el residuo catiónico de arginina y el residuo aniónico de aspartato se atraen entre sí lo que conduce a un plegamiento característico de este tripéptido alrededor del residuo "espaciador" de glicina.
6.
Especificidad. El código proteico es específico. La función metabólica específica y fiable de una proteína es dependiente de la especificidad de sus secuencias de señales biológicas. El carácter de señal de un oligopéptido específico está determinado por una combinación característica de carga eléctrica y conformación estructural. Dentro de una proteína el tripéptido RGD y otros tripéptidos análogos pueden servir como fuertes anclas principales, mientras que el mensaje biológico específico está mediado por oligopéptidos de señal adicionales más grandes y más complejos.

Diferenciación. El código proteico proporciona la base para la diferenciación inmunológica entre individuos y especies. Sustituciones, omisiones, y otras variaciones de uno o más residuos de aminoácidos dentro de la secuencia señal de una proteína permiten a un organismo diferenciar entre lo "propio" y lo "extraño". Por tanto, el código proteico comprende el lenguaje básico de la inmunología.

Diversificación. El código proteico es clave para el desarrollo individual dentro de una especie, así como para la diversificación evolutiva de las especies. La eficacia de los oligopéptidos de señal para mediar mensajes biológicos específicos fue el último criterio para la ventaja evolutiva de una proteína y, por tanto, para la supervivencia evolutiva del gen que la codifica. Las mutaciones genéticas que conducen a la sustitución de uno o más residuos de aminoácidos dentro de una secuencia de oligopéptido de señal fueron un mecanismo económico y por tanto frecuente para modular y diferenciar la acción de las proteínas y por tanto promover la diversificación evolutiva.

Intercepción. El código proteico es interceptable. Los oligopéptidos de señal de una proteína son idénticos a los de los determinantes antigénicos potenciales. Los anticuerpos y otros mediadores de la respuesta inmune son interceptores de comunicación biológica específica. Los oligopéptidos de señal como promotores de la comunicación proteínica diferenciada y mediadores de la respuesta inmune como interceptores, pueden formar un retículo sofisticado de comunicación biológica. Descodificar los aspectos fisiológicos de este retículo de comunicación conducirá a una comprensión precisa de millones de interacciones metabólicas que incluyen los principios para el desarrollo y diferenciación del organismo. Descodificar los aspectos patofisiológicos de este retículo de comunicación conducirá al control terapéutico de muchas enfermedades y finalmente a su erradicación como causas de la mortalidad humana.
10.
Regulación. El código proteico podría también proporcionar el eslabón que falta en la regulación de la síntesis de proteínas. Las secuencias de señal de una proteína pueden tener importantes funciones de retroalimentación que modulan directa o indirectamente la tasa de síntesis de esta proteína.
II. Principios de la terapia de intercepción de péptidos (PIT) y de la terapia de regulación de péptidos (PRT)
1.
La interacción metabólica de proteínas está modulada principalmente por una o más secuencias de señal oligopeptídicas que determinan la interacción metabólica de esta proteína. La terapia de intercepción de péptidos (PIT) se define como la intercepción terapéutica específica de acciones de la proteína patológicas o no deseadas e interacciones por el uso terapéutico de análogos sintéticos a las secuencias de señal oligopeptídicas de esta proteína. La terapia de regulación de péptidos (PRT) hace un uso terapéutico de los análogos sintéticos a las secuencias de señal oligopeptídicas para disminuir la tasa de síntesis de una proteína no deseada mediante mecanismos de retroalimentación.

La terapia de intercepción de péptidos usa el descubrimiento del código proteico. Para promover la acción proteínica se requiere la estructura proteínica tridimensional completa (frase proteínica). Para interceptar la acción de la proteína solamente ha de ser bloqueado terapéuticamente el oligopéptido de señal (el verbo proteínico).

Estas secuencias de oligopéptido de señal están localizadas en la superficie de la proteína y están representadas por los máximos de hidrofilicidad dentro de la secuencia de aminoácidos de la proteína. Las secuencias de señal oligopeptídicas de una proteína pueden ser identificadas a partir de su estructura primaria mediante el uso de una base de datos de proteínas en combinación con un algoritmo adecuado, tal como para la hidrofilicidad o la probabilidad superficial. En este algoritmo los mayores valores de hidrofilicidad o probabilidad tienen que ser asignados a los aminoácidos cargados lisina, arginina, aspartato y glutamato seguidos por asparagina y glutamina.

Los análogos sintéticos a las secuencias de señal oligopeptídicas se pueden usar terapéuticamente. Pueden sintetizarse un número ilimitado de análogos de oligopéptidos de señal que cubran la secuencia completa de un pico de hidrofilicidad seleccionado o una de sus partes.

Los tetrapéptidos, pentapéptidos, hexapéptidos y péptidos más grandes de señal representan los candidatos principales para la terapia específica con péptidos. Los péptidos más cortos, tales como el tripéptido RGD, son menos específicos y sus efectos secundarios omnipresentes limitan su amplio uso terapéutico.

La terapia de intercepción de péptidos (PIT) se puede usar de dos modos principales: La PIT directa usa análogos sintéticos a los oligopéptidos de señal como inhibidores competitivos de interacciones metabólicas patológicas o indeseadas. La PIT indirecta usa estos análogos sintéticos como vacunas para estimular la producción de anticuerpos específicos. En la PIT indirecta los anticuerpos, y no el péptido terapéutico propiamente dicho, funcionan como interceptores de la comunicación de proteínas.

La terapia de intercepción de péptidos (PIT) directa usa análogos sintéticos para los oligopéptidos de señal como inhibidores competitivos directos de la comunicación de proteínas no deseada. El bloqueo directo de la comunicación de proteínas que median la patogenicidad conduce al control de la enfermedad o estado clínico relacionados. Este enfoque terapéutico sería usado preferentemente en estados agudos, por ejemplo, terapia antitrombótica o fibrinolítica. La PIT directa sería usada preferentemente por vía intravenosa en las dosificaciones terapéuticas más elevadas del péptido sintético. Pueden usarse para extender la gama de posibles aplicaciones terapéuticas análogos poli-oligopeptídicos, sistemas de administración con tiempos de liberación programados, y otras modificaciones de los mecanismos de administración de péptidos.

Terapia de intercepción de péptidos indirecta (PIT indirecta). Actualmente las vacunas están esencialmente limitadas a la profilaxis y terapia de enfermedades infecciosas. La PIT indirecta facilita la extensión del uso preventivo y terapéutico de las vacunas a todos los campos de la medicina. La gran ventaja de las vacunas oligopeptídicas, comparadas con las vacunas convencionales, es que no se usa la porción completa como vacuna sino solamente los análogos sintéticos de uno o más oligopéptidos de señal de la proteína seleccionada.

La PIT indirecta está basada en el uso terapéutico de anticuerpos producidos por el propio sistema inmune del paciente contra las secuencias de señal de las proteínas que median la patogenicidad para una acción metabólica no deseada. La PIT indirecta se usa preferentemente para terapia preventiva, para el tratamiento de estados crónicos o como adyuvantes para la PIT directa u otras formas de terapia aguda.

La PIT indirecta hace uso de dos principios de inmunología recientemente descubiertos: Primero, Los oligopéptidos de señal de una proteína dada son idénticos en sus regiones determinantes de antigenicidad potencial (epítopos antigénicos). Segundo, el mecanismo primario que determina la antigenicidad entre individuos diferentes y diferentes especies son sustituciones, omisiones y otras variaciones de residuos de aminoácidos dentro de la(s) secuencia(s) de señal oligopeptídica(s) de una proteína.

El diseño del péptido terapéutico para la PIT indirecta se basa en el siguiente principio: Los oligopéptidos de señal de una proteína dada en una especie son epítopos antigénicos de esta proteína para el sistema inmune de otra especie. Para bloquear la acción de la proteína que media la patogenicidad en el tratamiento de una enfermedad humana, la secuencia de residuos de aminoácidos de las vacunas oligopeptídicas debe ser homóloga a las secuencias de señal de la misma proteína, pero de otra especie. Una vacuna de oligopéptido de señal de glucagón para el tratamiento de pacientes diabéticos estaría basada en secuencias de señal de glucagón de conejos, ovejas, ratones u otras especies. El objetivo de esta manipulación de residuos es crear un epítopo antigénico sin comprometer la capacidad de los anticuerpos producidos de bloquear eficazmente la interacción metabólica de la proteína. El enfoque terapéutico mimetiza o imita el modo de la naturaleza de discriminar entre lo propio y lo extraño y hace uso terapéutico de dicho modo de discriminación.

El diseñador de los compuestos terapéuticos para la PIT indirecta también puede hacer uso del reloj evolutivo. Cuánto más apartadas estén dos especies en evolución, se produjeron más mutaciones de residuos de aminoácidos, incluyendo mutaciones en las secuencias de los oligopéptidos de señal y más antigénico es el péptido terapéutico que comprende dicha secuencia. Es posible una valoración de la eficacia terapéutica de la PIT indirecta: La PIT indirecta con análogos sintéticos a los oligopéptidos de señal del glucagón de una especie de pez es más eficaz que la que usa oligopéptidos de una especie de mamífero en bloquear la acción del glucagón humano.

La antigenicidad requerida para la PIT indirecta eficaz también puede ser estimulada del modo usual, por ejemplo, acoplando los péptidos terapéuticos a haptenos definidos u otros compuestos inmunógenos que potencian el reconocimiento de lo extraño

Un tercer mecanismo terapéutico que usa análogos sintéticos a los oligopéptidos de señal es como reguladores de retroalimentación para la síntesis de proteínas. Esta forma de terapia se denomina terapia de regulación de péptidos (PRT). La tasa de síntesis en muchas proteínas está determinada por la cantidad de proteína y producto final disponible. Las secuencias de señal de las proteínas que median la patogenicidad se pueden usar para disminuir la síntesis de esta proteína mediante mecanismos de retroalimentación directa o indirecta.

III. Ventajas de la terapia con péptidos
1.
Eficacia. La terapia con péptidos es una forma altamente eficaz de tratamiento. El descubrimiento del código peptídico proporciona el conocimiento racional para descifrar el código de comunicación de las enfermedades humanas y permite la intercepción de las interacciones patológicas con máxima eficacia.
2.
Especificidad. La terapia con péptidos permite la máxima especificidad terapéutica. Basándose en la comprensión precisa de la relación estructura/función de señales proteínicas específicas, la terapia con péptidos permite la dianización terapéutica con especificidad sin precedente.
3.
Seguridad. La terapia con péptidos es extremadamente segura. El uso de análogos sintéticos a los compuestos fisiológicos elimina esencialmente los problemas de toxicidad. Los posibles efectos secundarios biológicos no deseados deben ser controlables optimizando la longitud y composición del péptido terapéutico.
4.
Ahorro en investigación terapéutica y desarrollo. El tiempo y los gastos para el desarrollo de péptidos terapéuticos es una fracción de la investigación y desarrollo terapéutico convencionales. La identificación de potenciales péptidos terapéuticos lleva minutos; el escrutinio in vitro de potenciales péptidos es cuestión de semanas; los estudios en animales deben proporcionar primeramente resultados in vivo en unos pocos meses; lo que es más importante, el fundamento de la especificidad y seguridad sin precedentes de la terapia con péptidos permitirá la realización de estudios clínicos sin demora.

Ventajas de la terapia con péptidos comparada con la terapia convencional. Las ventajas de la terapia con péptidos llegan a ser incluso más obvia cuando el nuevo método terapéutico se compara con las terapias convencionales o terapias alternativas futuras. La terapia con fármacos convencionales está frecuentemente comprometida por un mecanismo terapéutico desconocido y por una amplia gama de efectos secundarios y una toxicidad considerable. La terapia génica está comprometida por una disponibilidad limitada, sus requerimientos tecnológicos y sus altos costos, que restringen la aplicación terapéutica a campos exclusivos en un futuro previsible. La terapia con anticuerpos heterólogos o sintéticos, es decir la aplicación de anticuerpos producidos fuera del cuerpo del paciente puede causar reacciones adversas incalculables por el sistema inmune del paciente contra los anticuerpos "extraños". En contraste la terapia con péptidos hace un uso elegante del propio sistema inmune del paciente excluyendo de este modo las reacciones inmunológicas adversas.

IV. Aplicaciones terapéuticas de la terapia con péptidos
Trastornos metabólicos. Un nuevo campo terapéutico para la terapia con péptidos es el tratamiento de trastornos metabólicos. El potencial de la terapia con péptidos se ejemplifica en la presente memoria para el tratamiento de la diabetes y la hipertensión.
Diabetes. La terapia convencional para la diabetes se dirige una la mayor disponibilidad de insulina. La terapia con péptidos permite un método nuevo y alternativo inhibiendo la acción del glucagón, el antagonista de la insulina. La inhibición del glucagón puede conseguirse usando los análogos de péptidos terapéuticos de la secuencia de señal del glucagón para la inhibición competitiva directa o como una vacuna (Figura 5).
En la presente patente se reivindican las siguientes secuencias de oligopéptidos de señal de la hidroxil-metil-glutamil-coenzima A-reductasa, así como su uso para fines terapéuticos.
