Terapia de presión negativa usando succión de
pared.
Esta invención se refiere a la terapia de
presión negativa y proporciona un dispositivo que puede usarse para
proporcionar tal terapia en combinación con una fuente de succión
existente, tal como una línea de vacío.
La anterior solicitud de patente WO 97/18007 de
los solicitantes describe un aparato portátil para el tratamiento
de heridas para estimular la curación de heridas. El aparato
descrito en la solicitud anterior comprende una almohadilla porosa,
que es permeable a los líquidos, para colocar dentro de o sobre la
herida, apósitos para cubrir y para proporcionar un sello hermético
alrededor de la herida y un tubo de drenaje que conecta la
almohadilla a una bomba de succión de manera que pueda aplicarse
presión negativa a la herida para drenar líquidos de la misma,
proporcionando un bote para recoger los líquidos succionados de la
herida.
El paciente puede utilizar el aparato descrito
en la solicitud anterior de los solicitantes en un arnés o en un
cabestrillo por lo que no estará confinado a un lugar particular
mientras se realiza la terapia. Existe, sin embargo, necesidad de
un equipo más básico que, aunque no tan sofisticado como el equipo
descrito en la solicitud anterior, proporcione algunos de los
beneficios de la terapia de presión negativa.
La mayoría de los hospitales tienen una línea de
succión que se suministra a todas las salas y que está disponible
para el personal de enfermería para una diversidad de objetivos,
tales como hacer funcionar los tubos de drenaje y succionar
líquidos corporales en general. Para tales usos, puede conectarse un
regulador de presión a la fuente de succión y este regulador puede
incluir un indicador de presión que indique la presión en la válvula
reguladora. Es, sin embargo, peligroso conectar tal fuente de
succión directamente con un paciente, sin proporcionar supervisión
continua. En muchos hospitales, la falta de personal dificulta o
hace imposible proporcionar una estrecha supervisión adecuada, y si
se intenta usar tal fuente para la terapia de presión negativa,
existe un peligro real de producir heridas al paciente.
En algunos hospitales, pueden usarse botellas de
vacío para ayudar drenar de las heridas, por ejemplo tras las
operaciones. Tales botellas son contenedores que están disponibles
en diversas capacidades y que se evacúan a presión baja. Las
botellas de vacío pueden usarse según la invención como una
alternativa al punto de succión de pared. Tienen la ventaja de
proporcionar un mayor grado de portabilidad que el aparato de
terapia de presión negativa, pero la desventaja de que necesita
reemplazarse la botella por una nueva una vez que la presión en la
botella ha aumentado hasta una presión cercana a la presión
ambiental.
Un objeto de la presente invención es
proporcionar un equipo que pueda usarse con una fuente de succión de
pared existente para proporcionar de manera segura la terapia de
presión negativa a los pacientes.
Según la invención se proporciona un aparato
para aplicar terapia de presión negativa al lugar de una herida,
que comprende una almohadilla de espuma de celdas abiertas para
aplicar a la herida, un tubo de succión que conecta la almohadilla
de espuma a un bote de recogida y un medio de detección de la
presión conectado al tubo de succión entre la almohadilla de espuma
y el bote para indicar cuando la presión en el tubo de succión
alcanza un nivel predeterminado, caracterizado porque el aparato
incluye un tubo para conectar el bote a un punto de succión de
pared o a una botella de vacío, y porque el bote incluye una válvula
de cierre que cierra la salida desde el bote cuando está

recogida del tipo mostrado en la Patente de EEUU Nº 5.669.892 en el
primer aspecto de la invención.

El aparato puede incluir medios de medición del
caudal para medir la velocidad a la que se succiona el líquido
desde el lugar de la herida.
El medio para medir el caudal puede comprender
un dispositivo para medir la velocidad a la que se llena el
bote.
El medio para medir el caudal puede ser un
dispositivo de medición de capacitancia eléctrica.
El aparato puede incluir medios para advertir
que el bote está lleno y/o para cerrar la conexión entre el bote y
el punto de succión de pared.
El aparato puede incluir además medios para
regular la presión entre el bote y la fuente de succión.
También pueden introducirse en el aparato otras
características según se describen para aportar otras
características deseables.
A continuación se describirán varias formas de
realización de acuerdo con la invención con referencia a los
dibujos que acompañan, en los que:
La Figura 1 es una representación esquemática de
una forma de realización de acuerdo con la invención;
La Figura 2 es una representación esquemática de
una segunda forma de realización; y
Las Figuras 3A y 3B son representaciones
esquemáticas de un bote de recogida para controlar los caudales de
líquidos succionados de la herida.
Con respecto a la Figura 1 de los dibujos, el
aparato para aplicar terapia de presión negativa comprende una
almohadilla de espuma (1) que se aplica sobre o dentro de una herida
a tratar y está conectada por medio de un tubo de succión (2) a un
bote (3). El bote (3) puede ser de diseño convencional con una
válvula de cierre (mostrada como un esquema en (140)), que se
cierra automáticamente cuando el bote está lleno. La almohadilla de
espuma puede comprender cualquier espuma celular interconectada
adecuada. Las espumas que se ha encontrado que son especialmente
adecuadas son las espumas de poliuretano y de alcohol polivinílico o
sus combinaciones, que tienen células interconectadas.
Como se muestra en las Figuras 1 y 2, la
almohadilla de espuma se coloca dentro de una herida abierta y se
conecta a una fuente de presión negativa por medio de un dispositivo
conector (100) que comprende una placa posterior flexible (110) y
un tubo conector integral (120). El tubo conector (120) se
dimensiona internamente para recibir el tubo o los tubos de succión
(2) como un ajuste sustancialmente hermético. La combinación de la
almohadilla de espuma y el conector se aseguran a la herida a tratar
por medio de un campo quirúrgico (130), que puede estar formado con
una abertura para permitir que la parte tubular (120) del conector
pase a través del campo.
Un conector de este tipo se describe en la
Solicitud de Patente del RU Nº 9819678.5, WO 99/ 13793 y la
correspondiente Solicitud de Patente de EEUU presentada el 9 de
julio de 1999 (ref. de agente: 1009.1094) titulada "Campo
quirúrgico y punta de succión para el tratamiento de
heridas".
Un bote adecuado de este tipo se describe en el
documento WO 97/18007, Publicación de Solicitud de Patente Europea
Nº 0358302 o en la Patente de EEUU Nº 6.142.982 (Solicitud Nº
09/078223). El bote está conectado también a través de otro tubo
(4) a un regulador de presión (5). El regulador de presión lleva un
indicador (6) y está conectado a una línea de vacío existente tal
como una fuente de succión de pared convencional del hospital (97).
En muchas instalaciones hospitalarias ya existe una válvula
reguladora (5) junto con un indicador de presión (6), fijados a una
fuente de succión existente o pueden fijarse a una salida existente
en el suministro de la fuente de succión. El aparato puede incluir
también un limitador de flujo opcional (8), que puede ajustarse
para proporcionar diferentes niveles deseados de flujo en el
sistema.
La presión en el tubo de succión (2) se mide por
medio de un tubo bifurcado (9) que está conectado al tubo de
succión y a un transductor (10). El transductor (10) está montado en
un tablero de control del proceso (PCB) (12) y éste puede estar
conectado a un visualizador (13). Una válvula de escape opcional
(11) puede estar conectada también en el tubo (9) y proporciona un
medio para controlar el nivel de presión negativa en el lugar de la
herida. La válvula de escape (11) puede configurarse de forma manual
para que la presión en el lugar de la herida no exceda un valor
predeterminado. En una versión más esotérica, la válvula de escape
puede controlarse electrónicamente desde el PCB para liberar la
presión en el lugar de la herida a presiones máximas configurables.
Muchos hospitales, además de tener una fuente de succión y un
regulador de presión tal como el regulador (5), tienen también
botes de recogida de líquidos (3) suministrados para otros
objetivos. Por consiguiente, puede ser posible suministrar al
hospital el aparato incluido en la línea de trazos que se muestra
en la Figura 1, junto con espumas y tubos de conexión para que
puedan conectar el aparato existente a un bote y a un regulador (5)
disponible en el hospital.
En la Figura 2 se muestra un sistema más
elaborado, que es similar a la disposición mostrada en la Figura 1
excepto por las siguientes características que se describen a
continuación. Los mismos números de referencia indican
características comunes para ambas formas de realización. Primero,
el regulador de presión (8) que conecta el aparato a la fuente de
succión de pares 17 está controlado electrónicamente por el tablero
de control del proceso (12). En segundo lugar, la presión en el
lugar de la herida se controla por medio de un transductor (20),
mientras que la presión en el tubo que conecta el bote al regulador
se mide por medio de un transductor (21). El transductor (20) está
conectado al lugar de la herida por medio de un tubo (23). En lugar
de proporcionar tubos (2) y (23) separados, puede usarse un tubo de
lumen único o lumen doble o múltiple como se describe en la
solicitud de patente WO 97/18007 en trámite, del solicitante. Una
válvula de escape (11) comunica el tubo (23) y permite que el
aparato funcione intermitentemente de una manera controlable al
reducir de manera intermitente el flujo a través del regulador (8)
y liberar presión a través de la válvula (11). La situación total
del bote se detecta al notar la presión diferencial entre el
transductor (20) y el transductor (21), o por medio de un sensor de
nivel de líquidos separado. La detección de la presión en el lugar
de la herida por medio de un transductor (20) también indica si hay
pérdida de presión o no hay terapia. Un bote fabricado a medida
(32) puede incluir medios para controlar electrónicamente cuando el
bote está lleno y debe reemplazarse, por ejemplo por medios de
medición de capacitancia (34). De preferencia, el bote está diseñado
para que se ajuste a un hueco en una carcasa a medida (30),

otro extremo al tubo o tubos que llevan a la almohadilla de espuma
(1) en el lugar de la herida.
En la forma de realización de la Figura 2, los
transductores (20 y 21), la válvula de escape (11) y el regulador
de presión (8) se controlan de preferencia electrónicamente por
medio de conexiones al PCB. Por ejemplo, la situación del bote
lleno se detecta por comparación de la diferencia de presión entre
los transductores (20 y 21) y esto puede señalarse en el
visualizador (13) y, opcionalmente, también por medio de una señal
sonora de advertencia.
Puede ser deseable medir la velocidad a la que
se succionan los líquidos desde el lugar de la herida. Esto se
logra de manera conveniente midiendo la velocidad a la que se llena
el bote con exudado de la herida. En la Figura 3 se muestra de
manera esquemática un dispositivo adecuado. En una configuración
mostrada en la Figura 3A, se apoya una funda (33) en íntimo
contacto con la superficie exterior del bote. Esta funda lleva un
único elemento sensor (35), por ejemplo un sensor de capacitancia
que puede proporcionar un medio para controlar la presencia de
líquido a diferentes niveles en el bote simplemente moviendo la
funda hacia arriba o hacia abajo del bote. El elemento sensor
detecta la presencia de líquido al proyectar un campo eléctrico en
el bote y detectar cualquier cambio en ese campo, por ejemplo por un
cambio en la capacitancia. La tasa de cambio de capacitancia sobre
la porción del bote controlada por el detector da una indicación del
caudal de líquido succionado desde el lugar de la herida.
En otra configuración mostrada en la Figura 3B,
una serie de elementos sensores (36) están separados uniformemente
en una funda (33) que está en contacto con la superficie exterior
del bote. A medida que el nivel de líquido aumenta dentro del bote,
se activan los elementos sensores. Esta información puede usarse por
medio del sistema de control en el PCB para deducir el caudal.
Claims (8)
1. Un aparato para aplicar terapia de presión
negativa a un lugar de una herida, que comprende una almohadilla de
espuma de células abiertas (1) para aplicar a la herida, un tubo de
succión (2) que conecta la almohadilla de espuma (1) a un bote de
recogida (3, 32), un medio de detección de presión (10, 20)
conectado al tubo de succión (2) entre la almohadilla de espuma (1)
y el bote (3, 32) para indicar cuando la presión en el tubo de
succión (2) alcanza un nivel predeterminado por el que el aparato
también incluye un tubo (4) para conectar el bote (3, 32) a un
punto de succión de pared (7) o a una botella de vacío, en el que el
bote (3, 32) incluye una válvula de cierre (140) que cierra la
salida desde el bote (3, 32) cuando está lleno, y
caracterizado porque el aparato incluye una válvula
limitadora del flujo (8) dispuesta entre el bote (3, 32) y la fuente
de succión (7).
2. El aparato según se reivindica en la
reivindicación 1, que incluye una válvula de escape de presión (11)
que está conectada al tubo de succión (2) entre la almohadilla de
espuma (1) y el bote (3, 32).
3. El aparato según se reivindica en una
cualquiera de las reivindicaciones anteriores que incluye un primer
transductor (21) para medir la presión en el tubo (5) que conecta el
bote (3, 32) al punto de succión de pared (7) o a una botella de
vacío, y un segundo transductor (20) para medir la presión en el
lugar de la herida.
4. El aparato según se reivindica en una
cualquiera de las reivindicaciones anteriores que incluye un medio
de medición del caudal (33, 34, 35, 36) para medir la velocidad a la
que se succiona el líquido desde el lugar de la herida.
5. El aparato según se reivindica en la
reivindicación 4 en el que el medio de medición del caudal (33, 34,
35, 36) comprende un dispositivo para medir la velocidad a la que se
llena el bote (32).
6. El aparato según se reivindica en la
reivindicación 5 en el que el medio de medición del caudal (33, 34,
35, 36) es un dispositivo de medición de capacitancia eléctrica.
7. El aparato según cualquiera de las
reivindicaciones anteriores que incluye un medio para advertir que
el bote (3, 32) está lleno y/o para cerrar la conexión entre el
bote (3, 32) y el punto de succión de pared (7).
8. El aparato según cualquiera de las
reivindicaciones anteriores que incluye además un medio (5) para
regular la presión entre el bote (3, 32) y la fuente de succión
(7).
