Abstract

Um eine Tibiakomponente für eine Kniegelenkendoprothese, wobei die Tibiakomponente eine Tibiaplatte mit einer Oberseite und einer Unterseite und einen von der Unterseite abstehenden Verankerungsvorsprung zum Einführen in den Markraum einer Tibia zum Verankern der Tibiakomponente an der Tibia umfasst, bereitzustellen, die eine verbesserte Ausrichtung an der Tibia ermöglicht, wird vorgeschlagen, dass der Verankerungsvorsprung an der Tibiaplatte verschwenkbar und/oder um eine Längsachse des Verankerungsvorsprungs verdrehbar gelagert ist. Ferner wird ein verbessertes Kniegelenkendoprothesensystem vorgeschlagen.

Description

Tibiakomponente und Kniegelenkendoprothesensystem
Die vorliegende Erfindung betrifft eine Tibiakomponente für eine Kniegelenk endoprothese, wobei die Tibiakomponente eine Tibiaplatte mit einer Oberseite und einer Unterseite und einen von der Unterseite abstehenden Verankerungs vorsprung zum Einführen in den Markraum einer Tibia zum Verankern der Ti biakomponente an der Tibia umfasst, wobei der Verankerungsvorsprung an der Tibiaplatte verschwenkbar und/oder um eine Längsachse des Veranke rungsvorsprungs verdrehbar gelagert ist, wobei die Tibiakomponente eine Fi xiereinrichtung umfasst zum Fixieren des Verankerungsvorsprungs in einer Implantationsstellung und dass die Fixiereinrichtung von einer Ausrichtstel lung, in welcher der Verankerungsvorsprung und die Tibiaplatte relativ zu einander bewegbar und ausrichtbar sind, in die Implantationsstellung über- führbar ist, in welcher der Verankerungsvorsprung und die Tibiaplatte relativ zueinander unbeweglich aneinander gehalten sind.
Ferner betrifft die vorliegende Erfindung ein Kniegelenkendoprothesensystem mit mindestens einer Femurkomponente zum Verankern an einem distalen Ende eines Femurs und mindestens einer Tibiakomponente zum Verankern an einem proximalen Ende einer Tibia, wobei die mindestens eine Femurkompo nente und die mindestens eine Tibiakomponente korrespondierend zueinander ausgebildet sind zur Ausbildung einer Kniegelenkendoprothese.
Kniegelenkendoprothesensysteme der eingangs beschriebenen Art werden ein gesetzt, um geschädigte Kniegelenke von Patienten durch Implantation einer für den Patienten passenden Kniegelenkendoprothese zu ersetzen. Trotz un terschiedlichster Prothesentypen gibt es nach wie vor eine nicht vernachlässig bare Anzahl von Patienten, die nach der Implantation einer Kniegelenkendop rothese mit dem Ergebnis unzufrieden sind. Ein Problem dabei ist insbeson dere die optimale Ausrichtung, auch als "Alignment" bezeichnet. Ein neuer Ansatz zur Verbesserung der Patientenzufriedenheit ist das soge nannte kinematische Alignment. Dieses basiert auf einer Positionierung der Kniegelenkendoprothese unter Berücksichtigung der natürlichen und individu ellen Bewegungsachsen des präarthrotischen Kniegelenks. Auf diese Weise werden die natürliche Gelenklinie und Beinachsenstellung in ihrer ursprüngli chen Ausrichtung wiederhergestellt. Theoretisch bleibt dadurch auch eine na türliche stabile Kapsel-Band-Spannung ohne Entlastung der Bänder durch ein sogenanntes "Release" erhalten. Im Gegensatz zu diesem neuen Ansatz wer den bei einer klassischen Ausrichtung der Kniegelenkendoprothesen die Im plantatkomponenten, also insbesondere die Tibiakomponente und die Femur komponente, senkrecht zur mechanischen Achse ausgerichtet.
Aus der DE 20 2007 004 508 Ul sind eine Tibiakomponente und eine Kniege lenkendoprothesensystem der eingangs beschriebenen Art bekannt.
Es ist daher eine Aufgabe der vorliegenden Erfindung, eine Tibiakomponente und ein Kniegelenkendoprothesensystem der eingangs beschriebenen Art be reitzustellen, die insbesondere eine einfache Handhabung ermöglichen.
Diese Aufgabe wird bei einer Tibiakomponente der eingangs beschriebenen Art erfindungsgemäß dadurch gelöst, dass die Fixiereinrichtung ein Fixierelement umfasst und dass das zweite Gelenkelement in der Implantationsstellung klemmend zwischen dem Fixierelement und dem ersten Gelenkelement gehal ten ist.
Die erfindungsgemäß vorgeschlagene Lösung ermöglicht es insbesondere, Ti biakomponenten bereitzustellen, die eine gewünschte Ausrichtung des Veran kerungsvorsprungs relativ zur Tibiaplatte ermöglichen. Der Verankerungsvor sprung kann an der Tibiaplatte insbesondere stufenlos verschwenkbar und/ oder verdrehbar gelagert sein. Alternativ ist es insbesondere auch möglich, ei ne Vielzahl von definierten Schwenk- und Rotationsstellungen des Veranke rungsvorsprungs relativ zur Tibiaplatte vorzugeben, also eine gerastete Ver schwenkung beziehungsweise eine gerastete Rotation. Diese ermöglichen auch eine Mehrzahl unterschiedlicher Ausrichtungen des Verankerungsvorsprungs relativ zur Tibiaplatte, jedoch nicht in einer stufenlosen Art und Weise. Durch die vorgeschlagene besondere Lagerung des Verankerungsvorsprungs an der Tibiaplatte ist es insbesondere möglich, den Verankerungsvorsprung stets op timal auszurichten, so dass er anatomisch korrekt in den Markraum der Tibia des Patienten eingeführt werden kann. So kann insbesondere ein Konflikt zwi schen dem Verankerungsvorsprung und einer den Markraum begrenzenden Knochenfläche der Tibia verhindert werden. Insbesondere kann so ein Veran kerungsvorsprung der Tibiakomponente in beliebiger Weise sowohl in anterior posteriorer Richtung als auch medial-lateraler Richtung ausgerichtet werden. So können Spannungsspitzen, die insbesondere durch einen Druck bezie hungsweise eine Kantenbelastung des Verankerungsvorsprungs im Markraum auf die Innenkortikalis der Tibia verursacht werden, minimiert werden. Durch die beschriebene optimale Ausrichtungsmöglichkeit des Verankerungsvor sprungs der Tibiakomponente kann insbesondere ein kinematisches Alignment der Kniegelenkendoprothese überhaupt erreicht werden, und zwar bei jedem Patienten. Dies ermöglicht insbesondere den Erhalt einer natürlichen stabilen Kapsel-Band-Spannung, ohne dass eine Bandspannung durch teilweise Durch trennung von Bändern am Knie des Patienten verringert werden muss. Dieses sogenannte "Release" ist dann also nicht erforderlich. Gemäß der Erfindung umfasst die Tibiakomponente eine Fixiereinrichtung zum Fixieren des Veranke rungsvorsprungs in einer Implantationsstellung, wobei die Fixiereinrichtung von einer Ausrichtstellung, in welcher der Verankerungsvorsprung und die Ti biaplatte relativ zueinander bewegbar und ausrichtbar sind, in die Implantati onsstellung überführbar ist, in welcher der Verankerungsvorsprung und die Tibiaplatte relativ zueinander unbeweglich aneinander gehalten sind. Die Fi xiereinrichtung ermöglicht es also insbesondere, den Verankerungsvorsprung relativ zur Tibiakomponente in einer gewünschten Ausrichtung zu fixieren. In der Ausrichtstellung sind die Tibiaplatte und der Verankerungsvorsprung anei nander gelagert beziehungsweise aneinander gehalten, jedoch relativ zueinan der bewegbar, so dass sie in gewünschter Weise relativ zueinander ausgerich tet werden können. Durch Überführen der Fixiereinrichtung von der Ausricht- Stellung in die Implantationsstellung wird eine Beweglichkeit des Veranke rungsvorsprungs und der Tibiaplatte relativ zueinander aufgehoben, so dass die Tibiakomponente in definierter weise dauerhaft mit einer Tibia verankert werden kann, und zwar ohne dass der Verankerungsvorsprung und die Tibia platte sich relativ zueinander bewegen können. Nachfolgend wird die Angabe "Fixierstellung" synonym für die Angabe "Implantationsstellung" verwendet. Günstig ist es, dass die Fixiereinrichtung ein Fixierelement umfasst und wenn das zweite Gelenkelement in der Implantationsstellung klemmend zwischen dem Fixierelement und dem ersten Gelenkelement gehalten ist. So bildet die Fixiereinrichtung insbesondere eine Klemmeinrichtung für das zweite Gelenk element, die es ermöglicht, das zweite Gelenkelement, beispielsweise einen kugelförmigen Gelenkvorsprung, klemmend in einer hohlkugeligen Gelenkauf nahme zu fixieren. Eine solche Klemmeinrichtung ermöglicht insbesondere eine einfache Handhabung der Tibiakomponente bei der Implantation. In der DE 20 2007 004 508 Ul ist eine Lösung beschrieben, um einen Verankerungs vorsprung an der Tibiaplatte unbeweglich in der Fixierstellung zu halten, die Zugkräfte nutzt. Hierfür kommt ein an einem Schaftadapter drehbar gelagerter Zuganker zum Einsatz. Im Gegensatz dazu werden durch die vorgeschlagene Weiterbildung der Verankerungsvorsprung und die Tibiaplatte jedoch allein durch Druckkräfte in der Fixierstellung unbeweglich aneinander gehalten. Das Fixierelement drückt in der Fixierstellung das zweite Gelenkelement klemmend gegen das erste Gelenkelement. Dies ermöglicht eine einfachere Handhabung, da der Verankerungsvorsprung beim Überführen der Tibiakomponente von der Ausrichtstellung in die Fixierstellung - anders als bei der in der DE 20 2007 004 508 Ul beschriebenen Tibiakomponente - nicht verdreht werden muss. Dadurch ist es überhaupt erst möglich, die Tibiakomponente erst dann von der Ausrichtstellung in die Fixierstellung zu überführen und ohne den Veranke rungsvorsprung und einen gegebenenfalls mit diesem gekoppelten Schaft dis talseitig zu halten, wenn der Verankerungsvorsprung bereits in eine Kavität der Tibia eingesetzt ist. So wird die Ausrichtung des Verankerungsvorsprungs vereinfacht und die für die Implantation benötigte Zeit verkürzt. Für eine stabile Verankerung der Tibiakomponente ist es günstig, wenn der Verankerungsvorsprung im Bereich einer Symmetrieebene der Tibiaplatte an geordnet oder ausgebildet ist.
Vorzugsweise ist der Verankerungsvorsprung in einem mittleren oder zentra len Bereich der Unterseite der Tibiaplatte angeordnet oder ausgebildet. So kann er insbesondere sicher in einen zentralen Markraum der Tibia eingeführt und verankert werden.
Günstig ist es, wenn der Verankerungsvorsprung bezogen auf seine Längs achse rotationssymmetrisch oder im Wesentlichen rotationssymmetrisch aus gebildet ist. Beispielsweise kann der Verankerungsvorsprung in Form eines zy lindrischen Stumpfes oder in Form eines Kegelstumpfes ausgebildet sein.
Vorteilhaft ist es, wenn der Verankerungsvorsprung an der Tibiaplatte gelenkig gelagert ist. Insbesondere kann der Verankerungsvorsprung scharniergelenkig oder kugelgelenkig an der Tibiaplatte gelagert sein. Abhängig von der Art der Lagerung kann so eine optimale Verstellbarkeit des Verankerungsvorsprungs relativ zur Tibiaplatte erreicht werden. Insbesondere gestattet eine kugelge lenkige Lagerung eines Endes der Verankerungsvorsprungs eine freie Ausrich tung eines freien Endes desselben in einem definierten Winkelbereich bezogen auf eine Flächennormale der Unterseite der Tibiaplatte und dies auch über ei nen Umfang von 360° der Rotation bezogen auf die Flächennormale.
Gemäß einer bevorzugten Ausführungsform kann vorgesehen sein, dass die Tibiakomponente eine Gelenkeinrichtung mit einem ersten Gelenkelement und einem zweiten Gelenkelement umfasst, dass das erste Gelenkelement an der Tibiaplatte angeordnet oder ausgebildet ist, dass das zweite Gelenkelement am Verankerungsvorsprung angeordnet oder ausgebildet ist und dass die erste Gelenkkomponente und die zweite Gelenkkomponente miteinander in Eingriff stehen. Eine solche Gelenkeinrichtung ermöglicht insbesondere eine definierte Verschwenkung und/oder Rotation der miteinander in Eingriff stehenden und zusammenwirkenden Gelenkelemente der Gelenkeinrichtung. Insbesondere kann die Gelenkeinrichtung ausgebildet sein, um eine scharniergelenkige oder eine kugelgelenkige Verbindung zwischen den Gelenkelementen herzustellen.
Vorzugsweise ist die erste Gelenkkomponente in Form einer Gelenkaufnahme ausgebildet und die zweite Gelenkkomponente in Form eines in die Gelenkauf nahme eingreifenden Gelenkvorsprungs. Eine derart ausgebildete Gelenkein richtung lässt sich auf einfache Weise hersteilen und ermöglicht eine flexible Ausrichtung des Verankerungsvorsprungs relativ zur Tibiaplatte.
Günstig ist es, wenn der Gelenkvorsprung kugelförmig ausgebildet ist und wenn die Gelenkaufnahme eine hohlkugelige Anlagefläche für den Gelenkvor sprung aufweist. Diese Ausgestaltung gestattet es auf einfache Weise, mit den zusammenwirkenden Gelenkkomponenten eine Kugelgelenkverbindung herzu stellen. Der Verankerungsvorsprung mit Gelenkkugel kann somit auf einfache Weise um 360° bezogen auf seine Längsachse in der Gelenkaufnahme ver dreht und zudem bezogen auf eine Flächennormale der Unterseite der Tibia platte verschwenkt werden, und zwar vorzugsweise innerhalb eines vorgege benen Winkelbereichs. Dieser Winkelbereich kann insbesondere durch geeig nete Anschläge am Verankerungsvorsprung und/oder an der Tibiaplatte be grenzt werden.
Vorzugsweise erstreckt sich die Gelenkaufnahme von der Unterseite der Tibia platte weg. Diese Ausgestaltung ermöglicht insbesondere die Ausbildung der Gelenkaufnahme an einem von der Unterseite abstehenden Vorsprung. Dadurch kann insbesondere ein maximaler Winkelbereich einer Verschwen kung des Verankerungsvorsprungs auf einfache Weise vorgegeben und maxi miert werden.
Günstig ist es, wenn die Gelenkaufnahme rotationssymmetrisch ausgebildet ist. Insbesondere kann sie rotationssymmetrisch bezogen auf eine Flächennor male der Unterseite der Tibiaplatte ausgerichtet sein. Eine solche Gelenkauf nahme lässt sich auf einfache Weise hersteilen und ermöglicht eine kugelge lenkige Verbindung des Verankerungsvorsprungs mit der Tibiaplatte. Um insbesondere den Verankerungsvorsprung auf einfache Weise an der Tibia platte montieren zu können, ist es vorteilhaft, wenn an der Tibiaplatte eine Durchbrechung ausgebildet ist, welche die Tibiaplatte von der Oberseite durch das erste Gelenkelement hindurch durchsetzt. So kann beispielsweise bei ent sprechender Dimensionierung des Verankerungsvorsprungs der Verankerungs vorsprung von der Oberseite der Tibiaplatte her kommend in durch die Durch brechung eingeführt und durch sie hindurchgeführt werden.
Besonders kompakt lässt sich die Tibiakomponente ausbilden, wenn die Durch brechung die Gelenkaufnahme umfasst. Mit anderen Worten kann also die Ge lenkaufnahme im Bereich der Durchbrechung angeordnet oder ausgebildet sein.
Günstig ist es, wenn der Verankerungsvorsprung derart dimensioniert ist, dass er von der Oberseite der Tibiaplatte her durch die Durchbrechung hindurch führbar ist und wenn die Gelenkaufnahme eine engste Stelle definiert, welche einen in proximaler Richtung wirkenden Anschlag für das zweite Gelenkele ment bildet. Diese Ausgestaltung ermöglicht es insbesondere, die Tibiaplatte an einer präparierten Tibia des Patienten zu positionieren und dann von der Oberseite der Tibiaplatte her den Verankerungsvorsprung durch die Durchbre chung in den Markraum der Tibia einzuführen. Der Verankerungsvorsprung kann sich dabei in gewünschter Weise relativ zur Tibiaplatte ausrichten. Durch die engste Stelle der Gelenkaufnahme wird insbesondere verhindert, dass der Verankerungsvorsprung durch die Durchbrechung hindurch rutschen kann. Mit anderen Worten wird so eine Bewegung des Verankerungsvorsprungs durch die entsprechend ausgestaltete Gelenkaufnahme in distaler Richtung begrenzt.
Gemäß einer weiteren bevorzugten Ausführungsform kann vorgesehen sein, dass seitlich neben dem Verankerungssprung mindestens ein Stabilisierungs vorsprung an der Unterseite der Tibiaplatte und von dieser weg weisend angeordnet oder ausgebildet ist. Insbesondere können auch zwei Stabili sierungsvorsprünge in der beschriebenen Weise angeordnet oder ausgebildet sein. Insbesondere können diese spiegelsymmetrisch zueinander seitlich neben dem Verankerungsvorsprung von der Unterseite der Tibiaplatte abste hen. Insbesondere können sie das erste Gelenkelement der Gelenkeinrichtung stabilisieren und von diesem seitlich abstehen.
Günstig ist es, wenn der mindestens eine Stabilisierungsvorsprung geradlinig oder gekrümmt ausgebildet ist. Insbesondere kann der Stabilisierungsvor sprung in anteriorer Richtung konvex gekrümmt ausgebildet sein. Auf diese Weise kann insbesondere eine optimale Stabilisierung einer Verbindung der Tibiakomponente mit der Tibia erreicht werden, also insbesondere kann eine Rotation der Tibiaplatte an der Tibia auf diese Weise einfach verhindert wer den.
Eine optimale Verankerung der Tibiakomponente an der Tibia kann insbeson dere dadurch erreicht werden, dass die Tibiakomponente einen mit dem Ver ankerungsvorsprung kraft- und/oder formschlüssig verbindbaren Schaft um fasst. Insbesondere kann der Schaft mit dem Verankerungsvorsprung ver schraubbar ausgebildet sein. Dies ermöglicht es einem Operateur, die Tibi akomponente abhängig von einer Größe des Markkanals des Patienten optimal an der Tibia zu stabilisieren, indem er einen Schaft auswählt, welcher an die Größe des Markraums angepasst ist, also insbesondere an dessen Länge und Querschnitt. Der Schaft kann dabei insbesondere geradlinig ausgebildet sein oder auch gekrümmt.
Der Verankerungsvorsprung und der Schaft lassen sich auf einfache Weise miteinander verbinden, wenn am Verankerungsvorsprung ein Außengewinde abschnitt ausgebildet ist, wenn der Schaft eine Verankerungsvorsprungauf nahme für den Verankerungsvorsprung aufweist und wenn an der Veranke rungsvorsprungaufnahme ein zum Außengewindeabschnitt korrespondierender Innengewindeabschnitt ausgebildet ist. Die Fixiereinrichtung lässt sich auf einfache Weise von der Ausrichtstellung in die Fixier- beziehungsweise Implantationsstellung überführen, wenn das Fixie relement ein Schraubelement umfasst mit einem Außengewinde und wenn die Durchbrechung ausgehend von der Oberseite der Tibiaplatte ein zum Außen gewinde korrespondierendes Innengewinde umfasst. Auf diese Weise kann das Schraubelement in definierter Weise an der Tibiaplatte angeordnet werden und das zweite Klemmelement gegen das erste Klemmelement drücken, um es - unter Ausübung einer Druckkraft - in der Fixierstellung unbeweglich an der Ti biaplatte zu fixieren. Das Schraubelement kann insbesondere eine Werkzeug elementaufnahme aufweisen, die von der Oberseite der Tibiaplatte weg weist. So kann beispielsweise mit einem Werkzeug das Schraubelement auf einfache Weise in die Tibiaplatte eingeschraubt werden.
Ein kompakter Aufbau der Tibiakomponente kann insbesondere dadurch er reicht werden, dass das Fixierelement in der Implantationsstellung die Durch brechung verschließt.
Um eine Bewegung des zweiten Gelenkelements relativ zum ersten Gelenkele ment in der Fixier- beziehungsweise Implantationsstellung zu verhindern, ist es vorteilhaft, wenn das Fixierelement eine Fixierelementklemmfläche auf weist, welche in der Implantationsstellung klemmend gegen das zweite Ge lenkelement gehalten ist.
Ferner kann es vorteilhaft sein, wenn das Fixierelement ein Klemmelement umfasst und wenn die Fixierelementklemmfläche am Klemmelement ausgebil det ist. So können insbesondere optimale Materialpaarungen zum Klemmen des zweiten Gelenkelements gegen das erste Gelenkelement gewählt werden, unabhängig davon, aus welchem Material beispielsweise das Schraubelement ausgebildet ist. Das Klemmelement kann insbesondere separat vom Schrau belement ausgebildet sein oder mit diesem eine Einheit bilden. Günstigerweise sind das Schraubelement und das Klemmelement einstückig oder als zwei voneinander getrennte Komponenten ausgebildet. Eine zweitei lige Ausbildung von Schraubelement und Klemmelement ermöglicht es insbe sondere, eine Verdrehbewegung des Klemmelements relativ zum zweiten Ge lenkelement zu vermeiden, wenn das Schraubelement und die Tibiaplatte mit einander verschraubt werden.
Günstig ist es, wenn das Klemmelement eine Schraubelementanlagefläche aufweist, die von der Fixierelementklemmfläche weg weisend ausgebildet ist und in der Implantationsstellung am Schraubelement anliegt. Insbesondere kann die Schraubelementanlagefläche eben ausgebildet sein. So kann ein gu ter flächiger Kontakt zwischen dem Schraubelement und dem Klemmelement hergestellt werden, um mit dem Schraubelement das Klemmelement in defi nierter Weise gegen das zweite Gelenkelement zu drücken und so lediglich durch Ausübung einer Druckkraft das zweite Gelenkelement klemmend gegen das erste Gelenkelement in der Fixierstellung zu halten.
Vorteilhaft ist es, wenn die Durchbrechung eine sich an die Gelenkaufnahme in proximaler Richtung anschließende Klemmelementaufnahme aufweist zum Aufnehmen des Klemmelements. Eine derart angeordnete Klemmelementauf nahme ermöglicht es, das Klemmelement in definierter Weise aufzunehmen und dadurch an der Tibiaplatte zu positionieren.
Vorteilhaft ist es, wen die Durchbrechung eine sich an die Klemmelementauf nahme in proximaler Richtung anschließende Schraubelementaufnahme auf weist zum Aufnehmen des Schraubelements. So kann auch das Schraubele ment in definierter Weise in beziehungsweise an der Durchbrechung positio niert werden.
Günstigerweise ist eine von der Schraubelementaufnahme definierte Quer schnittsfläche größer ist als eine von der Klemmelementaufnahme definierte Querschnittsfläche. So kann lediglich das Klemmelement in die Klemmele mentaufnahme eingesetzt werden, nicht jedoch das Schraubelement, wenn das Klemmelement und das Schraubelement entsprechend den jeweils vorge sehenen Aufnahmen dimensioniert sind. Zudem kann durch die Ausbildung der Schraubelementaufnahme und der Klemmelementaufnahme auch eine maxi male Bewegung des Klemmelements und des Schraubelements in distaler Richtung begrenzt werden, also insbesondere auch eine Position des Schrau belements und des Klemmelements in der Fixier- beziehungsweise Implantati onsstellung.
Auf einfache Weise lässt sich die Tibiakomponente ausbilden, wenn die Ober seite und/oder die Unterseite der Tibiaplatte eben oder im Wesentlichen eben ausgebildet sind.
Die eingangs gestellte Aufgabe wird ferner bei einem Kniegelenkendoprothe sensystem der eingangs beschriebenen Art erfindungsgemäß dadurch gelöst, dass die mindestens eine Tibiakomponente in Form einer der oben beschriebe nen Tibiakomponenten ausgebildet ist.
Ein Kniegelenkendoprothesensystem mit einer der oben beschriebenen Tibia komponenten auszustatten hat insbesondere den Vorteil, dass eine für einen Patienten optimal passende Kniegelenkendoprothese ausgebildet werden kann, um das eingangs diskutierte kinematische Alignment zu realisieren. Mit einer solchen optimierten Ausrichtung eines Kniegelenkersatzes kann insbesondere eine Zufriedenheit der Patienten nach einer Kniegelenkendoprothesenimplan tation signifikant verbessert werden.
Vorteilhaft ist es, wenn die Kniegelenkendoprothese mindestens eine mit der Tibiakomponente koppelbare Meniskuskomponente umfasst und wenn die Me niskuskomponente eine mit der mindestens einen Femurkomponente zusam menwirkende Gelenkfläche aufweist. So kann eine optimale Gleitpaarung zwi schen der Meniskuskomponente und der Femurkomponente ausgebildet wer den. Insbesondere lassen sich unterschiedlichste Kniegelenkendoprothesen auf diese Weise mit einer der oben beschriebenen vorteilhaften Tibiakomponenten ausstatten. Die Meniskuskomponente kann entweder unbeweglich oder beweg lich mit der Tibiakomponente gekoppelt werden.
Um eine optimale Anpassung einer Kniegelenkendoprothese an einen Patien ten zu ermöglichen, ist es günstig, wenn das Kniegelenkendoprothesensystem mehrere sich in ihrer Länge und/oder ihrem Querschnitt unterscheidende Schäfte zum wahlweisen Koppeln mit dem Verankerungsvorsprung der Ti biakomponente umfasst.
Die nachfolgende Beschreibung bevorzugter Ausführungsformen der Erfindung dient im Zusammenhang mit den Zeichnungen der näheren Erläuterung. Es zeigen:

In Figur 1 ist schematisch ein erstes Ausführungsbeispiel einer Kniegelenken- doprothese 10 eines Kniegelenkendoprothesensystems 12 dargestellt. Die Kniegelenkendoprothese 10 ist als Ersatz für ein degeneriertes natürliches Kniegelenk im Knie 14 eines Patienten implantiert.
Die Kniegelenkendoprothese 10 umfasst eine Tibiakomponente 16 und eine mit dieser zusammenwirkende Femurkomponente 18. Die Tibiakomponente 16 ist ausgebildet zum Verankern an einem proximalen, präparierten Ende einer Tibia 20. Die Femurkomponente 18 ist ausgebildet zum Verankern an einem distalen präparierten Ende eines Femurs 22. Die Tibiakomponente 16 und die Femurkomponente 18 sind korrespondierend zueinander ausgebildet zur Aus bildung der Kniegelenkendoprothese 10.
Das in den Figuren 1 und 2 dargestellte Ausführungsbeispiel der Kniegelenken doprothese 10 umfasst eine optionale Meniskuskomponente 24, die zwischen der Tibiakomponente 16 und der Femurkomponente 18 angeordnet ist. Bei dem beschriebenen Ausführungsbeispiel ist die Meniskuskomponente 24 wahl weise an der Tibiakomponente 16 festlegbar oder relativ zu dieser bewegbar.
Die Meniskuskomponente 24 weist eine mit der Femurkomponente 18 zusam menwirkende Gelenkfläche 26 auf. Diese liegt an einer von der Femurkompo nente 18 definierten Kondylenfläche 28 an, wobei die Kondylenfläche 28 an der Gelenkfläche 26 abrollen und/oder entlanggleiten kann.
Die Tibiakomponente 16 umfasst eine Tibiaplatte 30, die in einer Draufsicht bezogen auf eine Symmetrieebene 32 spiegelsymmetrisch und im Wesentli chen U-förmig oder nierenförmig ausgebildet ist. Die Tibiaplatte 30 definiert eine Oberseite 34 und eine in entgegengesetzte Richtung weisende Unterseite 36.
Von der Unterseite 36 steht ein Verankerungsvorsprung 38 ab, welcher ausge bildet ist zum Verankern der Tibiakomponente im schematisch in Figur 1 ge strichelt eingezeichneten Markraum 40 der Tibia 20. Der Verankerungsvorsprung 38 ist an der Tibiaplatte 30 verschwenkbar und/oder um eine von ihm definierte Längsachse 42 verdrehbar gelagert.
Der Verankerungsvorsprung 38 ist bezogen auf die Längsachse 42 rotations symmetrisch beziehungsweise im Wesentlichen rotationssymmetrisch ausge bildet. Er umfasst einen distalen Endabschnitt 44, welcher zylindrisch bezie hungsweise im Wesentlichen zylindrisch ausgebildet ist.
Die Tibiakomponente 16 umfasst eine Gelenkeinrichtung 46 mit einem ersten Gelenkelement 48 und einem zweiten Gelenkelement 50. Das erste Gelenkele ment 48 und das zweite Gelenkelement 50 stehen miteinander in Eingriff, ins besondere teilweise formschlüssig. Das erste Gelenkelement 48 ist an der Tibiaplatte 30 angeordnet oder ausgebildet. Das zweite Gelenkelement 50 ist am Verankerungsvorsprung 38 angeordnet oder ausgebildet und definiert ei nen proximalen Endabschnitt 52 desselben.
Das erste Gelenkelement 48 und damit auch der Verankerungsvorsprung 38 sind in einem Bereich der Symmetrieebene 32 der Tibiaplatte 30 angeordnet beziehungsweise ausgebildet. Zudem ist das erste Gelenkelement 48 und da mit auch der Verankerungsvorsprung 38 in einem mittleren oder zentralen Be reich der Unterseite 36 der Tibiaplatte 30 angeordnet beziehungsweise ausge bildet.
Durch die Gelenkeinrichtung 46 ist der Verankerungsvorsprung 38 an der Tibiaplatte 30 gelenkig gelagert, insbesondere scharniergelenkig beziehungs weise kugelgelenkig.
Bei dem beschriebenen Ausführungsbeispiel der Tibiakomponente 16 ist die erste Gelenkkomponente 48 in Form einer Gelenkaufnahme 54 ausgebildet.
Die zweite Gelenkkomponente 50 ist in Form eines in die Gelenkaufnahme 54 eingreifenden Gelenkvorsprungs 56 ausgebildet. Der Gelenkvorsprung 56 ist bei dem beschriebenen Ausführungsbeispiel kugelförmig ausgebildet und weist einen Durchmesser auf, welcher größer ist als ein Durchmesser des distalen Endabschnitts 44. Die Gelenkaufnahme 54 definiert eine hohlkugelige Anlage fläche 58. Auf diese Weise kann der in Form einer Gelenkkugel ausgebildete Gelenkvorsprung 56 in der Gelenkaufnahme 54 rotiert und um ein Zentrum 60 des Gelenkvorsprungs 56 verschwenkt werden.
Die Gelenkaufnahme 54 erstreckt sich von der Unterseite 36 der Tibiaplatte 30 weg und ist an einem hülsenförmigen Vorsprung 62 ausgebildet. Die Ge lenkaufnahme 54 ist rotationssymmetrisch ausgebildet, und zwar bezogen auf eine Flächennormale 64 der Unterseite 36 der Tibiaplatte 30.
An der Tibiaplatte 30 ist eine Durchbrechung 66 ausgebildet, die die Tibia platte 30 von der Oberseite 34 durchsetzt, und zwar durch das erste Gelen kelement 48 hindurch. Mithin umfasst also die Durchbrechung 66 die Ge lenkaufnahme 54, die im Bereich eines distalen Endes des Vorsprungs 62 aus gebildet ist.
Der Verankerungsvorsprung 38 ist derart dimensioniert beziehungsweise be messen, dass er mit dem distalen Endabschnitt 44 von der Oberseite 34 der Tibiaplatte 30 her kommend in die Durchbrechung 66 ein und durch diese hin durchführbar ist, und zwar so weit, bis der Gelenkvorsprung 56 an einer von der Gelenkaufnahme 54 definierten engsten Stelle 68 in Anlage kommt. Die engste Stelle 68 bildet einen in proximaler Richtung wirkenden Anschlag 70 für das zweite Gelenkelement 50.
Vom Vorsprung 62 stehen seitlich und symmetrisch zueinander bezogen auf die Symmetrieebene 32 zwei Stabilisierungsvorsprünge 72 ab. Sie stehen wie der Vorsprung 62 von der Unterseite 36 der Tibiaplatte 30 weg weisend ab.
Die Stabilisierungsvorsprünge 72 sind beim dargestellten und beschriebenen Ausführungsbeispiel gekrümmt ausgebildet, und zwar in anteriorer Richtung weisend konvex gekrümmt. Bei alternativen Ausführungsbeispielen sind sie geradlinig ausgebildet. Die Stabilisierungsvorsprünge 72 können insbesondere eine Rotation der Tibia platte 30 relativ zur Tibia 20 um die Flächennormale 64 verhindern. Sie bilden somit eine Art Verdrehsicherung für die Tibiakomponente 16.
Um eine gute Verankerung der Tibiakomponente 16 an der Tibia 20 zu ermög lichen, ist bei dem in den Figuren dargestellten Ausführungsbeispiel der Veran kerungsvorsprung 38 kraft- und/oder formschlüssig mit einem Schaft 74 ver bindbar. Der Schaft 74 ist im Wesentlichen langgestreckt zylindrisch bezie hungsweise im Wesentlichen zylindrisch ausgebildet und weist ein abgerunde tes distales Ende 76 auf.
Der Schaft 74 definiert eine Schaftlängsachse 87, die mit der Längsachse 42 zusammenfällt, wenn der Schaft 74, wie in den Figuren 4 und 5 schematisch dargestellt, mit dem Verankerungsvorsprung 38 verbunden ist.
Der Schaft definiert eine Länge 80 von einem proximalen Ende bis zu seinem distalen Ende 76 und einen Durchmesser 64 quer zur Schaftlängsachse 78.
Ausgehend vom proximalen Ende 82 ist am Schaft 74 eine Verankerungsvor sprungaufnahme 86 ausgebildet, die einen Innengewindeabschnitt 88 auf weist. Die Verankerungsvorsprungaufnahme 86 ist sacklochförmig ausgebildet und dient der Aufnahme des distalen Endabschnitts 44 des Verankerungsvor sprungs 38. Am distalen Endabschnitt 44 ist ein Außengewindeabschnitt 90 ausgebildet, und zwar korrespondierend zum Innengewindeabschnitt 88, so dass sich der Schaft 74 und der Verankerungsvorsprung 38 miteinander ver schrauben lassen.
Der Verankerungsvorsprung 38 kann mit der Tibiaplatte 30 wie oben beschrie ben gekoppelt werden, indem der distale Endabschnitt 44 von der Oberseite 34 her kommend in die Durchbrechung 66 eingeführt wird, bis der Gelenkvor sprung 56 an der Anlagefläche 58 anliegt. In dieser Stellung sind die Gelen kelemente 48 und 50 relativ zueinander bewegbar und somit ausrichtbar. Um den Verankerungsvorsprung 38 in einer definierten Ausrichtung, beispiels weise um einen Auslenkwinkel 92 bezogen auf die Flächennormale 64 ausge lenkt, zu fixieren, umfasst die Tibiakomponente 16 eine Fixiereinrichtung 94. Mit dieser kann der Verankerungsvorsprung 38 in einer Fixier- beziehungs weise Implantationsstellung, wie sie schematisch in den Figuren 4 und 5 dar gestellt ist, relativ zur Tibiaplatte 30 fixiert werden.
Die Fixiereinrichtung kann von einer Ausrichtstellung, in welcher der Veranke rungsvorsprung 38 und die Tibiaplatte 30 relativ zueinander bewegbar und ausrichtbar sind, in die Implantationsstellung überführt werden. Wie erwähnt, sind der Verankerungsvorsprung 38 und die Tibiaplatte 30 in der Implantati onsstellung relativ zueinander unbeweglich aneinandergehalten.
Die Fixiereinrichtung 94 umfasst ein Fixierelement 96, mit dem das zweite Ge lenkelement 50 in der Fixierstellung gegen das erste Gelenkelement 48 spann bar ist. Mit anderen Worten kann so in der Fixierstellung das zweite Gelenkele ment 50 klemmend zwischen dem Fixierelement 96 und dem ersten Klemm element 48 gehalten werden.
Das in den Figuren dargestellte Fixierelement 96 ist zweiteilig ausgebildet und umfasst ein Schraubelement 98 und ein Klemmelement 100.
Das Schraubelement 100 ist zylindrisch ausgebildet und weist ein Außenge winde 102 auf. Korrespondierend zum Außengewinde 102 ist an der Durchbre chung 66 ausgehend von der Oberseite 34 der Tibiaplatte 30 ein Innenge winde 104 ausgebildet. In einer vom Verankerungsvorsprung 38 weg weisen den ebenen Seitenfläche 106 des Schraubelements ist eine Werkzeugauf nahme 108 ausgebildet, und zwar in Form eines Innenmehrkants 110.
Das Schraubelement 98 kann wie beschrieben mit der Tibiaplatte 30 unter Zu hilfenahme eines Einschraubwerkzeugs, das in die Werkzeugaufnahme 108 eingreift, verschraubt werden und verschließt in der Fixierstellung die Durch brechung 66. Das Fixierelement 96 weist eine Fixierelementklemmfläche 112 auf, welche in die Fixierstellung unter Ausübung ausschließlich einer Druckkraft klemmend gegen das zweite Gelenkelement 50 drückt. Um eine flächige Anlage der Fi xierelementklemmfläche 112 an das zweite Gelenkelement 50, welches durch den kugelförmigen Gelenkvorsprung 56 gebildet ist, zu ermöglichen, ist die Fixierelementklemmfläche 112 korrespondierend zum Gelenkvorsprung 56 hohlkugelig geformt. Die Fixierelementklemmfläche 112 ist somit am Klemm element 100 ausgebildet.
Bei alternativen Ausführungsbeispielen sind das Schraubelement 98 und das Klemmelement 100 einstückig ausgebildet. Insbesondere kann das Fixierele ment 96 monolithisch ausgebildet sein.
Wie in Figur 3 gut zu erkennen, weist bei dem dargestellten Ausführungsbei spiel das Klemmelement 100 eine ebene Schraubelementanlagefläche 114 auf. Diese ist von der Fixierelementklemmfläche 112 weg weisend ausgebildet und liegt in der Fixierstellung am Schraubelement 98 an.
An die Gelenkaufnahme 54 schließt sich proximalseitig eine Klemmelementauf nahme 116 an, die einen größeren Durchmesser definiert als die Gelenkauf nahme 54. Die Klemmelementaufnahme 116 dient zur Aufnahme und Positio nierung des Klemmelements 100.
Ferner weist die Durchbrechung 66 eine sich an die Klemmelementaufnahme 116 in proximaler Richtung anschließende Schraubelementaufnahme 118 auf zum Aufnehmen des Schraubelements 98. Das Innengewinde 104 ist aus schließlich im Bereich der Schraubelementaufnahme 118 ausgebildet.
Eine von der Schraubelementaufnahme 118 definierte Querschnittsfläche be ziehungsweise ein Durchmesser derselben ist größer als eine von der Klemm elementaufnahme 116 definierte Querschnittsfläche beziehungsweise ein Durchmesser derselben. Zum Überführen der Fixiereinrichtung 94 von der Ausrichtstellung in die Im plantationsstellung wird das Schraubelement 98 in Richtung auf den kugelför migen Gelenkvorsprung 56 hin eingeschraubt, bis das Klemmelement 100 un ter Ausübung einer Druckkraft den Gelenkvorsprung 56 klemmend gegen die Gelenkaufnahme 54 drückt. Umgekehrt können die Tibiakomponente 56 bezie hungsweise die Fixiereinrichtung 94 wieder von der Implantationsstellung in die Ausrichtstellung überführt werden, indem das Schraubelement 98 in ent gegengesetzter Richtung gedreht und die Verschraubung etwas gelöst wird, so dass das Klemmelement 100 den Gelenkvorsprung 56 etwas freigibt und so eine Rotations- und Verschwenkbewegung des Verankerungsvorsprungs 38 relativ zur Tibiaplatte 30 ermöglicht.
Bei dem in den Figuren dargestellten Ausführungsbeispiel sind die Oberseite 34 und die Unterseite 36 der Tibiaplatte 30 eben oder im Wesentlichen eben ausgebildet.
Die Oberseite 34 ist in Form einer Tibiagelenkfläche 120 ausgebildet. Sie bildet mit einer Unterseite der Meniskuskomponente 24 bei einer beweglichen Lage rung der Meniskuskomponente 24 auf der Tibiakomponente 16 eine Gleitpaa rung aus.
Um Kniegelenkendoprothesen 10 individuell an die Physiologie eines Patienten anpassen zu können, umfasst das Kniegelenkendoprothesensystem 12 optio nal mehrere sich in ihrer Länge 80 und/oder ihrem Durchmesser 84 bezie hungsweise ihrem Querschnitt unterscheidende Schäfte 74, die wahlweise mit dem Verankerungsvorsprung 38 der Tibiakomponente 16 koppelbar sind.
Ferner kann das Kniegelenkendoprothesensystem 12 bei anderen Ausfüh rungsbeispielen auch mehrere sich in Form und/oder Größe unterscheidende Tibiakomponenten 16 sowie mehrere sich in Form und/oder Größe unterschei dende Femurkomponenten 18 sowie mehrere sich in Form und/oder Größe un terscheidende Meniskuskomponenten 24 umfassen. Auf diese Weise können optimal passende Kniegelenkendoprothesen 10 für Menschen unterschiedlicher Größe bereitgestellt werden.
Durch die an den beschriebenen Ausführungsbeispielen der Tibiakomponente 16 vorgesehene Gelenkeinrichtung 46 ist es möglich, unabhängig von einer Orientierung des Markraums der Tibia 20 die Tibiakomponente 16 optimal an der Tibia 20 zu verankern. Durch die beschriebene Gelenkeinrichtung 46 wird eine stufenlose Ausrichtung des Verankerungsvorsprungs mit optional ange ordnetem Schaft 74 ermöglicht, um das eingangs beschriebene kinematische Alignment zu erreichen. Auf diese Weise lässt sich eine Patientenzufriedenheit nach der Implantation der Kniegelenkendoprothese im Vergleich zu herkömm lichen Kniegelenkendoprothesen verbessern.

Claims

Patentansprüche
1. Tibiakomponente (16) für eine Kniegelenkendoprothese (10), wobei die Tibiakomponente eine Tibiaplatte (30) mit einer Oberseite (34) und einer Unterseite (36) und einen von der Unterseite (36) abstehenden Veranke rungsvorsprung (38) zum Einführen in den Markraum (40) einer Tibia (20) zum Verankern der Tibiakomponente (16) an der Tibia (20) um fasst, wobei der Verankerungsvorsprung (38) an der Tibiaplatte (30) ver- schwenkbar und/oder um eine Längsachse (42) des Verankerungsvor sprungs (38) verdrehbar gelagert ist, wobei die Tibiakomponente (16) eine Fixiereinrichtung (94) umfasst zum Fixieren des Verankerungsvor sprungs (38) in einer Implantationsstellung und dass die Fixiereinrich tung (94) von einer Ausrichtstellung, in welcher der Verankerungsvor sprung (38) und die Tibiaplatte (30) relativ zueinander bewegbar und ausrichtbar sind, in die Implantationsstellung überführbar ist, in welcher der Verankerungsvorsprung (38) und die Tibiaplatte (30) relativ zueinan der unbeweglich aneinander gehalten sind, dadurch gekennzeichnet, dass die Fixiereinrichtung (94) ein Fixierelement (96) umfasst und dass das zweite Gelenkelement (50) in der Implantationsstellung klemmend zwischen dem Fixierelement (96) und dem ersten Gelenkelement (48) gehalten ist.
2. Tibiakomponente nach Anspruch 1, dadurch gekennzeichnet, dass der Verankerungsvorsprung (38) a) im Bereich einer Symmetrieebene (32) der Tibiaplatte (30) ange ordnet oder ausgebildet ist und/oder b) in einem mittleren oder zentralen Bereich der Unterseite (36) der Tibiaplatte (30) angeordnet oder ausgebildet ist und/oder c) bezogen auf seine Längsachse (42) rotationssymmetrisch oder im Wesentlichen rotationssymmetrisch ausgebildet ist und/oder d) der Verankerungsvorsprung (38) an der Tibiaplatte (30) gelenkig gelagert ist, insbesondere scharniergelenkig oder kugelgelenkig.

Tibiakomponente nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, dass die Tibiakomponente (16) eine Gelenkeinrichtung (46) mit einem ersten Gelenkelement (48) und einem zweiten Ge lenkelement (50) umfasst, dass das erste Gelenkelement (48) an der Tibiaplatte (30) angeordnet oder ausgebildet ist, dass das zweite Ge lenkelement (50) am Verankerungsvorsprung (38) angeordnet oder aus gebildet ist und dass die erste Gelenkkomponente (48) und die zweite Gelenkkomponente (50) miteinander in Eingriff stehen.
4. Tibiakomponente nach Anspruch 3, dadurch gekennzeichnet, dass die erste Gelenkkomponente (48) in Form einer Gelenkaufnahme (54) aus gebildet ist und dass die zweite Gelenkkomponente (50) in Form eines in die Gelenkaufnahme (54) eingreifenden Gelenkvorsprungs (56) ausgebil det ist.

Tibiakomponente nach Anspruch 4, dadurch gekennzeichnet, dass a) der Gelenkvorsprung (56) kugelförmig ausgebildet ist und dass die Gelenkaufnahme (54) eine hohlkugelige Anlagefläche (58) für den Gelenkvorsprung (56) aufweist und/oder b) sich die Gelenkaufnahme (54) von der Unterseite (36) der Tibia platte (30) weg erstreckt und/oder c) die Gelenkaufnahme (54) rotationssymmetrisch ausgebildet ist, insbesondere bezogen auf eine Flächennormale (64) der Unterseite (36) der Tibiaplatte (30).

Tibiakomponente nach einem der Ansprüche 3 bis 5, dadurch gekenn zeichnet, dass an der Tibiaplatte (30) eine Durchbrechung (66) ausgebil det ist, welche die Tibiaplatte (30) von der Oberseite (34) durch das erste Gelenkelement (48) hindurch durchsetzt, wobei insbesondere a) die Durchbrechung (66) die Gelenkaufnahme (54) umfasst und/oder b) der Verankerungsvorsprung (38) derart dimensioniert ist, dass er von der Oberseite (34) der Tibiaplatte (30) her durch die Durchbre chung (66) hindurchführbar ist und dass die Gelenkaufnahme (54) eine engste Stelle (68) definiert, welche einen in proximaler Rich tung wirkenden Anschlag (70) für das zweite Gelenkelement (50) bildet.

Tibiakomponente nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, dass seitlich neben dem Verankerungssprung (38) min destens ein Stabilisierungsvorsprung (72), insbesondere zwei Stabilisie rungsvorsprünge (72), an der Unterseite (36) der Tibiaplatte (30) und von dieser weg weisend angeordnet oder ausgebildet ist, wobei der mindestens eine Stabilisierungsvorsprung (72) geradlinig oder gekrümmt, insbesondere in anteriorer Richtung weisend konvex ge krümmt, ausgebildet ist.

Tibiakomponente nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, dass die Tibiakomponente (16) einen mit dem Veranke rungsvorsprung (38) kraft- und/oder formschlüssig verbindbaren, insbe sondere verschraubbaren, Schaft (74) umfasst, wobei insbesondere am Verankerungsvorsprung (38) ein Außengewinde abschnitt (90) ausgebildet ist, dass der Schaft (74) eine Verankerungs vorsprungaufnahme (86) für den Verankerungsvorsprung (38) aufweist und dass an der Verankerungsvorsprungaufnahme (86) ein zum Außen gewindeabschnitt (90) korrespondierender Innengewindeabschnitt (88) ausgebildet ist.
9. Tibiakomponente nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, dass das Fixierelement (96) a) ein Schraubelement (98) umfasst mit einem Außengewinde (102) und dass die Durchbrechung (66) ausgehend von der Oberseite (34) der Tibiaplatte (30) ein zum Außengewinde (102) korrespon dierendes Innengewinde (104) umfasst und/oder b) in der Implantationsstellung die Durchbrechung (66) verschließt und/oder c) eine Fixierelementklemmfläche (112) aufweist, welche in der Im plantationsstellung klemmend gegen das zweite Gelenkelement (50) gehalten ist.
10. Tibiakomponente nach Anspruch 9, dadurch gekennzeichnet, dass das Fixierelement (96) ein Klemmelement (100) umfasst und dass die Fixier elementklemmfläche (112) am Klemmelement (100) ausgebildet ist.
11. Tibiakomponente nach Anspruch 10, dadurch gekennzeichnet, dass das Schraubelement (98) und das Klemmelement (100) einstückig oder als zwei voneinander getrennte Komponenten ausgebildet sind.
12. Tibiakomponente nach Anspruch 10 oder 11, dadurch gekennzeichnet, dass das Klemmelement (100) eine, insbesondere ebene, Schraubele mentanlagefläche (114) aufweist, die von der Fixierelementklemmfläche (112) weg weisend ausgebildet ist und in der Implantationsstellung am Schraubelement (98) anliegt.
13. Tibiakomponente nach einem der Ansprüche 10 bis 12, dadurch gekenn zeichnet, dass die Durchbrechung (66) eine sich an die Gelenkaufnahme (54) in proximaler Richtung anschließende Klemmelementaufnahme (116) aufweist zum Aufnehmen des Klemmelements (100), wobei insbesondere die Durchbrechung (66) eine sich an die Klemmele mentaufnahme (116) in proximaler Richtung anschließende Schraubele mentaufnahme (118) aufweist zum Aufnehmen des Schraubelements (98), wobei weiter insbesondere eine von der Schraubelementaufnahme (118) definierte Querschnittsfläche größer ist als eine von der Klemmelement aufnahme (116) definierte Querschnittsfläche.
14. Kniegelenkendoprothesensystem (12) mit mindestens einer Femurkom ponente (18) zum Verankern an einem distalen Ende eines Femurs (22) und mindestens einer Tibiakomponente (16) zum Verankern an einem proximalen Ende einer Tibia (20), wobei die mindestens eine Femurkom ponente (18) und die mindestens eine Tibiakomponente (16) korrespon dierend zueinander ausgebildet sind zur Ausbildung einer Kniegelenken doprothese (10), dadurch gekennzeichnet, dass die mindestens eine Tibiakomponente (16) in Form einer Tibiakomponente (16) nach einem der voranstehenden Ansprüche ausgebildet ist.
15. Kniegelenkendoprothesensystem nach Anspruch 14, dadurch gekenn zeichnet, dass a) die Kniegelenkendoprothese (10) mindestens eine mit der Tibi akomponente (16) koppelbare Meniskuskomponente (24) umfasst und dass die Meniskuskomponente (24) eine mit der mindestens einen Femurkomponente (18) zusammenwirkende Gelenkfläche (26) aufweist und/oder b) das Kniegelenkendoprothesensystem (12) mehrere sich in ihrer Länge (80) und/oder ihrem Querschnitt unterscheidende Schäfte (74) zum wahlweisen Koppeln mit dem Verankerungsvorsprung (28) der Tibiakomponente (16) umfasst.
