Description
Die Erfindung betrifft eine Vorrichtung zur kardiopulmonalen Massage und/oder Reanimation eines Patienten, mit
- einem Auflagerbrett, auf dem der Patient mit seinem Rücken aufliegt;
- einer unmittelbar an dem Auflagerbrett angeschlossenen Massagevorrichtung, die aufweist:
- eine Portalstütze;
- einen an der Portalstütze angeordneten, oberhalb der Brust des Patienten vorragenden Ausleger;
- einen an dem freien Ende des Auslegers angeordneten, reversierend antreibbaren Massagestempel, wobei
- der Massagestempel an einem Soll-Kontaktbereich am Brustkorb des Patienten zur Durchführung einer kardiopulmonalen Massage positionierbar ist.

Die bekannte Vorrichtung, bei der durch den Anschluss der Massagevorrichtung an dem Auflagerbrett die bei der durchgeführten Herzdruckmassage vom Massagestempel ausgeübten Druckkräfte direkt in das Auflagerbrett eingeleitet werden, auf dem der Patient aufliegt, ist gut geeignet, an einem Patienten maschinell eine Herzdruckmassage durchzuführen, solange die Vorrichtung im Wesentlichen stationär ist, sich die Position der Vorrichtung relativ zu einer Unterlage, also beispielsweise einer fest in einem Rettungswagen montierten Liege nicht oder jedenfalls nicht nennenswert verändert. Oft gibt es aber für Rettungssanitäter das Problem, dass sie einen Patienten beispielsweise aus einer Wohnung oder von dem Ort einen Unfallgeschehens zunächst mittels geeigneter Tragehilfen zu einem Rettungsfahrzeug oder einem Hubschrauber transportieren müssen, wobei es durchaus sein kann, dass während dieses Transportvorgangs die Herzdruckmassage nicht unterbrochen werden soll. Zwar ist die bekannte Vorrichtung durchaus in der Lage, die Massage mit dem Druckstempel auch während des Transports des Patienten beispielsweise mittels einer Trage oder auch eines Tragetuchs fortzusetzen. Es kann bei derartigen Transportvorgängen aber leicht dazu kommen, dass der auf dem Auflagerbrett liegende Patient seine Lage - u.U. auch sehr plötzlich - relativ zu diesem verändert, was in der Praxis beispielsweise beim Transport des Patienten über eine Treppe dann dazu führt, dass ein einigermaßen durchgehender Betrieb der Massagevorrichtung nicht möglich ist, da zu schnell zu große Änderungen der Lage der Massagevorrichtung relativ zum Soll-Kontaktbereich am Brustkorb des Patienten auftreten und somit eine effiziente Reanimation nicht möglich ist. Erfindungsgemäß ist ein am Brustkorb 18 des Patienten 11 an dem Soll-Kontaktbereich 17 positionierbares Halteplattenelement 19 vorgesehen, das mit vier an seinem Umfang angreifenden Spannmitteln 20 mit dem Auflagerbrett 12 in dessen Eckbereichen verbindbar und verspannbar ist. Hierzu weist das Halteplattenelement 19 vier an seinem Umfang etwa gleichmäßig verteilt angeordnete, einander diagonal gegenüberliegende und auf die Eckbereiche des Auflagerbretts 12 ausgerichtete Anschlusselemente 21 auf, an denen die vier an den Eckbereichen des Auflagerbretts befestigten Spannmittel 20 angreifen können. Bei den Spannmitteln 20 handelt es sich um Spanngurte, die mit Hakenverschlusselementen 22 versehen sind, die an den am Halteplattenelement 19 angeordneten, ihnen zugeordneten Anschlusselementen 21 lösbar angebracht werden können. Die ringförmige Ausgestaltung des Halteplattenelements 19 mit der zentralen Öffnung 28 erleichtert die Positionierung des Massagestempels für die Herzdruckmassage, indem zunächst das Halteplattenelement korrekt am Brustkorb des Patienten positioniert und dann mit dem unter dem Patienten liegenden Auflagerbrett 12 mithilfe der Spannmittel 20 fest verbunden wird, so dass es sich seitlich nicht mehr verschieben kann und den Brustkorb des Patienten zwischen sich und dem Auflage Brett fest eingespannt. Es ist möglich, das Ringelement 29 beispielsweise im Bereich der vier um jeweils 90° zueinander versetzt angeordneten Befestigungslaschen für die am Auflagerbrett anzuschließenden Spannmittel 20 oder zwischen diesen mit Markierungsstrichen (nicht dargestellt) zu versehen, die das korrekte Ausrichten des Halteplattenelements konzentrisch zum Soll-Kontaktbereich 17 erleichtern, der mit dem Mittelpunkt der zentralen Öffnung 28 zusammenfällt, also konzentrisch zu der Öffnung 28 ist. Nach dem korrekten Anbringen des Halteplattenelements muss dann nur noch der Massagestempel 16 mit seiner Druckfläche mittig in der zentralen Öffnung 28 des Ringelements 29 positioniert werden. Es ist aber auch möglich, das Halteplattenelement nicht ringförmig, sondern geschlossen auszugestalten und die Druckfläche des Massagestempels dann nicht unmittelbar auf der Brust des Patienten, sondern der Oberseite des Halteplattenelements anzusetzen, das die Auf- und Abbewegung des Massagestempels aufgrund der ihm innewohnenden Flexibilität in Richtung senkrecht zum Brustkorb des Patienten uneingeschränkt mitmacht. Bei einer derartigen Ausgestaltung ist es auch möglich, den Massagestempel an seiner Druckfläche beispielsweise mittels eines Klettverbinders mit dem Halteplattenelement zu koppeln und auf diese Weise eine feste Verbindung zwischen Massagestempel und dem von der Vorrichtung praktisch eingespannten Patienten in einer Richtung quer zur Betätigungsrichtung des Massagestempels zu schaffen.The annular design of the holding

- Device according to any of claims 1 to 5, characterised in that the holding plate element (19) is designed to be at least to the main extent inflexible in radial and/or circumferential direction and to be deformable in a direction perpendicular to the thorax of the patient (11).
- Device according to any of claims 1 to 6, characterised in that the holding plate element (19) has a central annular or disc element (29) made of flexible plastic material, which is preferably covered with a high-strength woven fabric (30) on which the connection elements (21) are provided.
- Device according to any of claims 1 to 7, characterised in that the connection elements (21) are arranged on fastening tabs (31) which are arranged on the holding plate element (19) to be approximately radially protruding.
