Claims (17)

  1. Appareil (40, 120) conçu pour comprimer une endoprothèse valvulaire cardiaque (10) à un diamètre externe maximal inférieur ou égal à environ 10 mm pour un chargement dans un cathéter de pose (12), l'appareil (40, 120) comprenant
    un premier étage de compression (100) pour comprimer l'endoprothèse valvulaire (10); et
    un second étage de compression (102) accouplé ou pouvant être accouplé au premier étage de compression (100) et conçu pour comprimer davantage une partie de l'endoprothèse valvulaire (10) après le passage par le premier étage de compression (100)
    dans lequel le premier étage de compression (100) comprend un canal creux (42), et le premier étage de compression (100) est conçu pour comprimer progressivement l'endoprothèse valvulaire en réponse à, ou en association avec, l'avancement longitudinal de l'endoprothèse valvulaire à l'intérieur du canal creux (42), caractérisé en ce que
    le second étage de compression (102) est conçu pour comprimer une partie de l'endoprothèse valvulaire au niveau du second étage de compression (102) sans l'avancement de l'endoprothèse valvulaire à l'intérieur du second étage de compression (102).
  2. Appareil (40, 120) selon la revendication 1, conçu pour une utilisation dans la compression d'une endoprothèse valvulaire cardiaque (10) ayant un diamètre externe maximal dans son état dilaté allant d'environ 25 mm à environ 35 mm.
  3. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel le second étage de compression (102) est conçu pour appliquer une force radiale d'au moins 50 N.
  4. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel le second étage de compression (102) est accouplé de façon libérable à une sortie du premier étage de compression (100), ou est destiné à l'être au niveau de celle-ci.
  5. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel le second étage de compression (102) comprend une sertisseuse.
  6. Appareil (40, 120) selon la revendication 5, dans lequel la sertisseuse comprend une pluralité d'éléments mobiles.
  7. Appareil (40, 120) selon la revendication 5 ou 6, dans lequel la sertisseuse comprend des mâchoires (104) définissant un mandrin.
  8. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel les premier et second étages de compression (100, 102) sont actionnés par des actionneurs rotatifs respectifs (46, 110) qui peuvent tourner sur un axe commun, éventuellement un axe longitudinal de l'appareil.
  9. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel le second étage de compression (102) peut être fixé de façon libérable au premier étage de compression (100) au moyen d'un actionneur fileté (110) pour actionner le second étage de compression (102).
  10. Appareil (40, 120) selon l'une quelconque des revendications précédentes, dans lequel le premier étage de compression (100) comprend un ou plusieurs parmi (i) un élément de déplacement (44) pour appliquer à une valve endoprothétique (10) à l'intérieur du canal (42), une force de poussée dans le sens radial à partir de l'extérieur du canal (42) ; et/ou (ii) un élément d'entraînement (46) pour appliquer une force de poussée dans le sens radial à l'extérieur du canal (42) vers une partir de l'élément de déplacement (44) dans le sens radial à l'extérieur du canal (42).
  11. Appareil (40, 120) selon la revendication 10, dans lequel l'élément de déplacement comprend un anneau duquel dépassent vers l'intérieur plusieurs éléments, les éléments coulissant dans des fentes à travers une paroi du canal, pour appliquer une force de poussée à une endoprothèse valvulaire à l'intérieur du canal.
  12. Appareil (40, 120) selon l'une quelconque des revendications précédentes, comprenant en outre :
    une pluralité de segments (122a, 122b) qui peuvent être assemblés autour d'une partie d'un cathéter de pose (12) afin de définir une forme tubulaire ; et
    au moins un élément de retenue pour retenir la pluralité de segments (122a, 122b) dans la forme assemblée.
  13. Appareil (40, 120) selon la revendication 12, dans lequel l'élément de retenue comprend au moins un écrou fileté (126) et/ou un surtube de support (124).
  14. Procédé de compression d'une endoprothèse valvulaire (10), le procédé comprenant :
    (a) la fourniture d'un appareil comprenant un premier étage de compression (100) ayant un canal longitudinal (42), et un second étage de compression (102) accouplé ou pouvant être accouplé au premier étage de compression (100) pour comprimer une partie de l'endoprothèse valvulaire après le passage par le premier étage de compression ;
    (b) l'actionnement d'un actionneur (46) du premier étage de compression par rotation, pour faire avancer l'endoprothèse valvulaire à l'intérieur du canal creux (42), afin de comprimer progressivement l'endoprothèse valvulaire en réponse à, ou en association avec, l'avancement longitudinal à l'intérieur du canal creux (42) caractérisé par
    (c) l'actionnement du second étage de compression (102) afin de comprimer davantage une partie de l'endoprothèse valvulaire sans faire avancer l'endoprothèse valvulaire plus loin à l'intérieur du second étage de compression.
  15. Procédé selon la revendication 14, dans lequel l'étape d'actionnement du second étage de compression comprend l'actionnement d'un actionneur (110) du second étage de compression par rotation.
  16. Système comprenant :
    un cathéter de pose (12) possédant une région de confinement pour accueillir une endoprothèse valvulaire cardiaque, des première et seconde gaines (30a, 30b) conçues pour se déplacer dans des directions opposées afin de se fermer sur des parties respectives de l'endoprothèse valvulaire dans un état comprimé au niveau de la région de confinement ; et
    un appareil (40, 120) destiné à être utilisé dans la compression de l'endoprothèse valvulaire (10) pour un chargement dans le cathéter de pose (12), l'appareil (40, 120) comprenant :
    un premier étage de compression (100) pour comprimer l'endoprothèse valvulaire (10); et
    un second étage de compression (102) accouplé ou pouvant être accouplé au premier étage de compression (100) et conçu pour comprimer davantage une partie de l'endoprothèse valvulaire (10) après le passage par le premier étage de compression (100)
    le premier étage de compression (100) comprenant un canal creux (42), et le premier étage de compression (100) étant conçu pour comprimer progressivement l'endoprothèse en réponse à, ou en association avec, l'avancement longitudinal de l'endoprothèse à l'intérieur du canal creux (42), caractérisé en ce que
    le second étage de compression (102) est conçu pour comprimer une partie de l'endoprothèse au niveau du second étage de compression (102) sans l'avancement de l'endoprothèse à l'intérieur du second étage de compression (102).
  17. Système selon la revendication 16, dans lequel l'appareil (40, 120) est un appareil (40, 120) selon l'une quelconque des revendications 1 à 13.
