Definitions
- the invention relates to an arrangement for the storage and administration of rinsing solutions, preferably in the medical field as for general and endoscopic operations, with a rinsing solution bag, which is arranged in a rigid container.
- the aim of this development is the preparation, storage and mobile administration of locally prepared rinsing solutions.
- a mobile fluid storage that is equipped with a sterile disposable item, an accurate volume determination and a sterile collection and administration to allow a simple and inexpensive application.
- Both rinse solutions for endoscopic and general surgical operations, such as gynecology, urology, arthroscopy can be prepared by the use of purisole, Ringer, saline concentrates, as well as solutions for therapeutic applications.
- the invention is in particular the object of specifying an arrangement for storing and administering rinsing solutions, which meets high hygiene requirements and allows sterile removal and easy operation and administration of the rinsing liquid.
- Medical rinse solutions are usually processed from distilled water as a raw material, which is produced centrally, in a central production process to rinse solutions and brought to the site with significant logistics costs.
- Necessary improvements, purpose and purpose of this invention are therefore the cost-effective, user-friendly on-site production of a rinsing solution with low personnel and a corresponding to the investigation or several operations rinsing volume. Of particular importance is the uninterrupted administration without additional additional human effort while maintaining the application temperature and hygiene of the solution.
- a space-saving technique for the preparation of the rinse solution and a mobile rinse solution container which contains the essential components for high hygiene, safety, ease of use and a constant flow and pressure to administer the rinse.
- a high availability of the devices in all measuring and monitoring tasks with regard to their intrinsic safety and only a remote probability of failure is important in order to avoid a catastrophic effect on the patient under all circumstances, or the quality or toxicity of the liquid produced in the assured acceptance criteria to monitor.
- This object is achieved according to the invention effectively in that for the preparation of the rinsing solution, the combination of a reverse osmosis membrane and two further filter stages, for example ultra or sterile filters, preferably as a capillary membrane, are used.
- This filter combination and other components are referred to below as the filling station.
- the aforementioned rinse solution is representative of e.g. for Ringer -, and / or other sodium chloride solutions, which can be used in particular in the field of surgery, but also in other medical or mentioned areas.
- the concentrates and their mixing ratios are to be adapted to the respective applications.
- the growth of germs of the concentrate is almost prevented by the high concentration.
- the concentrate container which is advantageously designed as a bag, is hung on prepared receptacles of the filling station concentrate scale and the mixing process is initiated. Initially, the balance is verified by the known bag weight.
- the Standstations Disposions of the concentrate as well as the rinsing solution bag to be described will be brought to self-cleaning, foolproof connection connectors of the filling station, which are executed in this application, for example, as flap solutions, but also on the device side can be designed as a flexible hose, brought by the user.
- a mobile rinsing solution container which is preferably designed as a pressure vessel, equipped with an insertable sterile rinsing liquid bag, which is filled with a correspondingly large volume.
- the rinsing solution bag contains a permanent connection connector, which can be pushed through and fixed by the lockable lid of the pressure vessel.
- the connection connector can be provided with continuing flexible hose lines, which are designed as filling or transfer lines.
- the Connector can be carried out with advantage only a hose which can be used either as a filling or as a transfer hose depending on the sterility requirements.
- a transfer system for example, can be connected to the transfer connection of the rinsing solution connector, for example with an endoscopy system.
- a connection to other systems commonly used in surgery, for example to irrigation suction systems is also practicable and possible.
- pressurized gas air
- pressurized gas air
- compressed gas control and monitoring within the mobile flushing solution container is advantageously established.
- Compressed gas production and feed can be generated for example by an in-house source, but also on the device side.
- the mobile flushing solution container advantageously includes a balance which monitors the filling state and which, for safety reasons, is to be automatically tested by means of reference weight.
- the data are written to the confirmation of operation and protection computer together with a checksum in the trend data memory, which may be preferably designed as Eprom, but also as another storage medium.
- the electronics of the mobile rinsing solution tank can be operated by means of rechargeable batteries, on the display of the mobile rinsing solution container all required parameters and their deviations such as weight, temperature and tank pressure are displayed.
- FIG. 1 shows the entire preparation up to the application.
- the liquid to be treated can be forwarded to the RO system (2), for example, via an optional prefiltration (1), which can be designed as particle and / or further filter stages for the elimination of hardness formers and chlorine.
- the RO (2) includes, for example, a disinfection unit (4) with which, without the user's help, a chemothermal disinfection is feasible.
- Canister (67) contains the disinfectant / cleaning agent which is advantageously used as citrate-containing solution.
- the further function of device (4) is derived from the representation and will not be described further here. Of course, there is the possibility of a hot cleaning of the RO system without the use of other disinfectants.
- the permeate produced by the RO system (2) is circulated above the primary side of the filter (3).
- the permeate released by the RO controller (58) by means of conductivity measurement (not shown) reaches the secondary side of the filter (3) and the permeate release valve (5) to the mixing unit (12).
- any pre-heated permeate already from the RO plant (2) is heated to the required rinsing solution temperature via the heater (9) and temperature regulator (8,13). Via line (11), the permeate is fed to a mixing chamber (15), into which by means of pump (23) concentrate from bag (26) and line (25), the connector (24) and device-side connection connector (22) is supplied.
- the concentrate flap (20) is opened, detector (19) reports "open” because magnet (21) has exceeded the required distance.
- the concentrate flushing valve (17) is opened only with the flap (20) closed and according to selected or preset flushing programs, in order to clean the connection connector (22). Concentrate bag (26) is hung with its suspensions (27) in corresponding hooks of the concentrate bag scale (28).
- the second conductivity and temperature measurement (16) detects the corresponding values for reasons of redundancy.
- the rinsing liquid homogeneously mixed and tempered by chamber (15) passes via line (29) to a second sterile filter (30). Defective flushing liquid is discarded via bypass valve (31) to drain (100).
- the valve (31) is closed and the rinsing solution release valve (33) is open, the rinsing liquid is conducted via the device-side rinse solution connector (35) to the connected pouch connector (38), line (39) to the mobile rinsing solution container (40), in which a sterile rinsing solution bag (82) is provided. is inserted.
- the possibility of taking a sample of rinsing solution consists of sampling (32).
- the mobile flushing solution container includes a balance (43), which registers the respective level or the weight of the flushing amount. Also, a temperature sensor (59) is mounted so that the Spül thoroughlykeitstemperatur is indirectly measurable.
- the device-side connector (35) is rinsed or disinfected with sterile liquid or cleaning solution via flushing outflow (99).
- the test of the filter (3/30) takes place with the valves (20/36) closed by filtered air supply by means of air pump (6) and can optionally by valve switching the secondary side of the filter (3) or the primary side of the filter (30) act on air.
- the liquid is partially displaced by the air. Due to the hydrophilic character of the filter membrane, only a very small pressure drop will take place when the filter characteristic is intact, which can be registered or monitored by means of a pressure sensor (14) and electronics (58).
- FIG. 1 schematically shows a possible transfer of the rinsing liquid to an endoscopic system (57).
- Compressed air connector (48) can be connected by means of flexible hose lines (49) to an in-house compressed gas source.
- the pressure control unit (47) includes an adjustable pressure regulator (50), a mushroom emergency release button (51), a manual pressure relief valve (52), a gauge gauge (53) and an electronic pressure transducer (54). the like all sensors and actuators by means of redundant electronics (58) can be evaluated and displayed.
- the low pressure control valve (50) can only be adjusted by means of tools.
- the pressure control unit (47) can be designed for a control range from 0 to 0.5 bar and is set for practical use to 0.3 bar delivery pressure, for example, prostate surgery.
- the thus regulated air is introduced via hose connection (66) in pressure vessel (45).
- the rinsing liquid in bag (82) is conveyed by the supplied pressure via transfer port (55) and a suitable transfer system (56) to the endoscopic system (57).
- FIG. 2 spatially illustrates the total unit of a mixing plant or filling station. Due to the assumed confined space in the hospital, the filling station (60) was constructed as flat as possible so as not to impair the passageways in corridors or in premises. This requires a vertical design of the RO system (2) with membrane (68), feed tank (69) and pump (70). Shown is also a cleaning canister (67). Above the RO system, the mixing unit (12) is mounted, in which only the position of the concentrate flap (20), the rinsing solution flap (36). the heater (9) and the sterile filter (30) is pointed to illustrate the handling. Where the flaps are shown here in the closed state.
- Concentrate bag scale (28) is mounted below the electronics (58) and shown in the form of a boom (71) with receiving hook for the concentrate bag.

Description
The invention relates to an arrangement for the storage and administration of rinsing solutions, preferably in the medical field as for general and endoscopic operations, with a rinsing solution bag, which is arranged in a rigid container.
The aim of this development is the preparation, storage and mobile administration of locally prepared rinsing solutions. A mobile fluid storage that is equipped with a sterile disposable item, an accurate volume determination and a sterile collection and administration to allow a simple and inexpensive application.
Both rinse solutions for endoscopic and general surgical operations, such as gynecology, urology, arthroscopy can be prepared by the use of purisole, Ringer, saline concentrates, as well as solutions for therapeutic applications.
An application of this development for other areas such as veterinary medicine, in the laboratory or in biology and in pharmacy as a high-purity rinsing liquid or approach medium for the production of drugs, cell cultures and the like is conceivable and practicable.
The invention is in particular the object of specifying an arrangement for storing and administering rinsing solutions, which meets high hygiene requirements and allows sterile removal and easy operation and administration of the rinsing liquid.
This object is achieved by the features of claim 1.
Advantageous embodiments of the invention are characterized in the subclaims.
Medical rinse solutions are usually processed from distilled water as a raw material, which is produced centrally, in a central production process to rinse solutions and brought to the site with significant logistics costs.
For medical use, for example, industrially produced rinsing solutions with volume 3l, 5l, 10l are made available to the hospital and with large in-house, staff logistics interim u. disposed of.
By way of example, for the duration of the surgery or exam, these bag volumes will not be sufficient for bladder surgery with approximately 60 L of rinse fluid, so a jumper outside the central OR area must be available to provide, warm and deliver the pouches.
The administration takes place partly gravimetrically or with bag pressure cuffs. In addition, often expensive disposable items such as pump segments or for the bag warmer are required.
A considerable disadvantage, for example in endoscopic examinations, is the lack of visibility through floating tissue or pulsating rinsing fluid, because, for example, the required rinsing fluid pressure between 0.1 bar and 0.3 bar is not kept constant.
To improve wound hygiene, rinse generously. This causes personnel and material costs.
The regulatory and normative demands on the quality of the raw material water are so high that it is not yet possible to produce verifiable medical rinsing solutions on-site, eg in hospitals. On the one hand, these are the high microbiological and, on the other hand, the required chemical requirements for the raw material water, which preclude a verifiable and verifiable normatively demanded quality of on-site demand-controlled production.
The decentralized production of medical rinsing solutions by hospital personnel requires safe procedures both in the operation and in the reliability of the technology with regard to the rinsing solution quality.
Eine besondere Bedeutung kommt der unterbrechungsfreien Verabreichung ohne zusätzlichen personellen Mehraufwand unter Einhaltung von Anwendungstemperatur und Hygiene der Lösung zu.Necessary improvements, purpose and purpose of this invention are therefore the cost-effective, user-friendly on-site production of a rinsing solution with low personnel and a corresponding to the investigation or several operations rinsing volume.
Of particular importance is the uninterrupted administration without additional additional human effort while maintaining the application temperature and hygiene of the solution.
In this case, a space-saving technique for the preparation of the rinse solution and a mobile rinse solution container are used, which contains the essential components for high hygiene, safety, ease of use and a constant flow and pressure to administer the rinse.
Restmengen sollen einfach zu entsorgen sein.Remaining quantities should be easy to dispose of.
A high availability of the devices in all measuring and monitoring tasks with regard to their intrinsic safety and only a remote probability of failure is important in order to avoid a catastrophic effect on the patient under all circumstances, or the quality or toxicity of the liquid produced in the assured acceptance criteria to monitor.
This object is achieved according to the invention effectively in that for the preparation of the rinsing solution, the combination of a reverse osmosis membrane and two further filter stages, for example ultra or sterile filters, preferably as a capillary membrane, are used.
This filter combination and other components are referred to below as the filling station.
To prepare, for example, about 60 l ready-to-use purisol solution, approximately 561 sterile-filtered permeate with approximately 3.6 l of highly concentrated purisoleconate are to be proportionally diluted or mixed in such a way that the resulting rinsing solution can be used without further tests for intraoperative and postoperative rinsing with the bladder ,
The aforementioned rinse solution is representative of e.g. for Ringer -, and / or other sodium chloride solutions, which can be used in particular in the field of surgery, but also in other medical or mentioned areas. The concentrates and their mixing ratios are to be adapted to the respective applications.
However, the described method and the components and volumes used are not reduced to this. Due to the high-purity active ingredients, the exact mixing and dilution, a wide range of rinsing solutions can be produced.
Advantageously, the growth of germs of the concentrate is almost prevented by the high concentration.
For the preparation of the rinsing solution, the concentrate container, which is advantageously designed as a bag, is hung on prepared receptacles of the filling station concentrate scale and the mixing process is initiated. Initially, the balance is verified by the known bag weight.
The Füllstationsseitigen connections of the concentrate as well as the rinsing solution bag to be described will be brought to self-cleaning, foolproof connection connectors of the filling station, which are executed in this application, for example, as flap solutions, but also on the device side can be designed as a flexible hose, brought by the user.
With great advantage, a mobile rinsing solution container, which is preferably designed as a pressure vessel, equipped with an insertable sterile rinsing liquid bag, which is filled with a correspondingly large volume.
The rinsing solution bag contains a permanent connection connector, which can be pushed through and fixed by the lockable lid of the pressure vessel. The connection connector can be provided with continuing flexible hose lines, which are designed as filling or transfer lines. Whereby the Connector can be carried out with advantage only a hose which can be used either as a filling or as a transfer hose depending on the sterility requirements.
For administration of the rinsing liquid at the point of use, a transfer system, for example, can be connected to the transfer connection of the rinsing solution connector, for example with an endoscopy system. A connection to other systems commonly used in surgery, for example to irrigation suction systems is also practicable and possible.
The object of the simple operation and administration with constant purge flow and pressure has been solved by pressurized gas (air) is preferably introduced either into the pressure vessel or optionally directly into the rinsing liquid bag.
In this case, the compressed gas control and monitoring within the mobile flushing solution container is advantageously established. Compressed gas production and feed can be generated for example by an in-house source, but also on the device side.
The Propörtionierung of concentrate and permeate by means of a concentrate and a rinsing solution container scale, the concentrate scale is verified in the filling station with each attachment of the filled concentrate container.
For this, the mobile flushing solution container advantageously includes a balance which monitors the filling state and which, for safety reasons, is to be automatically tested by means of reference weight.
For homogenization and temperature control, highly pure or approximately sterile permeate is heated and mixed with added concentrate in a mixing block.
Prior to introduction into a sterile rinse solution / bag, a second sterile filtration of the mixed solution was performed.
The cleaning of the system or germ prevention and reduction by the combination of a low-toxic, based on citrate disinfection u. Detergent with a warming water running. Wherein both the primary and the secondary side of the reverse osmosis are to be disinfected or cleaned separately by means of an additional pump without transmembrane flow.
In principle, all process-relevant data from both the operating and the protection computer are recorded and possibly calculated. The measurement results are sent from the factory to the protection computer and vice versa. Each computer compares the measurement results with its own and returns a confirmation.
The data are written to the confirmation of operation and protection computer together with a checksum in the trend data memory, which may be preferably designed as Eprom, but also as another storage medium.
The electronics of the mobile rinsing solution tank can be operated by means of rechargeable batteries, on the display of the mobile rinsing solution container all required parameters and their deviations such as weight, temperature and tank pressure are displayed.
By establishing a wireless data exchange between filling station and mobile rinsing solution container, for example, the monitoring of the filling, the proportionality and the temperature takes place.
Further details and advantages are described in the figures shown below.
Die Möglichkeit zur Entnahme einer Spüllösungsprobenmenge besteht an Probeentnahme (32).To eliminate microbiological contamination, the RO (2) includes, for example, a disinfection unit (4) with which, without the user's help, a chemothermal disinfection is feasible. Canister (67) contains the disinfectant / cleaning agent which is advantageously used as citrate-containing solution. The further function of device (4) is derived from the representation and will not be described further here. Of course, there is the possibility of a hot cleaning of the RO system without the use of other disinfectants.
The permeate produced by the RO system (2) is circulated above the primary side of the filter (3). The permeate released by the RO controller (58) by means of conductivity measurement (not shown) reaches the secondary side of the filter (3) and the permeate release valve (5) to the mixing unit (12).
Any pre-heated permeate already from the RO plant (2) is heated to the required rinsing solution temperature via the heater (9) and temperature regulator (8,13). Via line (11), the permeate is fed to a mixing chamber (15), into which by means of pump (23) concentrate from bag (26) and line (25), the connector (24) and device-side connection connector (22) is supplied.
The concentrate flap (20) is opened, detector (19) reports "open" because magnet (21) has exceeded the required distance. The concentrate flushing valve (17) is opened only with the flap (20) closed and according to selected or preset flushing programs, in order to clean the connection connector (22).
Concentrate bag (26) is hung with its suspensions (27) in corresponding hooks of the concentrate bag scale (28).
The second conductivity and temperature measurement (16) detects the corresponding values for reasons of redundancy. The rinsing liquid homogeneously mixed and tempered by chamber (15) passes via line (29) to a second sterile filter (30). Defective flushing liquid is discarded via bypass valve (31) to drain (100).
When the valve (31) is closed and the rinsing solution release valve (33) is open, the rinsing liquid is conducted via the device-side rinse solution connector (35) to the connected pouch connector (38), line (39) to the mobile rinsing solution container (40), in which a sterile rinsing solution bag (82) is provided. is inserted. The possibility of taking a sample of rinsing solution consists of sampling (32).
The mobile flushing solution container includes a balance (43), which registers the respective level or the weight of the flushing amount. Also, a temperature sensor (59) is mounted so that the Spülflüssigkeitstemperatur is indirectly measurable.
When the rinsing solution flap (36) is closed and when a suitable rinsing program is selected and initiated, the device-side connector (35) is rinsed or disinfected with sterile liquid or cleaning solution via flushing outflow (99). The test of the filter (3/30) takes place with the valves (20/36) closed by filtered air supply by means of air pump (6) and can optionally by valve switching the secondary side of the filter (3) or the primary side of the filter (30) act on air. The liquid is partially displaced by the air. Due to the hydrophilic character of the filter membrane, only a very small pressure drop will take place when the filter characteristic is intact, which can be registered or monitored by means of a pressure sensor (14) and electronics (58).
Through this test, both the filters (3/30) and the tightness of the flaps (20/36) can be verified.

Likewise is in
To ensure a constant flow of flushing fluid, the pressure control unit (47) includes an adjustable pressure regulator (50), a mushroom emergency release button (51), a manual pressure relief valve (52), a gauge gauge (53) and an electronic pressure transducer (54). the like all sensors and actuators by means of redundant electronics (58) can be evaluated and displayed.
The low pressure control valve (50) can only be adjusted by means of tools. The pressure control unit (47) can be designed for a control range from 0 to 0.5 bar and is set for practical use to 0.3 bar delivery pressure, for example, prostate surgery. The thus regulated air is introduced via hose connection (66) in pressure vessel (45).
The rinsing liquid in bag (82) is conveyed by the supplied pressure via transfer port (55) and a suitable transfer system (56) to the endoscopic system (57).
It is understood that other units than endoscopic systems can also be connected to system (56).
In addition, it is stated that a further sterile filter, not shown here, could be connected to line (55).
It would also be possible to introduce the controlled pressure gas medium directly in the flushing solution bag (41).

In order for a positive sealing with good sliding properties between connector (83) and connector receiving seal (78) can occur, there is preferably seal (78) consists of a Teflon insert (128), which with an O-ring (126) and a pressure plate (127) is pressed against connector (83) so that the aforementioned objects are achieved.
A positive and sealing connection of the cover (44) to the pressure vessel (45) is produced on the one hand by the cover seal (74) and the conical seal bearing (77) in the pressure vessel opening in the closed state.
For closing, hook (126) pulls lid lock (76) into position by means of locking handle (80). Latch lock (81) engages behind the pivot (124).
Lid clamping hinge (75) holds lid (44) in an open position in an upright position.
It is understood that the bag (41/82) is to be introduced into the container.
For vertical support two lateral guides (73) are mounted on the pressure vessel (45).
The compressed air supply (66) is mounted, for example, in the hinge region (75) by means of connection (84).
Connector lock (79) is open in the event of a fault from the outside via the rotary shaft (85) by means of tools.
Also shown in this figure is the filling line (39) with connector (38) which is to be connected in the filling process to connection (35). After filling, clamp (72) can be closed. For the differentiation of filling line (39) and transfer line (55) these are equipped with different connectors and in different lengths, as shown executed.
In the flap (36) there is a magnet (37) which activates a magnetic contact (34) when the flap is closed.
For flushing, the flap (36) is closed so that the flap locking hook (91) engages the flap latch (89) into the locking collar (96) of the connector (35).
By pushing back the lock (89) via pivot point (92) by means of flap locking handle (90), the locking spring (93) is compressed and the flap locking hook (91) releases the excavation process of the flap (36).
The flap swings upwards. This is supported by excavation spring (102), which engages laterally of the pivot point (101).
For complete flushing of the connector (35) presses the seal (94) with the flap closed positively on the outer cone (95) of the connector (35). The rinsing liquid passes via connection (29) via the inner cone (88) to the rinsing chamber (103) and from there via the circumferentially mounted rinsing bores (98) of the connector (35) into the annular gap (97) from which the rinsing drain (99) takes place ,
For reasons of security of confusion for the application, the technical design of the flushing solution connections was designed differently from those of the concentrate connections.
Underneath the rinsing solution flap (36) is a rinsing solution connector (35) made, for example, with an inner cone (88) 1 to 16 and a double-threaded external thread 13x8. The bag (82) has a rectangular, welded contour into which one side a bag and connector receptacle (107) is welded circular in the film. The connector receptacle (107) is internally provided radially with at least two ratchet teeth (110), in which the connector (83) is pressed so that a positive, sealing, non-releasable compression (109) is formed.
The connector (83) is in
For ease of insertion into the connector receptacle (78) of the lid (44), the front portion (114) includes a gate (121). At the rear, a receptacle (118) for the kink protection (115) of the inner conduits (111, 113) is provided.
When installed, the bag connector (83) is fastened vertically with connector lock (79) in retaining groove (87).
When the cover (44) is closed, the bag (82) is rotated by approximately 90 ° relative to the connector (83) and thus also a change in position of the inner transfer (111) and filling line (113). The anti-kink (115) includes hose guides (129), which should prevent any kinking of the same.
The transfer line (111) closes at the lower end with a hose weight (112) which has contours on all outer sides in order to ensure complete emptying of the bag when the film is compressed and to counteract any lift of the transfer line (111).
To avoid touch contamination, connectors (55) and (38) may include protective gaskets (122) for fumigation for sterility.
The films of the bag (82) are circumferentially welded so that a welding bar of min. greater than 2 mm is present to avoid a rupture.
Bereits dargestellt in
Wherein bag (82) is also executable in other than the description shown contour or shape. So it is e.g. possible to attach the two connections of the connector (83) at the front, ie at the top of the bag (82), or completely dispense with the connector (83) and weld the terminals (39/55) directly on top. It is then to use another form-fitting seal. In addition, when using an additional pump, not shown in the figures, a circulation of the liquid in the bag via the two terminals of the connector (83) feasible to improve homogeneity, temperature or a liquid already in the bag or prepare. The connections are to be connected according to the flow direction of the pump or the purpose.
The cone (135) allows the inclusion of different silicone tube diameters (136) to connect different, commonly used in surgery system.
The hose clamp (134) can be advantageously designed as a roller clamp for controlling the purge flow.
The length of the tube (133) can be freely selected and adapted in its diameter to the desired flushing flow, wherein for the system shown, a hose diameter (133) of about 7 mm was selected, which is glued into connector (132).
For reasons of sterility, port (132) was equipped with a gassable cap (131).
