Appareil médical pour la rééducation et la prévention des entorses.
Domaine technique de l'invention
L'invention concerne un appareil médical pour recréer des mouvements biomécaniques pour la rééducation et la prévention des entorses, comprenant :
- un élément déstabilisateur de l'arrière pied formé par un articulateur pouvant pivoter autour d'un axe de pivotement orienté suivant un angle compris entre 10° et 55° par rapport au plan sagittal longitudinal,
- et un chausson destiné à maintenir le pied sur l'articulateur et sur une pelote d'avant pied .
État de la technique
Le document EP 1221332 décrit un appareil de rééducation de la cheville, ayant un articulateur pour déstabiliser le pied, grâce à un axe de pivotement orienté de préférence à 45° par rapport au plan sagittal. Le positionnement de cet axe de pivotement est situé dans l'articulateur, et se trouve en- dessous de l'axe de Henke constituant l'articulation anatomique de la cheville. Objet de l'invention
L'objet de l'invention consiste à réaliser un appareil médical de rééducation et de prévention des entorses, permettant d'optimiser les mouvements biomécaniques de l'articulateur, et de faciliter le réglage de l'amplitude de ces mouvements.
L'appareil selon l'invention est caractérisé en ce que l'articulateur comporte : - une partie supérieure composée d'un berceau basculant demi-cylindrique équipé de pistes de roulement, et d'un plateau rotatif servant de moyen d'appui du talon de l'utilisateur,
- une partie inférieure en contact avec le sol et comprenant une base de support dotée de galets coopérant avec les pistes de roulement du berceau après assemblage des deux parties,
- et des moyens de réglage pour ajuster l'amplitude de la course de basculement du berceau. L'axe de basculement de l'articulateur est fictif, et est situé au niveau de l'axe de Henke correspondant à l'articulation de la cheville de l'utilisateur.
Selon un mode de réalisation préférentiel, les moyens de réglage comportent un levier relié à un disque doté d'un téton destiné à être positionné dans un cran du berceau basculant. Les pistes de roulement du berceau sont conformées en arcs de cercle, dont le rayon est centré de préférence sur l'axe de basculement. Le plateau circulaire est au-dessus du berceau et possède un degré de liberté en rotation d'axe vertical passant par son propre centre, avec une course en rotation comprise entre 10° et 30°, de préférence 20°.

Sur les figures, un appareil médical 10 permet de recréer des mouvements biomécaniques prédéfinis pour la rééducation et la prévention des entorses de cheville.
L'appareil médical 10 est attaché à un pied d'une personne comme une chaussure, et est composé à cet effet d'un chausson 11 et de deux éléments déstabilisateurs. Ces derniers sont formés respectivement par un articulateur 12 et une pelote d'avant pied 13, destinés à déstabiliser respectivement l'arrière pied et l'avant pied de la personne. Le chausson 11 maintient le pied sur l'articulateur 12 et sur la pelote d'avant pied 13 lors de l'exécution des mouvements biomécaniques. L'articulateur 12 de déstabilisation de l'arrière pied est composé d'une partie supérieure 14 en contact avec le talon de la personne, et mobile par rapport à une partie inférieure 15 prenant appui sur le sol.
La partie supérieure 14 de l'articulateur 12 comporte un berceau 16 basculant de forme demi-cylindrique, surmonté d'un plateau 17 rotatif avec interposition d'une rondelle 32 de frottement. La base du berceau 16 est équipée d'une paire de pistes de roulement 18, 19 conformées en arcs de cercle, fixées aux bords du berceau 16.
Le plateau 17 circulaire sert de moyen d'appui du talon de l'utilisateur, et possède un degré de liberté en rotation d'axe vertical passant par son propre centre. L'amplitude de sa course en rotation est environ dans une fourchette de 10° à 30°, de préférence 20°. Des lames ressorts 20 sont encastrées dans le berceau 16, et coopèrent par déformation élastique avec une encoche du plateau 17 pour assurer son rappel vers une position centrale lorsque aucune contrainte mécanique ne lui est appliquée. Une bille 21 de retenue souple et téléscopique est fixée dans une gorge 22 radiale du plateau 17, et vient se loger dans une cavité 23 du berceau 16 lorsque le plateau 17 se trouve dans la position centrale de 0°. La présence de la bille 21 constitue un point dur élastique qui rend cette position centrale stable, avec une légère résistance de retenue à 0°. Lors de la rotation du plateau 17 suite à un mouvement du pied de l'utilisateur, la bille 21 sort par déformation élastique de la cavité 23 du berceau 16, et autorise le mouvement de rotation limitée du plateau 17.
La partie inférieure 15 de l'articulateur 12 est en contact avec le sol, et est formée par l'assemblage d'une base de support 24 dotée de galets 25 en forme de rouleaux, d'un fond 26 et d'une semelle 27 souple. Les pièces base de support 23, fond 26 et semelle 27 sont clippées entre elles, et ne possèdent aucun degré de liberté les unes par rapport aux autres. Les rouleaux des galets 25 sont fixés sur la base de support 23 au moyen d'axes métalliques, de manière à tourner librement autour de ces axes.
Lors de la fixation de la partie supérieure 14 sur la partie inférieure 15, les pistes de roulement 18, 19 peuvent rouler sur les galets 25 en autorisant le mouvement de basculement du berceau 16. Un levier de réglage 28 relié à un disque 29, est posé sur le fond 26, et peut être actionné en rotation pour ajuster l'amplitude de la course de basculement du berceau 16. A cet effet, le disque 29 est pourvu d'un téton 30 destiné à être positionné dans l'un des trois crans de tailles différentes du berceau 16.
En position assemblée, les deux parties 14 et 15 de l'articulateur 12 sont inséparables, et la partie supérieure peut basculer au-dessus de la partie inférieure 15. L'inclinaison de la partie supérieure 14 par rapport à la partie inférieure 15 dépend de l'amplitude du mouvement de basculement du berceau 16 selon la position du levier de réglage 28.
En l'absence de contrainte sur l'articulateur 12, la partie inférieure 15 est rappelée vers une position de -7°, en considérant que la position 0° est une position horizontale de la surface du berceau 16. Ce rappel est rendu possible grâce à un ressort de traction 31 reliant la base de support 24 fixe au berceau 16 basculant.
L'axe de rotation XY de l'articulateur 12 est fictif, et s'étend au niveau de l'axe anatomique de Henke correspondant à l'articulation de la cheville. Cet axe de rotation XY est orienté à 45° par rapport au plan sagittal du corps, et à 0° par rapport au plan horizontal.
Les mouvements de basculement de l'articulateur 12 présentent trois réglages en amplitude, en considérant que 0° correspond à une position horizontale de la surface supérieure du plateau 17. Ces trois réglages sont les suivants : - (de -5° à +30°) permet d'effectuer un travail de prévention et de rééducation des entorses de cheville (proprioception statique et dynamique, et renforcement musculaire) ;
- (de -7° à +7°) permet d'effectuer un travail de posturologie ;
- (de -15° à +15°) permet d'effectuer un travail de prévention et de rééducation des entorses du genou.
En plus des mouvements de basculement autour de l'axe XY, le plateau 17 rotatif qui est mobile sur l'articulateur 12, peut également effectuer une course en rotation d'environ 20° de l'arrière pied d'axe vertical.
La pelote d'avant pied 13 est destiné à déstabiliser ou stabiliser l'avant du pied. Sur la figure 13, la face supérieure de la pelote présente une première zone Z1 de placement des doigts de pied, et une deuxième zone Z2 d'appui des métatarses. La face inférieure de la pelote est subdivisée en deux parties P1 , P2 formant entre elles un coin s'étendant selon une ligne 33. Une cale 34 amovible est intégrée dans la partie inférieure de la pelote 13. Sa géométrie lui permet de s'insérer de deux manière différentes dans la pelote, de sorte à générer soit une déstabilisation latérale de l'avant pied, soit une stabilisation de l'avant pied.

Claims

Revendications
1. Appareil médical pour recréer des mouvements biomécaniques pour la rééducation et la prévention des entorses, comprenant :
- un élément déstabilisateur de l'arrière pied formé par un articulateur (12) pouvant pivoter autour d'un axe de pivotement XY orienté suivant un angle compris entre 10° et 55° par rapport au plan sagittal longitudinal,
- et un chausson (11) destiné à maintenir le pied sur l'articulateur (12) et sur une pelote d'avant pied (13),
caractérisé en ce que l'articulateur (12) comporte :
- une partie supérieure (14) composée d'un berceau (16) basculant demi- cylindrique équipé de pistes de roulement (18, 19), et d'un plateau (17) rotatif servant de moyen d'appui du talon de l'utilisateur,
- une partie inférieure (15) en contact avec le sol et comprenant une base de support (24) dotée de galets (25) coopérant avec les pistes de roulement du berceau (16) après assemblage des deux parties (14, 15),
- et des moyens de réglage pour ajuster l'amplitude de la course de basculement du berceau (16).
2. Appareil médical selon la revendication 1 , caractérisé en ce que les moyens de réglage comportent un levier (28) relié à un disque (29) doté d'un téton (30) destiné à être positionné dans un cran du berceau (16).
3. Appareil médical selon la revendication 1 , caractérisé en ce que les pistes de roulement (18, 19) du berceau (16) sont conformées en arcs de cercle dont le rayon est centré sur l'axe de basculement.
4. Appareil médical selon la revendication 1 , caractérisé en ce que le plateau (17) circulaire est au-dessus du berceau (16) et possède un degré de liberté en rotation d'axe vertical passant par son propre centre, avec une course en rotation comprise entre 10° et 30°, de préférence 20°.
5. Appareil médical selon la revendication 4, caractérisé en ce que le berceau (16) est pourvu de lames ressorts (20) pour le rappel du plateau (17) vers une position centrale en l'absence de contraintes mécaniques, et que le plateau (17) comporte une bille (21) de retenue souple pour constituer
5 un point dur dans ladite position centrale.
6. Appareil médical selon la revendication 1 , caractérisé en ce que les galets (26) comportent des rouleaux montés à libre rotation sur des axes de la base de support (23).

7. Appareil médical selon l'une des revendications précédentes, caractérisé en ce qu'un ressort de traction (31) est ancré entre la base de support (24) fixe et le berceau (16) basculant pour le solliciter vers une position prédéterminée.
