Definitions

  • the invention relates in particular to an intervertebral implant and its tool for positioning in the intervertebral space.
  • an intervertebral disc is damaged as a result of compression or because of degeneration, such as those frequently encountered in diseases related to herniated discs, it is necessary to stabilize the disc to eliminate the pain felt by the patient.
  • the solution adopted is to make the two vertebrae in question together, for example using intervertebral implants and bone grafts: it is called bone fusion of these vertebrae.
  • Intervertebral implants are commonly used in bone surgery of the spine; most often the vertebrae are spread with distractors to introduce and position these implants, as explained in particular in US6942698, FR2767676 and WO99 / 38463.
  • distractors that must be implemented carefully, complicates the surgical procedure and is highly invasive.
  • Patent application US02 / 0029082 is also known, the use of an implant whose establishment is done without distractor, thanks to an insertion of a main part between the vertebral bodies followed by a rotation of 90 ° .
  • the part used has a contact area with the two vertebrae too small to ensure stable positioning of the prosthesis, without risk that it tilts and causes adverse effects for the patient.
  • the subject of the present invention aims to avoid all or part of the disadvantages previously mentioned.
  • the subject of the present invention is, according to a first aspect, an intervertebral implant designed to be placed in an intervertebral space by pivoting by 90 ° and without prior separation of the vertebrae, comprising on the one hand two bearing faces. provided to cooperate with the lower or upper parts of the vertebral bodies and, secondly, two lateral faces substantially perpendicular to said bearing faces, this implant being remarkable in that:
  • a threaded orifice able to cooperate with a tool making it possible to rotate this implant by 90 °
  • gutters arranged in the direction of the length of said lateral faces, adapted to receive lamellae of said tool, for passing said implant from one to the other of said configurations.
  • Such an implant is easy to use and inexpensive since it can be made in one piece and does not require the use of additional external parts for its maintenance in the intervertebral space. Once in place, the implant has a high stability due to the spacing of its side faces, under the effect of its elasticity alone.
  • Such an implant allows the operator to avoid, in addition to the steps of introduction of the implant into the body of the patient, final duty Iser implant placement intervention is by fixing steps different parts, either by liquid injection steps.
  • Such an implant therefore limits the medical complications due to the manipulation and introduction of various foreign bodies in the intervertebral space and close to the nerve structures.
  • the presence of the threaded orifice allows the implant to be brought into engagement with the tool by which this implant can be rotated, and the presence of the gutters allows the passage of the lamellae of this tool, which prevent the spacing of the two support faces of each other until the pivoting has been achieved.
  • said bearing faces comprise grooves: this allows the implant to be retained by friction between the two vertebral bodies; said lateral faces are smooth, outside the zone of said gutters: this makes it possible to slide the implant easily between the two adjacent vertebral bodies before pivoting it;
  • said lateral faces are associated with elastic branches interconnected by a bridge;
  • said implant comprises gutters arranged along the length of said lateral faces, and a threaded orifice pierced in said bridge; -
  • the side faces are adapted to pass in the conformation apart by pivoting said elastic branches about an axis passing through said bridge;
  • the lateral faces have substantially an isosceles trapezium shape whose shorter side belongs to said bridge: such a morphology of the implant is particularly adapted to the shape of the intervertebral space in certain areas of the implant. rachis;
  • the relative inclination between said bearing faces is typically 10-25 °: such an angular deflection between the two-by-side non-parallel faces of the isosceles trapezium, best respects the geometry intervertebral spaces in the lumbar area of the spine.
  • the subject of the present invention is a tool for placing an intervertebral implant as described above, remarkable in that it comprises: a handle; - A rod slidable in said sleeve comprising a smooth portion and at its end, a screw thread adapted to cooperate with said threaded hole;
  • FIGS. 1a, 1b, 1c and 1d respectively represent a perspective view, a side view on the front face, a side view on a lateral face and a top view of an implant in a configuration of FIG. service according to the present invention, intended to be placed preferably posteriorly in the spine;
  • FIGS. 2 to 6 show a perspective view of the implant illustrated in FIGS. 1a, 1b, 1c and 1d with its positioning tool, during the various stages of positioning the tool on the implant;
  • FIGS. 7 and 8 show a side view of two adjacent vertebrae during the steps of placing an implant as illustrated in FIGS. 1a, 1b, 1c and 1d in space intervertebral;
  • FIGS. 9 to 12 show a view from above of one or two implants, as illustrated in FIGS. 1a, 1b, 1c and 1d, during the steps of placement in an intervertebral space, the Figures 8 and 9 showing two different views of the implant just after its 90 ° pivot in the spine;
  • FIG. 1 3a, 1 3b on the one hand, and 14a, 14b on the other hand represent, in perspective and in plan view, a view of another embodiment of an implant according to the present invention, provided to be preferably installed laterally in the spine, respectively in setup configuration and in service configuration;
  • FIG. 15 represents a view from above of the implant illustrated in FIGS. 14a and 14b in a service configuration in an intervertebral space.
  • the intervertebral implant 1 shown in Figures 1a, 1b, 1c and 1d is designed to be implanted preferably between lumbar vertebrae of a patient, posteriorly.
  • This implant 1 comprises side faces 3a and 3b respectively belonging to two branches 5a and 5b. These lateral faces have substantially the shape of an isosceles trapezium whose sides of same lengths are adjacent to the bearing faces 7a and 7b ridged by grooves 8.
  • the implant 1 also comprises gutters 15a and 15b formed on the lateral faces 3a and 3b; outside these gutters, the side faces 3a and 3b are preferably smooth.
  • the bridge 13 is perforated by a threaded orifice 17.
  • the branches 5a and 5b are of different lengths and provided with extensions 18a and 18b which close the space between these branches.
  • the connection zones between the lateral faces 3a and 3b and the bearing faces 7a and 7b are preferably rounded.
  • a positioning tool 19 Prior to the placement of the implant 1 in the patient's spine, the operator positions a positioning tool 19 on the implant 1.
  • a positioning tool is shown in particular in Figures 2 to 6: it comprises a handle 21 in which a rod 23, comprising a smooth end 25, is mounted in sliding pivot relation; this rod 23 ends with a screw thread 27, visible in FIG.
  • the handle 21 is also equipped with two strips 29a and 29b placed on either side of the rod 23. At its opposite end (not shown) to that which carries the thread 27, the rod 23 leaves the handle 21 and is provided with a gripping means for the rotation and / or in translation of this rod relative to this handle.
  • FIGS. 2 and 3 show the screwing, by the operator, of the screw thread 27 in the threaded orifice 17 of the implant 1.
  • the operator slides the handle 21 on the rod 23, as shown in Figures 4 and 5.
  • applying a slight pressure on the handle 21 while firmly holding the implant 1 by means of the rod 23 it inserts the entirety of the smooth part 25 of the rod 23 in the handle 21, so that the lamellae 29a and 29b slide entirely into the grooves 15a and 15b of the implant 1 and bring the branches 5a and 5b closer together. one of the other.
  • Such configuration change of the implant 1 is illustrated by the transition from Figure 5 to Figure 6.
  • the branches 5a and 5b are in such a position close to each other, the implant 1 is said in "setup configuration".
  • FIGS. 7 to 12 are introduced posteriorly at the level of the spine of a patient, as illustrated in FIGS. 7 to 12.
  • the operator introduces the implant 1 into its placing configuration, so that the lateral faces 3a and 3b are placed in position. vis-à-vis the lower vertebrae 31a and upper 31b (see Figure 7).
  • This sliding introduction of the implant between the two adjacent vertebral bodies does not require any over-distraction, that is to say, no spacing beyond the normal relative position of these two bodies, because on the one hand that the two branches 5a and 5b are held against each other by the two strips 29a and 29b of the tool 19, and secondly because these strips, positioned in the gutters 15a, 15b, do not exceed the volume of the implant.
  • a second step as illustrated in FIGS. 8 and 9, the operator rotates the handle 21 so as to print a rotation of 90 ° to the implant, so that the bearing faces 7a and 7b come into view. respectively to the vertebrae 31a and 31b.
  • the operator pulls on the handle 21 while still holding the rod 23 stationary, as shown in FIG. 10, which has the effect of extracting the lamellae 29a and 29b from the gutters 15a. and 15b and causes the spontaneous spacing of the branches 5a and 5b relative to each other, simply because of the elasticity of the material forming the implant.
  • the implant 1 in such a configuration is then in "service configuration".
  • the area of contact between the implant and the vertebrae 31b and 31b is therefore larger and allows better stability of the implant 1 in the intervertebral space 33.
  • the transition from the set-up configuration to the service configuration takes place under the sole elasticity of the material forming the implant 1, which material may be metallic (titanium for example), polymer (PEEK - "poly-ether- etherketone "for example), composite or shape memory.
  • the operator unscrews the thread 27 of the threaded orifice 17 by rotating the rod 23 in the handle 21 and leaves the implant 1 in position in the patient's spine in its service configuration, as shown in FIG. Figure 11. It is also possible to introduce two implants 1 in the spine, as shown in Figure 12.
  • the implant 1 is particularly adapted to secure the vertebrae 31a and 31b in the lumbar region of the spine, because the geometry of the lateral faces 3a and 3b of the implant inserted posteriorly, as shown in FIGS. 7 and 8, allows to respect the curvature of the vertebral column at this level.
  • the relative inclination between the bearing faces 7a and 7b is between 10 and 25 °.
  • the implant can be substantially in the form of two implants 1 in accordance with the foregoing, put end-to-end.
  • Such an implant 41 comprises lateral faces 43a and 43b each belonging respectively to two pairs of branches 45a, 45b and 45c, 45d of beveled shape, arranged symmetrically with respect to a bridge 46 interconnecting these pairs of branches.
  • Such an implant 41 can be obtained by the combination of two implants 1 each conforming to the embodiment described above, assembled by their bridges 13 by means of a threaded male part cooperating by screwing with the orifices 17 of the bridges 13. ; this implant 41 may also be formed of a single block.
  • the bridge 46 may not be centered in the middle of the pairs of branches 45a, 45b and 45c, 45d.
  • the upper and lower surfaces of the branches 45a, 45b, 45c, 45d form two bearing faces 47a and 47b each comprising a set of grooves 48.
  • the branches 45a and 45b are provided with extensions 58a and 58b, and the branches 45c 45d are provided with extensions 58c and 58d comprising openings 59a and 59b intended to allow the introduction of the rod 23 of the tool 19 between these branches.
  • the implant 1 also comprises gutters dug along the entire length of the lateral faces 43a and 43b, such as the gutter 65, visible in FIGS. 13a and 14a, formed in the lateral face 43a.
  • the implant 41 is preferably suitable for placement and positioning in the spine in an intervertebral space 42 of the thoracic region, and in particular by the lateral route, as shown in FIG.
  • the present invention is not limited to the embodiments described and shown, provided as simple examples; and in particular concerning the mode of introduction of the implants according to the invention, the person skilled in the art knows how to modify the morphology of the implant to facilitate its insertion by the anterior approach into the intervertebral space.
