Definitions

  • the invention relates to a medical implant for closing vessel openings and / or shunt connections between body vessels, which can be positioned minimally invasively in a contracted state and converted into an expanded state by having two spaced-apart, enlarged diameter sealing elements that the Seal the vessel opening or shunt connection on both sides, wherein the implant is made of a tubular body by introducing wall openings at least in the region of expanded in the expanded state having a sealing element.
  • occluders implants are already known.
  • EP 0 808 138 B1 an occluder made of a metal mesh of interwoven metal strands.
  • the occluder is placed by means of a catheter at the desired location in the vascular system of a patient and brought into the expanded state.
  • vessels of a patient can be closed, for example septal defects, such as defects of the atrial septum.
  • a persistent duct (PDA) or a ventricular septal defect can also be treated with occluders.
  • Another similar occlusion instrument is from the DE 10 2005 053 958 Al known.
  • This occlusion instrument also consists of a metal mesh with two umbrella-like sealing elements, wherein in the implanted state one of the sealing elements rests on one side of the vessel opening and the other sealing element on the opposite side of the vessel opening on the vessel wall and to be closed Vessel opening seals.
  • the US 2008/0249562 A1 discloses the subject matter of the preamble of claim 1.
  • the known occluders have the disadvantage that their production of thin metal wires or polymer fibers is relatively expensive.
  • the present invention is therefore based on the object to provide a occlusion implant that is easy to manufacture and to handle.
  • a medical implant for closing vascular openings and / or shunt connections between body vessels, which can be positioned minimally invasively in a contracted state and converted into an expanded state by having two spaced-apart, diameter-expanded sealing elements, with which the vessel opening or shunt connection can be sealed on both sides, wherein the implant is made of a tubular body by introducing wall openings at least in the region of expanded in the expanded state having a sealing element and wherein in the expanded state, the webs of each sealing element in Substantially perpendicular to the longitudinal axis of the implant and form radial loops in the region of each sealing element, which are open to the longitudinal axis of the implant, wherein the loops of each sealing element in each case substantially in one plane lie and the loops of the two sealing elements in the central region are connected to each other via substantially straight and mutually parallel webs.
  • the webs running essentially at right angles to the longitudinal axis of the implant in the region of the sealing elements make it possible to produce the implant with a very short length extension.
  • a further length reduction results from the fact that the webs of the sealing elements form radial loops, which in each sealing element for essentially lying in one plane. This feature is also essential for a short design of the implant.
  • the arrangement of the webs in open to the longitudinal axis of the implant loops leads to a umbrella-shaped or flower-like configuration of the sealing elements, which can optionally be easily covered with a membrane.
  • the loops allow a certain spring effect, which is why the implant can very easily adapt to the opening to be closed, for example in the septum of the heart. Even irregular openings are thus easy to close.
  • the substantially straight course of the webs in the central region which connect the loops of the two screens, allows an optimal adaptation to the thickness of the septum.
  • the straight and parallel course of the webs in the central region, which connect the loops of the two sealing elements together, also allows the variable production of the implants, i. the individual adaptation to the physiological conditions of the respective patient.
  • the implants according to the invention are thus not made of fabrics or nets which have previously been produced in relatively complicated processes from thin threads or wires.
  • the implant is made of a solid tubular body, which is provided with wall openings at least at the locations which form the sealing elements in the expanded state.
  • the wall openings can be preferably introduced by laser cutting in the tubular body. This method is highly accurate and relatively inexpensive.
  • the mutual distance of the wall openings can also be chosen such that the intervening wall webs of the tubular body are thicker than the threads or strands of the known occluders. As a result, implants according to the invention can be produced with a higher dimensional stability than the known occluders.
  • the wall openings are slot-shaped. This shape of the wall openings allows easy deformability of the tubular body, so that in the expanded state having an enlarged diameter sealing elements are easy to produce. Further advantages are obtained if a plurality of mutually parallel wall openings are provided. This measure also facilitates the deformability of the implant.
  • the mutual distance of the wall openings and their distribution over the circumference of the tubular body can be selected according to the desired properties of the implant.
  • the configuration of the wall openings determines the properties of the sealing elements.
  • the wall openings may extend substantially parallel to the longitudinal axis of the implant. As a result, they are easy to introduce into the tubular body and also ensure easy transfer of the implant in the expanded state. However, the wall openings may also extend at an acute angle to the longitudinal axis of the implant or extend helically over the circumference of the implant.
  • the implant is made of a shape memory alloy, in particular nitinol.
  • Nitinol also has resilient properties, so that the implant can be brought into the contracted state for insertion into the body of the patient and, for example, introduced by means of a catheter. At the target site, the catheter may be withdrawn, whereby the implant assumes the stored expanded form by being able to seal the vessel opening or shunt connection.
  • a shape memory polymer can also be used to produce the implant according to the invention.
  • Polymers also have excellent shape memory properties and are even easier to deform than nitinol.
  • liquid-tight membranes can be arranged in or on the sealing elements. These membranes are positioned with the help of the sealing elements on both sides of the vessel openings and held in this position.
  • the membranes can be made of different materials. In order to achieve high body compatibility, ePTFE is preferred as the material for the membranes.
  • ePTFE is preferred as the material for the membranes.
  • Such membranes can be sewn firmly to, for example, openings in the webs in the region of the loops and / or in the central region.
  • the slot-shaped wall openings of the central area merge into the slot-shaped wall openings in the area of the sealing elements.
  • the wall openings are thus made as a continuous, extending to the end portions of the tube slots.
  • the omission of a non-slotted tube section between the central region and the sealing element regions also leads to an easier deformability of the central region. It can thus be adapted more easily to different sizes and shapes of vessel openings or shunt connections. Often these have no round, but an oval or slit-shaped cross-section.
  • a single set of implants can accommodate a whole range of opening sizes and shapes.
  • the previously required storage of implants for different diameter sizes of the openings to be closed in a 1-millimeter grid can thus be significantly reduced.
  • An implant can now be used to supply diameters of openings of several millimeters.
  • the wall openings in the central region can extend in a spiral parallel to the longitudinal axis of the implant and in the region of the two sealing elements. After being transferred to the expanded state, the webs remaining between the wall openings in the sealing element regions form loops extending radially outwards.
  • the central area can also be upset, making it a bulbous shape. By twisting about the longitudinal axis, it can also be shortened in length become. The circumference of the central area adjusts to the circumference of the vessel opening or shunt connection after positioning of the implant at the site to be occluded.
  • the implant according to the invention may preferably be positionable in the body with the aid of a catheter or a helix.
  • the implant according to the invention can be guided and positioned through a guide wire.
  • the catheter or coil the implant can be moved with high accuracy through the patient's vascular system and positioned at the desired site of use.
  • the catheter or the helix can be connected to one of the ends of the implant by means of a screw connection or by means of a bayonet closure.
  • Fig. 1 shows a tubular body 20 for producing an implant 30, which has over the two sealing portions 31 and 32 and an intermediate central region 33 extending wall openings 38, which are also relatively wide, so that only a few wall webs 36 remain therebetween.
  • the wall openings 38 are thus made as a continuous, up to the end portions 34 and 35 of the tube extending slots.
  • the omission of non-slotted pipe sections between the central region 33 and the sealing element regions 31 and 32 leads to a slight deformability of the central region 33. It can thus be easily adapted to different sizes and shapes of vessel openings or shunt connections.
  • Fig. 2 showing the implant 30 in the expanded state.
  • the wall webs 36 form in the region of the sealing elements 31 and 32 radially outwardly projecting loops 37.
  • the wall webs 36 are parallel to each other, but have been pressed radially outwardly so that they compared to the pipe ends 34, 35 a significantly larger Include diameter between them, in the case shown about five times the diameter.
  • the radially outwardly projecting loops 37 of the sealing elements 31 and 32 extend approximately parallel and in the same direction.
  • the loops of a sealing element lie substantially in one plane.
  • Loops 37 of different sealing elements are connected to each other in the central region 33 by straight webs 36.
  • the same direction course of the loops of the respective sealing elements and the connection via webs 36 in the central region 33 results in a certain spring action, which causes a simple adaptation of the sealing elements and the implant to the opening to be closed.
  • Fig. 1 is still the opening of individual wall webs 36 in the region of the sealing elements 31 and 32 and shown in the central region 33, the sewing a liquid-tight membrane to the sealing elements enable.
  • Fig. 3 is the front view of the implant 30 is shown, from which the arrangement of a dot-dash line membrane 40 in the sealing region 31 is clear.
  • the membrane 40 made of a liquid-tight material can be stretched between the two halves of the radial loops 37 and fastened, for example, to the outer arcs of the loops 37. It is thus able to safely close a vessel opening whose diameter corresponds approximately to the diameter of the circle 41.
  • On the circle 41 which generally has approximately 2 to 15 times and in particular 4 to 10 times the diameter of the tube 20, are the portions of the wall webs 36 ( Fig. 2 ) in the central region 33 of the implant 30, which protrudes through the vessel opening after insertion of the implant 30.
  • a membrane 40 is arranged, so that the vessel opening is sealed from both sides.
  • the implants 10 'and 30 can also be used in vessel openings or shunt connections, which have no circular, but a more oval or slit-shaped cross-section. By a corresponding deformation of the wall webs 36 in the central region 33, these can also be brought into contact with the edge of the opening in the case of such vessel openings, thereby centering the implant 30.
  • the implants may be made, for example, from Nitinol or other metal alloy with shape memory or even from a shape memory polymer.
  • the membranes 40 in the region of the sealing elements may preferably be made of ePTFE, since this material is characterized by a very good body compatibility.
  • the implants 30 shown in the figures were produced by introducing slit-shaped wall openings 38 which run parallel to the longitudinal axis of the tubular body 20. However, this is not mandatory.
  • the openings 38 can with the longitudinal axis of the tubular Body 20 also form an angle, whereby the webs 36 form loops whose start and end points are offset in the tangential direction to each other. Spirally extending wall openings or other courses are possible.
