Description
- La présente invention concerne un dispositif de conditionnement et d'administration per-muqueuse de principes actifs.
- Il existe des procédés de diffusion de principes actifs par perméation sublinguale et/ou para-gingivale qui offrent de nombreux avantages.
- En effet, ces procédés de diffusion permettent d'obtenir des actions beaucoup plus rapides, avec des dosages beaucoup plus faibles de principes actifs et de façon plus ciblée en éliminant de très nombreux effets secondaires. Ces effets secondaires sont notamment engendrés par le fait que les médicaments à administration par voie orale sont métabolisées par les différents organes digestifs avant de diffuser vers les cibles visées par les principes actifs.
- Si cette voie d'administration per-muqueuse est particulièrement attractive, elle reste néanmoins liée à des contraintes auxquelles il convient de pallier pour en permettre une mise en oeuvre aisée par les patients.
- Les principes actifs mis en solution, notamment mis en solution dans des solutions alcooliques, doivent être conditionnés en unidoses, ledit conditionnement devant aussi permettre une administration per-muqueuse soit sub-linguale soit para-gingivale.
- Une première contrainte est le volume extrêmement réduit de la dose à administrer, de l'ordre de 0,25 ml à 2 ml pour donner un ordre de grandeur.
- La solution est également fragile au regard de la lumière et/ou de l'air avec des phénomènes d'oxydation et sensible aux phénomènes d'évaporation. Cette solution peut aussi faire l'objet de phénomènes d'adsorption ou d'ionisation au contact des parois du contenant, d'où la nécessité de recourir à des matériaux inertes et particulièrement protecteurs.
- Or la solution notamment dans le cas de solutions alcooliques doit conserver son degré alcoolique et une stabilité de dissolution constante du ou des principes actifs conditionnés, paramètres qui sont adaptés pour une perméation optimisée. De plus, le degré alcoolique est ajusté pour conserver les principes actifs eux-mêmes.
- Il importe de conserver le degré alcoolique initialement prévu.
- En outre, pour certaines substances pharmacologiques instables, leur mise en dissolution ne doit être pratiquée qu'extemporanément, au moment même de l'administration, faute de quoi ils seraient altérés, donc inactifs.
- Une contrainte omniprésente pour un tel conditionnement unidose est le coût de fabrication qui doit être extrêmement réduit compte tenu du nombre de conditionnements à réaliser et du fait d'un usage unique par principe même. Son recyclage après usage doit également être favorisé. Néanmoins, le conditionnement doit conserver les impératifs de sécurité, d'asepsie, en plus de la praticité et de l'ergonomie recherchées, pour garantir une mise à disposition aisée en automédication. Divers dispositifs de conditionnement sont connus de
US 3 802 604 ,US 4 648 532 ouEP 0293290 par exemple. - C'est pourquoi, selon la revendication 1, la présente invention vise un contenant pour la préparation et/ ou l'administration d'un très faible volume de principes actifs sous forme liquide comprenant une tête apte à prendre une première position de conservation dans laquelle ladite tête est en position distale par rapport au contenant et une seconde position d'administration dans laquelle ladite tête est en position proximale par rapport au contenant, cette tête comportant une canule, avec au moins un canal d'écoulement, la longueur étant adaptée pour que l'extrémité libre du canal d'écoulement puisse permettre une délivrance par perméation sub-linguale et/ou para-gingivale, le contenant étant tenu à la main.
- Par très faible volume on entend un volume à administrer compris entre 0,1 et 5 ml.
- Ce dispositif est particulièrement adapté à la préparation et l'administration de principes actifs et/ou de substances antigéniques à des fins de traitement pharmaceutique, homéopathique et/ou de désensibilisation allergique.
- Le conditionnement selon la présente invention est maintenant décrit en détail suivant un mode de réalisation particulier, non limitatif, en regard des dessins annexés, dessins sur lesquels les différentes figures représentent :

figure 1 , on a représenté un contenant 10 qui peut être en toute matière évitant l'évaporation à travers la paroi et apte à éviter l'action de la lumière sur le contenu. - Un tel contenant peut être réalisé en tout matériau adapté aux substances conditionnées et aux applications visées, tel que du verre, du métal, du plasmique ou une association de ces matériaux.
- Le contenant est avantageusement en verre, de préférence en verre épais, opacifié, de qualité pharmaceutique, avec une haute résistance mécanique, de section carrée, ovale, rectangulaire, triangulaire ou ronde.
- Afin de rendre les dessins explicites, la proportion des échelles n'est volontairement pas respectée.
- Ce contenant 10 comprend une tête 12, solidarisée de façon mobile au moins en translation par rapport audit contenant.
- Cette tête est apte à prendre une première position P1 de conservation dans laquelle ladite tête 12 est en position distale par rapport au contenant 10 et une seconde position P2 d'administration dans laquelle ladite tête 12 est en position proximale par rapport au contenant 10.
- La tête 12 comporte une canule 14, avec au moins un canal 16 d'écoulement.
- La longueur est adaptée pour que l'extrémité libre du canal 16 d'écoulement puisse venir se loger notamment sous la langue de l'utilisateur ou en para-gingival, le contenant étant tenu à la main.
- La canule 14 comprend en plus du canal 16 d'écoulement, au moins une prise d'air 18, par exemple un second canal 20. Ce second canal 20 peut être coaxial et externe ou juxtaposé au premier canal 16 par exemple. La prise d'air peut aussi s'effectuer par le même premier canal, suivant son diamètre, la solution liquide étant une solution à base d'alcool, donc de très faible viscosité.
- Cette canule 14 comporte à son extrémité libre un couvre embout buccal 22. Ce couvre embout buccal 22 est un bouchon scellé de sécurité par exemple scellé et ruptable par rotation pour le meilleur mode, ou par traction ou par découpe.
- La canule 14 est solidaire de la tête 12 et avantageusement venue de fabrication avec ladite tête. Le matériau peut être une matière polymère adaptée.
- La canule 14 comprend en pied des moyens 24 de perforation.
- Le contenant 10 et la tête 12 sont équipés de moyens 26 de déplacement en translation de ladite tête 12 de la position distale à la position proximale.
- Le contenant est aussi muni de moyens 28 de verrouillage de sécurité de façon à interdire tout déplacement involontaire, en translation, de la tête 12 par rapport au contenant 10.
- Dans le mode de réalisation préféré, les moyens 26 de déplacement en translation comprennent un ensemble pas de vis 30 porté par le contenant 10, plus particulièrement par le col 32 de ce contenant et un filetage 34 de profil conjugué du pas de vis du contenant porté par la tête 12 de façon à coopérer par vissage.
- Les moyens 28 de verrouillage comprennent une bague 36 amovible, interposée entre la tête 12 en position distale et le contenant 10, plus particulièrement l'épaulement 38 de ce contenant lorsqu'il en est muni, ainsi que représenté sur le mode de réalisation des
figures 1 et2 . - Cette bague 36 a un profil en C qui vient se monter élastiquement sur le pas de vis 30 porté par le contenant 10, interdisant le mouvement en translation de la tête 12 par rapport à ce contenant 10.
- De façon complémentaire, l'ouverture du col 32 du contenant 10 est munie d'un opercule 40 étanche et apte à être perforé par les moyens 24 de perforation.
- De façon avantageuse, afin d'assurer une administration de l'intégralité de la composition médicamenteuse sous forme liquide, la tête 12 comprend des moyens 42 d'écoulement total. Ces moyens 42 d'écoulement comprennent, en partie interne, un fond conique, la canule 14 étant munie, en plus des moyens 24 de perforation, de trous 44 de passage, disposés immédiatement en amont du fond par rapport au sens d'écoulement, en liaison avec la canal 16 d'écoulement. Dans ce cas, les moyens 24 de perforation comprennent uniquement une pointe. Ainsi, le contenant 10 est empli de la composition médication sous forme liquide à administration per-muqueuse puis le contenant 10 est obturé de façon étanche par la pose de l'opercule 40 qui joue aussi le rôle de membrane d'étanchéité, surtout dans le cas d'une solution alcoolique.
- La bague 36 est disposée sur le col, autour du pas de vis 30, puis la tête 12 est rapportée par vissage jusqu'à venir en butée sur la bague 36.
- Le couvre embout buccal 22 est en place sur la canule 14, obturant le canal 16 d'écoulement et le canal 20 de la prise d'air 18.
- Dans cette position de conservation, le conditionnement n'est pas utilisable et peut être conservé, sans altération du contenu et sans risque d'accessibilité aux jeunes enfants.
- Lorsque l'utilisateur souhaite s'administrer ou administrer la composition médicamenteuse, il lui suffit de retirer la bague 36 par simple traction puis de visser la tête 12.
- Il se produit alors un déplacement en translation de ladite tête qui amène les moyens de perforation à déchirer l'opercule qui assurait le scellage, permettant au liquide de couler vers la canule lorsque le contenant est retourné.
- Le retrait de l'embout buccal 22 avant retournement, par exemple par rotation provoquant la désolidarisntion de cet embout, permet de délivrer la composition par l'extrémité de la canule 14 positionnée sous la langue ou entre la gencive et la langue ou encore entre la joue et la gencive, la longueur de la canule 14 étant adaptée à cet effet.
- Ainsi, la composition est dispensée exactement à l'endroit où l'efficacité est optimale, ceci pour la totalité du volume contenu.
- On note que la prise d'air assure une distribution rapide du volume, qui même faible doit être dispensé en un temps très court, de l'ordre de la seconde pour donner un ordre de grandeur.
- A cet effet, le diamètre interne du canal 16 d'écoulement doit être relativement important, de l'ordre de 2 millimètres pour les quantités mentionnées ci-avant, comprises entre 0,25 et 2 ml. Ce diamètre proportionnellement important peut aussi assurer la remontée d'air pour faciliter l'écoulement rapide.
- Dans le cas d'un recours à un contenant du type flacon en verre épais, il est possible de recourir à un agencement particulier.
- Soit le col porte un pas de vis 30 venu de fabrication avec le contenant, soit le col est lisse. Dans le cas d'un col lisse, les moyens de déplacement intègrent une virole qui est directement rapportée de façon fixe sur ledit col du contenant, par exemple par sertissage, ladite virole portant le pas de vis 30 nécessaire pour coopérer avec le filetage 34 de la tête 12.
- Dans ce cas, le contenant en verre est empli de la composition puis operculé. Ensuite, la virole est rapportée sur le col et sertie, puis la bague 36 est positionnée puis la tête 12 est mise en place à son tour.
- Pour l'utilisation, l'utilisateur agit exactement comme indiqué précédemment. Ainsi que cela vient d'être décrit, les dimensions du flacon on été maximisées pour permettre de faire apparaître les détails de constitution mais il faut prendre en compte le fait qu'un contenant de 0,25 à 2 ml est extrêmement petit et difficile à manipuler.
- Aussi la présente invention propose un perfectionnement au contenant qui consiste à adjoindre une palette 46 de préhension, disposée avantageusement en partie inférieure du corps du contenant 10.
- Cette palette 46 de préhension permet une bonne saisie en pince bidigitale, malgré la petitesse du contenant afin de permettre à l'utilisateur de manoeuvrer la tête 12 en rotation.
- De même, cette tête 12 peut comporter sur sa surface extérieure périphérique des moyens 48 de préhension comme des ailettes 50, représentées sur les dessins.
- L'utilisateur est ainsi prêt à manoeuvrer en exerçant un couple entre le contenant 10 et la tête 12.
- On note aussi que la palette 46 de préhension présente un autre avantage de manipulation après rotation de la tête par rapport au contenant et après retrait du couvre embout buccal, celui d'autoriser une manipulation aisée du contenant prêt à l'usage pour positionner la canule 14 à l'endroit désiré.
- En variante, la bague 36 peut être une ceinture plastique apte à être déchirée ou dessertie. Afin de faciliter ces opérations de retrait de la ceinture, une languette externe préhensible de traction peut être adjointe à cette ceinture. De même le remplissage avant pose de l'opercule peut être réalisé sous atmosphère de gaz inerte pour préserver le contenu.
- Selon l'invention, comme représenté sur la
figure 4 , le contenant 10 comprend au moins deux compartiments 52, chaque compartiment étant séparé par un opercule étanche 54. - Chaque compartiment contient au moins un principe actif, un excipient, une substance antigénique et/ou un solvant.
- Le dispositif comprend également des moyens 24 de perforation aptes à déchirer le ou les opercules 40 et/ou 54, disposés en pied de canule 14 de façon à pouvoir obtenir des mélanges des substances dans un ordre déterminé.
- Lorsque l'utilisateur souhaite s'administrer ou administrer la composition médicamenteuse, il retire la bague 36 par traction puis visse la tête 12. Il se produit alors un déplacement en translation de ladite tête qui amène les moyens de perforation à déchirer le ou les opercules 40 et/ou 54, permettant aux substances contenues dans chaque compartiment 52 de se mélanger pour obtenir un liquide qui coule vers la canule lorsque le contenant est retourné.
- La tête 12 contient également un principe actif, un excipient, une substance antigénique et/ou un solvant, au niveau de la canule 14. La tête 12 peut également présenter plusieurs compartiments séparés par un opercule 66. La substance contenue dans la tête 12 peut se présenter sous forme de poudre, de comprimé, de lyoc, de microcapsule liquidienne, etc. Lorsque la tête se déplace en translation, les moyens de perforation déchirent le ou les opercules 40 et/ou 54 et/ou 66 permettant aux substances contenues dans la tête 12, notamment dans la canule 14, et dans chaque compartiment 52 de se mélanger. De même si la substance contenue dans la tête 12 est encapsulée, les moyens de perforation déchirent la membrane encapsulant ladite substance avant de perforer le ou les opercules 40 et/ou 54 et/ou 66.
- La substance contenue dans la tête 12 peut également se présenter sous forme d'un liquide contenu dans la canule 14 de la tête 12, comme représenté sur la
figure 5 . La canule 14 comporte alors une partie haute 56 renfermant le liquide scellée par un opercule étanche 62, et une partie basse 58, séparées par un pas de vis 60. Une première rotation de la tête 12 sur le pas de vis 30 amène la partie 58 à déchirer l'opercule 40 qui assurait le scellage du contenant 10. Une rotation supplémentaire entraîne la butée de la partie 58 sur le pas de vis 30. La canule 14 tourne alors autour du second pas de vis 60, ce qui permet de déchirer l'opercule 62 qui assurait le scellage de la canule. Le liquide de la tête 12 est ainsi libéré et les substances qui étaient séparées peuvent se mélanger. - Ces formes de contenant selon l'invention avec au moins deux compartiments 52 et/ou une tête 12 contenant une ou plusieurs substances, sont particulièrement adaptées aux principes actifs et substances instables en solution. C'est le cas par exemple des principes actifs lipophiles, qui, bien que parfaitement solubles dans des solutions hydroalcooliques, présentent des évolutions instables avec des risques de dénaturation et de formation de composés indésirables.
- Le contenant selon l'invention permet de protéger le principe actif, excipient et/ou substance antigénique sous forme de poudre, de lyoc, de comprimé, de microcapsule liquidienne, etc., et de réaliser une dissolution extemporanée dans un solvant juste avant l'administration. Il est ainsi possible pour certains composants complexes et altérables entre eux, de constituer un dispositif à plusieurs compartiments séparés permettant un assemblage successifs de ces différentes substances peu compatibles juste avant leur administration, ceci dans un ordre déterminé.
- Le contenant selon l'invention peut être utilisé pour la préparation et/ou l'administration de principes actifs et/ou de substances antigéniques à des fins de traitement pharmaceutique, homéopathique et/ou de désensibilisation allergique, notamment par voie trans-muqueuse.
Claims (9)
- Dispositif unidose de conditionnement, de préparation et d'administration per-muqueuse buccale d'un volume d'une solution alcoolique contenant au moins un principe actif lipophile comprenant un contenant (10), une tête (12) apte à prendre une première position P1 de conservation dans laquelle ladite tête (12) est en position distale par rapport au contenant (10) et une seconde position P2 d'administration dans laquelle ladite tête (12) est en position proximale par rapport au contenant (10), cette tête (12) comportant une canule (14), avec au moins un canal (16) d'écoulement, la longueur étant adaptée pour que l'extrémité libre du canal (16) d'écoulement puisse permettre une délivrance par perméation sub-linguale et/ou para-gingivale, le contenant étant tenu à la main, ledit dispositif comprenant au moins deux compartiments (52 ; 56) séparés par un opercule (40 ; 54 ; 62) étanche, chaque compartiment (52 ; 56) contenant au moins un principe actif et/ou un excipient et/ou une substance antigénique et/ou un solvant, ledit dispositif comprenant des moyens (24) de perforation du ou des opercule(s) (40 ; 54 ; 62), lesdits moyens (24) de perforation étant disposés en pied de canule (14) de façon à pouvoir obtenir des mélanges des substances contenues dans lesdits compartiments dans un ordre déterminé, le volume dudit contenant étant compris entre 0,25 ml et 2 ml, caractérisé en ce que la canule (14) contient avant actionnement un principe actif, une substance antigénique, un excipient et/ou un solvant.
- Dispositif selon la revendication 1, comprenant des moyens (26) de déplacement en translation de la tête (12), ces moyens comportant un ensemble pas de vis (30) porté par ledit contenant 10, et un filetage (34) de profil conjugué du pas de vis du contenant porté par la tête (12) de façon à coopérer par vissage.
- Dispositif selon la revendication 1 ou 2, comprenant des moyens (28) de verrouillage de sécurité de façon à interdire tout déplacement involontaire, en translation, de la tête (12) par rapport au contenant (10).
- Dispositif selon la revendication 3, dans lequel les moyens (28) de verrouillage comprennent une bague (36) amovible, interposée entre la tête (12) en position distale et le contenant (10).
- Dispositif selon l'une quelconque des revendications précédentes, dans lequel la canule (14) est munie d'un couvre embout buccal (22) de sécurité, ruptable.
- Dispositif selon l'une quelconque des revendications précédentes, comprenant un col (32), l'ouverture de ce col étant munie d'un opercule (40) étanche.
- Dispositif selon l'une quelconque des revendications précédentes, comprenant une palette (46) de préhension, disposée en partie inférieure du corps du contenant (10) de façon à permettre une bonne saisie en pince bidigitale.
- Dispositif selon l'une quelconque des revendications précédentes, dans lequel la tête (12) comporte sur sa surface extérieure périphérique des moyens (48) de préhension.
- Dispositif selon l'une quelconque des revendications précédentes, dans lequel ledit contenant est adapté pour l'administration de principes actifs et/ou substances antigéniques à des fins de traitement pharmaceutique, homéopathique et/ou de désensibilisation allergique.
