Definitions

  • prostheses which consist of two cover plates each to be connected to an adjacent vertebral body and a prosthesis core, the prosthesis core interacting with one or both cover plates via spherical, complementary articular surfaces (EP-B 298 233).
  • the cover plates have connection surfaces, by means of which they are connected to the adjacent vertebral bodies. It is desirable that the joint center of the prostheses is arranged in such a way that the movement conditions made possible by the prosthesis are as close as possible to the natural ones and a uniformly distributed force transmission can take place between the vertebral bodies and the prosthesis.
  • the desired spatial relationship to the vertebral bodies is only achieved if the anatomical conditions correspond to the standard conditions required in the construction of the prosthesis.
  • the prosthesis in question or a cover plate of this prosthesis must be used in a manner different from the normal spatial relationship with the vertebral body, which is difficult and risky.
  • the object of the invention is therefore to make the insertion of the prosthesis easier and safer for these cases. This is achieved through the features of claim 1 and preferably the features of the subclaims.
  • a system of intervertebral disc prostheses is required, which includes standard and correction prostheses.
  • the standard prosthesis Sen are the same in every size class. There are usually several size classes; however, this is not absolutely necessary.
  • the prostheses consist of a prosthesis core and at least one cover plate.
  • the prosthesis core interacts with at least one cover plate via articular surfaces.
  • the prosthesis core preferably has two articular surfaces on opposite sides, via which it interacts with complementary articular surfaces of two cover plates. However, it is also possible for one of the cover plates to interact with the prosthesis core via holding surfaces in a non-rigid manner.
  • the cover surfaces have a connection surface for connection to a vertebral body.
  • the correction prostheses have at least one correction cover plate, the core mating surface of which is offset in the ventro-dorsal direction in comparison with the standard prostheses with respect to the connection surface.
  • the prosthesis is offset from the vertebral body on whose side the correction cover plate is inserted, compared to the standard prostheses in the ventro-dorsal direction.
  • the displacement can be ventral or dorsal, depending on the type and direction of installation of the cover plate. If the connection surfaces of the correction cover plates are symmetrical with respect to their medio-lateral center line, they can be used optionally with a center offset to the ventral or dorsal. Since an offset to the dorsal is predominantly an important embodiment of the correction plate according to the invention is characterized in that the Joint center is displaced dorsally, provided that the dorsal side can be seen on the cover plate.
  • the intervertebral disc prosthesis tends to undesired misalignment due to strong spinal curvature or due to high ventro-dorsal forces acting between the affected vertebrae.
  • the cover plates are able to translationally shift a little against each other under lateral relative forces, with the prosthesis core being skewed.
  • the application of the invention is particularly advantageous because the correction plates enable the articulation center of one cover plate to be arranged relatively displaced relative to the articulation center of the other cover plate, thereby compensating for the offset mentioned.
  • the diameter of the articular surfaces is chosen to be as large as possible within the limits of the cover plate
  • the joint surface formed on the cover surface has only a slightly smaller ventro-dorsal extension than the cover plate itself.
  • the offset of the joint surface from the connection surface can only take place on the condition that a smaller diameter is selected for the joint surface. If the radius of curvature of the articular surface is left unchanged, the diameter reduction reduces the depth of the articulated surface provided in the cover plate and thus its ability to transmit lateral forces (parallel to the plane of the cover plate). This may be acceptable in some cases, so this execution the invention is not excluded.
  • the diameter and radius of curvature of the articulation surface of the correction cover plate are selected in accordance with the articulation surfaces which belong to a different, smaller size class.
  • the smaller size class not only is the diameter of the articular surfaces smaller, but also the radius of curvature.
  • the depth of these smaller articular surfaces is therefore greater than the depth of an articular surface of the larger size class in which only the diameter, but not the radius of curvature, has been reduced.
  • the correction cover plate is used together with prosthesis parts that belong to the same size class.
  • prosthesis parts belonging to the lower size class based on the model of which the articular surface of the correction cover plate was chosen, can be used instead.
  • the prosthesis core is expediently a standard prosthesis core in all cases.
  • the second cover plate belonging to the prosthesis is also expediently a standard cover plate.
  • the second cover plate can also be a correction cover plate, which is used, however, rotated by 180 ° with respect to the first, so that the offset amounts of both plates add up.
  • corrective cover plates are used on both sides in the same orientation.
  • the offset arrangement of the core mating surface results in a distance between the mating surface on one side of the core mating surface and the edge of the plate. If this part of the area is not required to provide the largest possible connection area, the plate can be shortened on this side.
  • the connection surface is then as wide in the mediolateral direction as the standard cover plates of the same size class, but their dimension in the ventro-dorsal direction is smaller and can correspond to the smaller size class. This can result in adaptation advantages in cases where the vertebral bodies are wider than normal in the medio-lateral direction and narrower in the ventro-dorsal direction.

All dimensions of the first size class (FIG. 1 and 2) are larger than those of the second size class (FIG. 3 and 4). In addition to the two size classes shown, there may be other size classes within the system.
  • the components of the prosthesis can consist of materials which have proven themselves for endoprostheses, for example metal, ceramic, polyethylene, the cover plates 1, 2 preferably being made of rigid material (for example metal) and the prosthesis core made of polyethylene.
  • the cover plates 1, 2 have a connection surface 7 which is intended for connection to the end surface of a vertebral body. It can be equipped with means, not shown, for a firm connection to the bone, for example teeth. It is more extensive in the ML direction, which is indicated by line 10 in FIG. 2, than in the VD direction 11. In the standard prostheses, the diameter 12 of the articular surface 4 is chosen to be as large as the dimension of the cover plate in the direction 11 permits.
  • such prostheses are suitable for transmitting the forces running in the direction of the spine and, to a certain extent, also transverse forces from vertebra to vertebra and at the same time allowing pivoting movements. If they are used between vertebrae, between which strong transverse forces act or a strong change in direction takes place, as is often the case, for example, between the last lumbar vertebra and the sacrum, the cover plates and the vertebrae may shift. This can be compensated for by using a correction prosthesis 14. For example, it is constructed as shown in FIG. 5 or in FIG.
