Abstract
Eine Anordnung zur Bestimmung des effektiven pulmonalen Blutdurchflusses (PBF) mittels partieller CO2-Rückatmung enhält einen Endotrachealtubus dessen Leitung von Patienten zum Beatmungsgerät zwischen einem steuerbaren Dreiwegeventil (4) und einem Y-Stück (5) in zwei Stränge aufgeteilt wird, wobei ein Strang einen grosseren Totraum (6) für die CO2-Rückatmung bildet. Für die Messung von CO2-Elimination und endexpiratorischen partiellen CO2-Druck ist ein CO2-sensor (3) und ein Beatmungsstromsensor (1) am Endotrachealtubus vorgesehen. Für die Berechnung des effektiven pulvonalen Blutdurchflusses dient ein Microprozessor/Kontroller (7), der auch zur Steuerung zur Umschaltung zwischen den beiden Strängen durch das Dreiwegeventil (4) vorgesehen ist.
Description
Anordnung zur Bestimmung des effektiven pulmonalen Blutdurchflusses
Die Erfindung betrifft eine Anordnung zur Bestimmung des effektiven pulmonalen Blutdurchflusses gemäß dem Oberbegriff des Anspruchs 1.
Aus der Druckschrift (Steinhart, C. M., Burch, K. D., Bruno, S., Parker, D. H.: Noninvasive determination of effective (nonshunted) pulmonary blood flow in normal and injured lungs, Crit. Care Med., 1989, Vol. 17, No. 4, S. 349-353 ist die Multiple- Inerte-Gas- Methode, mit Rückatmung von Helium, Acetylene und Kohlenmonoxide in Sauerstoff und Stickstoff aus einem Atembeutel bekannt. Nachteil dieser Messung ist der relative große Aufwand der Vorbereitung von Gasmischungen für die Rückatmung, der Bedarf für spezielle Meßgeräte, die die Konzentrationen von Gasen in der beatmeten Luft bzw. in dem Beatmungsstrom messen können, und die Notwendigkeit, daß eine Person den Atmembeutel an den Patienten anschließt und betreut. Damit kann die Messung des effektiven pulmonalen Blutdurchflusses nur von Spezialisten durchgeführt werden und dient fast ausschließlich Forschungszwecken.
Weiterhin bekannt sind nach der Druckschrift Inman, M D., Hughson, R.L. and Jones, N.L.: Comparison of cardiac Output during exercise by single-breath and CO2 rebreathing methods, J. Appl. Physiol., Vol. 58, pp. 1372-1377, 1985 die totale CO2-Rückatmungsmethode und die sogennante „single-breath" Methode. Nachteile dieser Methoden sind ausgeprägte Erhöhungen des CO2- Druckes im arteriellen Blut, da die CO2-Elimination unterbrochen wird und die Beeinträchtigung der Beatmung. Weitere Nachteile entsprechen denen der Multiple-Inerte- Gas-Methode und zwar: Vorbereitung von Gasmischungen für die Rückatmung mit bestimmten CO2-Konzentrationen, der Bedarf für spezielle Meßgeräte, die die Konzentrationen von Gasen in der beatmeten Luft bzw. den Beatmungsstrom messen können, und die Notwendigkeit, daß ein Spezialist den Atembeutel an den Patienten anschließt und betreut.
Bekannt ist auch eine partielle CO2-Rückatmungsmethode (nach Gedeon A., Forslund, L., Hedenstiema, G. und Romano, E.: A new method for noninvasive bedside determination of pulmonary blood flow, Med. & Biol. Eng. & Comput, 1980, Vol. 18, S. 41 1- 418) mittels Variation des Ateminutenvolumens. Nachteile dieser Technik ist die Variation des mittleren Atemwegdruckes und des Atemwegdruckes am Ende der Ausatmung. Infolge dieser Variation im Atemwegdruck ändert sich auch der pulmonale Blutdurchfluß und es werden sowohl die mechanische Stabilität der Lungen, als auch der Gasaustausch beeinträchtigt.
Aus der Druckschrift Capek, J. M. und Roy, R. J.: Noninvasive measurement of cardiac output using partial CO2 Rebreathing; IEEE Transactions on Biomedical Engineering, 1988, Vol. 35, No. 9, S. 653-661 ist die partielle CO -Rückatmungsmethode mittels Wechsel des Totraumes des Apparates bekannt, die mit Massenspectrometer und speziellen Beatmungsstromsensoren an den Endotrachealtubus durchgefühlt wird und das gesamte Herzzeitvolumen messen kann. Der Wechsel zwischen zwei verschiedenen Toträume erfolgt durch ein elektromagnetisches Ventil, das computergesteuert ist. Der partielle CO2- endexpiratorische Druck- und die Cθ2-EIimination wird für die Beatmung durch beide Toträume determiniert. Der partielle CO2-Druck am Ende der Ausatmung wird in die arterielle CO2-Konzentration umgerechnet, und das gesamte Herzzeitvolumen läßt sich aus der Division der CO2-EHminationsdifferenz und der arteriellen CO2- Konzentrationsdifferenz berechnen. Nachteil dieser Methode ist der apparative Aufwand für die Messunges der CO2-Konzentration in der beatmeten Luft (Massenspectrometer) und des Beatmungsstrom am Endotrachealtubus (Fleisch Pneumotacograph).
Die Aufgabe der vorliegenden Erfindung besteht nun darin, eine klinisch anwendbare Anordnung für die nichtinvasive Determination des effektiven pulmonalen Blutdurchflusses anzugeben, mit der nur der Anteil des Herzzeitvolumens, der zum Gasaustausch zur Verfügung
steht, ermittelt werden kann. Diese Anordnung soll apparativ nicht aufwendig sein, das Beatmungsmuster nicht wesentlich beeinflussen und automatisierbar sein.
Erfindungsgemäß wird die Aufgabe durch die im Anspruch 1 genannten Merkmale gelöst. Weiterbildungen und Ausgestaltungen sind Gegenstand der abhängigen
Unteransprüchen.
Vor allem durch die Wahl der Parameter ist es erstmals gelungen, den effektiven pulmonalen Blutdurchfluß während der Beatmung zu bestimmen, d. h. nur den Anteil des Herzeitvolumens, der zum Gasaustausch zur Verfüngung steht.
Mit der erfindungsgemäßen Anordnung, die durch einen Microprozessor oder einen Kontroller gesteuert wird, werden die Signale eines Hauptstrom-CO -Sensors und eines Beatmungsstromsensors aufgenommen. Der Hauptstrom-CO2- Sensor dient zur Messung der CO2-Konzentration in der Atemluft und der Beatmungsstromsensor zur Messung des Beatmungsstromes (Fig. l). Der Beatmungsstromsensor wird zwischen dem Endotrachealtubus und dem CO2-Sensor angeordnet. Zwischen dem Endotrachealtubus und dem Y-Stück des Beatmungsgerätes befindet sich ein steuerbares Dreiwegeventil. Dieses Ventil wird über den Microprozessor oder den Kontroller umgeschaltet, damit der Patient durch einen kleinen oder einen größeren Schenkel (sogenannter Totraum) beatmet wird (Fig. 2).
Die Messungen der Cθ2-Elimination und des endexpiratorischen partiellen CO2-Druckes, wird zuerst während der Beatmung durch den kleinen Totraum durchgeführt. Diese Phase dauert ca. 60 Sekunden und wird Nicht-Rückatmungsphase genannt. Nach dieser Phase und während eines inspiratorischen Zyklus wird das Dreiwegeventil umgeschaltet, damit der Patient durch den größeren Totraum (großer Schenkel) beatmet wird und einen Gasgemisch, das aus seiner eigenen ausgeatmeten Luft und frischer Luft aus dem Beatmungsgerät besteht, zurückatmet. Dadurch entfallt die Verwendung einer separaten CO2-Quelle für die Rückatmung. Der Zeitpunkt zur Umschaltung des Dreiwegeventils wird durch die Abwesenheit von CO2 in der inspiratorischen Luft erkannt. Damit wird keine wesentliche Veränderung des Atemdruckes

verursacht. Die anschheßende Phase dauert ca. 30 Sekunde und wird Rückatmungsphase genannt. Die CO2-Elimination und der endexpiratorische partielle CO2-Druck dieser Phase werden als Mittelwerte der entsprechenden Variablen während eines Plateaus, das sich im Bereich von 15 bis 30 Sekunden während dieser Phase bildet (zweite Hälfte), gemessen.
Mit der erfindungsgemäßen Anordnung besteht nun die Möglichkeit das Beatmungsmuster des Patienten im Beatmungsgerät so einzustellen, daß der maximale pulmonale Blutdurchfluß mit dem niedrigsten mittleren und endexpiratorischen Atemwegsdruck erreicht wird. Das bedeutet für den Patienten eine Verminderung des Risikos eines Barotraumas, d.h. Lungenschäden hervorgerufen durch erhöhten Atemwegsdruck werden vermieden und gleichzeitig wird eine Optimierung des Sauerstoffangebotes an die Organe erreicht. Die Lösung bietet auch die Möglichkeit, die Hämodynamik des Patienten nichtinvasiv zu überwachen und automatisiert zu dokumentieren. Wenn gleichzeitig Messungen des Herzzeitvolumens durchgeführt werden, besteht die Möglichkeit mit der gefundenen Lösung, den prozentuellen Anteil des nichtbelüfteten Herzzeitvolumen (sogenannter intrapulmonaler Shunt) zu messen, ohne daß die inspiratorische Sauerstoffkouzentration erhöht und Blut abgenommen werden muß.
Die Anordnung wird nachfolgend an einem Ausführungsbeispiel näher erläutert. In den Zeichnungen zeigen: Fig. 1 eine Anordnung eines Microprozessors oder eines Kontrollers für die Messung des effektiven pulmonalen Blutdurchflusses Fig. 2 eine erfindungsgemäße Anordnung mit Microprozessor/Kontroller und Beatmungsgerät Fig. 3 ein mit der erfindungsgemäßen Anordnung gemessenes Beispiel einer CO -
Konzentrations- und Beatmungsstromkurve Fig. 4 ein mit der erfindungsgemäßen Anordnung gemessenes Beispiel einer endexpiratorischen partiellen CO2-Druck- und CO2-Eliminationskurve, sowie den berechneten pulmonalen
Blutdurchfluß
In Fig. 1 ist eine Anordnung eines Microprozessor/Kontroller für die Messung des effektiven pulmonalen Blutdurchflusses dargestellt, das aus vier Teilen besteht. Die einzelnen Teile werden über Kabel und Stecker miteinander verbunden. Gemäß Fig. 1 besteht das System aus einem Microprozessor/Kontroller 7 und einer Analog-Digital-Wandlerkarte 8, die die CO2- Konzentrations- und Beatmungsstromsignale vom CO2-Sensor 3 bzw. vom Beatmungsstromsensor 1 aufnimmt und bearbeitet.
In Fig. 2 ist eine erfindungsgemäße microprozessor- oder kontrollergesteuerte Anordnung für die Messung des pulmonalen Blutdurchflusses dargestellt, das aus sieben, wahlweise weniger oder mehr Teilen, besteht. Gemäß der Fig. 2 wird der Endotrachealtubus des Patienten (Tubus) an die eine Seite eines Beatmungsstromsensors 1 angeschlossen. Au der anderen Seite des Beatmungsstromsensors 1 wird eine CO2-Küvette 2 angeschlossen. In die CO2-Küvette 2 ist ein CO2-Sensor 3 eingesetzt. An der anderen Seite der CO2-Küvette 2 wird ein steuerbares Dreiwegeventil 4 angeschlossen. Der eine Wege vom Dreiwegeventil 4 wird an ein Y- Stück 5 und der andere Weg wird an den Totraum 6 für die Rückatmung angeschlossen. An das Y-Stück 5 wird noch ein Beatmungsgerät sowie der Totraum 6 für Rückatmung angeschlossen. Der Totraum 6 für die Rückatmung beträgt ca. 200 ml, wahlweise mehr oder wemger, je nach Beatmungsmuster des Patienten. Zu dem Dreiwegeventil 4 besteht eine Verbindung, durch die mittels Druck oder Strom die interne Membran des Ventils 4 bewegt werden kann. Die Kontrolle des Dreiwegeventils 4 sowie die Aufnahme und Bearbeitung der CO2-Konzentration- und Beatmungstromsignale wird durch den Microprozessor/Kontroller 7 durchgeführt.
In der Fig. 3 sind die Verläufe der CO2-Konzentration in der Atemluft und des Beatmungstromes während einer Messung dargestellt. Während der Ausatmung wird CO2 über den Endotrachealtubus ausgeatmet. Die CO2- Konzentration (FCO2) in der ausgeatmeten Luft,
die dem partiellen CO2-Druck (PCO ) in der Luft entspricht, steigt mit dem ausgeatmeten Volumen an, und erreicht einen maximalen Wert am Ende der Ausatmung. Der partielle CO2- Druck in diesem Zeitpunkt, der sogennante endexpiratorische partielle CO2-Druck (PetCO2), entspricht ungefähr dem partiellen CO -Druck in den belüfteten pulmonalen Kapilllaren. Wälrrend der Nicht-Rückatmungsphase werden PetCO2-Werte gemessen, die sich kaum voneinander unterscheiden. Während der Rückatmungsphase wird ein Teil des ausgeatmeten CO zurückgeatmet. Dadurch modifiziert sich der Verlauf des partiellen CO2-Druckes in der Atemluft, und der PetCO2 steigt an.
In der Fig. 4 sind die Verläufe des endexpiratorischen partiellen CO2-Druckes, der dem höchsten CO2-Konzentrationswert während der Ausatmung entspricht, und der CO2- Elimination pro Atemzug, während einer Messung mit der erfindungsgemäßen Anordnung dargestellt. Die CO2-Elimination nimmt während der partiellen CO2-Rückatmung ab und der endexpiratorische partielle CO2-Druck in der Atmenluft nimmt zu, bis ein Plateau erreicht wird, üblicherweise nach ca. 15 Sekunden. Aus den vier Parameter die in Fig. 4 dargestellt sind, wird der effektive pulmonale Blutdurchfluß nach folgender Gleichung berechnet:

Unter VCO2(NR) wird die CO2-Elimination in ml/min während der Nicht-Rücktamungsphase, gemessen als der Mittelwert der CO2- Elimination vollständiger Atemzüge innerhalb von 60 Sekunden bis unmittelbar vor dem Beginn der Rücktamungsphase verstanden. Die CO2- Elimination eines Atemzuges wird aus dem Produkt vom Beatmungsstrom (nil/min) und CO2- Konzentration über die Zeit berechnet.
PetCO2(R) ist der endexpiratorische partielle CO -Druck in mmHg in der Atmenluft während der Nicht-Rücktamungsphase, gemessen als der Mittelwert der endexpiratorischen partiellen CO2- Druckwerte vollständiger Atemzüge innerhalb von 60 Sekunde bis unmittelbar vor dem Beginn der Rücktamungsphase.
Unter VCO (R) wird die CO2-Elirnination in ml/min während der Rückatmungsphase, gemessen als der Mittelwert der CO2-Ehmination vollständiger Atemzüge innerhalb 15 bis 30 Sekunden nachdem der Patient durch den größeren Totraum 6 beatmet wird, verstanden. Also nach der Umschaltung des Dreiwegeventils 4 in die Rückatmungsposition. Die CO2- Elimination eines Atemzuges wird aus dem Produkt vom Beatmungsstrom in ml/min und CO2-Konzentratiou in % über die Zeit berechnet.
PetCO (R) ist der endexpiratorische partielle CO -Druck in mmHg in der Atmenluft während der Rücktamungsphase, gemessen als der Mittelwert der endexpiratorischen partiellen CO2- Druckwerte vollständiger Atemzüge innerhalb von 15 bis 30 Sekunde nachdem der Patient durch den größeren Totraum 6 beatmet wird (nach der Umschaltung des Dreiwegeventils 4 in die Rückatmungsposition).
Die Funktion f (PetCO2(R), PetCO2(NR), Hb) ist schließlich die standardisierte CO2- Dissotiationskurve im Blut. Durch diese in der Literatur bereits beschriebene Funktion (McHardy, G.J.R.: The relationship between the differences in pressure and content of carbon dioxide in arterial and venous blood, Clin. Sei., 1967 32, 299-309) wird aus den PetCO (R)- und PetCO2(NR)- Werten, und auch anhand der Hämoglobin-Konzentration (Hb, g/dl), die mit einem anderen Gerät in einer Blutprobe des Patienten gemessen werden muß, die CO2- Gehaltdifferenz
(ΔCCO2, Vol% - ml/100 ml Blut) im pulmonalen kapillaren Blut zwischen der Rückatmuugs-
und der Nicht-Rückatmungsphase nach folgender Gleichung berechnet:
f (PetCO2(R), PetCO2(NR), Hb) = ΔCCO2 = 1 1,02 x (PetCO2(R)ü 396 - PetCO2(NR)° 3%) - 0,015 x ( 15 - Hb) x (PetCO2(R) - PetCO2(NR)).
Femer ist in der Gleichung Fs ein Skala factor für die Repräsentation des effektiven pulmonalen
Blutdurchflusses in 1/min. Fs ist auf 10 festgelegt.
Nach der Berechnung wird der effektive pulmonale Blutdurchfluß PBF in 1/min erhalten.

Claims
1. .Anordnung zur Bestimmung des effektiven pulmonalen Blutdurchflusses mittels partieller
CO2-Rückatmung, dadurch gekennzeichnet, daß
- die Leitung vom Endotrachealtubus des Patienten (Tubus) zum Beatmungsgerät zwischen einem steuerbaren Dreiwegeventil (4) und einem Y-Stück (5) in zwei Stränge aufgeteilt wird,
- ein Strang einen größeren Totraum (6) für die CO -Rückatmung bildet, wobei für die Messung von CO2-Elimination und endexpiratorischen partiellen CO2-Druck ein CO2-Sensor (3) und ein Beatmungsstromsensor (1) am Endotrachealtubus des Patienten und die Berechnung des effektiven pulmonalen Blutdurchflusses ein Microprozessor/Kontroller (7) vorgesehen ist,
- die Umschaltung zwischen den beiden Strängen durch das Dreiwegeventil (4) erfolgt, das über den Microprozessor/Kontroller (7) ansteuerbar ist,
- die Berechnung des effektiven pulmonalen Blutdurchflusses PBF nach der Beziehung:
( VCO2(NR)- VCO2(R)) PBF = f (PetCO2(R), PetCO2(NR), Hb) x Fs
erfolgt, wobei
- VCO2(NR) die CO2-Elimination (ml min) während der Nicht-Rücktamungsphase, gemessen als der Mittelwert der CO2-Elimination vollständiger Atemzüge innerhalb von 60 Sekunden bis unmittelbar vor Beginn der Rücktamungsphase ist,
- PetCO2(R) der endexpiratorische partielle CO2-Druck (mmHg) in der Atmenluft während der Nicht-Rücktamungsphase, gemessen als der Mittelwert der endexpiratorischen partiellen CO2- Druckwerte vollständiger Atemzüge innerhalb von 60 Sekunde bis unmittelbar vor dem Beginn der Rücktamungsphase ist,
ERSATZBLÄTT (REGEL 26) - VCO2(R) die CO2-Elimination (ml min) während der Rückatmungsphase, gemessen als der Mittelwert der CO2-Elimination vollständiger Atemzüge innerhalb 15 bis 30 Sekunden nachdem der Patient durch den größeren Totraum (6) beatmet wird (nach Umschaltung des Dreiwegeventils (4) in die Rückatmungsposition) ist,
- PetCO2(R) der endexpiratorische partielle CO2-Druck (mmHg) in der Atmenluft während der Rücktamungsphase, gemessen als der Mittelwert der endexpiratorischen partiellen CO2- Druckwerte vollständiger Atemzüge innerhalb von 15 bis 30 Sekunde nachdem der Patient durch den größeren Totraum (6) beatmet wird (nach Umschaltung des Dreiwegeventils (4) in die Rückatmungsposition) ist,
- Fs ein Skala-Faktor für die Repräsentation des effektiven pulmonalen Blutdurchflusses in 1 min und Fs = 10 ist,
- PBF der effektive pulmonale Blutdurchfluß (1/min) ist, wobei
- f(PetCO2(R), PetCO2(NR), Hb) die .standardisierte CO2-Dissotiationskurve im Blut ist, die durch Einsetzen der gemessenen Werte in die Gleichung
f (PetCO2(R), PetCO2(NR), Hb) = ΔCCO2 = 11,02 x (PetCO2(R)° 396 - PetCO2(NR)0396) - 0,015 x (15 - Hb) x (PetCO2(R) - PetCO2(NR))
berechnet wird.
2. Anordnung nach Anspruch 1, dadurch gekennzeichnet, daß das Volumen des Totraumes (6) 100, 200, 300, 400 oder 500 ml beträgt.
3. Anordnung nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß der Beatmungsstromsensor (1) in die Leitung am Endotracheltubus des Patienten (Tubus) eingebracht ist.
4. Anordnung nach Anspruch 1, 2 oder 3, dadurch gekennzeichnet, daß der CO -Sensor (3) ein Infrarotsensor ist.
5. Anordnung nach Anspruch 4, dadurch gekemizeichnct, daß der Infrarotsensor in einer CO2 Küvette (2) angeordnet ist.
6. Anordnung nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß der Totraum (6) ein auswechselbarer Schlauch oder Zylinder ist.
