Abstract
La présente invention a pour objet le perfectionnement d'un système d'implants dentaires destinés à être fixés dans les maxillaires. L'assemblage implant-moignon se fait par indexage multiple, l'implant (2) comportant un xn pans femelle et le moignon (8) un n pans mâle. Selon un deuxième mode de réalisation de l'invention, l'implant comporte le n pans mâle et le moignon le xn pans femelle.
Description
IMPLANT DENTAIRE EVOLUE ET EVOLUTIF
La présente invention a pour objet le perfectionnement d'un système d'implants dentaires destinés à être fixés dans les maxillaires pour soutenir une dent artificielle.
La technique d'implantation dentaire s'est développée depuis les travaux de Branemark qui proposait, outre un matériau biocompatible et une forme définie de la partie intraosseuse, une mise en charge retardée afin de favoriser l'ostéointégration. En effet, la mise en place d'une dent artificielle nécessite un implant de fondation intraosseux destiné à être fixé dans un maxillaire. Cet implant recevra un support transgingival (ou moignon) pour maintenir la prothèse dentaire. Cette dernière doit être strictement perpendiculaire à l'axe du maxillaire. Si l'implant a une bonne position (40 % des cas) (Fig. 8), la prothèse l'aura aussi par l'intermédiaire d'un moignon droit. Par contre s'il ne l'a pas, il est nécessaire de donner au moignon une angulation qui rattrapera l'obliquité de l'implant (Fig. 9).
Il est connu de réaliser, à l'heure actuelle, un support fixé de façon rigide ou par indexage à 6 pans sur l'implant selon un angle de rattrappage prédéterminé, afin de rectifier cette malposition. Cependant, un fort risque de correction insuffisante, dû à une absence de précision dans la prédétermination de cet angle, existe toujours, car, l'indexation à 6 pans ne propose que 6 possibilités d'angulation de 60° en 60° ce qui est nettement insuffisant.

L'état de l'art montre qu'il est possible d'améliorer les performances des implants existants, dans la mesure où quel que soit le type, aucun implant ne présente simultanément les qualités de facilité de mise en place, de simplicité d'utilisation, de précision du positionnement des moignons.
La présente invention concerne un système implant-support facile à mettre en place, et permettant une orientation du support avec beaucoup de précision..
Elle a pour objet des systèmes d'implants de diamètres et de longueurs différents constitués de deux parties : un implant de fondation, un moignon (ou support transgingival de prothèse dentaire), selon un mode préférentiel de l'invention, l'assemblage implant- moignon se fait par indexage multiple, l'implant comportant un xn pans femelle et le moignon un n pans mâle, ce qui implique xn positions du moignon sur l'implant, (avec x > 1) . Ceci s'explique par le fait que tout polygone est impactable dans un polygone comportant un multiple de nombres de pans.
Selon une autre caractéristique de l'invention, d'autres moignons comportent des multipans mâles décalés par rapport à la position d'origine, ce qui permet de multiplier le nombre de positions désirées.
L'application industrielle de cette invention est de proposer un certain nombre de moignons comportant des multipans mâles de positions différentes que l'utilisateur utilisera au choix.

Cependant, il est certain que le multipans mentionné dans cette description peut être remplacé par toute autre forme permettant un indexage multiple (systèmes à cannelures à flans parallèles ou non, de clavetage, dentés rec.tilignes ou non, de type multilobes, etc ...).
On dispose ainsi d'un système permettant une mise en place rapide, efficace, dans le maxillaire avec une précision optimale d'orientation du moignon.
Dans sa forme la plus simple, l'implant de fondation est de forme cylindrique filetée ou lisse à impacter. Il est creusé en son centre du multipans femelle dans lequel se fixera le multipans mâle du moignon.
Le moignon comporte outre une embase multipans mâle, une pièce transgingivale positionnée par rapport à l'axe de l'implant selon un angle prédéterminé.
L'implant et le moignon s'assemblent donc par le multipans, ce qui permet de les encastrer parfaitement l'un dans l'autre. Les extrémités du polygone mâle venant s'impacter une fois sur x dans les extrémités du polygone femelle. Selon un deuxième mode de réalisation de l'invention , le moignon susmentionné comporte le xn pans femelle et l'implant comporte le n pans mâle.
D'autres caractéristiques du système d'implants dentaires vont maintenant être décrites, mais sans caractère limitatif en référence aux dessins annexés sur lesquels :
Les figures la et lb représentent un implant fileté ou impacté. Les figures 2a et 2b montrent un moignon à sceller et à visser sur et dans l'implant.

Les figures 4a et 4b montrent respectivement les 2 moignons des figures 3a et 3b impactés dans l'implant.
La figure 5 représente ainsi les 24 positions possibles du moignon dans l'implant.
Les figures 6 et 7 illustrent un système d'implants,
. dont le moignon est scellé sur l'implant, . dont le moignon est fixé sur l'implant par l'intermédiaire d'une vis.
La figure 8 représente un implant supportant un moignon droit.
La figure 9 représente un implant incliné et un moignon angulé récupérant la verticalité. Les figures l a et lb représentent l'implant de fondation de forme cylindrique filetée (2) ou lisse à impacter (3) . Il est terminé en son segment apical en une demi-sphère (4), permettant ainsi une bonne répartition des contraintes. Cet implant présente dans son segment supérieur un col lisse (5) qui favorise en cas de résorption une meilleure hygiène contre la plaque bactérienne.
La partie filetée (2) ou lisse (3) de l'implant est pourvue de rainures verticales (6) et traitée par microbillage, favorisant l'antirotation et l'ostéointégration.

Dans sa partie interne, cet implant présente une cavité cylindrique taraudée (7), destinée à recevoir la partie inférieure du moignon (8) ou la vis de fixation (9) . L'ouverture supérieure (10) est combinée à un mécanisme de 12 pans femelle (1 1) dans lequel sera impacté le 6 pans mâle (12).
Le support de prothèse, fixé à l'implant de fondation et représenté par les figures 2a et 2b, est généralement composé de 4 éléments : une partie inférieure (8) destinée à être placée dans la partie interne filetée de l'implant, (7) une embase ( 12) pourvue d'un multipans mâle destinée à être fixée dans le multipans femelle ( 1 1) de l'implant, une partie gingivale conique ( 13), une pièce transgingivale constituée d'un cône long positionné par rapport à l'embase selon un angle prédéterminé (14) (15 ou
25°) , ce cône pouvant avoir des diamètres et des longueurs différents. Selon un mode préférentiel de réalisation de l'invention, le moignon est scellé sur l'implant de fondation en sa partie inférieure. Comme les illustrent les figures 2a et 6, cette pièce cylindrique présente une gorge de révolution (15) chanfreinée à son extrémité inférieure, aidant à l'antiextraction après collage.

Selon un deuxième mode de réalisation de l'invention (figures 2b et 7), le moignon peut être fixé sur l'implant par une vis transfixante appelée vis d'assemblage (9).
Dans le cas représenté par les figures 3 et 3a, le moignon à 6 pans mâle (Fig. 4 et 4b) (n=6) vient s'impacter dans l'implant à 12 pans femelle (Fig. 3c) (nx= 12), ce qui permet de positionner le moignon sur l'implant selon 12 orientations différentes (Fig. 4a) . La rotation du moignon sur l'implant se fait par indexage de 360°/ 12, ce qui équivaut à une rotation de 30°. Le nombre de positionnements double par l'utilisation d'un autre moignon dont le polygone est décalé par rapport au polygone d'origine. La figure 3b illustre un moignon comportant un indexage décalé de 15°. L'utilisateur pourra donc choisir l'orientation qui lui convient le plus par rapport aux deux moignons et 24 positions proposés (Figure 5) . Ce système quadruple donc les possibilités actuelles.

2. Système d'implants, selon la revendication 1 caractérisé en ce que d'autres moignons comportent des multipans mâle décalés par rapport à la position d'origine.
3. Système d'implants, selon les revendications 1 et 2, caractérisé en ce que le puits interne de l'implant de fondation comporte une section à 12 pans destinée à se conjuguer avec le 6 pans du moignon.
4. Système d'implants, selon l'une des 3 revendications, caractérisé en ce que. le multipans est remplacé par toute autre forme permettant un indexage multiple (systèmes à cannelures à flans parallèles ou non, de clavetage. dentés rectilignes ou non, de type multilobes).
5. Système d'implants , selon l'une des 4 revendications, caractérisé en ce que l'implant de fondation et le moignon présentent des longueurs et des diamètres différents.
6. Système d' implants selon l'un e des 5 revendications caractérisé en ce que l'implant de fondation peut comporter un n pans mâle et le moignon un xn pans femelle.
