Description

The present invention relates to a surgical material storage equipment specially intended for use in a surgical operating theater comprising an operating room whose walls are devoid of interior storage volumes and a method for the implementation of this equipment. Such equipment, which comprises several trolleys consisting of a rigid shelf adapted to receive removable baskets and a tray and a removable chassis is known from DE-A-1 919 896.

According to a more or less conventional arrangement of operating theaters, a so-called "clean" zone is provided which includes the operating rooms themselves, an anti-preparation room for surgeons and their assistants, possibly an anesthesia room and resuscitation, and a room in which sterile equipment is stored and handled, called "clean side sterilization" or "sterile stock", this room communicating with an area called "dirty side sterilization" or "washing side" via '' a disinfection and sterilization set whose entrance is in the dirty area.

After being sterilized, the equipment is stored in the operating room (s), at least one wall of which is fitted out for this purpose and includes display cases, cupboards, drawers, catgutiers, and possibly through counters.

The need to provide storage elements has led to taking advantage of the thickness arrangements on the wall or walls of the operating room to also embed certain devices such as the electrosurgical unit, the surgical suction system, the negatoscope, etc. .

These storage elements and these built-in devices consequently have many recesses and spaces that are difficult to access, their formaldehyde tightness is not always satisfactory and whose disinfection always poses problems.

In addition, these wall fittings are complicated, very time-consuming to install and consequently impose a heavy burden on construction and installation costs for operating theaters.

The object of the invention is to remedy these drawbacks by providing equipment specially intended for use in a surgical operating theater comprising an operating room whose walls are devoid of interior storage volumes, and a method for implementing it. of this equipment, making it possible to remove the elements and volumes of storage in operating rooms while putting within immediate reach of the surgeon the materials and the instrumentation which he needs for the interventions, and to simplify the handling of these instruments and accessories after use and especially during their disinfection and sterilization.

The equipment according to the invention comprises several removable trolleys suitable for receiving removable baskets and a tray P, a device for mounting said trolleys, a device for dismantling said trolleys and a movable trolley carrier suitable for receiving elements from several said disassembled carriages, in adjacent positions so as to have a minimum bulk, and at least one fixed support adapted to receive baskets.

The subject of the invention is also a method for implementing the equipment defined above, in a surgical operating theater comprising an operating room whose walls are devoid of interior storage volumes, characterized in that at least one dismounted trolley is used and at least one batch of preconditioned instruments and accessories placed in a sterile stock located in the clean area of the operating theater, and in which, prior to the intervention, the assembly of at least one trolley on which said preconditioned batch is loaded, the assembly is returned after the intervention to a dirty zone, as known per se, the trolley is dismantled in this dirty zone, said batch of instruments is repackaged and said disassembled carriage and the batch of instruments are simultaneously treated in a disinfection and sterilization assembly.

This method for implementing the equipment is further characterized in that there is provided a storage station for dismantled cart elements, a cart assembly station, a station for storing batches of preconditioned instruments. , in sterile stock, located in one and the same sterile zone, a car dismantling station and a washing station located in one and the same so-called dirty zone, in the vicinity of a disinfection and sterilization assembly disposed between said sterile area and the dirty and isolated area thereof in a known manner.

It is understood that thanks to the equipment, and to the process according to the invention, it is possible to eliminate from the operating room all the apparatus or installations which can be made mobile, that is to say on the one hand all the elements storage: drawers, cupboards, showcases, catgutiers, as well as the counters replaced by the equipment according to the invention, and on the other hand the surgical suction system (niche with bottles), the electrosurgical unit, and any bench-top , only remaining fixed: the X-ray viewer, surgical lighting and electrical and medical gas outlets.

The X-ray viewer and the sockets can be easily waterproofed, so the operating room is practically devoid of of all the wall installations used so far, which allows the room to be made with perfectly smooth walls and therefore can be very easily cleaned and disinfected.

In addition, the construction of the operating theater and the operating room are considerably simplified, which makes it possible to significantly lower the overall cost price of the installation.

The elimination of storage at the level of the operating room naturally leads to the need to prepare the equipment necessary for each intervention at the level of the sterile stock.

Consequently, with the equipment and the method according to the invention, baskets adapted for use with the carts and containing lots of equipment, instrumentations and the like, will be prepared in advance in the sterile storage room. accessories necessary for specific interventions, thus constituting listed assemblies prepared in advance and stored in a sterile state, ready for immediate use.

The installation according to the invention provides for this purpose a room located in a so-called "clean" area comprising special support and storage elements provided with means identical to those of the carriages to support the packed baskets and thus allowing storage in stock. sterile sets of baskets listed.

1 to 5 the various elements of the equipment according to the invention have been shown. This equipment comprises a removable carriage 1, a device 20 for mounting and dismounting the carriages, a dismantled carriages carrier 33 and a fixed support 41.

Fig. 1 shows a trolley 1 designed and specially adapted for use in a surgical operating theater, comprising an operating room whose walls are devoid of interior storage volumes, this trolley consisting of two identical end elements 2 each comprising two parallel uprights 3 interconnected by spaced transverse crosspieces 4 constituting supports for baskets 5 as will be described below.

The uprights 3 of the end elements, which are conveniently constituted by hollow stainless steel tubes of rectangular section and closed at their ends, have on their facing faces, near their upper and lower ends, means for attachment 6, one of which is shown on a larger scale in FIG. 2.

These fixing means each comprise a folded fitting 7 so as to delimit a roughly cubic parallelepiped open at its upper part and fixed, for example welded by a lateral face on the face of the upright 3 directed towards the corresponding face of the opposite symmetrical upright . The cube delimited by the fitting 7, the upper face of which is open, has in one of its side faces adjacent to the upper face a slot 9 extending vertically from the upper edge of the cube 7 and ending slightly near the center of face 8.

In its face opposite to face 8, the cube 7 has an opening 10 of semi-circular shape, the rounded end of which is tangent to the underside of the cube 7 and opening in the upper edge of this cube.

The fixing means 6 are arranged on each upright 3 while being oriented in the same direction.

The end elements 2 finally comprise an orientable roller 11 fixed to one end of each upright 3 below a fixing device 6, opposite its open face. In other words, the open upper faces of the fastening means are oriented upwards.

Two identical end elements 2 are arranged face to face and connected together by means of four identical spacers 12 each connecting the corresponding fixing means 6 of two opposite end elements.

Each spacer 12 is constituted by a bar which is for example made of stainless steel, of advantageously cylindrical section whose diameter is equal to the width of the opening 10 of each fixing means 6. Each spacer 12 is closed at its end and has a central axial rod 13 threaded in outwardly projecting onto which is screwed a disc 14 preferably having a diameter greater than that of the bar 12 and also preferably made of stainless steel.

As shown in FIG. 2, the bar constituting each spacer 12 is disposed with its ends introduced respectively into an opening 10 of each cubic fitting 7 with its threaded rod 13 disposed in the slot 9 and projecting outside the wall 8, the disc 14 forming handle being disposed outside the wall 8. It is understood that the end of each spacer is then disposed against the internal face of the wall 8. The spacers 12 being held without play in the fittings 7 it suffices to tighten each handle 14 to obtain a perfectly rigid assembly of the two end elements 2 of the carriage 1 in an extremely simple and rapid manner, without any tool and by simple means, thus putting the assembly of the carriages 1 within the reach of a nurse or 'An assistant, even without any knowledge or particular physical strength, sufficient tightening being provided effortlessly by threading the rods 13 having an appropriate pitch.

Each of the crosspieces 4 connecting together two uprights 3 advantageously consists of a slide fastened to the uprights 3 so as to have two horizontal wings extending relative to the fittings 7 on the same side as the openings 10 thereof. The ends 15 of each slide 4 are slightly raised and folded outwards as seen in FIG. 1 so as to delimit between them a rectilinear horizontal part 16 parallel to the plane containing the ends 15 and slightly spaced from this plane.

The horizontal parts 16 of the crosspieces 4 which face inward constitute supports adapted to receive the baskets 5.

Each basket 5. is constituted by a grid of stainless steel wire having two horizontal end portions 17 forming a handle, the width of each basket, and also of each of its handles 17, being equal to the length of the horizontal part. 16 of each crosspiece 4, between the ends 15 of the latter, the length of each basket 5 being approximately equal to the distance between the end elements 2, determined by the length of the spacers 12.

We understand that thanks to this arrangement, it is easy to introduce horizontally a basket 5 into a carriage 1 mounted so that its handles 17 are supported by the parts 16 of the crosspieces 4, the ends of the handles 17 being stopped by the ends 15 forming stops, projecting upwards. The basket in place can be easily removed by lifting it slightly to escape the end stops 15, on one side or the other of the carriage while being perfectly maintained without risk of falling or slipping when the carriage is moved on its wheels, or when a basket is partially pulled, such as a drawer.

Preferably some of the swivel casters 11 comprise a braking device which can be actuated by foot (not shown) of a conventional known type.

Each carriage is finally completed by a tray P of rectangular shape also made of stainless steel, having a slightly hollow shape and resting on the two upper spacers 12, which hold it between them and between the upper ends of the uprights 3.

The fastening means 6 adjacent to the rollers 11 are located at a sufficient distance above them to allow the carriage to be brought above the lower part of an assembly or disassembly device designated as a whole by reference 20 in FIG. 3.

The mounting device 20 comprises a plate 21 of roughly rectangular shape mounted on four orientable rollers 22 and is provided laterally with at least two immobilizing screw jacks 23.

On one of its sides, the plate 21 has two parallel vertical uprights 24 spaced at the ends of which is fixed an upper horizontal crosspiece 25 whose length is equal to the length of a carriage 1.

A second horizontal crosspiece 26 with a length equal to the crosspiece 25 is fixed parallel between the crosspiece 25 and the plate 21.

Horizontal arms 27 of the same length are fixed at right angles to the ends of the crosspieces 25 and 26 so as to extend parallel in the same direction.

The ends of the arms 27 situated on the same side with respect to the uprights 24 are connected together by vertical bars 28 of U-shaped section arranged so that their wings extend parallel in the same direction as the arms 27.

A support 29 is fixed on each end of the horizontal crosspiece 25 and on a part of the corresponding arm 27, and a folding arm 30 is articulated by one end at the top of each support 29 by means of a pin 31. At its opposite end at the support 29 each arm 30 comprises a retaining member 32 constituted by a hollow element having in section a rectangular shape, open at the bottom and whose section is a little greater than that of an upright 3 of the carriage 1.

Advantageously, the device 20 is entirely made of stainless steel and its swivel casters include a braking device.

This device makes it possible to easily mount a carriage as described above: there are two end elements 2 facing each other vertically by introducing an upright 3 into each of the vertical fittings 28 of U-shaped section, and the corresponding arm 30 is folded down. so that the retaining member 32 comes to cap the upper end of the upright 3 opposite to that held in the fitting 28. The two end elements 2 are thus held face to face vertically at the desired distance from one another. The upper spacers 12 are then placed in place in the fixing devices 6 by passing them under the arms 30 and by simultaneously tightening with each hand the end handles 14 of the spacers. The lower spacers 12 are then arranged in the same way by passing them between the lower crosspiece 4 and the plate 21 of the device 20, the carriage 1 thus being completely assembled and it is released by raising the arms 30.

It is understood that this extremely simple and rapid operation can be carried out easily by unqualified personnel.

To dismantle a carriage, the same operations are carried out in reverse.

When a carriage is dismantled, the six elements which constitute it, namely the two end elements 2 and the four spacers 12, can be arranged on a movable carriage carrier designated as a whole by the reference 33 in FIG. 4.

The carriage holder 33 comprises means for suspending and retaining the elements of several disassembled carriages.

These means comprise a frame constituted by a rectangular lower frame comprising two longitudinal members 34 connected by four crosspieces 35. This lower frame comprises at its ends two parallel vertical uprights 36 joined at their upper end by a crosspiece 35 '.

The midpoints of the crosspieces 35 "are interconnected by a beam 34 'supported by two intermediate uprights 36' and carrying two spaced pairs of parallel transverse horizontal arms 37.

The arms 37 of each pair are spaced a distance slightly less than the length of the crosspieces 4 of the end elements 2 of the carriages.

The movable carriage carrier 33 further comprises at each end a rack consisting of a crenellated metal strip 38 fixed horizontally on the upper part of each cross 35 'so that the tabs delimiting the slots extend horizontally outward .

The carriage carrier finally comprises vertical retaining means 39 constituted by metal rods curved in a U fixed vertically and parallel by the ends of their branches in an intermediate part of the length of the longitudinal members 34, transverse rods 39 'being arranged between the ends of each rod 39.

The trolley carrier 33 like the trolley 1 is entirely made of stainless steel elements and comprises at the corners of its lower frame orientable rollers 40 which are also advantageously of the type provided with a brake.

The movable trolley holder 33 is intended to store and carry with the smallest possible dimensions of the elements of a number of trolleys.

The end elements 2 of the carriages are suspended on the pairs of arms 37 on either side of the beam 34 'on which they are supported by the upper cross member 4 of each element, the spacers 12 are suspended vertically from the racks 38 in the slots of which the threaded rods 13 are introduced, the handles 14 being tightened to hold them in place.

The removable upper plates P of the carriages are arranged vertically between the retaining members 39.

It will be understood that, thanks to this arrangement, a large number of disassembled carriages can be placed on a single carriage carrier 33.

The last element of the equipment according to the invention consists of a fixed support designated as a whole by the reference 41 and shown in FIG. 5, intended to be installed in the sterile stock of the installation which will be described below.

The fixed support 41 is simply made up of two parallel vertical end elements 42 comprising parallel horizontal crosspieces 43 which are identical to the uprights 3 and to the crosspieces 4 of the end elements 2 of the carriages, and can thus be produced simultaneously with them. Ci, two corresponding uprights 42 which can be fixed along a wall by any suitable means such as fittings 45.

The crosspieces 43 are thus adapted to receive baskets such as the baskets 5 described above arranged one above the other with a spacing 44 just sufficient to allow the introduction and removal of the baskets individually.

The fixed supports 41 are intended to be mounted fixed in the sterile stock in which the baskets filled with lots of instruments are stored as indicated above with reference to the process according to the invention.

Referring now to FIG. 6 which shows the sequence of the different phases in the implementation of the method according to the invention.

Zone I delimited in solid lines represents the so-called "clean" zone, that is to say sterile. In phase 1 of the process, elements of dismantled trolleys placed on a movable trolley carrier are taken from a sterile storage room in sterile stock. In phase 2, the trolley elements are placed on a mounting device (Fig. 4) and the various elements of the carriages are assembled.

In phase 3, a number of baskets each containing a portion are taken from fixed supports (Fig. 6) mounted in the storage room in sterile storage. instruments, accessories and others, previously listed, necessary for a surgeon to perform a given intervention and, in phase 4, these baskets are loaded onto the mounted carriages.

In phase 5 of the process, these loaded trolleys are then transported to the operating rooms. The surgeon then performs his intervention using the instruments and accessories contained in the baskets using the carts as service pedestal tables, in phase 6.

In phase 7, the room personnel take the trolleys out of the operating room and bring them into sterilization on the "dirty" side delimited in phantom lines and designated in FIG. 1 by reference II in which, in phase 8, the baskets of the carriages are unloaded, these are placed in a dismantling device in phase 9 and they are dismantled, in phase 10 the various elements of the carriages are then placed on a movable trolley holder adapted to carry the elements of several dismantled trolleys, and in phase 11 the baskets and the material they contain are washed and the batches of instruments are reconditioned in the baskets.

Phase 12 is carried out in an area delimited in broken lines in FIG. 1 and designated by the reference III which represents a set of disinfection and sterilization, phase in which the disassembled carriage carriers are disinfected and in phase 13 the baskets and their contents are sterilized.

In the last two phases, 14 and 15 of the process, the sterile baskets leaving the sterilization assembly are stored in phase 14 in the sterile storage room and they are placed in the fixed supports provided in this room while in the phase 15 the movable trolley carriers carrying the disinfected trolley elements are stored in said storage room, at the outlet of the disinfection assembly, ready to be reused in a new sequence of the process phases.

It is noted from the list of the different phases of the process that at no time is it necessary to use equipment, instruments or accessories from an outside source; all the equipment necessary for surgeons and their assistants arriving in operating rooms from the sterile stock and leaving the entire operating room to be reconditioned for a new operation.

In other words, we understand that thanks to the process according to the invention, all the equipment necessary for a surgeon and his assistants to perform a given intervention travels through a circuit provided in an installation, passing through during only one of the phases ( phase 6) of the process by the operating room from which it emerges after the operation to return to its starting point.

We will now describe with reference to FIG. 7 is a diagram of an installation for a particular embodiment of the method according to the invention.

Referring to FIG. 7 the installation comprises a so-called sterilization zone on the "clean" side or sterile stock designated by the reference and delimited in solid lines, a so-called sterilization zone on the "dirty" side designated by the reference II and delimited in broken lines, and an intermediate zone III delimited in broken lines and which consists of a set of disinfection and sterilization.

The clean zone I comprises a sterile stock 51 in which is installed a station 52 for storing disassembled and disinfected cart elements, this station being constituted by a movable cart carrier 33 described above which is arranged at the most convenient in sterile stock 51, location in which it is immobilized thanks to the braking system of its rollers. This storage station 52 is the one from which elements of the carriages are removed in the phase of the process described above.

The sterile stock 51 also includes a station 53 for mounting the carriages, constituted by a mounting device 20 as described above, immobilized by means of the braking devices of its rollers, this mounting station 53 being that used in phase 2 of the above process.

There is also provided in the stock 51 at least one station 54 for storing baskets filled with their instrumentation batches listed sterilized, this station being constituted by at least one fixed support such as the support 41 described above. This item 54 is that used in phase III of the process.

Room 51 can open onto a corridor 55 serving for example two operating rooms 56a and 56b via the usual premises used for the preparation of the surgeon and his assistants and an anesthesia chamber according to an arrangement classic not part of the invention.

The operating rooms include exit doors which lead to a corridor 57 which is part of the so-called dirty zone, this corridor leading to the entrance 58 of zone III constituted by the disinfection and sterilization assembly.

There is provided in the corridor 57 in the vicinity of the entrance 58 of zone III a station 59 for dismantling the carriages, constituted by a device such as the device 20, immobilized by means of the braking systems of its rollers in the vicinity of a washing station 60, this station 59 for dismantling the carriages being that which is used for phase 9 of the process described above, and which is also advantageously used for phase 8 of this process, it is indeed logical to bring directly the cart mounted at the dismantling station before unloading the baskets as planned in phase 8 of the process.

There is also provided at the end of the corridor a station 61 for loading the elements of dismantled carriages, this station 61 being constituted by a movable carriage carrier 33 immobilized by braking its rollers in the vicinity of the entrance 58 of the assembly disinfection and sterilization. This station 61 is the one used for phase 10 of the process.

The washing station 60 is only mentioned for information, this station already existing at one point or another in any operating room in the vicinity of the disinfection and sterilization assembly, which is also conventional. It should be noted, however, that this washing station and this disinfection installation, although not forming part of the invention, are however quite naturally part of the process for treating surgical materials according to the invention and naturally find their place in the installation for the implementation of this process.

Note however that the disinfection and sterilization assembly constituting zone III must be provided so as to be able to receive at least one movable carriage carrier 33 as described above carrying the elements of several disassembled carriages to be disinfected therein.

The washing station 60 is used normally for the execution of phase 11 of the process, while the disinfection and sterilization assembly of zone III is used for the execution of the phases 12 of disinfection of the movable carriage carrier carrying elements of dismantled carts and phase 13 of sterilization of reconditioned baskets.

Phase 14 of the process, that is to say the storage of sterile baskets with batches of instrumentation and accessories that they contain is carried out at the exit of the sterilization unit of zone III, at the post 54 of storage already used during phase 3, while phase 15 of storage of the carriages is carried out at the storage station 52 used in phase 1. It is in fact understood that the movable carriages carrying the elements of dismantled carts should be moved from time to time. In fact, when a trolley carrier 33 located at the mounting station 53 has been completely stripped of the elements it was carrying, it should be brought back into the dirty zone II, at station 61 to replace the one which has been fully loaded with elements of trolleys and which passed into room 51 via zone III in which it was disinfected. This transfer can be carried out either between two interventions by crossing one of the operating rooms 56A or 56B to be brought back by the corridor 57, or even by making it pass through the disinfection assembly in the opposite direction.

It appears from the foregoing that the equipment and the method for implementing this equipment according to the invention allow rational treatment of the instrumentation with equipment of extremely simple use and having all facilities with regard to the possibilities of disinfection and sterilization. Thanks to the invention it is now possible to produce operating rooms with perfectly smooth walls and devoid of any continuity solutions or built-in equipment whose installation is expensive, the operating rooms thus produced in a simple and economical manner. can be disinfected and sterilized in their entirety without the slightest difficulty while considerably reducing the costs of installing operating theaters.
