Beschreibung
Die Erfindung betrifft eine Schraubpfanne, einen Pf an-^ neneinsatz und ein Eindrehwerkzeug der im Oberbegriff des Anspruchs 1 angegebenen Art.
Eine Schraubpfanne dient zur chirurgischen Rekonstruktion des Acetabulums und nimmt die Gelenkkugel beweglich auf. Eine derartige Schraubpfanne weist ein - bevorzugt selbstschneidendes - Gewinde auf, wobei ein Werkzeug notwendig ist, welches drehmomentschlüssig in die Pfanne eingreift und die übertragung des mittels eines Ratschenschlüssel aufgebrachten Einschraubmoments ermöglicht* Die Mittel zur Schraubmomentübertragung sind kraftschlüssig sowohl in Ein- als auch in Ausschraubrichtung, um ein nachträgliches Entfernen der Pfanne zu ermöglichen.
Nachdem die Pfanne ihre Position im Acetabulum eingenommen und der Chirurg - bei bevorzugter ringförmiger Ausgestaltung der Pfanne - durch die innere öffnung die korrekte Positionierung kontrolliert hat, wird der Pfanneneinsatz aus Kunststoff in die Metallpfanne eingeschnappt, wobei ein Sprengring oder eine entsprechend geformte umlaufende Kante den Pfanneneinsatz in Position hält.
Bei den bekannten Anordnungen sind an der Pfanne Ausnehmungen zur Drehmomentübertragung in Wechselwirkung mit entsprechenden Anformungen an dem Eindrehinstrument vorgesehen, wobei nachteilig ist, daß die Ausnehmungen zusätzlich zu Mitteln vorhanden sind, welche die Pfanneneinsatz

oegen Verdrehung sichern* und somit den Materialquerschnitt der Pfanne schwächen.
Der im Anspruch 1 angegebene Erfindung liegt die Aufgabe zugrunde in gegenseitiger Anpassung eine Einschraubpfanne, einen Pfanneneinsatz und ein Einschraubwerkzeug für die Pfanne zu schaffen, welche als System verwendbar sind und bezüglich der zur übertragung vüi'i DLöhfliüiüeniten bzw. Drehsicherung vorgesehenen Anformungen bzw. Ausnehmungen derart ausgebildet sind, daß die Zahl der entsprechenden an der Pfanne vorgesehenen Elemente möglichst gering ist.
Die Erfindung beruht auf der Erkenntnis, daß sowohl die Übertragung von Schraubmomenten als auch die Drehsicherung der Pfanne vergleichbare Maßnahmen erfordert, so daß diejenigen Elemente, welche zum Ein- bzw. Ausschrauben benutzt werden, auch zur Drehsicherung der Pfanne dienen können. Diese Anformungen bzw. Ausnehmungen können die Funktion der Drehsicherung ohne Komplikationen mit übernehmen, so daß sich insgesamt eine wesentliche Einsparung ergibt.
Während die bisher vorgesehenen Mittel zur übertragung des Drehmoments vom Einschraubwerkzeug zur Pfanne derart angebracht sein mußten, daß sie den Pfanneneinsatz in seiner späteren Position nicht behindern, d.h. insoweit den Pfannenquerschnitt schwächende Ausnehmungen bilden mußten, so sind jetzt bevorzugt Erhebungen vorgesehen, welche in den Pfanneneinsatz derart eingreifen, daß sie mit dessen Oberfläche bündig abschließen bzw. davon verdeckt sind. Im funktionsbereiten Zustand der Pfanne treten sie nicht als Erhebungen in Erscheinung.

Die Anformungen der Pfanne bilden bevorzugt Ansätze mit jeweils zwei Flächen, welche einen tangential gerichtete/i Flächenvektor aufweisen und und den Weg entsprechender mit kongruenten Anschlagflächen versehenen Ausnehmungen der Pfanne bzw. des Eindrehwerkzeugs in diesen Richtungen begrenzen* Sie sind drehsymmetrisch auf dem äußeren Umfang angeordenet, damit einerseits ein Einsetzen von Pfannenemsätz üüd EindtehwäckZoüy in unterschiedlichen Winkel"· richtungen möglich ist und andererseits die übertragbaren Momente bei kleinem Materialquerschnitt möglichst groß sind. Die Anformungen bilden in günstiger Weise prismatische Erhebungen, welche zum von der Einschraubpfanne entfernten Ende hin eine parallel zur Pfannenunterseite gerichte-La Abschlußfläche bilden, welche bündig an die untere Abschlußfläche des eingesetzten Pfanneneinsatzes abschließt. Das Eindrehwerkzeug weist eine entsprechende - aber durchgehende Fläche auf, an die der Werkzeugstiel anschließt, mit dem die Ratsche verbunden ist.
Vorteilhafte Weiterbildungen der Erfindung sind in den Unteransprüchen gekennzeichnet bzw. werden nachstehend zusammen mit der Beschreibung der bevorzugten Ausführung der Erfindung anhand der Figuren näher dargestellt.

welche selbstschneidend ausgebildet sind und so bei sphärischer Oberfläche der Pfanne ein Einschrauben in unterschiedlichen Richtungen ohne Vorschneiden eines Gewindes gestatten.
Die Einschraubpfanne ist ringförmig ausgebildet, so daß durch eine öffnung 4 die Innenfläche des Acetabulums sichtbar wird und der Chirurg erkennen kann, ob die Pfanne eine Position erreicnt hat, welche die Einleitung der im Gebrauch auftretenden Kräfte gestattet.
Die Pfanne weist an ihrer im übrigen ebenen, kreisförmig ausgebildeten Unterseite 5 Anformungen 6, 7, 8 und 9 auf, welche symmetrisch an der Außenkante angeordnet sind, so daß der Eingriff in entsprechende Geyenstücke in unterschiedlichen Winkelrichtungen möglich ist. Die Anformungen bilden Prismen, welche innen und außen jeweils von zylindrischen Flächenteilen begrenzt sind. Die Deckflächen erstrecken sich horizontal, während die restlichen Begrenzungsflächen einen tangentialen Flächenvektor aufweisen, so daß sie die in Verbindung mit Drehmomenten auftretenden Druckkräfte durch daran anliegende Gegenelemente übernehmen. Das Eindrehwerkzeug 10 besteht aus einem Stiel 11 mit einem mittleren Griff 12 und einem oberen Endgriff 13, wobei zwischen den beiden Griffen ein Ratscheninstrument 14 ansetzbar ist, welches die Einleitung des Einschraub- bzw. Ausdrehmomentes in den Stiel 11 ermöglicht.
Am unteren Ende des Stiels 11 befindet sich ein scheibenförmiger Ansatz 15, welcher an seinen Randbereichen Ausnehmungen 16 bis 19 aufweist- e an die Anformungen 6 bis

9 der Schraubpfanne 1 angepaßt sind, so daß die Scheibe im Anschlag an der Fläche 5 duch die Anformungen 6 bis 9 einerseits gegen seitliche Verschiebungen gesichert ist und andererseits die übertragung von Drehmomenten ermöglicht.

Durch das Vorsehen der Eingriffselemente am äußeren Durchmesser sind die übertragbaren Kräfte bei vorgegebenen räumlichen Abmessungen ein Maximum. Die derart relativ groß dimensionierte Anlagefläche ermöglicht auch eine optimale Führung zur Ausrichtung der Einschraubrichtung der Pfanne.
Nach dem Einschrauben der Pfanne und Entfernen des Werkzeugs wird in die untere öffnung ein Pfanneneinsatz 20 eingefügt, welche eine - in der Zeichnung nicht dargestellte - Kante aufweist, die in das Pfanneninnere einschnappt und den Pfanneneinsatz festhält. Diese Kante, welche auch durch einen Sprengring oder dergleichen gebildet sein kann, beinhaltet jedoch keine Drehsicherung des Pfanneneinsatzes, da die Abmessungen, welche eine Verdrehung des Einsatzes sperren könnten, gering sind.
Der Pfanneneinsatz weist eine kreisringförmige Begrenzungsfläche 21 auf, in die eine sphärische Ausnehmung 22 zur Aufnahme der Gelenkkugel eingelassen ist. Der scheibenförmige Ansatz 21 liegt an der unteren Kreisrxngflache 5 im montierten Zustand an, 'vobei Ausnehmungen 23, 24, 25 und 26 entsprechend den Ausnehmungen 16 bis 19 des Werkzeugs mit den Anformungen 6 bis 9 der Einschraubpfanne 1 in Wechselwirkung treten und den Pfanneneinsatz gegen Verdrehung sichern.

Die Stärke des scheibenförmigen Ansatzes des Pfanneneinsatzes 20 entspricht dabei der Höhe der Anformungen 6 bis 9 der Schraubpfanne 1. Damit schliessen Anformungen und Pfanneneinsatz bündig ab, so daß die Anformungen den scheibenförmigen Ansatz 21 zu einer Kreisringscheibe ergänzen, deren Randbereich vollständig kreisrund verläuft und dessen Deckfläche in diesem Bereich ebenfalls nicht unterbrochen ist. Die Erfindung beschrankt sich in ihrer Ausführung nicht auf das vorstehend angegebene bevorzugte Ausführungsbeispiel. Vielmehr ist eine Anzahl von Varianten denkbar, welche von der dargestellten Lösung auch bei grundsätzlich anders gearteten Ausführungen Gebrauch machen.

1. Schraubpfanne für das Acetabulum, in die Pfanne einschnappbarer Einsatz mit Sicherung gegen unbeabsichtigtes Verdrehen und/oder drehmomentschlüssig mit der Pfanne verbindbares Eindrehwerkzeug, unter gegenseitigem Eingriff von Anformungen bzw. Ausnehmungen,

dadurch gekennzeichnet,
daß die Schraubpfanne (1) mit einem einzigen Satz Anformungen bzw. Ausnehmungen (4 bis 8) versehen ist, welche 5 sowohl kongruent zu einem Satz entsprechender Anformungen
bzw. Ausnehmungen (23 bis 26) zur Verdrehsicherung des Pfanneneinsatzes (20) als auch kongruent zu einem Satz
j entsprechender Anformungen bzw. Ausnehmungen ,'16 bis 19)
zur Drehmomentübertragung beim Einschraubwerkzeug (10)
: 10 ausgebildet sind.
2. Schraubpfanne, Pfanneneinsatz und Eindrehwerkzeug nach Anspruch 1, dadurch gekennzeichnet, daß die ineinander eingreifenden Anformungen bzw. Ausnehmungen (6 bis 9, 16 bis 19 und 23 bis 26) auf dem Außenumfang der miteinander in Wechselwirkung tretenden Teile (1, 10, 20) symmetrisch in gleichmäßiger Winkelteilung angeordnet sind.

3. Schraubpfanne, Pfanneneinsatz und Eindrehwerkzeug nach einem der vorangehenden Ansprüche, dadurch
gekennzeichnet, daß bei der Pfanne (1) Anformungen (6 bis 9) im Randbereich der die Pfannenunterseite bildenden Fläche (5) vorgesehen sind, während an Pfanneneinsatz (20) und Einschraubwerkzeug (10) entsprechende Ausnehmungen (16 bis 19, 23 bis 26) angebracht, sind.

4. Schraubpfanne, Pfanneneinsatz und Exndrehwerkzeug nach einem der vorangehenden Ansprüche, dadurch
gekennzeichnet, daß die Anformungen (6 bis 9) prismatisch ausgebildet sind und Stirnflächen mit tangentialem Flächenvektor aufweisen.
5. Schraubpfanne und Pfanneneinsatz nach einem der vorangehenden Ansprüche, dadurch gekennzeichnet, daß die Ausnehmungen (23 bis 26) des Pfanneneinsatzes (20) an die Anformungen an der Pfanne (1) bündig anschließen oder diese abdecken.
