Description
- Hintergrund der Erfindung
- Fachgebiet der Erfindung
- Die Erfindung bezieht sich auf eine Vorrichtung zur Behandlung von Körpergewebe. Insbesondere bezieht sich die vorliegende Erfindung auf eine Vorrichtung zur Behandlung, einschließlich Vaporisation und Koagulation, von Körpergewebe unter Verwendung von Elektroden, die aus porösem Metall in Verbindung mit einem leitenden Fluid, wie zum Beispiel einer Salzlösung bestehen.
- Beschreibung des naheliegenden Standes der Technik
- Die Elektrochirurgie wird zur Behandlung von Körpergewebe, einschließlich der Koagulation, dem Austrocknen und dem Vaporisieren von Gewebe verwendet. Ein übliches Verfahren zur Durchführung der Elektrochirurgie umfaßt das Injizieren eines leitenden Fluids, wie zum Beispiel einer Salzlösung in die Operationsstelle. Das leitende Fluid dient dazu, einen elektrischen Pfad zwischen den Elektroden und dem Körpergewebe herzustellen.
- Ein kontinuierliches Nachfüllen des leitenden Fluids kann während verschiedener Eingriffe wichtig sein. Das Fluid kann durch den Körper des Patienten absorbiert werden. Weiterhin kann das. Fluid verunreinigt werden und so dessen Leitfähigkeit und/oder andere wünschenswerte Eigenschaften, wie zum Beispiel die Durchsichtigkeit gefährden.
- Wenn eine ausreichende Spannung über den Elektroden anliegt, dann können die Elektroden aufgeheizt werden und das gesamte Fluid kann beim direkten Kontakt mit den Elektroden verdampfen. Die Elektroden können dann bis zu einem Punkt aufgeheizt werden, bei dem sie beschädigt werden.
- Um einen konstanten Zufluß des leitenden Fluids an der Operationsstelle aufrechtzuerhalten, kann das Fluid der Operationsstelle kontinuierlich zugeführt werden. Jedoch kann das Einbringen des Fluids in den Bereich unmittelbar angrenzend an die Elektrode, ohne dabei die Elektrodenfunktion nachteilig zu stören, kritisch für die Funktion des Systems sein.
- Die US-Patentschrift Nr. A-5.643.197 beschreibt einen Ablationskatheter, der eine Spitzenkonstruktion aufweist, in der Wege ausgebildet sind, zum Beispiel indem die Spitzenkonstruktion aus einem Sintermetall gefertigt ist, das eine Vielzahl von nach dem Zufallsprinzip ausgebildeten Durchgängen enthält, um ein Fluid von einem mittigen Lumen in der Spitzenkonstruktion zu Bereichen der Außenfläche der Spitzenkonstruktion zu lenken, um so um die Spitzenkonstruktion herum eine Fluid-Schutzschicht zu schaffen.
- WO-A-9715238 beschreibt einen hochfrequenten Ablationsapparat in Form eines Elektrodenpaars, das ein mittiges Lumen aufweist, durch das eine Elektrolytlösung oder ein Elektrolyt-Gel zu einer ausgewählten Gewebe-Ablationsstelle durch in der Elektrode ausgebildete Fluid-Verteiler-Öffnungen befördert wird. Zur Beförderung des Fluids durch das mittige Lumen zu beiden Elektroden wird eine Pumpe verwendet. Die zurückkehrende Elektrolytlösung oder das zurückkehrende Elektrolyt-Gel wird in einen Abfallbehälter geleitet.
- WO-A-9607360 beschreibt einen hochfrequenten Ablationsapparat in Form einer porösen Elektrode, die den Fluidfluß einen hohen Widerstand aussetzt, um eine Ablationsstelle mit einem erhitzten leitenden Fluid von einer unter Druck stehenden Fluidquelle zu versorgen. In dem Apparat sind zwei weitere Leitungen vorgesehen, um ein Kühl- oder Heizfluid einzuspeisen und zu entfernen, um das leitende Fluid auf einer gewünschten Temperatur zu halten.
- Folglich besteht das Bedürfnis, ein Fluid effizient zu einer Operationsstelle zu führen und besonders über die elektrochirurgischen Elektroden, ohne die Funktion der Elektroden ungünstig zu beeinflussen. Die vorliegende Erfindung erfüllt dieses Bedürfnis.
- Kurze Beschreibung der Erfindung
- Kurz und ganz allgemein sieht die vorliegende Erfindung ein elektrochirurgisches Instrument, wie in Anspruch 1 definiert, und ein System, wie in Anspruch 4 definiert, vor.
- Das Instrument kann Teil eines elektrochirurgischen Systems sein und eine Stromversorgung zur Erzeugung einer Spannung zwischen der ersten Elektrode und der zweiten Elektrode enthalten. Das System kann weiterhin eine Fluidquelle enthalten, um die Operations stelle über eine Fluid-Zufuhrleitung mit einem Fluid zu versorgen. Eine Steuereinheit kann verwendet werden, um die Stromversorgung, die Fluidquelle und die Fluid-Ableitung zu überwachen und zu steuern.
- Während eines chirurgischen Eingriffs ist die erste Elektrode angrenzend an dem zu behandelnden Gewebe positioniert. Man führt ein leitendes Fluid zu der Operationsstelle, indem man Fluid von einer Fluidversorgungsleitung durch das poröse Metall der ersten Elektrode injiziert.
- Eine elektrische Spannung wird an der ersten Elektrode angelegt, um die gewünschte Behandlung zu bewirken.
- Weitere Merkmale und Vorteile der vorliegenden Erfindung werden am besten verständlich durch Bezugnahme auf die folgende detaillierte Beschreibung der Erfindung in Verbindung mit den dazugehörigen Zeichnungen.
- Kurze Beschreibung der Zeichnungen

- Die vorliegende Erfindung wird mit Bezug auf die
1 und1a beschrieben für die Anwendung bei der Behandlung von Körpergewebe, einschließlich der Anwendung bei der Behandlung des menschlichen Uterus. Die vorliegende Erfindung ist jedoch nicht auf die Anwendung an dem weiblichen Fortpflanzungssystem beschränkt und kann bei verschiedenen elektrochirurgischen Eingriffen verwendet werden. - Es wird jetzt auf
1 Bezug genommen. Eine Anordnung, die nicht eine Ausführungsform der vorliegenden Erfindung bildet, umfaßt ein elektrochirurgisches Instrument12 , das einen generell langgestreckten Schaft14 enthält, der an dem distalen Ende18 eine Elektro denbaugruppe16 aufweist. Man sieht die Elektrodenbaugruppe16 an einer gewünschten Operationsstelle20 . Die Elektrodenbaugruppe16 enthält eine erste Elektrode22 , die aus einem Sintermetall besteht. - Die Struktur eines Sintermetalls wird typischerweise so hergestellt, daß man ein Metallpulver oder eine Ansammlung von sehr kleinen Metallkugeln (wie zum Beispiel BB-Pellets) in einen Ofen einbringt. Indem man die Temperatur im Ofen bis zu dem Punkt erhöht, bei dem die Oberfläche der Pellets den Erweichungspunkt, aber nicht ganz den Schmelzpunkt erreicht, dann werden die benachbarten Kugeln an ihren Berührungspunkten aneinander geschweißt und werden so gesintert. Die Leerstellen zwischen den Kugeln bleiben vorzugsweise offen und bilden auf diese Weise eine schaumartige Struktur mit generell offenen Poren. Da die Kugeln nicht vollständig schmelzen, sintern sie nicht zu einer einzigen generell massiven Masse. Die sich ergebende Struktur ist eine poröse Metallstruktur mit einer Vielzahl von kleinen Durchlässen.
- Eine Alternative zu dem Sintermetall besteht darin, in einer Metallstruktur eine Vielzahl von Löchern zu bilden, wie zum Beispiel durch Bohren, und dadurch eine poröse Metallstruktur herzustellen. Zum Beispiel könnte eine Vielzahl von Löchern in ein Rohrstück gebohrt werden und dadurch eine Vielzahl von Durchlässen erzeugt werden, damit ein Fluid aus dem und/oder in das Rohr fließen kann.
- Die Elektrodenbaugruppe
16 kann eine zweite, auf dem Schaft14 positionierte Elektrode24 enthalten. Die zweite Elektrode24 und die erste Elektrode22 sind vorzugsweise voneinander isoliert. Zum Beispiel besteht der Katheter14 in der Anordnung von1 aus einem generell nicht leitenden Material. Auf diese Weise dient der Katheter-Schaft26 zwischen der ersten Elektrode22 und der zweiten Elektrode24 als Isolator. - Die erste Elektrode
22 und die zweite Elektrode24 sind elektrisch über eine erste elektrische Leitung28 bzw. eine zweite elektrische Leitung30 mit einer Stromquelle32 verbunden, die eine Spannung zwischen der ersten Elektrode22 und der zweiten Elektrode24 erzeugt. In der dargestellten Anordnung überwacht und steuert eine Steuereinheit34 die Ausgangsleistung gemäß vorprogrammierter Anweisungen und/oder Eingaben vom Chirurgen oder einem anderen Benutzer. - In der Anordnung von
1 ist die erste Elektrode22 am distalen Ende18 des Katheterschafts14 positioniert. Eine Fluidversorgungsleitung36 in dem Katheterschaft führt zu der ersten Elektrode22 , so daß das Fluid, das in die Fluidversorgungsleitung36 von einer Flu idquelle38 eingespeist wird, aus dem porösen Metall der ersten Elektrode22 herausfließt, und somit das Fluid in die Operationsstelle20 einbringt. Das Fluid ist vorzugsweise ein leitendes Fluid. - Es kann eine Fluid-Ableitung
40 vorgesehen sein, durch die das Fluid von der Operationsstelle20 durch eine Ableitungsvorrichtung, wie zum Beispiel die in1 dargestellte Fluid-Absaugpumpe42 abgesaugt werden kann. Die Fluid-Ableitung40 wird mit einer Einströmungsöffnung44 , die auf dem Katheterschaft14 proximal zur ersten Elektrode22 positioniert ist, dargestellt. - In der in
1a gezeigten Ausführungsform der Erfindung besteht die zweite Elektrode24a aus einem porösen Metall. Die zweite Elektrode24a ist über der Einströmungsöffnung44 positioniert, so daß das Fluid durch das poröse Metall der zweiten Elektrode24a abgesaugt wird. - Es wird nochmals auf
1 Bezug genommen. Die Steuereinheit34 kann die Fluidquelle38 überwachen und steuern und, im Falle einer Fehlfunktion von entweder der Fluidquelle38 oder der Stromversorgung32 , kann sie aktiv werden und den Betrieb abschalten. So kann beispielsweise die Steuereinheit34 automatisch die Stromversorgung32 abschalten, falls die Fluidquelle38 kein Fluid mehr an die Operationsstelle20 liefert. - In der in
1 gezeigten Anordnung ist die Elektrodenbaugruppe16 eine bipolare Elektrodenbaugruppe, die eine erste Elektrode22 und eine zweite Elektrode24 enthält. - Obgleich eine bevorzugte Ausführungsform der Erfindung beschrieben und dargestellt wurde, ist die Erfindung für Modifikationen und Bearbeitungen gemäß der Fähigkeiten des Fachmanns und ohne Ausübung einer erfinderischen Tätigkeit zugänglich. Folglich sollte selbstverständlich sein, daß verschiedene Veränderungen in Form, Detail und Anwendung der vorliegenden Erfindung ausgeführt werden können, ohne vom Schutzumfang der Erfindung abzuweichen. So ist nicht beabsichtigt, daß die Erfindung eingeschränkt wird, außer durch die angefügten Ansprüche.
Claims (4)
- Elektrochirurgisches Instrument (
12 ) zur Behandlung von Gewebe an einer ausgewählten Operationsstelle (20 ), wobei dieses Instrument (12 ) umfaßt: eine erste (22 ) und eine zweite Elektrode (24a ) beide aus porösem Metall, welche leitfähig mit einer ersten (28 ) bzw. zweiten elektrischen Leitung (30 ) verbunden sind; eine Fluid-Zufuhrleitung (36 ) in einer Fluid-Verbindung mit dem porösen Metall der ersten Elektrode (22 ), wobei das Fluid der Operationsstelle (20 ) über die Fluid-Zufuhrleitung (36 ) und das poröse Metall der ersten Elektrode (22 ) zugeführt werden kann; dadurch gekennzeichnet, daß das Instrument ferner umfaßt: eine Fluid-Ableitung (40 ) in einer Fluid-Verbindung mit dem porösen Metall der zweiten Elektrode (24a ), wodurch das Fluid über das poröse Metall der zweiten Elektrode (24a ) und die Fluid-Ableitung (40 ) von der Operationsstelle (20 ) abgeleitet werden kann. - Instrument nach Anspruch 1, bei welchem das poröse Metall Sintermetall ist.
- Instrument nach Anspruch 1 oder 2, welches ferner einen Katheterschaft (
14 ) mit einem distalen Ende (18 ) und einem proximalen Ende umfaßt, wobei die erste Elektrode (22 ) am distalen Ende (18 ) des Schaftes und die zweite Elektrode (24a ) auf dem Schaft (14 ) proximal von der ersten Elektrode (22 ) positioniert ist. - System (
10 ) zur Behandlung von Gewebe an einer ausgewählten Operationsstelle (20 ), wobei dieses System (10 ) umfaßt: ein elektrochirurgisches Instrument (12 ) nach einem der bisherigen Ansprüche; eine Stromversorgung (32 ) zur Erzeugung einer Spannung zwischen der ersten Elektrode (22 ) und der zweiten Elektrode (24a ); eine Einrichtung (38 ) zur Lieferung eines Fluids an die Fluid-Zufuhrleitung und eine Einrichtung (42 ) zur Ableitung des Fluids aus der Fluid-Ableitung.
