Description

  • Diese Erfindung betrifft eine Prothesenbaugruppe, insbesondere eine Schale zum Einsetzen in das Acetabulum (Hüftgelenkpfanne) eines Patienten und einen Einsatz zur Schaffung einer Lagerfläche für ein Hüftgelenk.
  • Es sind Hüftgelenk-Prothesen bekannt, bei denen das Hülsen-Bauteil am Acetabulum eines Patienten mittels Zement bzw. mit oder ohne Schraubenbefestigung direkt am Knochen angebracht ist. Ein Einsatz in Form einer Schale ist innerhalb der Hülse positioniert. Die Innenfläche der Schale ergibt eine Lagerfläche für den Femoralkopf.
  • Das Europäische Patent Nr. 0 436 317 beschreibt einen Einsatz mit einer breiten Lagerfläche, indem sich der Rand des Einsatzes von der Schalenform weg nach oben erstreckt. Der in diesem Dokument beschriebene Einsatz ist aus Metall oder einem Polyethylen-Material geformt. Die Dokumente WO 85/00284, FR 2 682 588, FR 2 677 878 und FR 2 651 995 beschreiben ebenfalls Einsätze mit verbreitertem Rand.
  • Vorzugsweise wird der schalenförmige Einsatz aus einem Keramikmaterial geformt, welches ein besseres Verschleißverhalten und eine bessere Bioverträglichkeit ausweist als andere Materialien, aber wegen seiner geringen Bruchzähigkeit sehr spröde ist. Die Sprödigkeit der Keramikmaterialien bedeutet, daß schalenförmige Einsätze aus Keramikmaterialien keine ausgedehnte Lagerfläche als einen Bereich haben können, wo die Schale nicht von der Hülse gestützt wird und somit bruchanfällig ist.
  • Nach der vorliegenden Erfindung ist eine Prothesenbaugruppe vorgesehen, welche umfaßt: ein Befestigungselement zur Anbringung am Knochen des Patienten; ein Keramik-Lagerschalen-Bauteil mit einem Rand, wobei ein Teil des Randes verstärkt ist und die Verstärkung die Form einer Schulter hat, die sich allgemein in radialer Richtung vom Lagerschalen-Bauteil aus erstreckt, wodurch ein Bereich des Randes mit größerer Breite entsteht.
  • Das Keramikmaterial kann Aluminiumoxid oder Zirkoniumoxid sein.
  • Das Befestigungselement ist vorzugsweise aus synthetischem Kunststoff oder Metall geformt.
  • Vorzugsweise existiert zwischen dem Befestigungselement und dem Lagerschalen-Bauteil eine Kegelpassung. Das Lagerschalen-Bauteil kann eine Außenfläche haben, welche zu einem Außenkegel geformt ist. Das Befestigungselement kann eine Innenfläche haben, welche zu einem Innenkegel geformt ist, der dem Außenkegel des Lagerschalen-Bauteiles entspricht.
  • Vorzugsweise hat der Rand des Lagerschalen-Bauteiles eine im wesentlichen ovale Form.
  • Vorzugsweise liegt die innere Lagerfläche des Lagerschalen-Bauteiles exzentrisch innerhalb des ovalen Randes desselben.
  • Vorzugsweise ist die Prothesenbaugruppe eine Hüftgelenkpfannen-Baugruppe.
  • Nun soll eine Ausführungsform einer Prothesenbaugruppe entsprechend der vorliegenden Erfindung unter Bezugnahme auf die beigefügten Zeichnungen beschrieben werden, welche darstellen:
  • 1 ist ein Schnitt durch die Prothesenbaugruppe.
  • 2 ist eine perspektivische Ansicht der Prothesenbaugruppe.
  • 3 ist eine Draufsicht auf das Lagerschalen-Bauteil der Prothesenbaugruppe.
  • 4 ist eine perspektivische Ansicht des Lagerschalen-Bauteiles der Prothesenbaugruppe.
  • Bezug nehmend auf die Zeichnungen ist eine Prothesenbaugruppe 1 vorgesehen bestehend aus einem Befestigungselement in Gestalt einer Hülse 2 und eines Lagerschalen-Bauteiles in Gestalt eines Einsatzes 4.
  • Die Hülse 2 hat eine abgestufte Außenfläche 6, welche am Knochen eines Patienten angebracht wird. Bei diesem Beispiel ist die Prothesenbaugruppe 1 eine Hüftgelenkpfannen-Baugruppe für ein künstliches Hüftgelenk, und die Hülse 2 wird am Acetabulum des Patienten angebracht.
  • Die Hülse 2 wird durch eine Preßpassung am Acetabulum angebracht und kann zusätzliche Schrauben-Befestigungen aufweisen. Die Schrauben-Befestigungen werden in Durchgangslöcher 24 in der Hülse 2 eingeführt. Die Köpfe der Schrauben-Befestigungen sind in der Hülse 2 versenkt, um eine Kollision mit dem Einsatz 4 zu vermeiden. Als Alternative kann die Hülse 2 auch im Acetabulum des Patienten einzementiert werden.
  • Die Hülse 2 hat allgemein eine Halbkugelform mit einem flachen Rand 26. Die Hülse 2 hat angrenzend an den Rand 26 einen Kegelbereich 14.
  • Der Einsatz 4 hat die Form einer Schale mit einer Innenfläche 12, welches die Lagerfläche für die Gelenk-Komponente, in diesem Beispiel für den Femoral-Kopf, ist. Die Außenfläche 28 des Einsatzes 4 hat einen Kegelbereich 16, welcher den Außenkegel bildet, der in den Innenkegel 14 der Hülse 2 paßt. Der restliche Bereich der Außenfläche 28 des Einsatzes 4 ist derart bemessen, daß er nicht auf der Innenfläche der Hülse 6 aufliegt. Der Raum zwischen der Außenfläche 28 des Einsatzes 4 und der Innenfläche 22 der Hülse 2 ist zur Wirksamkeit des Kegels 6 zwischen der Hülse 2 und dem Einsatz 4 erforderlich.
  • Der Einsatz 4 hat einen Rand 8, welcher einen verstärkten Teil 10 aufweist, der sich in radialer Richtung von der Schalenform des Einsatzes 4 aus erstreckt. Der verstärkte Teil des Randes 10 bildet eine Schulter, die sich nach außen über den flachen Rand 26 der Hülse 2 erstreckt. Zwischen dem flachen Rand 26 der Hülse 2 und dem verstärkten Teil des Randes 10 ist ein Spalt 18 vorgesehen.
  • Wie aus 3 zu erkennen ist, hat der Rand 8 des Einsatzes 4 eine ovale Form mit halbkugelförmigen Lagerfläche 12, die exzentrisch zur ovalen Form liegt. Wie in den 1 und 2 dargestellt, kann der Rand 8 des Einsatzes 4 abrupt in den verstärkten Teil des Randes 10 übergehen.
  • Zwei keramische Materialien sind gegenwärtig für orthopädische Vorrichtungen verfügbar, nämlich Zirkoniumoxid und Aluminiumoxid.
  • Aluminiumoxid ist ein wesentlich härteres Material als Zirkoniumoxid und zur Erzielung der besten Ergebnisse wird die Verwendung des gleichen Materials für den Femoral-Kopf und für den Einsatz empfohlen. Es ist jedoch auch möglich, den Keramik-Kopf durch einen Kopf aus Kobalt-Chrom zu ersetzen. Die Gelenkpaarung Aluminiumoxid/Aluminiumoxid hat die besten Eigenschaften, aber das Material ist sehr spröde. Es ist bekannt, daß die Gelenkpaarung Kobalt-Chrom/Polyethylen einen jährlichen Verschleiß zwischen 0,2 mm und 0,5 mm aufweist. Eine Gelenkpaarung Aluminiumoxid/Aluminiumoxid hat einen jährlichen Verschleiß zwischen 0,0001 mm und 0,005 mm.
  • Der verstärkte Einsatz 4 ist erforderlich, weil es oft schwierig ist, die Hülse in ihrer exakten Position am Acetabulum anzufügen. Dies kann seine Ursache in der Anatomie des Patienten oder auch in anderen Dingen haben. Es ist daher unerläßlich, den Einsatz 4 mit einer exzentrischen Lagerfläche 12 zu versehen, welche das Problem einer ungenauen Positionierung der Hülse 2 überwindet. Der Bewegungsbereich des Gelenkes kann vergrößert und das Risiko einer Verlagerung kann reduziert werden.
  • Keramische Einsätze sind bekannt und daher gibt es praktisch keine Gefahr eines Brechens des Einsatzes, wenn er mit der Oberfläche der Hülse bündig abschließt. Wenn die Lagerfläche des Einsatzes vergrößert wird, bleibt ein Teil des Randes ungestützt und daher ist die Gefahr eines Bruches des keramischen Einsatzes sehr hoch.
  • Wenn der Chirurg operiert, befestigt er zuerst die Hülse im Acetabulum und sucht dann die optimale Position für den verstärkten Einsatz 4 zu finden, indem er einen Versuchs-Einsatz 4 aus einem biokompatiblen Kunststoff verwendet. Wenn der Chirurg die exakte erforderliche Position für den Einsatz 4 gefunden hat, drückt er den Keramik-Einsatz 4 in die Hülse 2. Der Einsatz 4 wird in der Hülse 2 durch das Zusammenwirken der Kegelflächen 14 und 16 von Einsatz 4 bzw. Hülse 2 arretiert. Ein Kegel-Arretierungs-Mechanismus eröffnet die Möglichkeit einer Drehung des Einsatzes um 360 Grad in der Hülse 2, so daß sich eine vollständige Positionierungsfreiheit ergibt. Der Chirurg prüft dann den Bewegungsbereich und kann die Position des Einsatzes 4 mit dem verstärkten Rand in der Hülse 2 geringfügig ändern. Wenn er zufrieden ist, schlägt der Chirurg den Einsatz 4 in die Hülse 2, um die beiden Kegelteile 14 und 16 sicher untereinander zu blockieren.
  • Die vorliegende Erfindung ändert die Festigkeit des verstärkten Einsatzes 4 durch Vergrößerung der Dicke des Randes 10 am höchsten, nicht gestützten Bereich des Randes B. Die erhöhte Festigkeit des verstärkten Einsatzes 4 ermöglicht es, daß dieser verstärkte Einsatz aus keramischen Material, wie Aluminiumoxid und Zirkoniumoxid, geformt wird.
  • Es muß ein kleiner Spalt 20 zwischen dem verstärkten Teil 10 des Randes und dem flachen Rand 26 der Hülse 2 bleiben, um die Kegelpaarung wirksam werden zu lassen.
  • Die Analyse nach der Methode der finiten Elemente ist bei einem verstärkten Einsatz aus keramischen Material nach dem Stand der Technik und bei einem erfindungsgemäß ver stärkten Einsatz durchgeführt worden. Die Ergebnisse haben gezeigt, daß die Spannungen bei dem verstärkten Einsatz nach vorliegenden Erfindung vermindert werden und somit das Gesamt-Risiko eines Bruches des Einsatzes 4 vermindert wird.
  • An dem oben Beschriebenen können Abwandlungen und Verbesserungen vorgenommen werden, ohne den Schutzumfang der vorliegenden Erfindung zu verlassen.

Claims (9)

  1. Prothesenbaugruppe (1) umfassend: ein Befestigungselement (2) zur Anbringung am Knochen des Patienten; ein Lagerschalen-Bauteil (4) mit einem Rand (8), wobei ein Teil (10) des Randes (8) verstärkt ist und die Verstärkung (10) die Form einer Schulter hat, die sich allgemein in radialer Richtung vom Lagerschalen-Bauteil (4) aus erstreckt, wodurch ein Teil des Randes (8) mit größerer Breite entsteht, dadurch gekennzeichnet, daß das Lagerschalen-Bauteil (4) aus einem keramischen Material geformt ist.
  2. Prothesenbaugruppe (1) nach Anspruch 1, bei welcher das keramische Material Aluminiumoxid oder Zirkoniumoxid ist.
  3. Prothesenbaugruppe (1) nach einem der bisherigen Ansprüche, bei welcher das Befestigungselement (2) aus synthetischem Kunststoffmaterial oder aus Metall geformt ist.
  4. Prothesenbaugruppe (1) nach einem der bisherigen Ansprüche, bei welcher es zwischen dem Befestigungselement (2) und dem Lagerschalen-Bauteil (4) eine Kegelpassung gibt.
  5. Prothesenbaugruppe (1) nach Anspruch 4, bei welcher das Lagerschalen-Bauteil (4) eine Kegel-Außenfläche (28) aufweist, welche einen Außenkegel (16) bildet.
  6. Prothesenbaugruppe (1) nach Anspruch 5, bei welcher das Befestigungselement (2) eine Kegel-Innenfläche aufweist, welche einen Innenkegel (14) bildet, der dem Außenkegel (16) des Lagerschalen-Bauteiles (4) entspricht.
  7. Prothesenbaugruppe (1) nach einem der bisherigen Ansprüche, bei welcher der Rand des Lagerschalen-Bauteiles (4) eine im wesentlichen ovale Form hat.
  8. Prothesenbaugruppe (1) nach Anspruch 7, bei welcher die innere Lagerfläche (12) des Lagerschalen-Bauteiles (4) exzentrisch innerhalb der ovalen Form desselben liegt.
  9. Prothesenbaugruppe (1) nach einem der bisherigen Ansprüche, welche eine Hüftgelenkpfannen-Baugruppe ist.
