Description
Die Erfindung betrifft eine Hüftgelenkpfanne zur zementfreien
Implantation in einer Knochenausnehmung mit einer Vielzahl von
Löchern zur Verankerung einwachsender Knochensubstanz.
Es ist bekannt, die zementfreie Haftung eines Implantats am
Knochen dadurch zu verbessern, daß die Implantatoberfläche mit
einer Vielzahl von kleinen oder großen Ausnehmungen versehen
wird, in die später der Knochen einsprossen kann. Feinporige
Oberflächenstruktur wird beispielsweise in der DE-OS 26 14 169
empfohlen. Die meisten Entwicklungen bevorzugen grobere
Öffnungsstrukturen, die durch Hinterschneidungen oder hinter
einem durchbrochenen Bauteil liegende Erweiterungen gekenn
zeichnet sind, so daß das durch einen verengten Querschnitt
einwachsende Knochenmaterial sich in der dahinterliegenden
Erweiterung verankern kann. Beispielsweise empfiehlt die DE-OS
27 30 004 die Belegung der Prothesenoberfläche mit einer
Schicht von sphärischen Verankerungskörpern. Die DE-OS
29 32 744 offenbart eine Implantatoberfläche mit einer Viel
zahl von Vertiefungen, die sich hinter einer engen Stelle
erweitern. Die DE-OS 26 34 954 und DE-OS 28 51 598 empfehlen
einen rohrförmigen Prothesenschaft, der Bohrungen enthält. Die
DE-OS 28 42 847, DE-OS 24 04 214, DE-OS 27 58 541 und DE-GM
76 19 986 schlagen vor, um einen Prothesenschaft herum einen
von einem Lochblech oder einem Drahtgewebe gebildeten Mantel
zu bilden, wobei das Knochengewebe durch die Öffnungen dieses
Mantels hindurch wachsen und sich in dem weiteren Zwischenraum
zwischen dem Mantel und dem Prothesenschaft verankern kann.
Die DE-OS 21 38 881 offenbart ein offenporiges Gewebe, das am
Implantat zur Verankerung des Knochengewebes anvulkanisiert
ist. Schließlich ist es aus Ztschr. Orthopädie f. 1974, 27-33,
bekannt, das Implantat mit groben Rippen zu versehen, zwischen
die der Knochen einwachsen kann.
Infolge der Er
weiterung dieser Hafträume nach außen hin entstehen breitbasige,
poröse Annagelungsspitzen, so daß die gesamte Oberfläche ein
igelförmiges Aussehen erhält. Diese Spitzen werden beim Ein
bringen der Pfannenprothese in die spongiöse Knochenmasse
des Pfannenlagers eingetrieben, so daß eine feste primäre
Fixation vorliegt. Das einwachsende Knochenmaterial kann
sich entsprechend dem üblichen physiologischen Aufbau von
Knochen ausbilden, d. h. in den weiten Bereichen der Haft
räume als vielfach verstrebtes Fachwerk aus kleinen Knochen
bälkchen, von denen im weiteren Vordringen immer mehr in
die spongiösen Hohlräume der Prothesenwandung abgezweigt
werden, so daß im Idealfall nur noch ein Knochenbälkchen
vor der Zentralperforation an der inneren Grenzschicht endet.
Das bedeutet, daß die unregelmäßigen, porösen Knochen
pyramiden mit den Spitzen an der inneren Grenzschicht und
die unregelmäßigen, porösen Pfannenpyramiden mit den Spitzen
in entgegengesetzter Richtung sich zunächst grobmechanisch
verzahnen und dann aber auch auf Grund der Mikroporosität
beider Komponenten feinmechanisch durchdringen und verflechten.
Die Prothese ist auf Grund ihres Konstruktionsprinzips in
besonderem Maße geeignet, die an die heranwachsenden Knochen
bälkchen, die sich entsprechend den von mm2 zu mm2 wechseln
den Zug- und Druckverhältnissen verschieden organisieren,
mit ihren trichterförmigen Öffnungen aufzufangen und die
spezielle poröse Ausbildung ist zur Verankerung von Pfannen
prothesen mit besonderem Erfolg anwendbar. Infolge der Er
weiterung dieser Hafträume nach außen hin entstehen breitbasige,
poröse Annagelungsspitzen, so daß die gesamte Oberfläche ein
igelförmiges Aussehen erhält. Diese Spitzen werden beim Ein
bringen der Pfannenprothese in die spongiöse Knochenmasse
des Pfannenlagers eingetrieben, so daß eine feste primäre
Fixation vorliegt. Das einwachsende Knochenmaterial kann
sich entsprechend dem üblichen physiologischen Aufbau von
Knochen ausbilden, d. h. in den weiten Bereichen der Haft
räume als vielfach verstrebtes Fachwerk aus kleinen Knochen
bälkchen, von denen im weiteren Vordringen immer mehr in
die spongiösen Hohlräume der Prothesenwandung abgezweigt
werden, so daß im Idealfall nur noch ein Knochenbälkchen
vor der Zentralperforation an der inneren Grenzschicht endet.
Das bedeutet, daß die unregelmäßigen, porösen Knochen
pyramiden mit den Spitzen an der inneren Grenzschicht und
die unregelmäßigen, porösen Pfannenpyramiden mit den Spitzen
in entgegengesetzter Richtung sich zunächst grobmechanisch
verzahnen und dann aber auch auf Grund der Mikroporosität
beider Komponenten feinmechanisch durchdringen und verflechten.
Die Prothese ist auf Grund ihres Konstruktionsprinzips in
besonderem Maße geeignet, die an sie heranwachsenden Knochen
bälkchen, die sich entsprechend den von mm2 zu mm2 wechseln
den Zug- und Druckverhältnissen verschieden organisieren,
mit ihren trichterförmigen Öffnungen aufzufangen und die
entstehenden Knochenstrukturen miteinander zu verbinden und
damit den Vorgang zu simulieren, der sich natürlicherweise
in der Knochenkompakta vollzieht. Durch diese Berücksichtigung
der natürlichen Aufbauverhältnisse beim Knochen wird eine
quasi elastische Einbettung der Pfanne im Knochenmaterial
erreicht, wodurch der Gefahr eines Schrumpfens des Knochens
unter Druckbelastung weitgehend vorgebeut ist. Bei der Er
findung stellt die gesamte Pfannenwandung also eine Grenz
schicht dar, innerhalb der sich Prothesen- und Knochen
material gegenseitig durchdringen, wobei der Anteil an
Prothesenmaterial über die Wandstärke der Pfanne hinweg
gleichmäßig zugunsten des Knochenmaterials abnimmt. Als
Knochengrenzschicht funktioniert die Prothese um so besser
im Sinne der Dauerschwingfestigkeit, je mehr die Gesamt
elastizitätsverhältnisse von Pfannenmaterial und Pfannen
einbettung mit denen des Knochengewebes übereinstimmen.
Das Knochengewebe ist in dieser Beziehung jedoch kein
homogenes Gewebe, so daß es von vorne herein nicht möglich
ist, eine materialmäßig isoelastische Prothese herzustellen.
Es kann deshalb nur die Gesamtelastizität (aus Pfanne und
Pfanneneinbettung) mit der des Knochengewebes in Überein
stimmung gebracht werden, wobei die besondere Art der Kraft
einleitung im vorliegenden Falle als Regulativ dient. Das heißt,
je mehr die Elastizität des Pfannenmateriales zu dem des
Knochengewebes des Pfannenlagers kontrastiert, desto mehr
werden sich die in die Prothese einsprießenden Knochen
bälkchen im ganzen oder abschnittweise verstärken oder
abschwächen.
Die porige, siebförmige oder filterhafte Auflockerung der
Prothesenwandung schließlich macht diese für Stoffwechsel
vorgänge in beiden Richtungen durchlässig, so daß insbe
sondere die Gewebsflüssigkeit sich nicht wie bei den ge
schlossenen Prothesen in einem bindegewebigen Granulations
saum um die Prothese herum einen Weg suchen muß. Es fällt
also diese Gewebsschicht weg, die nicht tragfähig ist und
als Ursache und Ausgangspunkt vieler Prothesenlockerungen
angesehen werden muß.
Die Gelenkpfanne kann entweder aus einem einheitlichen
Material oder aus mehreren chemisch oder physikalisch ver
bundenen Komponenten bestehen. Das Material ist als ge
eignet anzusehen, wenn es über folgende Eigenschaften
verfügt: Biokompatibilität, Festigkeit, Gleit-Reibeigen
schaften, Mikro- und Makroporosität und Elastizitäts-Modul
nach dem neuesten Standard.
Als Ausgestaltung der Erfindung kann der Pfannenkörper aus einem
porösen Keramik-, Porzellan- oder Kohlenstoffmaterial,
Kunststoff, Metall, z. B. Titan oder Legierungen, bestehen,
dem durch entsprechende Techniken eine schwammartige
Porosität, d. h. ein offenes Porennetz verliehen ist. Aus
Stabilitätsgründen kann es sich dabei empfehlen, den
Pfannenkörper wenn nötig mit einem Bewehrungsnetzwerk aus
einem Metallgitter oder aus einem Kohlenstoffadengespinst,
-geflecht, -gewebe oder -gewirke zu versehen.
Die Hüftgelenkpfanne nach der Erfindung wird im folgenden anhand
mehrerer in der Zeichnung schematisch dargestellter Aus
führungsbeispiele erläutert. In der Zeichnung zeigt
Fig. 1 teils in Draufsicht, teils im Schnitt ein erstes
Ausführungsbeispiel einer porösen Hüftgelenkpfanne,
Fig. 2 in vergrößertem Maßstab einen Detailquerschnitt
der Pfanne nach Fig. 1 und
Fig. 3 und 4 jeweils in einem Teilquerschnitt ein
weiteres Ausführungsbeispiel.
Die Gelenkpfanne nach Fig. 1 ist einstückig aus einem porösen
Material, z. B. einer porösen Keramikmasse gefertigt und be
sitzt eine Vielzahl von Durchgangslöchern in Form von Poren 1,
die beidseits offen sind. Die Porigkeit, d. h. das spezifische
Porenvolumen nimmt von innen nach außen stark zu, was in
Fig. 1, links, durch die starke Erweiterung der einzelnen
Poren 1 nach außen hin und in Fig. 1, rechts, durch das
dunkle, zusammenhängende Lochbild versinnbildlicht ist. An
ihrer Innenseite ist die Prothese entsprechend ihrer Lager
funktion glatt geschliffen und poliert und kann zweckmäßiger
weise auch noch mit einer sehr verschleißfesten Keramikoxid
schicht 2, z. B. in einem Aufdampf- oder Plasmaspritzverfahren
versehen sein, allerdings unter Offenlassung der inneren,
kleinen Porenöffnungen 3. In die relativ weiten äußeren
Öffnungen 4 der Poren 1 wächst allmählich Knochenmaterial
ein, das mit der Zeit den gesamten Pfannenkörper durchdringt
und bis an die innere Lagerfläche heran sich ausbilden kann.
Beim Ausführungsbeispiel nach Fig. 3 ist der Pfannenkörper
mit einem Bewehrungsnetzwerk aus einem Kohlenstoffadenge
flecht 5 versehen, welches die mechanische Festigkeit des
Pfannenkörpers erhöht. Fig. 4 zeigt ein ähnliches Ausführungs
beispiel, bei dem der Pfannenkörper mit einem Drahtgitter
aus Längs- und Querdrähten 7, 8 bewehrt ist.
Claims (3)
1. Hüftgelenkpfanne zur zementfreien Implantation in einer
Knochenausnehmung mit einer Vielzahl von Löchern zur
Verankerung einwachsender Knochensubstanz, dadurch ge
kennzeichnet, daß die Löcher (1) sich von innen nach außen
zur Bildung einwachsender Knochenpyramiden stark erwei
tern und das zwischen den Löchern (1) befindliche Pfannenmaterial
die Form breitbasiger Pyramiden mit nach außen gewendeten
Spitzen aufweist, wobei die Löcher (1) auf der Pfannen
innenseite feine Perforationen und die Material-Pyramiden des
Pfannenmaterials auf der Außenseite stachelartige
Spitzen bilden.
2. Hüftgelenkpfanne nach Anspruch 1, dadurch gekennzeichnet,
daß das Pfannenmaterial porös ist.
3. Hüftgelenkpfanne nach Anspruch 1 oder 2, dadurch gekenn
zeichnet, daß die Pfanne in Abstand von ihrer knochenseitigen
Oberfläche ein Bewehrungsnetz enthält.
