Description

  • Gelenkkopf für eine Kugelgelenkprothese
  • Die Erfindung betrifft einen Gelenkkopf für eine Kugelgelenkprothese, der über eine konische Steckverbindung auf einem Verankerungsschaft aufgesteckt ist, wobei ein Mutterkonus des Gelenkkopfes den Zapfen des Verankerungsschaftes aufnimmt und zwischen beiden ein Hohlraum verbleibt.
  • Gelenkköpfe, die in der genannten Art auf dem Verankerungsschaft befestigt sind, sind - vor allem für Hüftgelenkprothesen - bekannt (CH-PS 568 753). Weiterhin ist bekannt, dass die als "Fremdkörper" wirkenden Prothesen nach der Implantation in relativ kurzer Zeit von bindegewerbeartigem Narbengewebe umwachsen und abgekapselt werden. Dieses Gewebe umschliesst dabei den Gelenkkopf relativ dicht; neben der Gelenkprothese ist der Raum innerhalb des Gewebes mit Körperflüssigkeit gefüllt, die gleichzeitig als Schmierflüssigkeit für das Gelenk dient, besonders bei Fraktur-oder Teilprothesen, bei denen nur der Femurkopf durch einen künstlichen Gelenkkopf ersetzt wird, der in der natürlichen Hüftgelenkspfanne gelagert wird. Die Praxis hat nun gezeigt, dass sehr häufig das für die Flüssigkeit zur Verfügung stehende Volumen innerhalb des Narbengewerbes zu gering ist, um eine für die Schmierung des Gelenkes notwendige Flüssigkeitsmenge zu speichern.
  • Aufgabe der Erfindung ist es, das für Flüssigkeiten innerhalb des von Narbengewerbes eingeschlossenen Raumes zur Verfügung stehende Volumen zu vergrössern. Dieses Ziel wird erfindungsgemäss dadurch erreicht, dass der als Kugelschale ausgebildete Gelenkkopf schirmartig auf einer den Mutterkonus enthaltenden Konusbüchse sitzt, wobei zwischen der Innenwand der Kugelschale und der Konusbüchse ein freier Ringraum ausgebildet ist.
  • Durch diese Massnahme wird gegenüber den bisherigen Konstruktionen nicht nur das Volumen für die "Schmier"-Flüssigkeit vergrössert, sondern auch die Masse an körperfremden Material im lebenden Gewebe verringert, wodurch gleichzeitig das Gewicht des Gelenkkopfes reduziert wird.
  • In neuerer Zeit wird bei den Implantationen sehr häufig der Gelenkkopf erst während der Implantationsoperation auf den Zapfen des bereits in den Femurknochen eingesetzten Verankerungsschaftes aufgesteckt, um eine Anpassung an den individuellen Patienten zu erreichen; diese Anpassung erfolgt durch Auswählen eines Gelenkkopfes aus einer Serie, bei der die Lage des Gelenkkopfmittelpunktes relativ zu dem Mutterkonus variiert ist. Sehr häufig ist es bei der Operation nicht möglich, in dem Gelenkkopf den Hohlraum für den Mutterkonus frei von biologischen Verunreinigungen zu halten, was unter Umständen zu Gewebezersetzungen im Innern des Hohlraumes und - bei metallischen Gelenkköpfen -zu Materialkorrosionen führen kann. Diese Schwierigkeiten können vermieden werden, wenn aus dem Hohlraum mindestens zwei Durchtrittsöffnungen durch die Konusbüchse hindurch in den Ringraum führen, so dass der im Mutterkonus verbleibende Hohlraum dauernd von Körperflüssigkeit durchspült wird.
  • Als Materialien für den neuen Gelenkkopf, der mit seiner Konusbüchse aus einem Stück besteht, eignen sich alle in der Implantat-Technik üblichen und bewährten Werkstoffe: also z.B. Metalle, Metall-Legierungen, Kunstoffe, unter Umständen faserverstärkt, keramische Materialien und pyrolytischer Kohlenstoff.
  • Im folgenden wird die Erfindung anhand eines Ausführungsbeispiels im Zusammenhang mit der Zeichnung näher erläutert.
  • Fig. 1 zeigt einen erfindungsgemäss ausgebildeten Gelenkkopf auf einem Verankerungsschaft einer Hüftgelenkprothese, wobei der Gelenkkopf im Schnitt dargestellt ist; Fig. 2 und 3 sind die Schnitte II-II und III-III von Fig. 1.
  • Ein Verankerungsschaft 1 einer Hüftgelenkprothese, der zum Einsetzen in den Femur bestimmt ist, trägt an seinem oberen Ende einen konisch zulaufenden Zapfen 2. Auf diesen ist mit einem entsprechenden Mutterkonus 3 ein Gelenkkopf 4 aufgesetzt.
  • Der Gelenkkopf 4 besteht aus einer relativ dünnen, schirmartigen Kugelschale 5, an die als "Stiel" eine Konusbüchse 6 angesetzt ist. Die Konusbüchse 6 nimmt den Mutterkonus 3 auf, der bei einzelnen Gelenkköpfen 4 unterschiedlich "tief" in der Konusbüchse 6 angeordnet ist, um - wie erwähnt - den Abstand, den die Kugelschale 5 in Richtung der Achse des Zapfens 2 von einer willkürlich gewählten Bezugslinie 7 hat, variieren zu können.
  • Der schirmartige Aufbau des Gelenkkopfes 4 ergibt im Innern der Kugelschale 5 einen freien Ringraum 8, der nach unten offen ist und nach der Implantation im wesentlichen von Narbengewebe abgeschlossen wird.
  • Auch bei auf dem Zapfen 2 aufgestecktem Gelenkkopf 4 verbleibt zwischen der Kugelschale 5 und dem Zapfen 2 im Innern der Konusbüchse 6 ein Hohlraum 9, der über kanalartige Durchtrittsöffnungen 10 mit dem Ringraum 8 in Verbindung steht, so dass aus dem Ringraum 8 durch die Oeffnungen 10 und den Hohlraum 9 zurück in den Ringraum 8 dauernd Körperflüssigkeit fliessen kann, um so eine Spülung des Hohlraumes 9 zu erreichen. Das Vorhandensein des Ringraumes 8 stellt sicher, dass Innerhalb der erwähnten, jedoch nicht dargestellten Kapsel aus Narben- oder Bindegewebe, das die Prothese als Fremdkörper im lebenden Gewebe umschliesst, ein für eine derartige Spülung ausreichende Flüssigkeitsmenge zur Verfügung steht.

Claims (2)

  1. Patentansprüche 1.) Gelenkkopf für eine Kugelgelenkprothese, der über eine konische Steckverbindung auf einen Verankerungsschaft aufgesteckt ist, wobei ein Mutterkonus des Gelenkkopfes den Zapfen des Verankerungsschaftes aufnimmt und zwischen beiden ein Hohlraum verbleibt, dadurch gekennzeichnet, dass der als Kugelschale (5) ausgebildete Gelenkkopf (4) schirmartig auf einer den Mutterkonus (3) enthaltenden Konusbüchse (6) sitzt, wobei zwischen der Innenwand der Kugelschale (5) und der Konusbüchse (6) ein freier Ringraum (8) ausgebildet ist.
  2. 2. Gelenkkopf nach Anspruch 1, dadurch gekennzeichnet, dass aus dem Hohlraum (9) mindestens zwei Durchtrittsöffnungen (10) durch die Konusbüchse (6) hindurch in den Ringraum (8) führen.
