Prothetisches Schultergelenk
Die Erfindung bezieht sich auf ein prothetisches Scnultergelenk, das zum vollständigen Ersetzen eines durch Krankheit
oder Verwundung angegriffenen Schultergelenks bestimmt ist
Mit den üblichen Schultergelenkprothesen ist noch kein erfolgreicher Versuch bekanntgeworden, der ein vollständiges
Ersetzen eines Schultergelenks zuließe. Die erhältlichen Prothesen umfassen Oberarmknochenprothesen, nicht jedoch
zugehörige prothetische Gelenkpfannen. Darüber hinaus erfordert das Einpflanzen der bekannten Oberarmknochenprothesen gewöhnlich
einen Samen der Schwenkmuskeln, und die eingesetzte Prothese kann sich verschieben oder versetzen.
Der Erfindung liegt die Aufgabe zugrunde, diese Situation dadurch zu erleichtern, daß eine prothetische Gelenkpfanne
geschaffen wird, die mit einer Oberarmknochenknochenprothese zusammenwirkt.

Bei einem prothetischen Schultergelenk gemäß der Erfindung ist; vorgesehen, daß von angenähert einer Hälfte der äußeren
Oberfläche einer im wesentlichen becherförmig ausgebildeten, künstlichen Gelenkpfanne mit dieser starr verbundene oder
einstückige Befestigungsmittel wegragen.
Die prothetische Gelenkpfanne ist normalerweise mindestens auf ihrer Innenseite im wesentlichen halbkugelförmig ausgebildet.
Bei einer bevorzugten Ausführungsform der Erfindung umfassen die Befestigungsmittel eine Vielzahl von divergierenden stiftartigen,
sowohl langen als auch kurzen Gliedern. Die langen stiftartigen Glieder ragen von einer Folge von benachbart der
gang s radialen Mittelebene der einen Hälfte gelegenen Auspunkten weg
und divergieren von dieser Ebene im gleichen Sinne.
Vorteilhafterweise werden die langen Befestigungsteile zum Rad der Gelenkpfanne hin zunehmend langer und divergieren
von der Mittelebene weg zunehmend stärker.
Die kurzen stiftartigen Glieder sind beidseits der langen
einen Glieder auf der yGelenkpfannenhälfte gelegen.

Innendurchmesser von etwa 19 mm und einen Außendurchmesser von etwa 25 mm, während die längeren stiftartigen Glieder
etwa 12 bis 25 mm lang sind und um 9° bis 13° gegenüber der Mittellinie geneigt sind und die kürzeren stiftartigen Glieder
etwa 3 mm lang sind»
Als Befestigungsmittel müssen nicht stiftartige Glieder verwendet werden. Beispielsweise können die längeren Stifte durch
einen zweckmäßig gestalteten Stützträger und die kürzeren Stifte durch Rippen ersetzt sein.

Eine zum Zusammenwirken mit der Gelenkpfanne bestimmte Oberarmknochenprothese kann jede geeignete Form haben, sofern
sie nur ein kugelförmiges Kopfteil aufweist, das bei eingepflanzter Prothese mit der Gelenkpfanne ein geeignetes Gelenk "
bildet.
Eine bevorzugte Oberarmknochenprothese umfaßt ein intrameduBär
(im Knochenmark) zu befestigendes längliches Befestigungsteil sowie ein Kopfteil mit einem Kugelkopf, wobei die Längsachse
des Befestigungsteils um etwa 45 ° zu derjenigen des Kopfteils geneigt ist.
Die Erfindung sowie vorteilhafte Einzelheiten der Erfindung sind im folgenden anhand schematischer Zeichnungen ,
an einem Ausführungsbeispiel näher erläutert.

Die Gelenkpfanne nach der Erfindung umfaßt ein im wesentliches halbkugelförmiges becherförmiges Teil 1 und eine
Vielzahl von divergierenden stiftartigen Teilen oder Stiften 2 bis 6 in der oben beschriebenen bevorzugten Ausgestaltung.
Die'stifte 2 bis 6 erstrecken sich im wesentlichen von einer
Hälfte der äußeren Oberfläche des . becherförmigen Teils,die von der anderen Hälfte durch eine diametrale Ebene in Richtung
des Pfeils A in Fig. 1 abgeteilt ist. Dabei erstrecken sich
Punkte die längeren Stiften 2, 3 und 4 von einer Folge benachbarter /
einer sich in Richtung des Pfeils B erstreckenden radialen Mittelebene und divergieren von dieser Mittelebene im gleichen
Sinne. Die kürzeren Stifte 5 und 6 sind auf entgegengesetzten Seiten der Stiftreihe mit den langen Stiften angeordnet. Die
beiden Stifte 2 und 3 sind etwa gleich lang und bezüglich der radialen Mittelebene etwa um dieselben Winkel geneigt, während
der nächst dem Rand des becherförmigen Teils gelegene Stift 4 länger ist und eine größere Neigung zu der radialen Mittelebene
aufweist als die Stifte 2 und 3.
Weiter ins einzelne gehende Angaben zu der dargestellten Gelenkpfanne ergeben sich aus der folgenden Maßtabelle, wobei
jedem Maß ein auch in den Zeichnungen angegebenes Bezugszeichen mit einer Zahl und einem Buchstaben.zugeordnet ist, wobei die
Zahladie zugehörige Figur und der Buchstabe deu in der Figur
bezeichneten Maß angibt, und die Maßangaben in Millimetern angegeben sind, sofern sie nicht mit der Bezeichnung für

Die dargestellten Gelenkteile werden zweckmäßig aus einer Kobalt-Chromlegierung hergestellt, welche üblicherweise
für derartige Prothesen verwendet wird. Die Gelenkpfanne kann ebenso jedoch auch aus Titan hergestellt sein und eine
hochfeste Polythen-Einlage als Lagereinsatz aufweisen. Der Lagereinsatz hat ebenfalls zweckmäßigerweise eine halbkugelförmige
Gestalt und ist mittels eines in einer Ringnut am Rand des becherförmigen Teils sitzenden Sprengringes gehalten.
Die Gelenkprothesenteile können aus jedem zweckmäßigen Material, sei es Metall, Kunststoff, ein Keramikwerkstoff,
eine Legierung oder eine Kombination davon, hergestellt sein. In den Fig. 16 und 17 ist gezeigt, in welcher Weise das
prothetische Schultergelenk eingepflanzt ist.
Die natürliche Gelenkpfanne wird unter Entfernung spongiöser Knochenteile ausgehöht, so daß die künstliche Gelenkpfanne
in der richtigen Stellung einzementiert oder vergossen werden kann, z.B. mit einem Acryl-Harz oder einem anderen
zweckmäßigen Bindewerkstoff. Bei richtigem Sitz umschließt die Gelenkpfanne einen Winkel von etwa 60° zur Horizontalebene und von oben gesehen zur Ebene der Schultergelenkaushöhlung
einen Winkel von 10 bis 13° ein.
Die Oberarmknochenprothese ist in gleicher Weise in ein im Oberarmknochenmark einpräpariertes Loch einzementiert oder
eingegossen und der Kugelkopf in der dargestellten Weise in der Gelenkpfanne aufgenommen.

Claims (13)
1. ' Prothetisches Schultergelenk, dadurch gekennzeichnet , daß von angenähert einer Hälfte der
äußeren Oberfläche einer im wesentlichen becherförmig ausgebildeten, künstlichen Gelenkpfanne (1) mit dieser starr
verbundene oder einstückige Befestigungsmittel (2-6) wegragen.
2. Prothetisches Schultergelenk nach Anspruch 1, dadurch gekennzeichnet , daß die Gelenkpfanne (1)
mindestens auf ihrer Innenseite im wesentlichen halbkugelförmig ausgebildet ist.
3. Prothetisches Schultergelenk nach Anspruch 1 oder 2, dadurch ge. kennzeichnet , daß die Befestigungsmittel
(2-6) verhältnismäßig lange Befestigungsteile (2-4l
umfassen, die von einer Folge von benachbart der radialen Mittelebene der einen Hälfte gelegenen Ausgangspunkten wegragen
und von dieser Ebene in gleichem Sinne divergieren.
4. Prothetisches Schultergelenk nach Anspruch 3» dadurch gekennzeichnet , daß die langen Befestigungsteile
(2-4) zum Rand der Gelenkpfanne (1) hin zunehmend langer werden und von der Mittelebene weg zunehmend stärker divergieren.
5. Prothetisches Schultergelenk nach Anspruch 4, dadurch gekennzeichnet , daß die langen Befestigungsteile
(2-4) eine kürzeste Länge von etwa 12mm und eine größte Lange von etwa 25 mm, eine kleinste Neigung von etwa 9° und
eine größte Neigung von etwa 13 haben.

1 bis 10, dadurch gekennzeichnet , daß die Gelenkpfanne zwei lösbar miteinander verbundene Teile umfaßt,
wobei das äußere Teil aus Metall und das innere Teil aus Kunststoff besteht.
12. Prothetisches Schultergelenk nach einem der Ansprüche 1 bis 11, gekennzeichnet -durch eine Qberarmkaochenprothese
(Fig. 12, 13) mit einem intraraedullär zu befestigenden, länglichen Befestigungsteil sowie einem Kopfteil
mit einem Kugelkopf, wobei die Längsachse des Befestigungsteils um etwa 45° zu. derjenigen des Kopfteils geneigt ist.
13. Prothetisches Schultergelenk nach Anspruch 12, dadurch gekennzeichnet , daß mindestens das Kopfteil
der Oherarmknochenprothese aus Metall hergestellt ist.
