Abstract

Eine in den Operationstisch integrierte Einrichtung zur Erwärmung von Patienten während operativer Eingriffe, dadurch gekennzeichnet, dass, über die durch Wasser erwärmte, IR und Röntgentransparente Auflagefläche (1) im Bereich des Rumpfsegmentes des OP-Tisches hinaus, mit Infrarotstrahlen der Wellenlängen 780–1400 nm (IR-A, „nahes” IR-Licht) zusätzlich Wärmeenergie in die Haut des Patienten eingebracht werden kann.

Description

  • Das Wasserkissen (1) hat drei wichtige Funktionen:
    Die gleichmäßige Druckverteilung,
    die Kontrolle der Hautoberflächentemperatur
    und eine Filterwirkung zum Hautschutz (Wasserfilter nach Manders).
  • Dazu wird das Wasser, in dem transparenten Kissen aus IR- und Röntgentransparentem Material, permanent umgewälzt, dabei kann, z. B. durch Thermoelemente (Peltier) nach Bedarf und thermostatisch kontrolliert, erwärmt oder gekühlt werden, wodurch auf einen Festwasseranschluß verzichtet werden kann.
  • Um die Oberfläche des Polsters vor Verschmutzung zu schützen wird eine Packung von Abreißfolien aufgelegt, wobei vor jedem Patientenwechsel die oberste Folie einfach abgezogen werden kann.
  • Die Kissenauflage (2) muss aus ebenfalls Röntgen- und IR-transparentem Material bestehen, entweder aus Kunstoffplatten oder einem hochfesten, elastischen Netzverbund (z. B. Polyethylenfasern).
  • Die IR-Strahler (3) können aus einfachen Glühwendelstrahlern („Glühbirnen”) bestehen, da hierbei das gewünschte IR-Spektrum mit emittiert wird. Die gruppenweise Anordnung erlaubt eine subtile Steuerung der applizierten Energiemenge durch individuelle Schaltzeiten. Dabei müssen die vorgegebenen Zyklen, aus Sicherheitsgründen, durch einen sog. „Totmannschalter” (deadmen-switch) für jeden Zyklus erneut bestätigt werden.
  • Ein (4) Schwenkmechanismus (z. B. Scherengitter) kann die Strahler so nach außen gegen den Tischrahmen (5) schwenken, daß der Tisch auf „Beckenbreite” für intraoperative Röntgenkontrollen zur Verfügung steht.
  • Die untere Abschlussplatte (6) muss röntgentransparent sein, schützt die Strahler vor Verschmutzung und Beschädigung ist aber kein zwingender Bestandteil der Konstruktion.

Claims (6)

  1. Eine in den Operationstisch integrierte Einrichtung zur Erwärmung von Patienten während operativer Eingriffe, dadurch gekennzeichnet, dass, über die durch Wasser erwärmte, IR und Röntgentransparente Auflagefläche (1) im Bereich des Rumpfsegmentes des OP-Tisches hinaus, mit Infrarotstrahlen der Wellenlängen 780–1400 nm (IR-A, „nahes” IR-Licht) zusätzlich Wärmeenergie in die Haut des Patienten eingebracht werden kann.
  2. Dazu kann das Wasserpolster (1) über eine integrierte Umwälzpumpe und Thermoelemente (Peltier-Prinzip) aktiv temperaturgesteuert werden, wodurch ein Wärmemanagement, von der Vorwärmung des Polsters und der aktiven Kühlung zur Unterstützung von operativen Eingriffen in künstlicher Unterkühlung (Hypothermie) ermöglicht wird.
  3. Damit bei röntgenkontrollierten Eingriffen der Strahlengang durch den Tisch nicht gestört wird, können die Strahler über einen Schwenkmechanismus (4) (z. B. Scherengitter) nach den Seiten wegbewegt werden.
  4. Die Steuerung der Strahler (Glühwendelstrahler, z. B. 20 W 24 V Glühbirnen) erfolgt über getaktetes Aus- und Einschalten, wobei ein sicherer Zyklus mit definierter elektrischer Leistung (= Wärmemenge) vorgegeben ist. Ein erneuter Zyklus kann nur durch Betätigung eines Schalters durch den Anästhesisten (deadmen-switch) ausgelöst werden. Wasser und Hauttemperatur, sowie ggf. (bei Gefäßoperationen) die Temperatur der unteren Extremitäten sollten in Echtzeit dokumentiert werden.
  5. Zum Schutz des Auflagepolsters vor Verschmutzung wird ein Paket von 5–10 sehr dünnen, transparenten, elastischen („Abreiß-”)Folien zu Betriebsbeginn über den Polstern angebracht, wobei, nach dem Eingriff, nur jeweils die oberste Folie abgezogen werden muß.
  6. Durch die integrierte Auslegung wird außerhalb des Tisches keine zusätzliche Wärmebelastung des OP-Ambiente erfolgen, die Raumtemperatur kann sogar gesenkt werden.
