Abstract
Description
The present invention relates to a tube for insertion into a patient's trachea, with a central lumen for ventilation or for breathing and with at least one inflatable cuff provided on the outside of the tube, which is the seal serves the outside of the tube against the wall of the trachea.
Such a tube can be a tracheotomy tube, which is cut through a trachea or an endotracheal tube that is inserted through the mouth or nose Ventilation of an anesthetized patient or in a coma.
Such tubes are used very frequently in intensive care medicine. A special Problem, especially with long ventilation with the help of an appropriate tube, is in the development of pneumonia, which is comparatively common in intensely cared for patients occur and apparently have some connection with this type of ventilation.
When using a corresponding tube, this is, as already mentioned, in the A patient's trachea inserted and positioned so that the area of the tube, which is equipped on the outside with the inflatable cuff in the area of the Trachea and generally located just below the larynx. About one separate hose or a line integrated into the wall of the tube is then the Inflated cuff, the pressure in the cuff quite a few tens of millibars can be. This causes the cuff to inflate and touches its outer wall the inner surface of the trachea. However, this will make the regular transportation of secretions along the wall of the trachea (from bottom to top) and moreover is blocked by the Foreign bodies may strongly stimulate secretion formation. The forming and to Examples of secretions accumulating above the cuff in the area around the tube are with the conditions of temperature and humidity there you can see good nutrient media for various types of bacteria and other pathogens. You could believe that a lung infection is nevertheless prevented because the cuff is tight bears against the wall of the windpipe and thus prevents that with strong pathogens Enriched secretion, which is at the outward end of the cuff accumulates, can not penetrate the lower bronchial tubes, but it has emphasized that pneumonia occurs again and again, which leads to a considerable Part are due to the use of such a tube.
In particular, studies seem to show that the cuffs, despite their flexibility and mobility and despite inflating to relatively high pressures, which at least a few tens may be millibars, but not lie close to the wall of the trachea and / or also tend to form wrinkles, these folds being a channel for secretions and Form disease germs, which can then lead to serious infections of the lungs.
It has already been proposed, especially to avoid such wrinkling to use thin-walled and / or particularly elastic cuffs, but these have Cuffs have the problem that they are relatively sensitive and are very easily damaged can, in which case they completely lose their sealing and holding function.
Alternatively, you could try increasing the pressure in these cuffs, which However, under certain circumstances it could lead to excessive stress on the windpipe Surface cells (mucous membrane) due to the high pressure to which they would then be exposed may perish, possibly including deeper necrosis including the Cricoid cartilage can arise.
Compared to this prior art, the object of the present invention is to create a tube with an appropriate inflatable cuff that addresses the problem the inadequate seal is eliminated and yet is neither particularly sensitive nor requires particularly high inflation pressures.
This object is achieved in that the cuff on its outer surface Has coating which is swellable and / or limited flow.
If you put such a coating after the cuff in a patient's trachea has been inflated or inflated, causes swelling, so through the swelling material any leaks that may still be present like, closed. In particular, the coating material that swells up possibly located in any folds and the like so that these folds no longer form channels through which anything else can pass the cuff can. Even if the coating is not necessarily swellable, but at least limited flowable, a similar effect can be achieved, namely on mechanically stressed Places, for example along the crease lines or the outer surfaces pressed against each other in the area of folds or the like, which displaces the coating material and is distributed to other, non-binding areas. This also creates emerging Folds filled with the coating material, so that the formation of any Transport channels in the longitudinal direction of these sleeves can be prevented.
It is understood that the coating material adheres sufficiently well to the Material of the cuff must not have to, due to the small pressure differences that between the two ends of the cuffs in the trachea can occur from the sealing surface to be displaced into the end face areas of the cuff, so that the sealing function could be lost.
For the same reason, the material should not be too free-flowing and could be used Example in the state of use the consistency of a gel or a cream, possibly also one very viscous oil. A viscosity of the material at body temperature is preferred tur (37 ° C) of at least 10 Pa.s (Pascal second), preferably of more than 40 Pa.s.
The coating material can also be more or less one unit with the material of the Form the cuff by, for example, partially coating the coating material into the wall of the Cuff is diffused or diffused or by a chemical bond with the material of the cuff. Furthermore you can targeted thin binding layers are applied to the cuff material in turn adhere well to the cuff material, but at the same time also a good one Provide an adhesive base for the sealing coating. The transition from that though elastic but nevertheless tear-resistant and practically non-flowable material So cuff to the coating material can be more or less fluid, what the adhesion properties of the coating material to the sleeve material verbes sert. The coating material can also be one in which the normal Storage at room temperature does not cause swelling or significant Flowability (in the range between 1 and 10 Pa.s) is present. The swelling ability or fluidity can be triggered, for example, by moisture and heat, in particular due to the moisture and Temperature conditions, but also by reagents, by catalysts, by Irradiation with X-rays, UV radiation, light or ultrasound or through other mechanical treatment. Otherwise, reagents, catalysts and Irradiation or mechanical treatments of the aforementioned type also used for this a good connection of the coating to the base material of the membrane achieve.
An embodiment of the invention is particularly preferred in which the coating material is swellable with water, the swollen coating material also limited can be flowable. The wall of the cuff itself is preferably made of a polymer and is relatively thin if possible, i.e. H. the thickness of the wall should preferably no longer be than 500 µm, better not more than 200 µm and particularly preferably not more than 30 µm.
Very diverse substances can be considered as coating materials, in particular Mucilage and slime formers or gelling agents, as well as foaming agents. in the Individuals can use these non-ionic mucilages, such as Salep Mannan, Guaran, Carobin, starch from wheat, corn etc. xanthan gum, chitosan, collagen, carmellose, Hypromellose, macrogols, etc., as well as anion-active slime formers for example gum arabic, tragacanth, karaya gum, pectins, alginic acid, carragen, agar, Agarose, especially rehydrable agarose gels, gelatin type B and carboxymethyl cellulose in question. Cation-active mucilages (gelatin type A) can also be used for the coating be used. Furthermore, mucilages from drugs, such as Radix Althaeae, Semen Lini, Semen Psylli, Semen Foenugreci, Semen Isphaghulae, seed pods from Plantago Ovata, inorganic gelling agents as well as water-swellable gels made of synthetic gelling agents Origin, their modification products and combinations of all of the aforementioned substances. Of further polymethacrylic acid and its salts, polyacrylic acid and its salts, polydimethylsiloxa ne and mixtures thereof in combination with glycerin, waxes, fats or petroleum jelly, as well as emulsifiers. Another important group of coating materials for the present invention are polyethylene glycols.
Furthermore, the coatings can be made from auxiliary substances such as association colloids (amphiphilic, non-ionogenic, anionic or cationic in nature) and / or solubilizers (ionogenic or non-ionic in nature). Several coatings can also be applied to one another be provided, which may be the desired sealing property on its own not yet have, but the mechanical load on the double layer mix or react with each other and thereby form a layer that the desired properties.
Basically, all water-swellable gels, natural or synthetic substances, Polymers or combinations thereof in question. Thixotropes are also particularly suitable Substances, d. H. Materials that change their flow behavior under load, i.e. H. for larger ones Load and are easier to flow when shear forces occur than with less Burden. The foaming agents already mentioned can also have the property of Ab improve sealing layer.
Regarding these foaming agents mentioned above, it should be noted that small Bubbles that form in an appropriate layer between the cuff and the wall of the Form trachea, significantly impede any liquid transport. This depends together with the relatively large capillary forces or surface tensions, which at the Bubble surface of foam occur, so that liquid secretion by appropriate Foam bubbles are bound due to these adhesive forces and the further flow of Prevent secretions.
Polymeric foams are also particularly suitable for the coatings according to the invention and in particular those foams which, if possible, consist of the same or a very similar basic material exist as the wall of the membrane itself on the one hand very good connection of the coating consisting of a foam material and, on the other hand, the foam layer particularly prevents, as already mentioned effectively any secretion flow between the outer wall of the membrane and the wall the trachea, as well as in the channels formed in folds of the membrane. Here are before Open-pore foams or foam layers are also very suitable. The foam layers also have the advantageous property that they are very compressible and insofar as they are well suited to fill in the cavities forming in folds during the Foam is strongly compressed and compressed in some fold areas, it can in areas with larger cross sections, typically at the bottom of corresponding folds arise, remain extended and thus completely fill the resulting cavity.
As an alternative to the water-swellable gels, hydrophobic, spreadable preparations can also be used tion forms with a sufficient viscosity can be used, for example coal hydrogen gels, such as lipogels or silicone gels, such as those used as solution, suspension, emulsion, or suspension emulsion lipogels are known. Likewise, stearate ointments or Creams suitable as coatings. It is useful if the coating at Room temperature is relatively firm and poorly fluid, but melts at body temperature or becomes softer.
The cuffs should, if possible, be wrinkle-free when inflated and nestle against the wall of the trachea, with particularly thin-walled cuffs from Are advantage. The pressures applied should be well below 1 bar.
Also polymer materials that have a glass transition point at a temperature above 30 ° C, are suitable as coating materials.
In a further preferred embodiment of the invention it is provided that the Cuff consists of an elastic material and is dimensioned such that it is not in the inflated state practically without folds and relatively close to the outer wall of the tube abuts while inflated under an inflation pressure between 20 and 30 millibars has a diameter to be measured perpendicular to the axis of the tube, which is at least 50% larger than the outer diameter of the tube or the previous one Diameter of the cuff when not inflated, this diameter in inflated condition can also be 100% larger.
By using an appropriately elastic material, which is already used for one relatively low inflation pressure of a few tens of millibars, an increase in the cuff radius If you experience 50% or more, you can see that the cuff is wrinkle-free Ensure that the cuff is inflated, as the cuff is already in the uninflated state is essentially wrinkle-free. This also prevents the occurrence of the above described, especially along folds occurring channels, along which possible wise secretions from the area above the cuff to the area below the Cuff can penetrate. Under certain circumstances, this embodiment could also be entirely without an additional coating agent on the outer surface of the cuff be used and still close tightly.
In combination with cuffs, which are made of polymer material, there are coatings gene from water-swellable gels from gel formers with non-covalent linkage of the Polymer chains (in the manner of homopolar half-valence gels, secondary valence gels or through Coulomb forces cross-linked substances etc.) have been found to be very suitable.
Cremes).Even coatings made from water-free, spreadable preparation forms, but water can record, then swell and for example from natural or syn thetic substances or polymers and / or combinations consist of them proven to be well suited. Another group of suitable coatings consists of hydrophobic, spreadable preparation forms with a viscosity of 2 Pa.s and more (for example hydrocarbon gels such as lipogels or silicone gels in the form of solution, Suspension, emulsion or suspension emulsion lipogels and stearate ointments or Creams).
Generally, most of the coatings in question can be divided into at least one classify into the following groups, with the relevant properties of course should remain permanently in the trachea.
B. 40 Pa.s oder mehr.a) The viscosity of the coating (in the state inserted in the trachea and under the prevailing temperature and / or humidity conditions) at least 2 Pa.s, better still at least 10 Pa.s, e.g. B. 40 Pa.s or more.
- b) das Beschichtungsmaterial weist eine relativ große Haftkraft an dem Material der Manschette auf, wobei diese Haftkraft zum Beispiel so groß ist, daß eine Druckdifferenz von zum Beispiel 30 millibar an den Enden eines zum Beispiel 200 µ breiten Abdichtungs spalts nicht zu einem Herausfließen des Schichtmaterials aus dem Spalt führt,b) the coating material has a relatively large adhesive force to the material of the Cuff, this adhesive force being so great, for example, that a pressure difference of, for example, 30 millibars at the ends of a, for example, 200 µ wide seal gap does not cause the layer material to flow out of the gap,
- c) das Material ist in hohem Maße quellfähig (mindestens 5-fache Volumenvergrößerung),c) the material is highly swellable (at least a 5-fold increase in volume),
- d) das Material ist in einer Schaumschicht aufgebracht (mit einer Mindeststärke von zum Beispiel 200 µ) und es ist bei geringen Flächendrücken von einigen 'zig millibar um mindestens einen Faktor 3 komprimierbar.d) the material is applied in a foam layer (with a minimum thickness of Example 200 µ) and it is around a few tens of millibars at low surface pressures compressible at least a factor of 3.
Es zeigen:Further advantages, features and possible uses of the present invention will become apparent clear from the following description of a preferred embodiment and the associated figures. Show it:
Man schette, Fig. 1 schematically shows a tube according to the invention with an inflated cuff Man,
Fig. 2 eine Schnittansicht mit Blickrichtung entlang der Achse des Tubus und mit einer Schnittebene, die etwa entlang der in Fig. 1 mit II-II bezeichneten, strich punktierten Linie verläuft und Fig. 2 is a sectional view looking in the direction of the axis of the tube and with a sectional plane that runs approximately along the dash-dotted line designated in Fig. 1 and II
Fig. 3 shows an enlarged detail according to the circle designated III in Fig. 2.
It can be seen in Fig. 1 is a endotracheal or tracheostomy tube 1 and which consists essentially of a hose or a tube 1, an inflatable cuff 2 is provided at its outer circumference in a central portion. The cuff 2 can optionally be firmly welded to the hose or tube 1 , but it can also be glued or made in one piece with the tube 1 . A provided in the wall of the tube 1 , here not recognizable channel has in the outer wall of the tube 1 an opening in the area of the cuff 2 , so that the cuff 2 inflatable via this outlet opening of the channel provided in the wall of the tube 1 , but possibly also is ventable. Breathing or ventilation takes place through the central lumen 4 of the tube 1 .
Figs. 1 to 3 show the subject invention only schematically and also the formation of folds 3 that can be seen in all three figures, is only schematically shown here and the wrinkles must of course not a extending over the entire longitudinal extent of the sleeve, uniform Have course.
In Fig. 2 the tube in the area of the cuff 2 is shown in section. One can see hatched the wall of the tube 1 , a central lumen 4 , and two channels 5 provided in the wall of the tube 1 , which can be used for inflating, deflating or also for supplying or aspirating liquid or secretion, depending on where the lower ends of these channels 5 open, which can open devices at the upper end of the tube in appropriate suction or flushing.
A corresponding channel 5 also opens into the outer wall of the tube 1 in the area of the cuff 2 .
In the figures, the inflated cuff, which may lie in the illustrated state on the wall of a trachea, is shown with a whole series of longitudinal folds 3 . It goes without saying that these folds 3 do not all have to run parallel and that such folds do not necessarily have to extend over the full length of the sleeve 2 . Rather, the illustration is only schematic and is intended to show that there are any folds 3 at all and that due to these folds 3 there can also be channels, albeit with a very small cross-section, along which possibly secretions from the area above the cuff 2 into the The area below the cuff 2 can flow so that pathogens, which can find favorable growth conditions above the cuff in the secretion, can penetrate into the deeper airways and can cause pneumonia.
According to the invention, this is prevented by a coating 6 on the outer surface of the sleeve 2 , which can only be seen in the enlargement according to FIG. 3. As already mentioned, FIG. 3 shows the area encircled in FIG. 2 in a further enlarged state, the illustration here also being only schematic and in particular the thickness of the coating layer 6 in relation to the thickness of the wall of the sleeve 2 shown only as a line is possibly exaggerated, but on the other hand, corresponding thickness ratios should not be excluded.
In the enlarged area of FIG. 3, two folds 3 can be seen , namely a slightly less deep fold 3 on the left in FIG. 3 and a comparatively deep fold 3 on the right in FIG. 3. A coating 6 is also shown hatched, which, for example, consists of a water-swellable gel.
If the tube 1 is inserted into the trachea of a patient together with the cuff 2 , which has not yet been inflated and lies tightly against the tube 1 or saggingly, the layer 6 is still very thin and hardly recognizable, ie it would be shown possibly recognize the Fig. 3 only as a layer that could be represented together with the wall of the sleeve 2 only through a slightly larger stroke weight.
In contact with the water-containing secretion in a patient's trachea, this coating material can swell to form a layer 6 , which is shown in broken lines in FIG. 3. The individual folds 3 form a cavity or a channel with the cross section of a drop, especially in the vicinity of their base, where the wall material of the sleeve has the greatest curvature. Ie that the two opposite wall sections of the sleeve 2 in the area of such a fold 3 near the bottom of such a fold are not tight and close to each other, which is probably related to the elastic restoring forces that act in the material of the sleeve 2 and lead to the fact that the kink at the bottom of the fold 3 can not be arbitrarily sharp, but has a finite curvature, which leads to this channel formation. If, however, a coating is provided in this area that swells under the influence of moisture or, for example, heat, this swelling coating material 6 , which can be plastically deformable or flowable as desired, closes this channel. In addition, of course, the coating material 6 also swells in the areas where the wall sections of the sleeve 2 abut against one another with their outer surfaces in the fold area, and possibly push them apart a little there, so that the layer material 6 is continuously formed in all folds and the at any point, the fold channel that may be forming is sealed. The same also applies in the gusset area at the transition from the outer, more or less cylindrical circumference of the sleeve to one of these folds 3 . The swelling layer 6 also expands there and forms a continuous, homogeneous layer which lies between the inner surface of the trachea and the sleeve material 2 . A similar effect is also achieved if the layer is spread thinly on the outer surface of the sleeve 2 as a more or less flowable layer in the form of a cream or a gel. Due to the pressure that the adjacent wall sections of a fold 3 exert on the flowable coating material 6 , this coating material 6 is partly squeezed out of the fold area, but partly also pressed into the bottom of the fold 3 , where otherwise the channels already described above are typically located form. Corresponding channels are thereby blocked or prevented. The coating material is distributed on the surface between the trachea and cuff wall 2 in accordance with the available space.
Apart from embodiments in which the coating 6 is relatively firm and in particular abrasion-resistant during storage at room temperature and in the dry state, and these layers first obtain the desired flowability and by means of radiation, moisture, heat, a reagent or also in connection with catalysts Assuming sealing properties, the coatings can also permanently have the gel-like, flowable state and they can then be covered for the purpose of storage and transport, for example with a thin protective film or protective sleeve, which extend over the cuff and possibly also take up the entire tube. Such a sleeve or cover preferably then consists of a material which in any case has no greater adhesiveness or adhesive force in relation to the coating agent than the sleeve material. The latter consists preferably of a polymeric material. If, moreover, the wall material of the sleeve 2 is as thin-walled as possible, it is ensured that the channels which may be present in the form of the folds 3 have a very small cross-section from the outset, so that these small cross-sections can be filled particularly easily and with little coating material.
