Abstract

Gelenkprothese mit einem Prothesenschaft (12), an dessen einem Ende ein Kugelkörper (11) befestigt ist,
dadurch gekennzeichnet,
daß die Stirnseite des kugelkörperseitigen Endes des Prothesenschaftes (12) als Mulde (15) ausgebildet ist,
daß die Mulde (15) und der Kugelkörper (11) auch an seiner Schaftseite (16) kugelförmig sind und annähernd denselben Radius aufweisen, so daß die Schaftseite (16) des Kugelkörpers (11) spaltfrei in der Mulde (15) sitzt, und
daß eine Befestigungsvorrichtung (19) die kugelförmigen Berührungsflächen aneinandergedrückt hält.

Description

  • Die Erfindung bezieht sich auf eine Gelenkprothese mit einem Prothesenschaft, an dessen einem Ende ein Kugelkörper befestigt ist.
  • In Gelenkprothesen, beispielsweise in künstlichen Hüftgelenken, bildet eine Gelenkkugel das Gegenstück zu einer Gelenkpfanne. Wegen ihrer komplexen Form und dem Wunsch, verschiedene Materialien verwenden zu können, sind Gelenkkugeln üblicherweise aus zwei Teilen zusammengesetzt, nämlich dem meist aus Metall gefertigten Prothesenschaft und dem aus anderen Materialien gefertigten Kugelkörper.
  • Aus DE 690 00 445 T2 ist ein prothetisches Kugelgelenk bekannt, dessen keramischer Kugelkörper eine tiefe konische Ausnehmung aufweist, in der das konische Ende des Gelenkschaftes festgeklemmt ist. Bei einer axialen Belastung, wie sie beispielsweise bei Hüftgelenkprothesen auftritt, werden die axialen Kräfte aus schließlich durch die Konuswände übertragen. Der Kugelkörper wird dadurch bei zunehmender axialer Belastung auseinandergedrückt und kann schließlich zerbrechen.

  • Aufgabe der Erfindung ist es, die mechanische Belastbarkeit einer Gelenkprothese zu verbessern.
  • Die Aufgabe wird erfindungsgemäß durch die Merkmale des Anspruchs 1 gelöst.
  • Die Stirnseite des kugelkörperseitigen Endes des Prothesenschaftes ist als Mulde ausgebildet. Die Mulde und die Schaftseite des Kugelkörpers sind kugelförmig und mit annähernd demselben Radius ausgebildet, so daß die Schaftseite des Kugelkörpers annähernd spaltfrei in der Mulde sitzt. Der Kugelkörper wird großflächig von der Muldenfläche, die ungefähr senkrecht zur axialen Hauptbelastungsachse orientiert ist, abgestützt. Durch die axialen Druckkräfte wird der Kugelkörper auf Druck beansprucht, wodurch auch sehr große Gelenkkräfte ohne Zerstörungsgefahr für den Kugelkörper übertragen werden können.
  • Wegen der hohen konstruktionsbedingten Stabilität können kleinere Kugelkörper und damit kleinere Gelenkprothesen verwendet werden. Die Gefahr des Berstens des Kugelkörpers und der Gewebeverunreinigung durch Splitter ist nicht mehr gegeben.
  • Die kugelförmige Ausbildung der Berührungsflächen des Kugelkörpers und des Prothesenschaftes gewährt auch eine gute seitliche Abstützung, so daß auch nicht exakt axial eingeleitete Druckkräfte über die kugelförmigen Berührungsflächen übertragen werden.
  • Durch eine Befestigungsvorrichtung werden die kugelförmigen spaltfrei aneinanderliegenden Berührungsflächen der Mulde und des Kugelkörpers aneinandergedrückt. Die Befestigungsvorrichtung dient ausschließlich der Fixierung der Kugel in der Mulde und nicht der Übertragung von axialen Druckkräften. Die Befestigungsvorrichtung kann daher auf geringe durch sie zu übertragende Kräfte ausgelegt werden. An den Berührungspunkten und Flächen der Befestigungsvorrichtung in oder an dem Kugelkörper werden nur kleine Haltekräfte übertragen, jedoch keine von der Gelenkbelastung herrührenden Kräfte.
  • Vorzugsweise ist der Radius des Kugelkörpers kleiner als der Radius der Mulde, beträgt jedoch mindestens 99% des Radius der Mulde. Dadurch ist ein Sitz des Kugelkörpers in der Mulde gewährleistet, der sowohl klemmfrei als auch spielfrei ist.
  • Die Befestigungsvorrichtung ist vorzugsweise eine Schraube, mit der der Kugelkörper in der Mulde kontinuierlich spannbar ist. Der Kugelkörper kann dadurch mit einer gewünschten Vorspannung in der Mulde festgezogen werden. An dem Befestigungsort Schraube-Kugelkörper treten nur geringe Zugkräfte auf. Bei axialer Druckbelastung der Gelenkprothese wirken die Druckkräfte den Zugkräften sogar entgegen, so daß die Zugkräfte bei zunehmender Gelenkbelastung geringer werden. Dadurch wird die Gefahr der Zerstörung des Kugelkörpers bei hoher Druckkraftbelastung durch unverträgliche Zugkräfte ausgeschlossen.
  • Der Kugelkörper kann eine Bohrung aufweisen, in der eine Gewindebuchse sitzt, in der die Befestigungsschraube eingeschraubt wird. Durch Verwendung einer Gewindebuchse kann eine Schraubverbindung auch bei Ku gelkörpermaterialien verwendet werden, bei denen das Einschneiden eines Gewindes nicht möglich oder sinnvoll ist.
  • Die Mulde kann in der Muldenmitte eine Gewindebohrung aufweisen, in die ein Gewindestift eingeschraubt ist, der mit seinem anderen Ende in die Buchse eingeschraubt wird. Bei einem Auswechseln des Kugelkörpers kann gleichzeitig der Gewindestift ausgewechselt werden, wodurch die Bruchgefahr des Gewindestiftes wegen wiederholter Benutzung reduziert wird.
  • Der Kugelkörper besteht vorzugsweise aus Keramik und der Prothesenschaft aus Metall. Der Keramik-Kugelkörper ermöglicht eine sehr genaue spiel- und spaltfreie Lagerung in einer vorzugsweise keramischen Gelenkpfanne. Zusammen mit dem geringen Reibungswiderstand Keramik-Keramik wird die Belastung der Verbindung zwischen dem Prothesenschaft und dem Kugelkörper reduziert.
  • Die Buchse ist vorzugsweise eine Metallhülse, deren Wärmeausdehnungskoeffizient gleich dem des Kugelkörpermaterials ist. Dadurch werden Spannungen aufgrund verschiedener Temperaturverhalten des Buchsen- und des Kugelkörpermaterials vermieden.

  • Im folgenden wird unter Bezugnahme auf eine Zeichnung eine Ausführungsform der Erfindung näher erläutert.
  • Die Figur zeigt im Längsschnitt das obere Ende eines Prothesenschaftes mit einem Kugelkörper einer Gelenkprothese.
  • Die in der Figur gezeigte Gelenkprothese 10 wird von einem Keramik-Kugelkörper 11 und einem Metall-Prothe senschaft 12 gebildet. Der Prothesenschaft 12 wird mit seinem Schaftkörper in einem Knochen implantiert, während der Kugelkörper 11 in eine prothetische Gelenkpfanne eingesetzt wird (nicht dargestellt). Das kugelkörperseitige Ende des Prothesenschaftes 12 ist als konvexe sphärische Mulde 15 ausgebildet, deren tiefster Punkt ungefähr in der Mitte der Mulde 15 liegt.
  • In der Mulde 15 liegt spalt- und spielfrei der sphärische Kugelkörper 11, so daß die gesamte Fläche der Mulde 15 an der muldenseitigen Außenwand 16 des Kugelkörpers anliegt. Der Kugelradius der Mulde 15 und des Kugelkörpers 11 ist annähernd gleich. In der Mitte der Mulde 15 ist eine axiale Gewindebohrung 18 angeordnet, in die ein Gewindestift 19 eingeschraubt ist.
  • Der Kugelkörper 11 weist eine radiale Sackbohrung 21 auf, in die eine Gewindebuchse 22 mit einem Innengewinde 23 eingelassen ist. Die Gewindebuchse 12 besteht aus Metall und wird in der Gewindebohrung 18 befestigt, indem sie zunächst gekühlt wird und widerstandsfrei in die Sackbohrung 21 eingesetzt wird. Durch die anschließende Erwärmung auf die Umgebungstemperatur dehnt sich die Gewindebuchse 22 aus und sitzt dann mit festem Klemmsitz in der Sackbohrung 21. In die Gewindebuchse 22 ist der Gewindestift 19 eingeschraubt. Der Gewindestift 19 hält den Keramik-Kugelkörper 11 in der Mulde 15, so daß die Kugelflächen der Mulde 15 und des Kugelkörpers 11 aufeinandergedrückt werden.
  • Zum Lösen wird der Kugelkörper 11 von Hand oder mittels eines Werkzeugs in Löserichtung der Gewindeschraube 19 gelöst und abgeschraubt. Beim Auswechseln eines Kugelkörpers 11 wird ein neuer Kugelkörper 11 mit einem neuen Gewindestift 19 in der Mulde 15 festgeschraubt. Die Steigung des Gewindes des Gewindestiftes 19, der Gewindebohrung 18 und der Sackbohrung 21 ist so flach, daß es selbsthemmend wirkt. Nachdem der Kugelkörper 11 mit einem vorgegebenen Drehmoment festgezogen wurde, verhindern die Gewindereibungskräfte und die Reibungskräfte zwischen der Mulde 15 und der muldenseitigen Außenwand 16 des Kugelkörpers 11 ein ungewolltes Lösen des Kugelkörpers 11.
  • Die Übertragung axialer Druckkräfte zwischen dem Kugelkörper 11 und dem Prothesenschaft 12 erfolgt großflächig und über eine Fläche, die nur einen kleinen Winkel zur Senkrechten der Hauptdruckrichtung einnimmt, d.h. die Gelenkkräfte werden annähernd senkrecht durch die großflächigen Berührungsflächen übertragen.
  • Durch die muldenartige Ausbildung der druckaufnehmenden Fläche der Mulde 15 werden jedoch auch Druckkräfte, die nicht genau axial eingeleitet werden, großflächig übertragen, ohne daß der Kugelkörper 11 durch die Druckkräfte aus der Mulde 15 herausgedrückt und durch den Gewindestift 19 in der Mulde gehalten wird.
  • Die Gelenkkräfte bewirken in dem Kugelkörper 11 hauptsächlich Druckkräfte. Dies ermöglicht den Einsatz von Keramikmaterialien, die im Gegensatz zu Zugkräften hohe Druckkräfte zerstörungsfrei übertragen können.
  • Der Kugelkörper 11 und der Prothesenschaft 12 können auch aus anderen Materialien, beispielsweise Kunststoffen und Metallen, bestehen.

Claims (7)

  1. Gelenkprothese mit einem Prothesenschaft (12), an dessen einem Ende ein Kugelkörper (11) befestigt ist, dadurch gekennzeichnet, daß die Stirnseite des kugelkörperseitigen Endes des Prothesenschaftes (12) als Mulde (15) ausgebildet ist, daß die Mulde (15) und der Kugelkörper (11) auch an seiner Schaftseite (16) kugelförmig sind und annähernd denselben Radius aufweisen, so daß die Schaftseite (16) des Kugelkörpers (11) spaltfrei in der Mulde (15) sitzt, und daß eine Befestigungsvorrichtung (19) die kugelförmigen Berührungsflächen aneinandergedrückt hält.
  2. Gelenkprothese nach Anspruch 1, dadurch gekennzeichnet, daß der Radius des Kugelkörpers (11) kleiner als der Radius der Mulde (15) ist, jedoch mindestens 99% des Radius der Mulde (15) beträgt.
  3. Gelenkprothese nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Befestigungsvorrichtung (19) eine Schraube ist, mit der der Kugelkörper (11) in der Mulde (15) kontinuierlich spannbar ist.
  4. Gelenkprothese nach Anspruch 3, dadurch gekennzeichnet, daß der Kugelkörper (11) eine Bohrung (21) aufweist, in der eine Gewindebuchse (22) sitzt, in der die Befestigungsschraube (19) eingeschraubt ist.
  5. Gelenkprothese nach Anspruch 4, dadurch gekennzeichnet, daß die Mulde (15) in der Muldenmitte eine Gewindebohrung (18) aufweist, in die ein Gewindestift (19) eingeschraubt ist, der mit seinem anderen Ende in die Gewindebuchse (22) eingeschraubt ist.
  6. Gelenkprothese nach einem der Ansprüche 1–5, dadurch gekennzeichnet, daß der Kugelkörper (11) aus Keramik und der Prothesenschaft (12) aus Metall besteht.
  7. Gelenkprothese nach einem der Ansprüche 1–6, dadurch gekennzeichnet, daß die Gewindebuchse (22) eine Metallhülse ist, deren Wärmeausdehnungskoeffizient gleich dem des Kugelkörpermaterials ist.
