Abstract
Die vorliegende Erfindung betrifft neue Sprayformulierungen von Levothyroxin. Des Weiteren beschreibt die Erfindung auch ein Verfahren zur Herstellung derartiger Formulierungen.
Description
- Hintergrund der Erfindung
- Thyroxin-aktive Wirkstoffe sind sowohl zur therapeutischen als auch zur prophylaktischen Behandlung von Schilddrüsenerkrankungen bekannt. Die Schilddrüse erzielt ihre Regulierungsfunktionen durch Produktion der Hormone L-Triiodthyronin (Liothyronin; T3) und L-Thyroxin (Levothyroxin; T4). Die physiologischen Wirkungen von Schilddrüsenhormonen werden vorwiegend durch T3 erzielt, von dem der Großteil (ca. 80%) von T4 durch Deiodierung im peripheren Gewebe abgeleitet wird.
- Durch Verabreichung von Levothyroxin-Natrium wird einem Patienten T4 bereitgestellt. Nach Absorption im Körper verhält sich das verabreichte T4 identisch zu T4, das ansonsten von der Schilddrüse des Patienten abgeschieden würde. Es bindet sich an dieselben Serumproteine, stellt einen Vorrat an zirkulierendem T4-Thyroglobulin im Patienten bereit. Das verabreichte T4 kann in vivo zu T3 deiodiert werden. Infolgedessen zeigen sich in einem Patienten, der geeignete Dosen Levothyroxin-Natrium erhält, normale T3-Werte im Blut, selbst wenn die Schilddrüse des Patienten entfernt wurde oder nicht funktioniert.
- Levothyroxin-Natrium wird zur Schilddrüsenhormonersatztherapie bei verminderter oder nicht mehr gegebener Schilddrüsenfunktion verschrieben, zum Beispiel bei Krankheiten wie Myxödem, Kretinismus und Adipositas. Levothyroxin-Natrium ist ziemlich instabil, hygroskopisch und zerfällt schnell, wenn es hoher Luftfeuchtigkeit, Licht oder hohen Temperaturen ausgesetzt ist. Aufgrund der physikalisch-chemischen Eigenschaften des Wirkstoffes haben Levothyroxin-Natrium-Formulierungen äußerst geringe Haltbarkeit.
- Levothyroxin-Natrium ist in Form von Kapseln, Tabletten und als Injektion erhältlich. Die Injektionsformulierung ist als steriles lyophilisiertes Produkt zur parenteralen Verabreichung mit 100 Mikrogramm/Durchstechflasche, 200 Mikrogramm/Durchstechflasche und 500 Mikrogramm/Durchstechflasche erhältlich.

U.S.-Patenten Nr. 2,889,363 und 2,889,364 (Ginger et al.) sind Verfahren zur Herstellung von Thyroxin-Natrium offenbart. - In den
U.S.-Patenten Nr. 5,955,105 und 6,056,975 (Amit et al.) sind feste pharmazeutische Präparate mit einem Thyroxin-Wirkstoff offenbart. - Im
U.S.-Patent Nr. 9,006,289 - In der
U.S.-Patentanmeldung Nr. 20140073695 (Yannis et al.) ist ein Verfahren zur Herstellung einer oralen Levothyroxin-Zusammensetzung offenbart. - Bei den oralen Formulierungen wird jedoch der Wirkstoff nicht so schnell ins Blut abgegeben wie bei den Injektionen. Andererseits ist die Verabreichung der Injektionsformulierungen für den Patienten schmerzhaft. Die Erfinder der vorliegenden Anmeldung haben stabile orale Sprayformulierungen von Levothyroxin entwickelt, mit denen sich die Nachteile der Formulierungen aus dem Stand der Technik überwinden lassen. Diese Formulierungen können im Vergleich zu oralen und anderen Verabreichungsformen des Wirkstoffes erhebliche Vorzüge bieten, beispielsweise schnelleres Auftreten des pharmazeutischen aktiven Wirkstoffs im Blut, verbesserte Dosierungsverlässlichkeit, verbessertes Sicherheitsprofil, höhere Bioverfügbarkeit und verbesserte Compliance des Patienten. Darüber hinaus verringert sich die Zeit für das Herstellungsverfahren und im Gegensatz zu den im Handel erhältlichen Formulierungen sind dafür keine speziellen Geräte erforderlich, was das Herstellungsverfahren kostengünstig macht.
- Zusammenfassung der Erfindung
- Eine Aufgabe der Erfindung liegt in der Bereitstellung stabiler oraler Sprayformulierungen von Levothyroxin und Verfahren zur Herstellung derartiger Formulierungen.
- Gemäß einem weiteren Aspekt der Erfindung werden stabile orale Sprayformulierungen mit Levothyroxin-Natrium, Puffermitteln oder Mitteln zur pH-Einstellung, einem oder mehreren Lösungsmitteln und anderen pharmazeutisch verträglichen Trägerstoffen bereitgestellt.
- Ausführliche Beschreibung der Erfindung
- Die vorliegende Erfindung betrifft eine stabile orale Sprayformulierung von Levothyroxin. Insbesondere betrifft die Erfindung eine stabilisierte Levothyroxin-Sprayformulierung mit Puffermitteln, Lösungsmitteln und anderen pharmazeutisch verträglichen Trägerstoffen.
- Im Kontext dieser Erfindung bezieht sich „Levothyroxin“ auf pharmazeutisch verträgliche Salze, Solvate, Hydrate und wasserfreie Formen davon. Die Formulierungen der vorliegenden Erfindung umfassen vorzugsweise Levothyroxin-Natrium.
- Der hierin verwendete Begriff „orale Sprayformulierung von Levothyroxin“ bezieht sich auf eine Formulierung, die Levothyroxin in gelöster oder solubilisierter Form enthält und zur oralen Verabreichung gedacht ist. Die orale Sprayformulierung von Levothyroxin kann eine klare Lösung sein, die zum Einsprühen direkt in den Mund, auf oder unter die Zunge vorgesehen ist.
- Levothyroxin-Natrium ist ziemlich instabil, hygroskopisch und zerfällt schnell, wenn es hoher Luftfeuchtigkeit, Licht oder hohen Temperaturen ausgesetzt ist. Sein Zerfall wird in Anwesenheit von Wasser noch beschleunigt. Die Erfinder der vorliegenden Erfindung haben trotz der schnellen Zerfallseigenschaft von Levothyroxin erfolgreich eine stabile orale Sprayformulierung dieses Wirkstoffs entwickelt.
- Eine Ausführungsform der Erfindung betrifft eine orale Sprayformulierung von Levothyroxin mit Levothyroxin-Natrium und pharmazeutisch verträglichen Trägerstoffen.
- Eine weitere Ausführungsform der Erfindung betrifft orale Sprayformulierungen von Levothyroxin, umfassend:
- i. Levothyroxin
- ii. Puffermittel oder Mittel zur pH-Einstellung
- iii. ein oder mehrere Lösungsmittel
- Eine noch weitere Ausführungsform der Erfindung betrifft eine orale Sprayformulierung von Levothyroxin, umfassend:
- i. Levothyroxin
- ii. Puffermittel oder Mittel zur pH-Einstellung
- iii. ein oder mehrere Lösungsmittel
- iv. Cyclodextrin
- Eine bevorzugte Ausführungsform der Erfindung betrifft eine orale Sprayformulierung von Levothyroxin, umfassend:
- i. Levothyroxin-Natrium,
- ii. Puffermittel oder Mittel zur pH-Einstellung
- iii. ein oder mehrere Lösungsmittel, ausgewählt aus der Gruppe umfassend Wasser, Polyethylenglykol, Ethanol, Polypropylenglykol und Glycerin
- In einer meist bevorzugten Ausführungsform umfasst die orale Sprayformulierung gemäß vorliegender Erfindung folgendes:
i. Levothyroxin-Natrium 0,0025 - 5% ii. Puffermittel oder Mittel zur pH-Einstellung 0,0015 - 5% iii. Lösungsmittel qs iv. gegebenenfalls andere pharmazeutisch verträgliche Trägerstoffe, einschließlich Cyclodextrin, Süßungsmittel, Geschmacksstoffe, Konservierungsstoffe, Penetrationsverstärker und Stabilisatoren. - Geeignete Puffermittel sind unter anderem, jedoch nicht beschränkt auf, Phosphat, Citrat, A-conit, Natriumkarbonat, Natriumbikarbonat, Tartrat, Benzoat, Acetat, Borsäure, Milchsäure, Glutarsäure, Maleinsäure, Bernsteinsäure und Kohlensäure, Alkali- oder Erdalkalisalz einer dieser Säuren, Tris, Meglumin, Aminosäurepuffer wie Arginin, Alanin, Histidin, Glycin, Lysin und dergleichen.
- Geeignete Lösungsmittel sind unter anderem, jedoch nicht beschränkt auf, Dimethylacetamid (DMA), Dimethylsulfoxid (DMSO), N-Methylpyrrolidon, Dimethylisosorbid, Ethanol, Propylenglycol, Polyethylenalkohol, Glycerol, Propylenglycolester, Polyethylenglycole (PEG) und Wasser. Bevorzugte Lösungsmittel sind Wasser und Propylenglycol.
- Geeignete Mittel zur pH-Einstellung sind unter anderem, jedoch nicht beschränkt auf, Natriumhydroxid, Kaliumhydroxid, Ammoniumkarbonat, Chlorwasserstoffsäure, Citronensäure, Milchsäure, Phosphorsäure und Schwefelsäure. Bevorzugte Mittel zur pH-Einstellung sind Natriumhydroxid und Chlorwasserstoffsäure.
- Geeignete Cyclodextrine sind unter anderem, jedoch nicht beschränkt auf, α, β und γ-Cyclodextrin sowie Cyclodextrine, die mit Alkyl-, Hydroxyalkyl-, Dialkyl- und Sulfoalkylether modifiziert sind, beispielsweise Methyl- oder Hydroxypropyl-β-Cyclodextrine (HPCD), Methyl- und Ethyl-β-Cyclodextrin, Sulfoalkylether-substituiertes beta-Cyclodextrin, Sulfobutylether-β-Cyclodextrin (SBECD) und dergleichen. Substituierte beta-Cyclodextrine schließen diejenigen ein, die durch eine oder mehrere hydrophile(n) Gruppe(n) substituiert sind, beispielsweise Monosaccharid (z.B. Glucosyl), Carboxyalkyl (z.B. Carboxymethyl, Carboxyethyl), Hydroxyalkyl-substituiert (z.B. Hydroxyethyl, 2-Hydroxypropyl) und dergleichen. Vorzugsweise wird Sulfobutylether-β-Cyclodextrin (SBECD) verwendet.
- Die pharmazeutischen Formulierungen der vorliegenden Erfindung können auch ein oder mehrere Antioxidantien und Konservierungsstoffe enthalten, zum Beispiel, jedoch nicht beschränkt auf, butyliertes Hydroxyanisol, butyliertes Hydroxyltoluol, Citronensäure, Milchsäure, Benzoesäure, Tocopherol, Monothioglycerol, Ascorbinsäure, L-Cystein, Methylparaben, Propylparaben, Benzylalkohol, Propylgallat, Thioglycolsäure, Citronensäure, Weinsäure, Phosphorsäure, Gluconsäure, Thiodipropionsäure, acetonische Dicarbonsäure, Sorbinsäure und deren Salze, Chelatbildner und dergleichen.
- Die Formulierung kann zusätzlich einen oder mehrere Geschmacksstoffe oder geschmacksüberdeckende Mittel und Süßungsmittel enthalten. Beispiele für geschmacksüberdeckende Mittel oder Geschmacksstoffe sind unter anderem, jedoch nicht beschränkt auf, synthetisches oder natürliches Pfefferminzöl, Spearmintöl, Citrusöl, Fruchtaromen (z.B. Citrus, Orange, Zitrone, Erdbeere, Melone und Mischungen) und Süßstoffe (z.B. Zucker, Sorbitol, Mannitol, Dextrose, Sucrose, Fructose, Levulose, Aspartam, Natriumcyclamat, Saccharin und Sucralose).
- Die Formulierung kann auch Absorptionsverstärker oder Penetrationsverstärker wie beispielsweise, jedoch nicht beschränkt auf, Tenside, zum Beispiel Natriumlaurylsulfat, Natriumdodecylsulfat, Dioctylnatriumsulfosuccinat, Tweens, Polysorbate, Cetylpyridiniumchlorid, Chitosan, Trimethylchitosan, Poly-L-Arginin und L-Lysin; Fettsäuren und Derivate; Gallensalze, Chelatbildner, Sulfoxide wie beispielsweise Dimethylsulfoxid (DMSO), Decylmethylsulfoxid; Polyole und Alkohole enthalten.
- Fakultativ können auch Stabilisatoren, Löslichkeitsverstärker und viskositätsmodifizierende Mittel zugegeben werden.
- Die orale Sprayformulierung lässt sich in jedwede geeignete Behältnisse füllen. Bevorzugte Behälter sind Flaschen aus pharmazeutisch verträglichem Glas, Polyethylenterephthalat (PET), hochdichtem Polyethylen (HDPE), Crystal Zenith (CZ), Polyolefine wie Polypropylen (PP) oder Polyethylen (PE), Cycloolefinpolymer (COP), Cycoolefincopolymer (COC), Pfeiffer-Pumpen oder jedwede andere Art von pharmazeutisch verträglicher Packung, Behälter, Pumpe oder Flasche.
- Eine vergleichende Auflösungsstudie wurde mit einer erfindungsgemäß hergestellten Levothyroxin-Natrium-Formulierung und im Handel erhältlichen Levothyroxin-Natrium-Tabletten durchgeführt (AEB1997, Thyronorm 150 Mikrogramm). Die Auflösungsstudie wurde in 500 ml 0,01N HCl+0,2% SLS (Natriumlaurylsulphat) Auflösungsmedium bei 50 U/min durchgeführt. Die Auflösungsprofile des Vergleichs wurden aufgezeichnet und sind in Tabelle 1 tabellarisch aufgeführt. Tabelle 1: Auflösungsprofilvergleich

- Ultrareines Wasser wurde in ein Gefäß zur Mischungsherstellung gefüllt und Puffermittel (wie in Beispiel 3 erwähnt) zugegeben und verrührt. Erforderlichenfalls erfolgte die Zugabe von Mannitol (F1). Der pH-Wert der Lösung wurde durch Zugabe von Natriumhydroxidlösung auf 11 ± 1,0 eingestellt. Dann wurde die Feststofflösung auf 2°C bis 8°C abgekühlt. Levothyroxin-Natrium wurde zugegeben und verrührt, bis sich eine klare Lösung ergab, wobei die Temperatur auf 5±3°C gehalten wurde. Die Lösung wurde gefiltert und anschließend in geeignete Behälter gefüllt.
- Die gemäß Beispiel 3 hergestellten Levothyroxin-Formulierungen wurden bei 25±2°C/60±5% RH (1 Woche); 40±2°C/75±5%RH (1 Woche) und 60±2°C (3 Tage) auf ihre Stabilität getestet. Die Daten sind in Tabelle 2 zusammengefasst. Tabelle 2: Stabilität der Formulierung

- Ultrareines Wasser wurde in ein Gefäß zur Mischungsherstellung gefüllt und SBECD, Levothyroxin, Arginin, Kaliumsorbat und Natriumjodid zugegeben und verrührt. Claims (9)
- Orale Sprayformulierung von Levothyroxin umfassend Levothyroxin-Natrium und pharmazeutisch verträgliche Trägerstoffe.
- Formulierung gemäß
Anspruch 1 , umfassend: (i) Levothyroxin-Natrium (ii) Puffermittel oder Mittel zur Einstellung des pH-Werts (iii) ein oder mehrere Lösungsmittel (iv) fakultativ einen oder mehrere pharmazeutische verträgliche Trägerstoffe. - Formulierung gemäß
Anspruch 2 , umfassend:(i) Levothyroxin-Natrium 0,0025 - 5% (ii) Puffermittel oder Mittel zur Einstellung des pH-Werts 0,0015 - 5% (iii) Lösungsmittel qs (iv) fakultativ einen oder mehrere pharmazeutisch verträgliche Trägerstoffe. - Formulierung gemäß
