Abstract

Die vorliegende Anmeldung betrifft einen winkelstabilen, verschraubbaren Verriegelungszapfen als einen Hüftpfannendorn, für die zusätzliche Fixation einer vorzugsweise zementlos implantierbaren Hüftgelenkpfannen-Endoprothese, mit einer speziellen Funktionsgeometrie für die Erzeugung einer Oberflächenvergrößerung und sichernder bzw. verspannender Kräfte in dem Beckenknochen-Implantatsystem zur Implantatsicherung und Vermeidung von postoperativen Rotationsbewegungen oder sonstigen Lageveränderungen der implantierten Hüftgelenkpfannen-Endoprothese, der unter besonderer Berücksichtigung der anatomischen und mechanischen Anforderungen einer Hüftgelenkpfanne, der einfachen intraoperativen Anwendung und der allgemeinen Sicherheitsaspekte optimiert ist. DOLLAR A Der winkelstabile, verschraubbare Hüftpfannendorn besteht in der bevorzugten Ausführung aus drei Elementen, die einen Bolzen, ein Gewinde und eine Adaptionsvorrichtung für ein OP-Instrument darstellen. DOLLAR A Die mechanische Sicherung bzw. Systemverspannung kann auf unterschiedliche Weise als zusätzliche Verankerung, als eine Oberflächenvergrößerung der Hüftgelenkpfannen-Endoprothese oder durch geometrische Unterschiede der Beckenknochenbohrung und der Geometrie des winkelstabilen, verschraubbaren Hüftpfannendorns bewirkt werden.

Description

  • Die bekannten Ausführungen zum Stand der Technik von Applikationen zur zusätzlichen Fixation von zementlos implantierten Hüftgelenkpfannen-Endoprothesen weisen verschiedene konstruktive Varianten in Form von Applikationen an Hüftgelenkpfannen-Endoprothesen wie starre Flügel oder Streben auf der Oberfläche der Hüftgelenkpfannen-Endoprothesen, schwenkbare, in die Hüftgelenkpfannen-Endoprothese geometrisch integrierte Keile oder auch zusätzliche Implantatelemente wie Spongiosa-Schrauben, verpressbare Stifte oder verspannte Drähte etc. auf.
  • Die Aufgabe der vorliegenden Erfindung ist es, einen Hüftpfannendorn als einen winkelstabilen, eingeschraubten Verriegelungszapfen für die zusätzliche Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese mit einer speziellen Funktionsgeometrie zu schaffen, der unter besonderer Berücksichtigung der anatomischen und mechanischen Anforderungen einer Hüftgelenkpfanne, der einfachen intraoperativen Anwendung und der allgemeinen Sicherheitsaspekte optimiert ist.
  • Die Innovation berücksichtigt den Stand der Technik und bezieht sich insbesondere auf den Aspekt der zusätzliche Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese ohne eine Erzeugung von Zugkräften, wie sie durch nicht winkelstabile Spongiosa-Schrauben hervorgerufen werden, die die Hüftgelenkpfannen-Endoprothese einseitig belasten und somit auf der gegenüberliegenden Seite den umfänglichen Pressfitt der Hüftgelenkpfannen-Endoprothese vermindern können.
  • Die vorliegende Anmeldung betrifft einen Hüftpfannendorn (1), als einen winkelstabilen, eingeschraubten Verriegelungszapfen, für die zusätzliche Fixation einer vorzugsweise zementlos implantierbaren Hüftgelenkpfannen-Endoprothese (7), mit einer speziellen Funktionsgeometrie für die Erzeugung einer Oberflächenvergößerung und sichernder bzw. verspannender Kräfte in dem Beckenknochen-Implantatsystem zur Implantatsicherung und Vermeidung von postoperativen Rotationsbewegungen oder sonstigen Lageveränderungen der implantierten Hüftgelenkpfannen-Endoprothese (7).
  • Das montierte Implantatsystem, bestehend aus einem oder mehreren winkelstabilen, verschraubbaren Hüftpfannendornen (1) und der Hüftgelenkpfannen-Endoprothese (7), erzeugt im Beckenknochen aufgrund der mechanischen Sicherung der Implantate in Bezug zum Beckenknochen eine Rotations- und Lagestabilität. Die mechanische Sicherung bzw. Systemverspannung kann auf unterschiedliche Weise hervorgerufen werden:
    • 1. indem eine zusätzliche Verankerung der Hüftgelenkpfannen-Endoprothese durch die Montage der winkelstabilen, verschraubbaren Hüftpfannendorne bewirkt wird
    • 2. indem eine Oberflächenvergößerung der Hüftgelenkpfannen-Endoprothese durch die Montage der winkelstabilen, verschraubbaren Hüftpfannendorne bewirkt wird,
    • 3. indem der Bohrungsdurchmesser im Beckenknochen (10) geringer als der Durchmesser des winkelstabilen, verschraubbaren Hüftpfannendornes (1) ist.
    • 4. indem die Achse (11) der in den Beckenknochen (10) eingebrachten Bohrung parallel nach medial zur Achse (6) des winkelstabilen, verschraubbaren Hüftpfannendornes (1) verschoben ist,
    • 5. indem die Bohrungsachse (11) der in den Beckenknochen (10) eingebrachten Bohrung nach medial zur Achse (6) des winkelstabilen, verschraubbaren Hüftpfannendornes (1) gewinkelt ist.
    • 6. indem Kombinationen der unter Punkt 1. bis 5. beschriebenen Ausführungen realisiert werden.
  • Der winkelstabile, verschraubbare Hüftpfannendorn (1), in der bevorzugten Ausführung, besteht aus drei, einstückig miteinander verbundenen Elementen, die einen Bolzen (2), ein Gewinde (4) und eine Adaptionsvorrichtung (5) für ein OP-Instrument darstellen.
  • Der winkelstabile, verschraubbare Hüftpfannendorn (1) im Bereich des Bolzens (2) ist in der bevorzugten Ausführung dadurch gekennzeichnet, dass er eine vorzugsweise glatte Oberfläche und einen kreisrunden Querschnitt aufweist, alternativ aber auch eine rauhe oder strukturierte Oberfläche bzw. einen ovalen oder vieleckigen Querschnitt oder zusätzliche Applikationen (3) in Form von Bohrungen (3), Kerben (3) oder Abflachungen (3) aufweisen kann um ein Implantatinterface darzustellen. Durch den Bolzen (2) des Hüftpfannendornes (1), dessen Oberfläche als Implantatinterface einen Kontakt zur Spongiosa des Beckenknochens (10) aufweist, werden, je nach der vorab unter Punkt 1-6 dargestellten Variante, radiale Druckkräfte in einem umfänglichen Winkelbereich von 180° bis 360° in die zirkulär anliegende Spongiosa des Beckenknochens (10) eingeleitet. Die eingeleiteten Kräfte können, je nach Ausführung des Bolzens (2) am winkelstabilen, verschraubbaren Hüftpfannendorn (1) und hinsichtlich seiner Position zu der Bohrung im Beckenknochen (10), auf seiner gesamten Länge gleich groß oder auch unterschiedlich groß sein. Durch die entstehenden, umfänglichen Druckkräfte wird keine Lageveränderung der Hüftgelenkpfannen-Endoprothese (7) bei der Montage oder postoperativ, als Schwenken der Hüftgelenkpfannen-Endoprothese (7) im Knochenlager in oder gegen die Montagerichtung des winkelstabilen, verschraubbaren Hüftpfannendornes (1), hervorgerufen, wie sie durch Zugkräfte, die durch Spongiosa-Schrauben in einem Knochen-Implantatsystem auftreten, entsteht. Durch die alternativ vorgeschlagene Oberflächengestaltung in Form einer rauhen oder makroporös strukturierten Oberfläche bzw. einem ovalen oder vieleckigen Querschnitt oder zusätzliche Applikationen (3) in Form von Bohrungen (3), Kerben (3) oder Abflachungen kann eine ungewollte, postoperative Rotation des winkelstabilen, verschraubbaren Hüftpfannendornes (1) im Gewinde (8) der Hüftgelenkpfannen-Endoprothese (7) nach bindegewebiger Einscheidung oder knöcherner Integration vermieden werden.
  • Zur Erzeugung einer winkelstabilen Verschraubung zwecks translatorischer Lagesicherung und Montage des winkelstabilen, verschraubbaren Hüftpfannendornes (1) in der Hüftgelenkpfannen-Endoprothese (7) weist dieser im Anschluss an den Bolzen (2) ein Gewinde (4) auf, das bei der intraoperativen Montage in einer Gewindebohrung (8) der Hüftgelenkpfannen-Endoprothese (7) plaziert wird. Eine derartige winkelstabile Verschraubung ist durch die Verwendung der bekannten Spongiosa-Schrauben nicht erreichbar, da der Kopfbereich der Spongiosa-Schraube, der Kontakt durch eine einfache Anlage zur Hüftgelenkpfannen-Endoprothese in der Aufnahme bzw. Kontaktzone in der Hüftgelenkpfannen-Endoprothese hat, die vorzugsweise als Stufenbohrung bzw. abgesetzte oder angesenkte Bohrung ausgeführt ist, keine winkelstabile Fixierung der Spongiosa-Schraube zuläßt. Zur einfachen und sicheren intraoperativen Montage ist im Gewinde (4) eine Adaptionsvorrichtung (5) für ein OP-Instrument integriert, die in der bevorzugten Ausführung durch einen Sechskant dargestellt wird.
  • Ein einzelner winkelstabiler, verschraubbarer Hüftpfannendorn (1) kann vor oder nach der Plazierung der Hüftgelenkpfannen-Endoprothese (7) montiert werden, weitere winkelstabile, verschraubbare Hüftpfannendorne (1) sind grundsätzlich nach der Implantation der Hüftgelenkpfannen-Endoprothese (7) zu montieren.
  • Die Einzelheiten zum winkelstabilen, verschraubbaren Hüftpfannendorn (1) für die Fixation einer vorzugsweise zementlos implantierten Hüftgelenkpfannen-Endoprothese (7) in der bevorzugten Ausführungsform als ein Gewindebolzen gemäß der vorangegangenen Beschreibung und den in den Schutzansprüchen formulierten Neuerungen sind in den beigefügten Zeichnungen dargestellt:

Claims (5)

1 Ein Hüftpfannendorn (1) als Gewindebolzen zur zusätzlichen Fixation einer vorzugsweise zementlos implantierbaren Hüftgelenkpfannen-Endoprothese (7), in der bevorzugten Ausführungsform als ein modularer in eine Hüftgelenkpfannen-Endoprothese winkelstabiler, eingeschraubter Verriegelungszapfen, ist dadurch gekennzeichnet, dass der Hüftpfannendorn (1) aus einem Bolzen (2), einem Gewinde (4) zur winkelstabilen Adaption des Hüftpfannendornes (1) in einem entsprechenden Gewinde der Hüftpfannen-Endoprothese (7) und einer Adaptionsvorrichtung (5) für ein OP-Instrument besteht.
Ein Hüftpfannendorn (1) als ein winkelstabiler, eingeschraubter Verriegelungszapfen zur zusätzlichen Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese (7) nach Schutzanspruch 1, ist dadurch gekennzeichnet, dass der Hüftpfannendorn (1) über eine Adaptionsvorrichtung (5) verfügt, die vorzugsweise als ein Sechskant ausgebildet ist, an die ein OP-Instrument gekoppelt werden kann, um eine Montage, vorzugsweise durch eine Verschraubung des Hüftpfannendornes (1) in eine entsprechende Gewindebohrungen (8) einer Hüftgelenkpfannen-Endoprothese (7), vorzunehmen.
Ein Hüftpfannendorn (1) als ein winkelstabiler, eingeschraubter Verriegelungszapfen zur zusätzlichen Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese nach Schutzanspruch 1, ist dadurch gekennzeichnet, dass durch die Montage eines oder mehrerer Hüftpfannendorne (1) eine mechanische Sicherung des Implantatsystemes, bestehend aus einem oder mehreren Hüftpfannendoren (1) und der Hüftgelenkpfannen-Endoprothese (7), zur Erzeugung einer Lage- und Rotationsstabilität durch Verankerung, Oberflächenvergrößerung, mechanischer Verspannung bzw. Verklemmung / Verpressung, erzeugt wird, indem der Bohrungsdurchmesser im Beckenknochen geringer als der Durchmesser des Hüftpfannendornes (1) ist und/oder die Achse (11) der in den Beckenknochen (10) eingebrachten Bohrung ungleich der Achse (6) des Hüftpfannendornes (1) ist indem diese parallel verschoben oder gewinkelt ist.
Ein Hüftpfannendorn (1) als ein winkelstabiler, eingeschraubter Verriegelungszapfen zur zusätzlichen Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese (7) nach Schutzanspruch 1, ist dadurch gekennzeichnet, dass der Hüftpfannendorn (1) im Bereich des Bolzens (2) in der bevorzugten Ausführung eine glatte Oberfläche aufweist, alternativ aber auch eine rauhe oder strukturierte Oberfläche aufweisen kann.
Ein Hüftpfannendorn (1) als ein winkelstabiler, eingeschraubter Verriegelungszapfen zur zusätzlichen Fixation einer zementlos implantierbaren Hüftgelenkpfannen-Endoprothese (7) nach Schutzanspruch 1, ist dadurch gekennzeichnet, dass der Hüftpfannendorn (1) im Bereich des Bolzens (2) in der bevorzugten Ausführung einen kreisrunden Querschnitt aufweist, alternativ aber auch einen ovalen oder vieleckigen Querschnitt oder zusätzliche Applikationen (3) in Form von Bohrungen (3) Kerben (3) oder Abflachungen (3) aufweisen kann.
