Abstract
Description
Gegenwärtiger Stand der TechnikCurrent state of the art
Draining tubes are inserted after surgery for the removal of wound secretions, bodily fluids, suppurations. These are to divert secretions via lateral openings in the drainage course in the sense of a gravity drainage and overflow drainage. Drains can be designed as a hose drainage or as a flat drainage. In the case of the latter, drainage also succeeds via capillary action. Also drainage hoses are inserted, which are connected to a vacuum. Wound drainage usually works only immediately after an operation, as fibrin precipitation, coagulation of blood and tissue damage cause a rapid obstruction of the drainage openings.
In the treatment of open secondary healing soft tissue wounds, the vacuum sponge therapy has become established in recent years. Here, a polyurethane sponge, which is inserted into a wound and sealed with a film dressing, used as a filter for durable longer-term over days to be performed wound drainage. The wounds are usually cleansed quickly and, in particular, it is also possible to heal problematic wounds.
Also for the treatment of internal wounds, which arise as a result of Anastomseninsuffizienzen or perforations on the intestine, the endoscopic vacuum sponge therapy is used successfully.
For the treatment of septic wounds in the abdominal cavity in, for example, peritonitis, polyurethane sponges are also used for secretion drainage. These partially large-area wound dressings are placed under a negative pressure with a film in the form of an occlusion dressing on the open abdomen with one or two drainage lugs.
The invention has several improved and expanded uses of vacuum sponge therapy.
Beschreibung und Einsatzmöglichkeit und Handhabung der ErfindungDescription and possible use and handling of the invention
The invention is an open-pore sponge provided with drainage tubing which divides into smaller subunits. The drainage hoses are connected to each other via connecting element.
For example, in abdominal surgery, the invention enables a targeted placement of vacuum sponge drains exactly at the actual septic intra-abdominal focus. In addition, drainage is possible from the "deepest points" where wound water can collect in the abdominal cavity. It is also the placement in hollow organs such as a intestinal lumen possible. This results in new treatment options.
The invention can be introduced into the abdominal cavity surgically in the form of large-area sponge packages, but it is also possible to divide it into several subunits. For this purpose, it is pre-perforated in order to accomplish the subunits by simply pulling the units apart. About connecting elements, the individual drainage hoses are connected to each other inside or outside the abdominal cavity, so that only a single hose must be connected to a vacuum. A vacuum can be applied to all subunits. It is possible to rinse the drainage hoses and use them as rinsing hoses to perform a so-called rinsing-suction treatment.
The fact that over the entire length of a sponge element, the perforated end of the drainage tube is secured in the sponge, an advantage of the invention is that it comes only slightly to the length after shortening the negative pressure to contraction of the sponge in width. In this way, the original placement of the sponge remains secured.
The invention is to be used in cases in which repeated short-term surgical procedures due to septic conditions are to be expected (so-called relaparotomies). The invention generally must be inserted and removed under conventional operative conditions (i.e., opening of the body cavity via a cut). Their use will initially be confined to cases of severe septic disease, a risk is an iatrogenic injury to organs by the direct suction on the sponge. Place the vacuum sponge drainage after opening the abdominal cavity. Subsequently, the abdominal cavity is closed again with a seam, so that a suction can be established through the resealed space. It is also the use of the invention in combination with an airtight film dressing possible for abdominal closure possible.
It is also the minimally invasive, such as laparoscopic placement of the invention possible.
The embodiments contained in the description and the following drawings are merely exemplary embodiments.
The invention will be explained with reference to the drawings according to structure and handling.
