Description

Degenerative Veränderungen der Netzhautperipherie können entweder allein oder in Verbindung mit krankhaften Veränderungen des Glaskörpers sowie bei Gewalteinwirkung von außen zu Rißbildungen in der Netzhaut führen. Durch diesen Riß dringt verflüssigter Glaskörper in den kapillaren Spaltraum zwischen Netzhaut und Aderhaut ein, wodurch sich die Netzhaut allmählich ablöst und die Sehfunktionen in den betroffenen Flächen weitgehend erlöschen.The retina is in the normal state only loose and separated by a capillary gap on the middle eyeball cover, also called choroid. Für die Wiederanlegung der Netzhaut an die Aderhaut sind mehrere Operationsverfahren bekannt. Ein bevorzugtes Verfahren ist die eindelleneOperation. Dabei wird eine sogenannte massive Plombe aus Sehnenmaterial, Silikongummi oder Schaumstoffmaterial im Bereich des Netzhautrisses dauerhaft auf die äußere Augapfelwandung aufgenäht, wodurch diese gemeinsam mit der Aderhaut der abgehobenen Netzhaut genähert und an letztere angelegt wird. Diese Operation stellt jedoch einen den Patienten stark belastenden Eingriff dar, der auch verschiedene Funktionen des Auges beeinträchtigen kann. Um diese Belastungen des Patienten und Schädigungen des Auges soweit als möglich einzuschränken, wurde kürzlich ein Verfahren bekannt, bei dem durch Einschub eines Ballons in den parabulbären Raum eine temporäre Eindellung des Augapfels erreicht wird, die nur solange belassen wird, bis sich durch eine vorangehende oder nachfolgende Lichtkoagulation bzw. Kryopexie eine feste Narbenbildung zwischen Netzhaut und Aderhaut herausgebildet hat. Durch den Wegfall der Aufnähung einer Plombe und Punktion äußerer Augenhüllen sowie durch zeitliche Begrenzung der Augapfeldeformierung ist dieses Verfahren, das heißt die Anwendung einer Ballonplombe, wesentlich schonender und für Patient und Arzt mit zahlreichen wesentlichen Vorteilen verbunden.For this purpose, the retina must first be re-attached to the choroid in the region of the tear and then tightly connected to it by scar formation provoked by means of light coagulation or cryopexy. Der Ballon ist um den Schlauch vulkanisiert. Das vordere Ende des Schlauches ist verschlossen und trägt eine Öse, die zum Festnähen der Ballonplombe an die Aufapfelhülle vorgesehen ist. Am enderen Ende des Schlauches ist ein Gummistopfenventil angebracht. Bei der zweiten bekannten Konstruktionslösung ist ein im gefüllten Zustand kugelförmiger Ballon von etwa 12 mm Durchmesser aus silikonisiertem Latex ebenfalls um einen hindurchziehenden Schlauch angeordnet, durch den er mit Flüssigkeit auffüllbar ist. Das vordere Ende des Schlauches überragt dabei den Ballon um einige Millimeter. Um diesem Ballon-Schlauchgebilde eine gewisse Festigkeit zu verleihen, ist am Endabschnitt des Schlauches, an dem sich der Ballon befindet, eine feine Stahlfeder eingearbeitet, die durch diese Gebilde hindurchgehend diesem eine gewise Führungsstabilität und Festigkeit verleihen und das Einschieben des Ballons erleichtern soll. Diese Nachteile sind bei der zweiten Konstruktionslösung weitgehend behoben. Der dünne Schlauch aus Silikongummi mit dem im geleerten Zustand manschettenartig anliegenden Ballon kann durch einen kleinen Bindehautschnitt in den parabulbären Raum eingeschoben werden.These two constructive solutions show significant defects. Nach ophthalmoskopischer Überprüfung des richtigen Sitzes des Ballons wird der zuführende Katheter verschlossen, die Lagebestimmungseinrichtung zurückgezogen und durch Aufklappen ihrer Teile entfernt und der Schlauchförmige Katheter mit seinem geschlossenen Ende an der Schläfe oder Stirn des Patienten in üblicherweise befestigt.
