The retina is in the normal state only loosely and separated by a capillary gap on the middle eyeball cover, also called the choroid, "Only in the area of the optic nerve entry and the front edge of the retina this is fused firmly with the choroid. In the remaining area, the choroid and the retina are fixed in their mutual position only by adhesive forces or by the contact pressure of the vitreous body. Degenerative changes of the retinal periphery, either alone or in conjunction with diseased changes in the vitreous body as well as external trauma, can lead to cracking in the retina. This rupture causes liquefied vitreous humor to enter the capillary gap between the retina and the choroid, causing the retina to gradually detach and the visual action in the affected areas to be largely extinguished.
Dadurch erfolgt' eine Resorption der subretinaleh Flüssigkeit ,und weitere FlüssigkeitThe principle of treatment of retinal detachment is a reassembly of the same to the choroid, which is achieved by a tamponade of the retinal tear from the outside. This results in a resorption of the subretinal fluid, and additional fluid

For this purpose, the retina must first be re-attached to the choroid in the area of the tear and then tightly connected to it by a scar formation provoked by means of light coagulation or cryopexy.
For the restoration of the retina to the choroid, several surgical procedures are known. A preferred method is the denting operation. A so-called massive seal of tendon material, silicone rubber or foam material in the area of the retinal tear is permanently sewn onto the outer wall of the eyeball, whereby it is approached together with the choroid of the lifted retina and applied to the latter. However, this surgery is a patient's stressful intervention that may affect various functions of the eye. In order to minimize this burden on the patient and damage to the eye as far as possible, a method has recently been known in which by inserting a balloon in the parabulary space, a temporary dehiscence of the eyeball is achieved, which is left only until a previous or subsequent photocoagulation or, cryopexy has formed a solid staining between retina and choroid. By eliminating the Aufnähung a seal and puncture outer eyelets and by limiting the time Augapfeldeformierung this method, that is, the use of a balloon seal, much gentler and connected to the patient and doctor with numerous significant benefits.
To carry out this process, two design solutions of a Bällonplombe have been known. The first is a thin-walled, filled ball-shaped rubber balloon about 30 mm in diameter, which can be filled with air through a tube passing through the balloon. The balloon is vulcanized around the tube. The front end of the tube is closed and carries an eyelet, which is provided for sewn the balloon seal to the eyeball cover. At the other end of the hose, a rubber stopper valve is attached. In the second known design solution, a balloon in the filled state of approximately 12 mm "
Made of siliconized latex also arranged around a pulling hose, through which it can be filled with liquid. The front end of the tube projects beyond the balloon usn a few millimeters "To give this balloon tube structure a certain strength, is at the end portion of the tube to which the balloon is located, a fine steel spring incorporated, which passes through this structure this a certain Provide leadership stability and strength and facilitate the insertion of the balloon · The other end of the tube is closed by an adapter with rubber diaphragm, can be filled or sucked through the liquid ·
These two constructive solutions show substantial deficiency. In the first balloon seal balloon and feeding tube are too large and allow an application only in individual cases »In addition, the conjunctiva must be wide opened in this balloon seal to sewn the end with the eyelet on the eyeball cover far back» These disadvantages largely eliminated in the second design solution. The thin tube made of silicone rubber with the sleeve cuffed in the emptied state can be inserted through a small conjunctival cut into the parabulbar space. However, in both constructive solutions the stiffening elements internally contained in the balloons or protruding beyond the balloon can lead to difficulties in introducing the balloon or the balloon , positioning and pressure damage to the surrounding tissue ·
Another disadvantage of particular importance is that with the balloon seals described above by their in the filled state spherical shape a dent of the outer eyelashes is possible only in a very localized space.

The application of the erfindungsgeroäßen device assumes that after trariskonjunktivaler cryopia around the retina in Lokalanaesthesie the tubular catheter with the attached and in the unfilled state zylinderförraigen balloon inserted partially in the orientation device state by a small conjunctival section of about 3 mm in length in the Tenon space introduced and positioned over the retina. By filling with 1 to 2 ml of physiological saline solution over the catheter fixation of the balloon takes place between the orbital wall and bulbus oculi, whereby the sclera is dented in accordance with "the best balloon shape can be selected according to the location, size and shape of the retinal tear, for example, as a ball -, cylindrical or bean shape is available. After ophthalmoscopic examination of the proper fit of the balloon, the feeding catheter is closed, the orientation device withdrawn and removed by unfolding their parts and the tubular catheter with its closed end attached to the temple or forehead of the patient in a conventional manner. The conjunctiva defect around the tubular catheter is suitably closed with a single button seam. The removal of the balloon catheter then takes place, depending on the anatomical findings, after 7 to 10 days after a slow, two-day evacuation of the balloon.
