Abstract
本发明涉及医疗器械技术领域,是一种可承载微型放射性粒子源的管道支架,主要用于中晚期胰腺癌或胆管癌的内照射治疗并具备引流功能。其由引流管(1)和引流管两端所设的相向的倒刺(2)组成。引流管内腔为引流腔(1.1),管壁内设有粒子孔道(1.2),粒子孔道直径与微型放射性粒子源外径匹配。粒子孔道的外侧壁设有微型放射性粒子源的辐照窗口(1.3)。辐照窗口长度和宽度均略小于微型放射性粒子源。本发明结构简单,可通过内窥镜微创置入体内,使用方便,兼具放疗和引流功能。
Description
技术领域
本发明涉及医疗器械技术领域,是一种可承载微型放射性粒子源的管道支架,主要用于中晚期胰腺癌或胆管癌的内照射治疗并具备引流功能。
背景技术
胰腺癌和胆管癌是一组病情凶险、治愈率低、预后极差的消化道恶性肿瘤,多数发现时已属中晚期,往往无法手术切除。肿瘤常常会压迫胰管、胆管,造成狭窄或阻塞。对于管道狭窄或阻塞,目前主要采用内镜下置入具备引流功能的管道支架来缓解临床症状。临床常用的管道支架由引流管和引流管两端外侧所设的相向的倒刺组成。通过十二指肠镜将管道支架置入患部,引流管支撑在狭窄或阻塞区域,胆汁或胰液可通过管内引流腔进行引流;两端倒刺用于固定支架位置,既可防止支架向肠腔滑脱,又可防止支架完全进入胰管或胆管内。支架可定期取出更换,以保持引流通畅。
对于胰腺癌和胆管癌的治疗,目前临床上已有用微型放射性粒子源进行局部放疗,疗效确定。医用微型放射性粒子是将放射性核素封装在钛合金外壳内制成的短杆状固体放射源。目前常用的粒子有碘(125I)、铱(192Ir)、钯(103Pd)粒子等,临床常用规格为长度4.5mm,直径0.8mm。将其置于肿瘤附近或植入瘤体内进行照射,具有治疗靶点局部剂量高,周围正常组织受量低以及安全、可靠、易防护等特点。但放射性粒子往往通过手术植入,或者通过B超、CT引导下穿刺植入,存在操作复杂、创伤大、不利于患者康复等缺点,且植入后无法取出,不能根据疗效及时调整治疗方案。
发明内容
本发明的目的是提供一种可承载微型放射性粒子源的管道支架。可通过十二指肠镜置入胰管或胆管内的患部,不仅置入时操作方便,创伤极小,而且既具有引流作用,又可对周围肿瘤进行局部持续照射,还可根据需要取出或定期更换。
本发明是对现有胰管和胆管支架的改进,由引流管和引流管两端所设的相向的倒刺组成。引流管的内腔为1条管径较粗的引流腔,引流腔外的管壁内设有与引流腔平行和等长的细小粒子孔道,孔道的内径与微型放射性粒子源的直径匹配,该孔道的外侧壁设有辐照窗口,辐照窗口的长度和宽度略小于微型放射性粒子源,以使微型放射性粒子源通过粒子孔道放置到辐照窗口后不会脱出。
本管道支架可设计成不同规格型号,如根据胆管和胰管粗细的不同可选择不同外径,根据癌瘤分布的部位不同可设计1条、2条或多条粒子孔道,根据放疗需要设计1个、2个或多个辐照窗口,辐照窗口的大小和间距也可因辐射源形状或强度大小而不同。使用时选择相应的规格型号,先将微型放射性粒子源置于本支架的辐照窗口,再按常规将支架通过十二指肠镜置入胆管或胰管的相应位置即可。
本发明成本低廉、结构简单、使用方便,既能扩张胆管或胰管起引流作用,又能在微创下将微型放射性粒子源置于癌瘤周围,对癌瘤进行近距离持续有效的放疗,还能在需要时适时取出或更换,有利于患者治疗和康复。
本发明也可通过内窥镜或其它途径置入其它部位,对癌瘤进行放疗。

具体实施方式
现结合附图和实施例对本发明作详细描述。
本发明由引流管1和引流管两端所设相向的倒刺2组成。引流管1内腔为引流腔1.1,周围管壁内设有粒子孔道1.2,其直径与微型放射性粒子源外径匹配。粒子孔道1.2的外侧壁设有辐照窗口1.3。辐照窗口1.3长度和宽度略小于微型放射性粒子源。
使用时,根据治疗方案,选择适当的微型放射性粒子源和相应规格型号的管道支架,逐一将微型粒子置入支架的粒子孔道1.2并使其固定于辐照窗口1.3。
再按常规通过内镜将支架置入胆管或胰管内,于X线透视下调整支架位置,使微型放射性粒子源位于病变区域附近。倒刺2可帮助固定支架位置。
实施例1.一种可承载微型放射性粒子源的胰管支架
本发明引流管的长度为54.4mm,外径为3.2mm,引流腔直径为1.8mm,管壁内设有1条粒子孔道。粒子孔道内径为0.8mm。粒子孔道的外侧壁设有3个辐照窗口,长宽高分为4.4×0.7×0.2(mm),相邻辐照窗口中心间隔10.0mm。两侧辐照窗口距引流管两端均为15.0mm。两根倒刺长度均为7mm,其根部距引流管两端各5mm。微型放射性粒子源长度4.5mm,直径0.8mm。单个粒子活度0.5mCi。这种设计治疗剂量为50Gy的承载放射性125I粒子源的胰管支架已成功通过内镜下支架置入术置入猪胰管内,并证实此支架安全有效。
实施例2.一种可承载微型放射性粒子源的胆管支架
本发明引流管的长度为69.4mm,外径为3.6mm,引流腔直径为1.5mm,管壁内设有左右对称的2条粒子孔道。粒子孔道内径为0.8mm。每条粒子孔道的外侧壁均设有6个辐照窗口,长宽高分为4.4×0.7×0.2(mm),相邻辐照窗口中心间隔5.0mm。头端辐照窗口距引流管起始端距离为25.0mm,尾端辐照窗口距引流管末端距离为15.0mm。两根倒刺长度均为7mm,其根部距支架两端各5mm。微型放射性粒子源长度4.5mm,直径0.8mm。单个粒子活度0.6mCi。这种设计治疗剂量为150Gy的承载放射性125I粒子源的胆管支架已成功通过内镜下支架置入术置入猪胆管内,并证实此支架安全有效。
Claims (3)
1、一种可承载微型放射性粒子源的管道支架,由引流管(1)和引流管两端相向的倒刺(2)组成,引流管(1)内腔为引流腔(1.1),其特征在于管壁内设有粒子孔道(1.2),粒子孔道(1.2)直径与微型放射性粒子源外径匹配,粒子孔道(1.2)的外侧壁设有辐照窗口(1.3),辐照窗口的大小与微型放射性粒子源匹配并使粒子源不会从窗口脱出。
2、按权利要求1所述的管道支架,其特征在于所说的粒子孔道为1-2个。
3、按权利要求1或2所述的管道支架,其特征在于所说的辐照窗口为1-6个。
