Abstract
本发明涉及护肝片的剂型改进。其药用原料是由柴胡626g，茵陈626g，板蓝根626g，五味子336g，猪胆粉40g，绿豆256g组成，上述药物制成颗粒剂。其制备方法为：柴胡、茵陈、板蓝根加水提取后，与部分绿豆粉混合，制成细粉A；五味子用75％乙醇回流提取，与剩余的绿豆粉混匀，制成细粉B；取猪胆粉，与上述细粉A、细粉B和适量的糊精、甜菊苷混匀，制成颗粒。其优点在于，不使用蔗糖、减少了辅料用量，提高了吸收度，增强了药效。其制备方法，具有提取纯度高，药物损失少，适合于工业化生产。
Description
(一)所属技术领域
本发明涉及一种治疗肝炎的药物，属于护肝片的剂型改进。
(二)背景技术
护肝片是由柴胡313g，茵陈313g，板蓝根313g，五味子168g，猪胆粉20g，绿豆128g制成的片剂。这种片剂的辅料采用蔗糖和滑石粉，对于大多数肝病患者来说，都厌油腻、不喜欢甜的食品，所以护肝片不易让患者接受。另外，片剂溶解后，在胃肠道局部药物浓度过高，刺激胃肠粘膜，产生不良反应，部分患者不适应。
(三)发明内容
本发明的目的，在于提供一种护肝片的新剂型，具有辅料含量少、不含蔗糖、药效好、吸收度高好的优点。
本发明的另一目的在于提供上述药物的制备方法。
本发明是通过以下措施来实现的：
本发明公开了一种治疗肝炎的药物，其药用原料是由柴胡626g，茵陈626g，板蓝根626g，五味子336g，猪胆粉40g，绿豆256g组成，其特别之处在于：上述药物制成颗粒剂。
上述本发明的药物，所述颗粒剂的辅料为糊精和甜菊苷。
上述本发明的药物的制备方法，包括以下步骤，
(1)柴胡、茵陈、板蓝根加水煎煮二次，每次2小时，滤过，合并滤液，静置48小时，取上清液，减压浓缩成80℃相对密度为1.30的清膏，与部分绿豆粉混合，减压干燥，粉碎成细粉A；
(2)五味子粉碎成粗粉，用75％乙醇回流提取三次，第一次3小时，第二次2小时，第三次1小时，合并提取液，静置24小时，取上清液，回收乙醇，并浓缩至相对密度80℃相对密度为1.28，与剩余的绿豆粉混匀，减压干燥，粉碎成细粉B；
(3)取猪胆粉，与上述细粉A、细粉B和适量的糊精、甜菊苷混匀，制成颗粒，装袋即得。
本发明的颗粒剂称为护肝颗粒，其处方来源于中医名著《伤寒论》中小柴胡汤和茵陈蒿汤的配伍原理，由柴胡、茵陈、板蓝根、五味子、胆汁粉、绿豆六味组成，其中君药柴胡选用生长三年以上的野生红柴胡，茵陈选用春季采集的干燥幼茵，板蓝根选用安徽地产，五味子选用东北产果实成熟的北五味子，质优品正。
护肝颗粒处方由柴胡、茵陈、板蓝根、五味子、猪胆汁、绿豆六味组成。功能疏肝理气，健脾消食。柴胡为君，性味苦辛微寒，归肝胆经，有和解退热，疏肝解郁之功；茵陈苦泄下降，清利湿热，板蓝根清热解毒，共为臣药，既解肝郁，又清郁热或虚热；五味子酸温，敛肺滋肾，且现代药理研究亦证实其能降低转氨酶。胆汁苦寒，归肺、肝、胆经，清热解毒；绿豆清胆养胃。柴胡为辛燥之品，佐以胆汁，免伤肝阴，佐以五味子，滋敛肾阴，绿豆更能健脾养胃，三味共为佐药。六味相配伍，共奏疏肝理气，清热解郁，健脾消食之功。
《中医内科学》中认为：胁痛之病，主要责于肝胆，又因肝主疏泄、性喜条达、凡情治失调、肝气郁结、或气郁日久、乏滞血瘀或脾失健运、湿热内郁、疏泄不利等，均可导致胁痛。因此在治疗上，多以疏肝理气，健脾消食，确定治则，进行治疗。本发明的药物，制备成颗粒剂，与护肝片的功能与主治相同，具有疏肝理气，健脾消食，降低转氨辅作用，用于慢性肝炎及早期肝硬化等。
甜菊苷是从甜叶菊干叶中提取的低热量、高甜度天然甜味剂，其热量为蔗糖的1/300、甜度为蔗糖的200-350倍，具有低热量、高甜度、在人体内不代谢、无残留、安全的特点。本发明的颗粒剂采用甜菊苷代替蔗糖，大大减少了辅料的用量。
药效学研究表明，护肝颗粒对四氯化碳、D-半乳糖胺所致大鼠肝损伤而引起的血清谷丙转氨酶升高，具有明显的降低作用；镜下观察，对四氯化碳所致的小鼠肝损伤有减轻作用；对氢化可的松所致的小鼠免疫功能低下有明显的对抗作用，可使单核吞噬细胞吞噬指数和吞噬活性明显增强。综上所述，护肝颗粒具有明显的保肝降酶，增强免疫功能的作用。
临床结论：护肝颗粒对降低血清谷丙转氨酶显效率达90％，总有效率达98％。对治疗慢性肝炎、迁延性肝炎的显效率达70％，总有效率达95.8％，对治疗肝硬化的显效率达63％，总有效率达82.5％，服药后食欲增加、腹胀消失达99％，胁痛消失、体力恢复达90％，肝大变至正常、硬变软者达90％。
经长期毒性试验，本品口服用药无明显的毒性反应。
本发明的药物采用无糖颗粒，从而减少患者服用蔗糖及滑石粉的大量无效辅料成分，对大多患者来讲，更加易于接受。经过改进颗粒以后，提高了药效及吸收度。制成颗粒剂后，避免了普通片剂在胃肠道局部药物浓度过高，刺激胃肠粘膜等缺点，减少不良反应，提高患者适应性。
本发明的制备方法，具有提取纯度高，药物损失少，适合于工业化生产。
(四)具体实施方式
下面结合具体实施方式对本发明作具体的描述。
本发明的颗粒剂，其药用原料是由柴胡626g，茵陈626g，板蓝根626g，五味子336g，猪胆粉40g，绿豆256g组成，辅料为糊精和甜菊苷。
制备方法，包括以下步骤，
(1)柴胡、茵陈、板蓝根加水煎煮二次，每次2小时，滤过，合并滤液，静置48小时，取上清液，减压浓缩成80℃相对密度为1.30的清膏，与部分绿豆粉混合，减压干燥，粉碎成细粉A；
(2)五味子粉碎成粗粉，用75％乙醇回流提取三次，第一次3小时，第二次2小时，第三次1小时，合并提取液，静置24小时，取上清液，回收乙醇，并浓缩至相对密度80℃相对密度为1.28，与剩余的绿豆粉混匀，减压干燥，粉碎成细粉B；
(3)取猪胆粉，与上述细粉A、细粉B和适量的糊精、甜菊苷混匀，制成颗粒，干燥，装袋2g/袋，制成500袋颗粒剂。
本品每袋含五味子以五味子乙素计，不得少于0.33mg。
用法与用量：口服，一次一袋，一日3次。

1.一种治疗肝炎的药物，其药用原料是由柴胡626g，茵陈626g，板蓝根626g，五味子336g，猪胆粉40g，绿豆256g组成，其特征在于：其药物制成颗粒剂。
2.根据权利要求1所述的治疗肝炎的药物，其特征在于：所述颗粒剂的辅料为糊精和甜菊苷。
3.一种权利要2所述药物的制备方法，其特征在于：包括以下步骤，
(1)柴胡、茵陈、板蓝根加水煎煮二次，每次2小时，滤过，合并滤液，静置48小时，取上清液，减压浓缩成80℃相对密度为1.30的清膏，与部分绿豆粉混合，减压干燥，粉碎成细粉A；
(2)五味子粉碎成粗粉，用75％乙醇回流提取三次，第一次3小时，第二次2小时，第三次1小时，合并提取液，静置24小时，取上清液，回收乙醇，并浓缩至相对密度80℃相对密度为1.28，与剩余的绿豆粉混匀，减压干燥，粉碎成细粉B；
(3)取猪胆粉，与上述细粉A、细粉B和适量的糊精、甜菊苷混匀，制成颗粒，装袋即得。
