Abstract

组织修复和再生的过程是复杂的,并在多水平上受到调节和组织(orchestrated)。通常个别的因子(例如细胞因子、受体、激素等)在此过程中起重要作用,并作为多种治疗形式可能的治疗方式被测试。但是,由于这种调节的复杂性质,单一因子或受体等,或甚至因子或受体的简单组合已被证明不足以提供有益效果。因此,本领域需要提供大量参与哺乳动物组织修复和再生复杂过程的不同分子。

发明祥述
在进行本发明之前,对下文中将要使用的某些术语进行定义可有助于理解本发明。
此处使用的术语“刺激”,指的是细胞表面部分连接诱导的初始反应。例如,对于受体,这样的刺激需要受体连接和后续的信号转导事件。关于T细胞刺激,这样的刺激指的是T细胞表面部分连接,在一个实施方案中其随后诱导信号转导事件,例如TCR/CD3复合体的结合。而且,刺激事件可活化细胞并上调或下调分子的表达或分泌,例如下调肿瘤生长因子β(TGF-β)。因此,即使没有直接信号转导事件,细胞表面部分的连接也可导致细胞骨架结构的重组或细胞表面部分的接合,其中每一个都能增强、修饰,或改变后续的细胞反应。

对于闭塞血管旁路的新生血管形成,“有效治疗量”是导致新血管形成的量,新血管至少能传输部分正常情况下要通过堵塞血管的血液。
T细胞组合物
T细胞有独特的生物学和功能。它们在表面上表达和分泌能与其他细胞/组织以特定方式相互作用的一系列重要分子,其能促进或调节这些细胞或组织的分化/去分化/成熟/组织形成和修复活性。特别的,T细胞在不同的活化状态(并因此表达不同的表面或分泌分子)能导致组织发育、分化或重组和修复,这能改善多种医疗状态。
T细胞,尤其是活化T细胞,具有很多可能参与哺乳动物组织修复和再生复杂过程中的分子,并满足本领域提供一系列复杂的调节分子的需要,这些分子为通过调节/控制此过程中其他细胞类型以提供组织生长和/或改造所必需。
一般地,由诱导活化的细胞表面部分连接而产生本发明的活化T细胞。通过活化T细胞群并用结合辅助(accessory)分子的配体刺激T细胞表面的辅助分子而产生活化T细胞,其描述于例如,标题为共刺激和细胞浓度的美国专利申请(申请于2002年4月26号),和美国专利申请08/253,694;08/435,816;08/592,711;09/183,055;09/350,202;和09/252,150,和专利6,352,694;5,858,358;和5,883,223,此处全部引用作为参考。
T细胞可从多种来源获得,包括外周血单核细胞、骨髓、胸腺、组织活检、肿瘤、淋巴结组织、消化道相关淋巴组织、粘膜相关淋巴组织、脾组织,或其他任何淋巴组织和肿瘤。T细胞可从T细胞系和自体的或同种异体的来源获得。T细胞还可从异种来源获得,例如,小鼠、大鼠、非人灵长类和猪。
优选,通过单采血液成分术或白细胞分离法从个体循环血中分离细胞。单采血液成分术产物通常包含淋巴细胞,包括T细胞、单核细胞、粒细胞、B细胞,其他有核白细胞、红细胞,和血小板。在一个实施方案中,通过单采血液成分术或白细胞分离法收集的细胞可洗涤以去除血浆成分,并为了后续的处理步骤将细胞置于适当的缓冲液或培养基中。在本发明一个实施方案中,细胞用磷酸盐缓冲液(PBS)洗涤。在另外的实施方案中,洗涤液可缺少钙和可缺少镁或如不缺少所有的二价离子,也可缺少很多二价离子。作为那些本领域普通的技术人员容易理解洗涤步骤可通过本领域熟知的方法完成,例如根据制造商的建议使用半自动化的“极限沉降”(flow-through)离心(例如,the Cobe 2991 cell processor,Baxter)。洗涤之后,细胞可用多种生物相容的缓冲液重悬,例如,无Ca++/Mg++的PBS。另外,可去除单采血液成分术样品中不需要的成分并将细胞重悬于培养基中。
在另一实施方案中,如前所述,通过溶解红细胞、分离和保存单核细胞或者例如,通过PERCOLLTM梯度离心而从外周血淋巴细胞中分离T细胞。T细胞的特异亚群,例如CD28+、CD4+、CD8+、CD45RA+和CD45RO+T细胞,可通过正或负选择技术进一步分离。例如CD3+、CD28+T细胞可用CD3/CD28缀合的磁珠进行正选择(例如,DYNABEADSM-450 CD3/CD28 T Cell Expander)。在本发明的一个方面,用针对负选择的细胞表面独特标志的抗体组合而完成通过负选择富集T细胞群。优选的方法是通过使用单克隆抗体混合物的负磁性免疫粘附或流式细胞术进行细胞分选和/或选择,其中单克隆抗体针对负选择细胞上的细胞表面标志。例如,通过负选择富集CD4+细胞,单克隆抗体混合物一般包括抗CD14、CD20、CD11b、CD16、HLA-DR,和CD8的抗体。
因此,在一个实施方案中,本发明使用的顺磁微粒的大小足够被吞噬单核细胞吞噬,其随后通过磁性分离去除。在某些实施方案中,顺磁微粒是可商业得到的小珠,例如,那些由Dynal AS生产的商品名为DynabeadsTM的小珠。这里可作实例的DynabeadsTM是M-280、M-450和M-500。一方面,用“无关”蛋白质(例如,血清蛋白质或抗体)包被顺磁微粒去除其他非特异细胞。无关蛋白质和抗体包括那些不能特异地靶向欲扩增的T细胞的蛋白质和抗体或其片段。在某些实施方案中,无关小珠包括用绵羊抗小鼠抗体、山羊抗小鼠抗体和人血清白蛋白包被的小珠。
刺激用T细胞的另一种制备方法是在洗涤步骤后冷冻细胞,它不需要单核细胞去除步骤。希望不被理论束缚,冷冻和后续的融化步骤通过去除细胞群中的粒细胞和一定程度的单核细胞而提供更单一的产物。在去除血浆和血小板的洗涤步骤后,细胞可悬浮在冷冻液中。许多本领域已知的冷冻液和参数在本发明中是有用的,一种方法涉及使用含20%DMSO和8%人血清白蛋白(HSA)的PBS,或其他适合的细胞冷冻介质。然后其用介质1∶1稀释以使DMSO和HSA的终浓度分别为10%和4%。然后细胞以每分钟1°的速度冷冻到-80℃并贮存在液氮贮存罐的汽相。可使用其他控制冷冻方法以及不加控制的在-20℃或液氮中立即冷冻的方法。
由诱导活化的细胞表面部分连接而产生本发明的活化T细胞。通过活化T细胞群并用结合辅助分子的配体刺激T细胞表面的辅助分子而产生活化T细胞,其描述于例如标题为共刺激和细胞浓度的美国专利申请(申请于2002年4月26号),专利申请08/253,694;08/435,816;08/592,711;09/183,055;09/350,202;和09/252,150,和专利6,352,694;5,858,358;和5,883,223,此处全部引用作为参考。
一般地,T细胞活化可通过细胞表面部分的连接完成,例如刺激T细胞受体(TCR)/CD3复合体或CD2表面蛋白质。商业上提供许多抗人CD3的单克隆抗体,可作实例的是,BC3克隆(XR-CD3;FredHutchinson Cancer Research Center,Seattle,WA)、OKT3,从American Type Culture Collection获得的杂交瘤细胞中制备,和单克隆抗体G19-4。同样地,抗CD2抗体的刺激形式是已知的和可获得的。用抗CD2抗体通过CD2的刺激一般至少用两种不同的抗CD2抗体的组合完成。抗CD2抗体的刺激组合已有描述,包括以下:T11.3抗体与T11.1或T11.2抗体组合(Meuer等人,Cell 36:897-906,1984),和9.6抗体(其与T11.1识别同样的表位)与9-1抗体组合(Yang等人,J.Immunol.137:1097-1100,1986)。也可使用与上述任何抗体结合同样表位的其他抗体。其他抗体或抗体组合,可用标准技术制备和鉴定。还可通过与抗原、肽、蛋白质、肽-MHC四聚体(参见Altman等人,Science 1996年10月4号;274(5284):94-6)、超抗原(例如,葡萄球菌肠毒素A(SEA)、葡萄球菌肠毒素B(SEB)、中毒性休克综合症毒素1(TSST-1))、内毒素的接触完成刺激,或通过与多种促分裂原,包括但不限于,植物凝集素(PHA)、乙酸肉豆蔻佛波醇(phorbolmyristate acetate)(PMA)和离子霉素、脂多糖(LPS)、T细胞促分裂原,和IL-2的接触完成刺激。

另外,根据本发明还可使用,无论是天然的还是由化学或重组技术合成的天然配体的结合同源物。其他因子可包括天然的和合成的配体。因子可包括,但不限于,其他抗体或其片段、肽、多肽、生长因子、细胞因子、趋化因子、糖肽、可溶性受体、类固醇、激素、促分裂原(例如PHA)或其他超抗原。
T细胞初始刺激信号和共刺激信号可由不同的方法提供。例如,提供每一种信号的因子可在溶液中或与表面偶连。当偶连到表面上时,因子可与同一表面(即,“顺式”结构)或与分离的表面(即,“反式”结构)偶连。另外,一种因子可偶连到表面上而其他因子在溶液中。在一个实施方案中,提供共刺激信号的因子结合到细胞表面上,提供初始活化信号的因子在溶液中或与表面偶连。在某些实施方案中,两种因子都在溶液中。在另一实施方案中,因子可是溶解的形式,然后交联到表面上,例如表达FC受体或抗体或其他与该因子结合的结合因子的细胞。在优选的实施方案中,这两种因子固定在小珠上,或在同一小珠上,即“顺式”,或在分离的小珠上,即“反式”。举例来说,提供初始活化信号的因子是抗CD3的抗体,提供共刺激信号的因子是抗CD28的抗体;两种因子以等同的分子数量固定在同一小珠上。在一个实施方案中,使用每一种抗体以1∶1的比例结合到小珠上用于CD4+T细胞扩增和生长。在本发明的某些方面,结合到小珠上的抗CD3∶CD28抗体使用的比例与使用1∶1的比例观察到的扩增相比可观察到T细胞扩增的提高。在一个特定的实施方案中,与使用1∶1的比例观察到的扩增相比观察到约从0.5到约3倍的提高。在一个实方案例中,结合到小珠上的CD3∶CD28抗体的比例范围从100∶1到1∶100及二者之间所有的整数值。在本发明的一个方面,结合到微粒上的抗CD28的抗体比抗CD3的抗体多,即CD3∶CD28的比例小于1。在本发明的某些实施方案中,结合到小珠上的抗CD28的抗体与抗CD3的抗体的比例大于2∶1。在一个特定的实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶100。在另一实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶75。在另一实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶50。在另一实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶30。在一个优选的实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶10。在另一实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为1∶3。在再一实施方案中,使用结合到小珠上的CD3∶CD28抗体的比例为3∶1。
为刺激T细胞和其他靶细胞,微粒与细胞的比例可使用从1∶500到500∶1及二者之间的任何整数值。作为本领域普通的技术人员容易理解,微粒与细胞的比例取决于微粒相对靶细胞的大小。例如,小尺寸的小珠只能结合几个细胞,而更大的小珠可结合许多细胞。在某些实施方案中细胞与微粒的比例范围从1∶100到100∶1及二者之间的任何整数值,并在另一实施方案中,包括1∶9到9∶1及二者之间任何整数值的比例,也可用于刺激T细胞。如上所述,可以改变引起T细胞刺激的抗CD3和抗CD28偶联的小珠微粒与T细胞的比值,但一些优选的比值至少包括1∶5、1∶4、1∶3、1∶2、1∶1、2∶1、3∶1、4∶1到6∶1,其中一个优选的比例是至少21∶1小珠微粒每T细胞。在一个实施方案中,使用的微粒与细胞的比例为1∶1或更小。在另一实施方案中,微粒与细胞的比例随刺激天数而变。例如,在一个实施方案中,微粒与细胞的比例在第一天为1∶1-10∶1,然后每天或隔天加入另外的微粒直到10天,最后的比例为1∶1-1∶10(基于添加当天的细胞计数)。在一个特定的实施方案中,在刺激的第一天微粒与细胞的比例为1∶1,并在刺激的第三和第五天调整到1∶5。在另一实施例中,根据刺激第一天的最终比例为1∶1,每天或隔天添加微粒,第三和第五天的比例为1∶5。在另一实施方案中,在刺激的第一天微粒与细胞的比例为2∶1,并在刺激的第三和第五天调整到1∶10。在另一实施方案中,根据刺激第一天的最终比例为1∶1,每天或隔天添加微粒,第三和第五天的比例为1∶10。本领域的一名技术人员应当理解其他多种比例适合在本发明中使用。特别的,比例将根据微粒大小和细胞的大小及类型改变。
使用某些方法学可在初始活化和刺激后,约12到约14天后将T细胞与刺激物分离,有利保持T细胞群的长期刺激。T细胞增殖的速率被定期监测(例如,每天),例如,通过检查T细胞的大小或测量体积,例如使用Coulter Counter。在这方面,静止T细胞的平均直径约6.8微米,刺激配体存在下起始活化和刺激,T细胞的平均直径将在4天时增加到超过12微米并在约6天时开始下降。当T细胞的平均直径下降到约8微米时,T细胞可被再活化和再刺激以诱导T细胞的进一步增殖。另外,可通过分析细胞表面存在的分子监测T细胞增殖的速率和T细胞再刺激的时间,例如,CD154、CD54、CD25、CD137、CD134、B7-1、B7-2,它们被诱导于活化T细胞上。
为诱导CD4+和/或CD8+T细胞群的长期刺激,需要用刺激因子再活化和再刺激T细胞几次以产生数量增加是原T细胞群约10到约1,000倍的CD4+或CD8+细胞群,例如,抗CD3的抗体和抗CD28的抗体(例如B-T3、XR-CD28(Diaclone,Besancon,法国))或单克隆抗体ES5.2D8。例如,在本发明的一个实施方案中,如此处所描述T细胞被刺激2-3次。在另一实施方案中,如此处所描述T细胞被刺激4或5次。
在另一实施方案中,可调节或裁剪(tailored)暴露到刺激因子例如抗CD3/抗CD28(即,CD3XCD28)包被小珠的时间以获得需要的T细胞表型。由于TH细胞的扩增可诱导需要的组织修复和/或再生,因此相对CD8+细胞毒或抑制T细胞(TC),研究人员可能需要更多的辅助T细胞(TH),一般为CD4+T细胞。CD4+T细胞,表达重要的免疫调节分子,例如GM-CSF、CD40L和IL-2。当优选CD4介导的辅助时,例如此处描述的那样,保持或增强CD4∶CD8比例的方法有重要作用。在本发明的一个方面,提高表达GM-CSF或IL-2的输注细胞数量是有用的,所有这些因子主要由CD4+T细胞表达。另外,在需要增加CD8+T细胞数量和需要较少CD4辅助细胞的情况下,也可使用此处描述的T细胞活化方法,例如,在刺激和/或培养之前预选CD8+T细胞。优选升高水平的IFN-γ时可能发生这样的情况。另外,在其他应用中,可能需要使用TH1型细胞群而不是TH2型细胞群(或反之亦然)或其上清。
为实行不同T细胞群的分离,可改变或脉动变化(pulsed)诱导活化的细胞表面部分连接的时间。例如,通过改变扩增时间和/或使用不同类型的刺激因子(例如,抗体或其片段、肽、多肽、MHC/肽四聚体、生长因子、细胞因子、趋化因子、糖肽、可溶性受体、类固醇、激素、促分裂原,例如PHA,或其他超抗原)以获得特定的目的表型。多种表型标志的表达随时间而变;因此,可选择特定的时间点或刺激因子以获得特定的T细胞群。所以,根据刺激的细胞类型,刺激和/或扩增的时间可以是4周或更短、2周或更短、10天或更短,或8天或更短(4周或更短包括从4周到1天(24小时)范围的所有时间)。在一些实施方案中,刺激和扩增可进行6天或更短、4天或更短、2天或更短,并在其他实施方案中少至24小时或更短,并优选4-6小时或更短(这些范围包括其中的任何整数值)。当对T细胞进行短时刺激时,T细胞群的数量可能并不显著升高,但该群细胞将提供更强壮和更健康的活化T细胞,它们能继续在体内增殖并更接近类似天然效应T细胞群。
暴露于不同刺激时间和因子的T细胞可具有不同的特性。例如,典型的血液或单采血液成分术的外周血单核细胞产物的辅助T细胞群(TH,CD4+)多于细胞毒或抑制T细胞群(TC,CD8+)。通过刺激CD3和CD28受体体外(exvivo)扩增T细胞产生的T细胞群,在约8-9天前主要由TH细胞组成,而在约8-9天后,该群T细胞主要由不断增加的TC细胞群组成。因此,根据治疗目的,将主要由TH细胞组成的T细胞群输入受试者可能是有益的。
另外,除CD4和CD8标志外,其他表型标志变化显著,但大部分,在细胞扩增过程期间是可重现的。因此,这种重现性能根据特殊的目的(例如,骨再生相对于血管发生)调整活化的T细胞产物。
在一个这样的例子中,在随时间重现性变化的重要表型标志中,有IL-2高亲和力受体(CD25)、CD40配体(CD154)和CD45RO(通过与TCR的优先结合而提高TCR抗原结合敏感性的分子)。作为一名本领域普通的技术人员容易理解,由于多种原因这些分子是重要的。例如,CD25构成了允许T细胞快速分裂的自分泌环的重要部分。CD154已被证明在下述过程中起重要作用:刺激抗原呈递树突细胞成熟;活化抗体产生B细胞;调节TH细胞增殖;增强TC细胞分化;调节TH细胞抗原呈递细胞的细胞因子分泌;和刺激共刺激配体的表达,包括CD80、CD86和CD154。
在体外扩增过程中,细胞因子、细胞表面受体和本发明组织修复和再生中的其他重要因子,它们的生成增加经常开始的很早。因此,由于已知细胞因子和其他因子在介导T细胞的活化和功能及细胞分化的调节中是重要的,因此这些因子在开发T细胞治疗产品中很可能是关键的。这方面的重要分子,包括,但不限于,IL-2、IL-4、TNF-α,和IFN-γ、转化生长因子(TGF)TGF-β、神经白细胞素(neuroleukin)(磷酸葡萄糖异构酶)、神经生长因子、NF-κB转录因子和CD40。因此,通过在扩增前几天获得T细胞群并将这些细胞输入到受试者中,或直接在损伤部位使用这些细胞或其上清,可出现治疗效果,其中在体内出现另外的T细胞活化和扩增和/或出现组织修复和再生。
在体外扩增过程中,除上面讨论过的细胞因子和标志外,已知在介导T细胞活化和免疫介导的靶细胞调节中重要的粘附分子的表达也变化显著,但是可重现的。例如,CD62L对于T细胞归巢到淋巴组织和运输T细胞到炎症部位是重要的。由于活化后的早期出现CD62L的下调,所以T细胞可进行短期扩增。相反地,长期培养产生的T细胞群具有更高水平的CD62L,并因此在其他优选条件下有更高的能力靶向活化T细胞到这些部位。多肽表达随时间改变的另一个例子是CD49d,一个从血液向炎症部位组织间隙输送淋巴细胞中涉及的粘附分子。CD49d配体与CD49d的结合也允许T细胞接受通过VCAM-1或纤连蛋白配体结合的活化和增殖共刺激信号。粘附分子CD54的表达,与T细胞-APC和T细胞-T细胞的相互作用和向炎症部位的归巢有关,也随扩增过程而改变。因此,可刺激T细胞与目的标志特性(profile)一致的选择时间,并随后收集和输入。也可输入包含活化T细胞上清的组合物。也可在损伤部位直接使用活化T细胞,或其上清。因此,可调整T细胞以表达据信为治疗适应症提供最好治疗效果的标志。
在多种实施方案中,本领域的一名普通技术人员理解从细胞中去除刺激信号取决于使用的表面类型。例如,如果使用顺磁珠,那么磁性分离是适宜的选择。顺磁珠的制造商说明书(例如,DYNALInc.,Oslo,挪威)详细描述了分离技术。另外,如果表面是可从细胞分离的足够大的小珠则可使用过滤。另外,商业上提供多种输血过滤器,包括20微米和80微米输血过滤器(Baxter)。因此,只要小珠大于滤器筛目的尺寸,这种过滤是非常有效力的。在相关的实施方案中,小珠可通过滤器,但可保留细胞而实现分离。
尽管此处描述的方法中使用的抗体易于从公共资源中获得,例如ATCC,还能通过标准技术制备抗T细胞辅助分子和CD3复合体的抗体。生产本发明方法中使用的抗体的方法学为本领域所公知。
在本发明的一个方面,可使用本领域公知的一些方法对T细胞进行基因修饰。T细胞可用很多本领域普通技术人员公知的RNA或DNA表达载体转染。基因修饰包括使用本领域公知的任何技术进行RNA或DNA转染,例如电穿孔法(例如使用,The Gene Pulser II,BioRad,Richmond,CA)、多种阳离子脂类,(LIPOFECTAMINETM,LifeTechnologies,Carlsbad,CA),或其他技术例如Current Protocols inMolecular Biology.,John Wiley & Sons,New York.N.Y.中所描述的磷酸钙转染。例如,500μl Opti-MEM中的5-50μg RNA或DNA与浓度为10到100μg的阳离子脂类混合,并在室温孵育20到30分钟。其他适合的脂类包括LIPOFECTINTM、LIPOFECTAMINETM。形成的核酸-脂类复合体然后被加入到总体积约2ml(例如,在Opti-MEM中)的1-3×106抗原呈递细胞中,优选2×106,并在37℃孵育2到4小时。T细胞还可用下述的病毒转导方法学进行转导。
另外可使用逆转录病毒转导技术对T细胞进行基因修饰。在本发明的一个方面,逆转录病毒载体可是兼嗜性逆转录病毒载体,优选具有长末端重复序列特征(LTR)的载体,例如,衍生自下述病毒的逆转录病毒载体:莫洛尼小鼠白血病病毒(MoMLV)、骨髓增生肉瘤病毒(myeloproliferative sarcoma virus)(MPSV)、小鼠胚胎干细胞病毒(MESV)、小鼠干细胞病毒(MSCV)、形成脾脏病灶病毒(SFFV),或腺伴随病毒(AAV)。大多数逆转录病毒载体衍生自小鼠逆转录病毒。然而,根据本发明,适合使用的逆转录病毒可得自任何禽类或哺乳动物来源的细胞。优选的这些逆转录病毒是兼嗜性的,意指它们能感染几种宿主细胞,包括人的。在一个实施方案中,要表达的基因取代了逆转录病毒的gag、pol和/或env序列。许多例证性的逆转录病毒系统已被描述(例如,美国专利5,219,740;6,207,453;5,219,740;Miller和Rosman(1989)BioTechniques 7:980-990;Miller,A.D.(1990)Human Gene Therapy 1:5-14;Scarpa等人(1991)Virology180:849-852;Burns等人(1993)Proc.Natl.Acad.Sci.USA90:8033-8037;和Boris-Lawrie和Temin(1993)Cur.Opin.Genet.Develop.3:102-109)。
T细胞组合物在组织修复和再生中的用途
本发明的活化T细胞可普遍应用到需要治疗的损坏组织或组织部位,其可包括多种不同的细胞、组织和器官。活化T细胞被“靶向”到损坏组织部位。本发明适于人类医学中广泛的多种损坏组织的修复。这些包括,但不限于骨坏死、退化软骨组织、心肌缺血、损坏内皮细胞、退化或其他损坏神经组织、灼伤部位、术后部位和器官/组织移植部位的修复和/或再生。
例如,使用本发明的活化T细胞或其上清、由该细胞产生或其上清中存在的细胞生长因子和/或其他分子,将通过与细胞表面信号受体的结合影响组织部位的其他细胞,因此刺激和放大一般与创伤愈合、组织修复或改型的过程相关联的生理事件级联。例如,将增加创伤愈合的速度,导致更快的上皮细胞再生和组织修复。最后的结果是促进组织修复和再生。本发明的细胞和/或上清可用于招募其他细胞到创伤部位或需要组织再生的部位,例如间充质干细胞、骨髓衍生的成血管细胞、神经干细胞或组织修复中涉及的任何形式的前体细胞,包括但不限于单核细胞。可根据需要的结果在体外或体内给予本发明的活化T细胞或其上清。
当目标是阻断疾病进程,以使组织发生自然愈合,或当目标是替换遗传缺陷的蛋白质功能时,本发明的活化T细胞或其上清也是有用的。
损坏组织可来自组织创伤,来自手术过程、感染、外伤损伤诱导或造成的组织损坏,或者一种疾病状态,包括但不限于,心肌梗塞、心肌缺血、骨坏死、退化软骨组织、退化神经组织、灼伤,或移植部位。可使用多种技术将本发明的活化T细胞或其上清转移到患者。例如,包括细胞和/或其上清的组合物可作为治疗植入物(implant)、注射液或经由局部应用的适当制剂由医师直接转移到创伤部位。另外,活化T细胞或其上清可局部给药,或为治疗远端患病组织而外科手术放置在正常组织部位。
创伤愈合过程是一系列协调的事件,它包括出血、血凝块形成、血块溶解并发损坏组织去除和肉芽组织作为初期修复物质沉积。肉芽组织是成纤维细胞和毛细血管的混合物。创伤愈合过程涉及不同的细胞群,包括内皮细胞、干细胞、巨噬细胞和成纤维细胞。已知创伤修复中涉及的调节因子包括全身的激素、细胞因子、生长因子、细胞外基质蛋白质和其他调节生长和分化的蛋白质。
T细胞组合物在骨修复和再生中的用途
骨在骨折后有基本的再生能力。复杂但有序的骨折修复顺序包括止血、血块溶解、肉芽组织内生、愈伤组织形成和愈伤组织改型到最佳结构(A.W.Ham.J.Bone Joint Surg.12:827-844,1930)。参与此过程的细胞包括血小板、炎性细胞、成纤维细胞、内皮细胞、周细胞、破骨细胞和成骨祖细胞(osteogenic progenitors)。
设计用于骨修复的技术在骨折治疗中是很有用的。骨折的骨的大部分仍通过铸造法(casting)治疗,它通过自然机制达到创伤修复。尽管近年在骨折治疗上有所进步,包括设备的改良、新的刺激或补充方法的发展,但此创伤修复机制仍将代表本领域的重要进展。
本发明的活化T细胞或其上清可用于促进骨折修复。此技术的其他方面包括细胞或其上清转移到治疗带有“弱骨”(weak bones)的患者中的用途(例如在象骨质疏松症的疾病中);改善由未知原因产生的不良愈合(例如,纤维不结合(fibrous non-union));促进移植整合和人工关节的功能;促进其他骨骼组织例如跟腱的愈合;并作为大的缺损修复的辅药。
转化生长因子(TGF),也在通过影响细胞增殖、基因表达和基质蛋白质合成而调节组织愈合中显示重要作用(Roberts & Sporn,1989,M.B.Sporn和A.B.Roberts,Eds.,Springer-Verlag,Heidelberg,95(Part 1))。例如TGF-β1和TGF-β2可起始软骨形成和骨生成(Joyce等人J.Cell Biol.110:195-2007,1990;Izumi等人J.Bone Min.Res.7:115-11,1992;Jingushi等人J.Orthop.Res.8:364-371,1992)。因此,本发明产生该因子的活化T细胞,可在本发明的实践中应用以影响骨折后新骨的形成。
在本发明的实施方案中本发明的活化T细胞外科手术植入到骨折部位。这样的手术过程可包括直接注射活化T细胞制剂到骨折部位、手术修补复杂骨折或关节镜手术。在活化T细胞或其上清应用于修复骨折的骨的情况下,哺乳动物修复细胞会自然移往骨损坏部位并在该部位增殖。
本发明还可用于刺激软组织的生长或再生,例如韧带、腱、软骨和皮肤。活化T细胞或其上清可用于治疗由外伤性损伤造成的骨骼结缔组织的损坏。活化T细胞产生的多种因子可促进软组织修复。这些包括,但不限于,TGF-β超家族成员(例如,TGF-β本身),其刺激编码胞外基质蛋白质的基因的表达,和其他细胞因子例如EGF和PDGF。在此过程中可能是重要的由活化T细胞产生的其他因子的实例包括(a)白细胞介素、趋化因子、干扰素、集落刺激因子;(b)细胞粘附分子家族;(c)核反式作用蛋白质(nuclear trans acting proteins)例如转录因子。
本发明的活化T细胞或其上清可置于哺乳动物宿主的结缔组织创伤区域。活化T细胞或其上清可直接注射到结缔组织创伤区域。另外,外科技术,例如关节镜手术,可用于传递细胞或上清到结缔组织创伤区。
在本发明的一个方面,活化T细胞或其上清,可用于刺激体外培养的骨细胞或其前体细胞的骨再生。与活化T细胞或其上清共培养可导致骨细胞的成熟、分化、功能提高和/或移入潜能(engraftmentpotential)增强。另外,多种细胞类型与本发明的活化T细胞或其上清体外共培养可导致非T细胞谱系细胞的功能改变。然后由共培养改变的细胞可施用于哺乳动物用于组织修复和/或再生。
活化T细胞和/或其上清还可用于修复骨转移(bone metastases)。有几种病理状态涉及钙和磷酸盐代谢紊乱。这些状态包含骨相关疾病,包括佩吉特病(Paget’s disease)和骨质疏松症,及骨转移中的骨质溶解。在许多经常发生的恶性肿瘤中骨转移表现为主要问题。高钙血症,因骨吸收引起,是恶性肿瘤常见的和非常重要的并发症,引起使人痛苦的症状,例如剧烈痛苦和自发性骨折,并可导致代谢性昏迷和死亡。而且,瘤细胞诱导的骨质溶解可决定骨肿瘤的定位和生长增加。(参见,G.R.Mundy,Bone,8,supp.1,S9-516(1987);和Calcium in BiologicalSystems,R.P.Rubin,G.B.Weiss,和J.W.Putney,Jr.eds.PlenumPress,N.Y.(1985))。体内由钙和磷酸盐异常沉积引起或导致的其他疾病状态,例如类风湿性关节炎和骨关节炎。本发明的活化T细胞组合物或其上清可与已知促进所需活性的化合物联合用于这些疾病的治疗,例如使用osteoprotegrin或二膦酸盐化合物。二膦酸盐化合物是一类已开发用于多种骨代谢病的药物,其靶标为过度骨吸收和不正常的钙化和骨化。(M.D.Francis和R.R.Martodam,“The Role ofPhosphonates in Living Systems”R.L.Hilderbrand,ed.,CRC Press,Boca Raton,Fla,1983,pp.55-96;和H.Fleisch,Bone,1987,8,Supp.1,S23-S28)。
T细胞组合物在血管生成中的用途
本发明还可用于调节血管的形成和扩展,或分别为血管发生和血管生成。这两种生理过程在创伤愈合和组织再生中发挥重要作用。
最初,胶原、基质和血管在创伤部位沉积并提供组织修复中的伤口强度。新血管的形成包括血管内皮细胞增殖、迁移和浸润,并已知被多种多肽生长因子调节。已鉴定出几种有内皮细胞生长促进活性的多肽,包括酸性和碱性成纤维细胞生长因子(FGF),血管内皮生长因子(VEGF)和血小板衍生生长因子(PDGF)。
活化T细胞表达因子例如,但不限于,TGF-β,其促进这些生长因子的表达,可将该活化T细胞施用到宿主血管系统内或需要创伤愈合/血管生成的部位,以刺激血管的形成和扩展。在一些实例中,可能需要通过组织损伤诱导创伤愈合过程。
本发明的活化T细胞或其上清,也可用于刺激在体外培养的心肌细胞、内皮细胞或这些细胞的前体细胞中的血管生成。共培养这些细胞可导致细胞的成熟、分化、功能提高,和/或移入潜能增强。另外,本发明的细胞与多种细胞类型体外共培养可导致非T细胞谱系细胞的功能改变。
血管生成因子,包括诱导哺乳动物内以血管发生调节为特征的生理改变的分子,例如,血管内皮生长因子(vasoendothelial growthfactor),也可与本发明的组合物联合使用。特征性的调节实例包括调节(促进或抑制)肿瘤生长、组织修复和组织改造。当与多价配体结合时调节肿瘤生长的肽被认为是生血管的。调节血管发生或内皮细胞表型表达涉及的细胞过程的分子也包括在血管生成因子的定义中。实例包括内皮细胞增殖、内皮细胞存活、内皮细胞运动性、内皮细胞结合。
评价血管生成的体外试验描述在Tolsma等人,J.Cell Biol.122:497(1993)和Vogel等人J.Cell.Biochem.53:74(1993),此处全部引用作为参考。还可使用此处全部引用的美国专利6,225,118描述的体外试验。简言之,其中描述的是使用双重培养并无需额外生长因子的基于合适细胞信号机制的体外血管生成试验。使用此技术可证明血管发生的刺激和抑制。
T细胞组合物在神经再生中的用途
在本发明的另一方面,活化T细胞或其上清用于刺激神经生长。为此,此处描述的组合物可直接应用到培养中的神经细胞或以适合体内给药的组合物提供。在本发明优选的一个方面,组合物用于体外神经再生。
根据另外的实施方案,刺激轴突向外生长的方法包括用神经营养因子处理病人或体外培养神经细胞的附加步骤。该实施方案包括当给药给病人时,本发明的组合物和神经营养因子以单剂量或分开的、多剂量形式给药。如果使用分开的剂量形式,它们可同时地、连续地或彼此在短于约5小时内给药。
本发明的方法和组合物可用于治疗由多种疾病或身体创伤引起的神经损伤。这些包括,但不限于,阿耳茨海默氏病、帕金森氏病、ALS、多发性硬化症、中风和局部缺血伴随中风、neural paropathy、其他神经退化疾病、运动神经元疾病、坐骨神经压伤(sciatic crush)、外周神经病变、糖尿病相关特殊神经病变(particularly neuropathyassociated with diabetes)、脊髓损伤和面神经压伤。
本领域已鉴定出很多神经营养因子,这些因子中的任何因子均可与本发明的活化T细胞或上清组合物联合使用。这些神经营养因子包括,但不限于,神经生长因子(NGF)、胰岛素生长因子(IGF-1)和其截短的活性衍生物例如gIGF-1、酸性和碱性成纤维细胞生长因子(分别地,aFGF和bFGF)、血小板衍生生长因子(PDGF)、脑源性神经营养因子(BDNF)、睫状神经营养因子(CNTF)、胶质细胞系衍生的神经营养因子(GDNF)、神经营养蛋白-3(NT-3)和神经营养蛋白4/5(NT-4/5)。本发明组合物中一个优选的神经营养因子是NGF。
利用美国专利号5,547,963描述的方法(此处全部引用以供参考)可测定本发明关于神经再生和修复的效果。简言之,人肌肉片段,清除其纤维鞘,切成小块,并在由199培养基和10%胎牛血清(FCS)及1%随时可用(ready-to-use)的抗生素和抗真菌素溶液(青霉素G钠、链霉素、两性霉素B[GIBCO])组成的条件培养基中孵育过夜。这些片段保存在由4体积条件培养基和1体积人血浆组成的滋养凝块中。然后外植块(explant)被转移到明胶包被的培养皿中,通过在37℃孵育1h增湿并固定在支持物上,并加入含10%FCS、2mM谷氨酰胺、10.mu.g/ml胰岛素、10ng/ml FGF和10ng/ml EGF的F14培养基(GIBCO)。大量卫星肌肉细胞(成人体内的肌纤维前体)移到外植块外。在1周的培养后这些细胞开始增殖和融合。在卫星细胞融合成肌管之前移去外植块。恰好在融合期前用胰蛋白酶处理细胞并传代培养以获得实验需要量。
最终接种细胞(20.000/cm2)。肌管形成后,13日龄大鼠胚胎的脊髓外植块被固定在肌细胞层上并在25%199培养基、67.5%MEM培养基(GIBCO)、5%FCS、10.mu.g/ml胰岛素和1%抗生素溶液中共培养。此培养基每周更新两次。试验化合物溶解在此培养基中。
在标准条件下,4个外植块中只有1个能与肌纤维建立功能接触。因此,这些是证明轴突生成(neuritogenesis)和突触形成的最佳实验条件。
通过测定下述参数测定本发明的活化T细胞或其上清的效果:
1)轴突长度
2)通过使用带目镜测微尺的相差显微镜(最终放大倍数200X)测定轴突长度。轴突长度从外植块中心测定并不计算这些丝状伸展的弯曲。分支的长度也被测定。至少在15个外植块中测定轴突总长度。

T细胞组合物在粘膜炎中的用途
经历化疗或放疗治疗的恶性肿瘤患者几乎总要面对由他们的治疗引起的中度或重度的副作用。癌症病人面对的一个普遍的副反应是诱导的粘膜溃疡性粘膜炎。此粘膜炎在口腔中特别突出。此副反应,尽管不象其他副反应例如贫血或免疫抑制那样有生命威胁,但经常在许多癌症患者的继续治疗中成为剂量限制因素。溃疡性粘膜炎以在口腔中形成缓慢愈合的开放性溃疡为标志,其在患者中导致很多痛苦和不适。吃、喝,和吞咽变得困难和痛苦,加上经常受影响的唾液腺也不适。在由于其治疗而经常是免疫抑制的这些患者中,口腔中出现的开放性溃疡经常导致细菌、病毒,和真菌来源的机会感染。这些口腔感染必需小心监控以避免它们扩散成威胁生命的、全身性的感染。
迄今,除停止治疗或进行减轻的和支持性的干涉外这样的粘膜炎还没有治疗方法。现在使用的一些减轻的治疗包括使用抗生素减少感染机会、使用抗组胺药和消炎药,和使用止痛药。所有这些治疗,或因伴随癌症治疗的停止或仅成功的部分缓解粘膜炎的痛苦而不能接受。
因此在本发明的一个方面,活化T细胞或其上清用于粘膜炎的治疗。为此,此处描述的组合物可直接应用到粘膜炎部位或以适合体内给药的药物组合物提供。另一方面,活化T细胞或其上清可用于抑制粘膜炎的发生。本发明的活化T细胞可在化疗之前、同时或之后使用。
T细胞组合物在恶病质中的用途
恶病质包括进行性体重减轻、贫血、水肿和食欲缺乏作为主要症状,其与恶性肿瘤、结核病、糖尿病、homodyscrasia、内分泌病、AIDS等相关“J.Parenteral and Enteral Nutrition,12,286-298,1988”和“American Journal of Medicine,85,289-291,1988”。
到目前为止已经尝试过,例如,胃肠外或肠内营养和内分泌治疗恶病质,但迄今还没建立起满意的治疗模式。尤其在恶病质由恶性肿瘤引起时,由于进行性的恶病质降低了患者对抗癌化疗的耐受性以至于治疗遭遇到严重挫折。另一方面,恶病质减少的营养性支持倾向于加重恶性肿瘤而缩短患者的存活期。虽然恶病质经常由恶性肿瘤诱导,使用抗肿瘤药物可引起抗肿瘤效果,但抗肿瘤药物的副作用被叠加而阻止恶病质的缓解是规律而非例外。在这种情况下,出现了对改善或抑制恶病质症状进展的例如体重减轻的治疗药物的需求。
在本发明的一个方面,活化T细胞或其上清用于恶病质的治疗。为此,此处描述的组合物可以适合体内给药的药物组合物提供。另一方面,活化T细胞或其上清可用于抑制恶病质的发展。本发明的活化的T细胞可以在化疗前、结合化疗或化疗之后给药。本发明的活化T细胞可与本领域提供的恶病质的其他疗法联合给药。
T细胞组合物在基因发现中的用途
此处描述的活化T细胞和/或其上清的体外共培养,还可用于基因发现。例如,活化T细胞和/或其上清与神经细胞、cariomyocytes、内皮细胞、骨细胞,和/或这些细胞的前体共培养,可导致靶细胞基因表达的改变。然后可使用技术人员公知的多种技术例如多种免疫选择方法分离感兴趣的细胞。这样的技术已被描述,例如,在CurrentProtocols in Immunology,John Wiley & Sons,New York.N.Y.中。然后用这种方法分离的细胞可用于建立基因文库。DNA或cDNA文库的构建技术为那些本领域的技术人员众所周知。通常的(custom)文库还可由很多公司商业产生,例如Clontech(Palo Alto,CA)。然后这些文库可由,例如PCR和直接测序筛选,以鉴定由活化T细胞或其上清引起的组织修复和再生过程中涉及的已知的和/或独特的活化基因。
制剂/药物组合物
本发明还提供药物组合物,其包括活化T细胞和/或与活化T细胞或其上清共培养后改变的细胞和药物可接受的载体。本发明的组合物可单独给药,或作为药物组分与稀释剂和/或与其他组分例如IL-2/或其他细胞因子或细胞群联合给药。简言之,本发明的药物组合物可包括此处描述的靶细胞群,结合一种或多种药物或生理可接受的载体、稀释剂或赋形剂。这样的组合物可包括缓冲液例如中性盐缓冲液、磷酸盐缓冲液等;碳水化合物例如葡萄糖、甘露糖、蔗糖或葡聚糖、甘露醇;蛋白质;多肽或氨基酸例如甘氨酸;抗氧化剂、螯合剂例如乙二胺四乙酸(EDTA)或谷光苷肽;佐剂(例如,氢氧化铝);和防腐剂。在某些方面,配制本发明的组合物用于静脉给药。
本发明的药物组合物可根据治疗(或预防)的疾病以适当方式给药。尽管合适的剂量可由临床试验决定,但给药的量和频率将由如患者的状态、患者的疾病类型和严重性这样的因素决定。
“治疗有效量”是指,本发明组合物的精确给药量,可由医师根据个体在年龄、体重、肿瘤尺寸、感染或转移的程度、患者状态上的个体差异决定。一般地,在继承性免疫治疗研究中,给药到患者的活化T细胞大约在2×109到2×1011细胞。(参见,例如,美国专利No.5,057,423)。在本发明的一些方面,尤其在使用同种异体或异种细胞中,可使用106/千克范围内(106-1011每患者)的低数量细胞。T细胞,或其他共培养后改变的细胞组合物可以这些范围内的剂量多次给药。对经历治疗的患者活化T细胞可是自体的或异种的。
受试的药物组合物可以任何方便的方式进行给药,包括通过气溶液吸入、注射、摄食、输液、植入或移植。本发明的组合物可通过皮下、皮内、肌内、静脉注射或腹膜内对患者给药。本发明的T细胞组合物优选通过静脉注射给药。该活化T细胞组合物可直接注射到组织损伤部位。
在另一实施方案中,药物组合物可在控制释放系统中送递。在一个实施方案中,可使用泵(参见Langer,1990,Science 249:1527-1533;Sefton 1987,CRC Crit.Ref.Biomed.Eng.14:201;Buchwald等人,1980;Surgey 88:507;Saudek等人,1989,N.Engl.J.Med.321:574)。在另一实施方案中,可使用聚合材料(参见Medical Applications ofControlled Release,1974,Langer和Wise(eds.),CRC Pres.BocaRaton,Fla.;Controlled Drug Bioavailability,Drug Product Designand Performance,1984,Smolen和Ball(eds.),Wiley,New York;Ranger和Peppas,1983;J.Macromol.Sci.Rev.Macromol.Chem.23:61;还参见Levy等人,1985,Science 228:190;During等人,1989,Ann.Neurol.25:351;Howard等人,1989,J.Neurosurg.71:105)。在另一实施方案中,控制释放系统可靠近治疗靶标放置,因此仅需要全身剂量的一部分(参见,例如,Medical Applications of ControlledRelease,1984,Langer和Wise(eds.),CRC Pres.Boca Raton,Fla.,Vol.2,pp.115-138)。
本发明的组合物还可使用很多基质给药。在组织工程中基质已被使用多年(参见,例如,Principle of Tissue Engineering(Lanza,Langer,和Chick(eds.)),1997)。本发明在新的实践中使用这样的基质作为人工淋巴器官以支持、保持,或调节免疫系统,一般通过调节T细胞。因此,本发明可使用在组织工程中已显示效用的那些基质组合物和制剂。因此,本发明的组合物、装置和方法可使用的基质类型实际没有限制,并可包括生物学的和合成的基质。在一个特定的实施例中,使用由美国专利:5,980,889;5,913,998;5,902,745;5,843,069;5,787,900;或5,626,561所述的组合物和装置,这些专利全部引用作为参考。当向哺乳动物宿主给药时基质包含的特征一般与是生物相容的相关。基质可从天然和合成材料产生。在希望在动物体内留下永久结构或可移去的结构的情况中,例如植入物,基质可是非生物降解的;或基质也可是生物降解的。基质可用海绵、植入物,管、telfa pads、纤维、中空纤维、冻干成分、凝胶、粉末、多孔成分,或纳米微粒。另外,基质可设计成允许持续释放接种的细胞或产生的细胞因子或其他活性因子。在某些实施方案中,本发明的基质是柔韧的和有弹性的,并可被描述为无机盐、水性流体和溶解的气体成分包括氧等物质可透过的半固体支架。
此处使用的物质作为生物相容性物质的实例。然而,本发明并不局限于这样的物质,无论术语基质出现在哪里这些术语都应被理解为包括允许细胞停留和通过的装置和其他物质,是生物相容性的,并能允许大分子直接通过,物质本身为半透膜的物质或用于和特殊的半透过物质连接的物质。
使用那些本领域普通技术人员已知的配制方法,可以提供此处描述的包括活化T细胞和/或其上清和/或已与活化T细胞或其上清共培养的细胞的组合物的药物可接受的制剂。这些制剂可由标准的途径给药。一般而言,组合物可通过局部、经皮、口腔、直肠或胃肠外的(例如,静脉、皮下或肌内)途径给药。另外,组合物可掺入生物可降解的聚合物以允许组合物的持续释放,聚合物被植入到需要释放的地方的附近,例如,在组织损伤部位。生物可降解的聚合物和其用途有详细描述,例如,在Brem等人J.Neurosurg.74:441-446(1991)中。
组合物的剂量取决于治疗的情况,和其他临床因素例如人和动物的体重和状态、组合物的性质,和组合物的给药途径。可以理解本发明有人和兽医应用的用途。
该制剂包括那些适合口腔、直肠、眼睛(包括玻璃体内或intracameral)、鼻腔、局部(包括含服和舌下)、阴道或胃肠外(包括皮下、肌内、静脉、皮内、气管内、硬膜外)给药的制剂。该制剂可便利地以剂量形式提供并可由常规的药学技术制备。这样的技术包括将活性成分与药物载体或赋形剂结合的步骤。一般而言,通过将活性成分与液态载体或精细分离的固体载体或与两者均匀和紧密结合制备制剂,然后,如果需要,定型产品。
适合皮肤局部给药的制剂可以为药物可接受的载体中含有给药成分的软膏剂、乳膏剂、凝胶剂和泥膏剂。优选的局部递送系统是含有给药成分的皮肤膜片(transdermal patch)。
直肠给药的制剂可以带有适当基质(base)的栓剂,包括可可脂(cocoa butter)或水杨酸酯。
适合鼻腔给药的制剂,其中载体是固体,例如,包括颗粒大小在20到500微米范围内的粗粉末,其通过给予鼻吸药的方式给药,即,从靠近鼻子举起的粉末容器中通过鼻道快速吸入。合适的给药制剂,其中载体是液体,例如,像鼻喷雾剂或滴鼻剂,包括活性成分的水性或油性溶液。
适合阴道给药的制剂可以是除活性成分外还含有本领域公知的合适载体的阴道栓剂、tamports、乳膏剂、凝胶剂、泥膏剂,泡沫或喷雾制剂。
适合胃肠外给药的制剂包括水性和非水性无菌注射液,其可含有抗氧剂、缓冲液、抑菌剂和使制剂与预期受体血液等渗的溶质;和包含悬浮成分和增稠成分的水和非水无菌混悬液。该制剂可以单剂量或多剂量包装提供,例如,封口安瓿和管形瓶(vial),并可以冷冻干燥(低压冻干)的状态贮存,其仅需要在临用之前,加入无菌液态载体,例如,注射用水。临时注射液和混悬液可从上述的此类无菌粉末、颗粒剂和片剂中制备。
优选的单位剂量制剂含有如以上所列举的,每日剂量或单位、每日亚剂量,或其适当的部分的给药成分。
应当理解除该成分外,尤其是上面提到的,本发明的制剂可包括有关所谈论的制剂类型的其他常规成分,例如,那些适合口服的可包括调味剂。
在本发明的一个实施方案中,通过使用顺磁珠和在靶组织内部或外部应用磁力,将包含本发明的细胞,活化T细胞或预先与活化T细胞共培养的细胞的组合物靶向需要的部位(例如,如美国专利6,203,487中所描述,此处全部引用作为参考)。简言之,在体内或体外或二者结合,将本发明的细胞,活化T细胞或预先与活化T细胞共培养的细胞,暴露于缀合适当表面标志的顺磁珠以便顺磁微粒与细胞的结合。如果在体外进行,包含与顺磁微粒结合的细胞和药物可接受的赋形剂的组合物被给药到哺乳动物。磁体可靠近靶组织放置,即,在其中需要局部细胞递送的身体区域或所选的组织或器官。在局部递送的靶组织的内部或外部使用外科或经皮的方法可将磁体定位于体表表面或放置到体表内部。递送结合细胞的磁粒可以通过直接注射进所选组织或远端使之被动循环到靶位点或由磁体或靶向作用的配体导入靶位点来进行。
本文中引用的所有文献此处全部引用作为参考。而且,此处使用的所有数值范围明确地包括此范围内的所有整数值,并根据特定的用途选择此范围内的特定数值。另外,下面提供的实施例用于说明,不用于限制。
实施例
实施例1
创伤愈合的动物模型
检查表面(皮肤)创伤的动物模型以研究本发明的组合物对创伤修复和愈合的效果。
根据由Staiano-Coico等人J Clin Invest.77(2):396-404(1986)描述的方法,在麻醉的猪的背上制造约2×2cm的剖开的厚皮伤口。由于猪皮最像人的皮肤因此在这样的研究中一般使用猪模型。包含本发明组合物的少量溶液放置到约11处伤口上并用封闭粘附敷料覆盖伤口。安慰剂溶液(盐水,50到100.mu.l)放置到11处“对照(mirror)”的每个上,也由封闭敷料覆盖的同样的伤口。3天后,麻醉动物,处死并移出它们包含的如原皮肤伤口两倍大的完整厚皮样本。编码样本以致盲病理学家分析的处理和对愈合的百分比评分。此评分方法主要测量从创伤开始在3天的修复和愈合中发生的上皮形成(由角质化细胞覆盖的伤口)数量,用本发明的组合物治疗的11处伤口的结果与盐水对照比较并表示为愈合百分比。较高的百分比反映较多的愈合。
实施例2
心肌缺血的动物模型
使用本发明的T细胞或其上清成功研究组织修复和再生的重要前提是(a)应用于临床的心肌缺血动物模型的结构可提供心肌缺血情况下关于血管生成机制的有用数据,和(b)精确评价此处描述的组合物的效果。使用模仿临床冠状动脉疾病的猪的心肌缺血模型,如美国专利6,174,871中所描述,此处全部引用。环绕左旋冠状动脉(LCx)放置ameroid缩窄器产生最小梗塞形成(左心室的1%,左旋冠状动脉床的4.+-.1%)的逐渐完全闭合(放置7天内)(Roth等人Circulation82:1778,1990;Roth等人Am J Physiol 235:H1279,1987;White等人Circ Res 71:1490,1992,Hammond等人Cardiol 23:475,1994;和Hammond等人J Clin Invest 92:2644,1993)。由于侧副血管的发生,由闭塞动脉以前灌注的区域(指的是作为局部缺血区域)中心肌功能和血流量一般是静止的,但当心肌氧需要增加时,储备的血流量不足以阻止局部缺血。因此,LCx床易发生局部缺血,类似临床心绞痛。在ameroid放置21天中侧副血管的发生和血流-功能的关系是稳定的,并四个月保持不变(Roth等人Circulation 82:1778,1990;Roth等人Am J Physiol 235:H1279,1987;White等人Circ Res 71:1490,1992)。由遥测术证明,动物全天都有在所述床中出现阶段性局部缺血功能障碍的危险,与进食、被人干扰等过程中(未公开数据)的心率突然升高有关。因此,该模型具有稳定但不足的侧副血管的床,易发生阶段性局部缺血。该模型另一独特的优势是灌注和功能正常的区域(LAD床)邻近灌注和功能异常的区域(LCx床),因此在每一个动物中提供对照床。
使用心肌心脏超声造影术(contrast echocardiography)评价局部心肌灌注。造影剂(contrast material)由半乳糖颗粒组成并提高图象的产生回声性(白)。颗粒以与血流成比例的方式分散到冠状动脉和心肌壁(Skyba等人Circulation 90:1513-1521,1994)。通过中心体(microsphere)测定显示造影剂的峰强度与心肌血流紧密相关(Skyba等人Circulation 90:1513-1521,1994)。环绕靠近ameroid的近端LCx放置液压套囊闭合器,以说明本发明使用的超声心动图像可精确鉴定LCx,和心肌的心脏超声造影术可用于评价心肌血流。
为通过显微术定量毛细血管生长,当动物被处死时,心脏被灌注固定(戊二醛、生理压力、原位)。在接受基因转移的动物心肌中用PCR检测生成血管的蛋白质DNA和mRNA。最后,用抗血管生成蛋白质的多克隆抗体,检查来自本发明组合物给药动物的细胞和心肌中血管生成蛋白质的表达。
治疗研究策略包括组合物给药时间、组合物给药途径和组合物类型(活化T细胞、其上清、与活化T细胞共培养后的细胞)。在心肌局部缺血的ameroid模型中,在稳定但不足的侧副血管形成后给予需要的组合物。那些本领域的技术人员会理解在猪中证明的结果是人的预期结果。猪有与人的冠状循环非常相似的天然冠状循环,包括缺少天然冠状侧副管。
实施例3
在两个接受活化T细胞(XCELLERATETM)+IL-2疗法的患者中
修复成骨细胞和溶解的骨损伤
三个患者进入转移肾细胞癌I期试验(XT00)并接受活化T细胞(XCELLERATETM)+IL-2疗法的治疗。患者(患者#003)颅骨有成骨细胞损伤,在用(XCELLERATETM)+IL-2疗法治疗后的其损伤完全消退(图1,圈点区域)。第二个患者(患者#004)在其骨盆中有一个大的7厘米的溶解损伤并在其肋骨中有第二个溶解损伤。在(XCELLERATETM)+IL-2治疗后,两个骨损伤都完全愈合,并在10个月的随访中保持愈合(图2,圈点区域)。
实施例4
使用活化T细胞的体外血管生成模型
如Iruela-Arispe等人Proc.Nat.Acad.Sci.USA 88:5026-5030,1991中所述(此处全部引用),使用体外血管生成模型。简言之,基本上如Sage H.等人Biochemistry 24:5433-5442,1979中所述分离牛的主动脉血管内皮细胞和大鼠血管内皮细胞(分别为,BAEC和RVEC)。选择表达芽殖表型的BAEC克隆和组成内皮细胞索(endothelialcords)RVEC克隆。两种类型细胞都在37℃含10%(体积/体积)热灭活FCS的Dulbecco’s改良的Eagle’s培养基中培养。使用BAEC的5-10代及RVEC的25-30代的细胞。内皮细胞索的自发形成一般发生在融合后的10-15天。
来自2周内细胞索(cord)的BAEC在12孔Coster平板(Corning)上铺板。一个细胞索定义为两个交叉(顶点)点之间的长度。为确定管(tube)的数量,在预标记的平板上检查10个显微视野(使用X10的物镜和X1的目镜)。对于每个试验,计数五个重复的培养物(0天)。然后用无血清的培养基洗涤该培养物三次并与活化T细胞或其上清孵育。优化使用的T细胞数量和用T细胞活化后的时间。
在0天和2天,计算每一批试验的细胞索±SEM的平均数。将每一培养物中0天细胞索的数量作为100%,数值还可表示为百分数(±SEM)。由配对样本t检验分析数据,当P≤0.025时认为差异有显著性。
Claims (9)
1.诱导组织修复和/或再生的方法,包含:
(a)提供细胞群,其中至少其一部分包含活化T细胞;
(b)将组织暴露到该T细胞或其上清;并因此诱导组织修复和/或再生。
2.根据权利要求1的方法,其中组织选自骨、心肌、上皮,和神经组织。
3.根据权利要求1的方法,其中组织再生包含血管生成。
4.根据权利要求1的方法,其中组织再生包含新骨生成。
5.根据权利要求1的方法,其中组织再生包含神经再生。
6.根据权利要求1的方法,其中组织修复包含损伤组织的新血管形成。
7.根据权利要求6的方法,其中损伤组织包括心脏组织。
8.由下面的方法产生的基因文库,该方法包含:
(a)活化T细胞;
(b)将T细胞与选自心肌、骨、骨髓、皮肤、神经、内皮和上皮的组织源共培养,持续的时间长度足以诱导基因表达的改变;
(c)收获该组织;
(d)从此产生基因文库;
9.在患者中或在体外神经细胞中刺激轴突生长的方法,包括给予所述患者或所述神经细胞神经营养量的包含活化T细胞或其上清的组合物的步骤。
