在许多医学症状中,包括糖尿病、烧伤、褥疮和伤口,氧化组织的能力降低且动脉氧也许不能到达受损伤的皮肤。每年美国有数万病人由于没有足够的氧传输到受影响的组织而引起的并发症死去。低水平的氧传输,具体在肢中,导致缓慢复原、感染、疤痕发展,在最坏的情况下,导致组织死亡和截除。

用“局部高压”氧化装置方法存在一些缺点。例如,外部氧源和通常用于呼吸治疗的呼吸器必须和设备一起提供。此外,间歇的限制和释放血流到治疗区域对已受损伤的组织也许不可取或忍受。
另一种“局部高压”治疗复原差的皮肤损伤的局部方法包括通过伤口敷料局部应用高水平的氧气。美国专利号5,792,090显示了氧产生伤口敷料和通过应用这样的绷带在表面伤口提高氧张力的方法。在此方法中,伤口敷料包括氧气渗透膜和能通过化学反应提供氧的容器。美国专利号5,855,570描述了另一种促进皮肤伤口复原的氧-产生绷带。此装置结合伤口敷料和生成高纯度氧的电化学、化学或热方式,并可调节以多种浓度、压力和剂量提供氧气给伤口上的区域。

1.3现有技术的缺点

2.0发明概述
提高组织氧化的常规方法使用氧气。与此截然相反的是,本发明揭示了通过局部应用超氧化组合物提高组织氧化的新方法。超氧化组合物通过促进有效的氧扩散入组织来迅速提高组织中氧分压水平。
因此,发明显示了在哺乳动物中增加组织氧化的方法,包括应用超氧化组合物于组织表面足够长的时间以使上皮下氧分压在基线pO2上从约30%提高到约120%。哺乳动物一般是人,但没有限制其在兽医应用中的使用,用于也许有组织损伤的小型和大型动物,损伤相应于提高组织氧化的治疗过程。
最普遍的应用是直接用于外部皮肤,但此方法同样地用于消化道的粘膜表面和器官表面。器官可在手术过程中暴露或从体腔中取出。可将器官浸入根据发明制备的超氧化溶液,使部分器官接触这种制剂,或用超氧化溶液灌注器官。在后一种情况中,这可以是旨在维持器官存活和减少局部缺血损伤的体外过程。
出于一些原因,可期望提高组织氧化水平,主要在组织受条件或疾病影响的症状中,如褥疮、伤口、烧伤或溃疡或其它往往降低正常组织氧化水平的情况。另外,它预期在治疗厌氧细菌感染中尤其有益,例如那些由假单胞菌属、拟杆菌属如脆弱拟杆菌(Bacteroides fragilis)、产黑素普雷沃氏菌(Prevotellamelaninogenica)、二路普雷沃氏菌(Prevotella bivia)、解糖胨普雷沃氏菌(Prevotella disien)、梭菌属(Fusobacterium)、放线菌属(Actinomyces)、乳杆菌属(Lactobacillus)、丙酸杆菌属(Propionibacterium)、真杆菌属(Eubacterium)、双歧杆菌属(Bifidobacterium)、蜘菌属(Arachnia)、消化球菌属(Peptostreptococcus)、韦荣氏球菌属(Veillonella)、梭菌属(Clostridium)如破伤风杆菌(C.tetani)、肉毒梭菌(C.botulinum)、产气荚膜梭菌(C.perfringens)、艰难梭菌(C.difficile)和卟啉单胞菌属(Porphyromonas)引起的感染。这些感染可在肺组织、口部或阴道粘膜内部溃烂,或包埋入器官表面如肝脏、肾和心。
因此,使用所示提高组织氧化水平方法的好处之一是对致病厌氧细菌的毒性。一种具体理想的应用是控制或杀死引起牙周疾病的厌氧细菌。超氧化嗽口水溶液可安全和方便使用,并可包装以维持超氧化溶液的稳定,维持稳定通过以单剂量分配或单独使用包装的形式使用加压容器。
本发明的超氧化组合物包括至少约55ppm氧,但有用浓度从约45到约220ppm。组合物的氧水平取决于一些因素,包括组合物的类型、温度和其它成分、活性或不是,这些因素可出于多种原因加入如稳定性、应用的方便或提高吸收。
熟知的是流体中气体浓度与温度成反比。当想用水基超氧化组合物时,温度不由化学因素决定,但由对活组织的潜在损伤和对更高氧浓度的需求决定。因此,当组合物局部应用于外部皮肤表面时;例如前臂损伤,约0℃的溶液温度一般认为是合适的。这会提供相对高的氧水平,通常在220ppm范围内。另一方面,病人可在约34℃的范围中以更舒适的温度全身浸入涡流浴。氧浓度必须小于220ppm,这不仅由于温度,也由于在这些机构如复原中心的涡流浴中通常所用的开放环境。

微泡组合物不需是纯水,组合物通常包括以水为基础的溶液如缓冲液或药学上可接受的载体,如果接触组织表面不会有害。如果使用缓冲液,较佳的是在7.2-7.4的生理pH范围内,但也可以更低的pH如醋酸缓冲液提供的,或以更加碱性的缓冲液如碳酸缓冲液中更高的pH。对于许多应用,超氧化组合物包括水和氧微泡。
在一些环境下当超氧化组合物应用于组织时,提供超氧化组合物搅拌也许是有利的。这会增加氧接触组织表面并可提高吸收效率。当组合物是涡流浴治疗一部分且可稍微补偿一些氧在开放大气环境中的减少和使用旨在提供病人舒适的温度时,搅拌是应用方法固有的。
发明的氧过饱和组合物可以多种方式应用,方式取决于应用的区域、情况的性质和治疗目的、受试者或待治疗的病人的健康情况。皮肤治疗通常作为溶液应用,溶液可结合入霜剂、糊剂、粉剂、软膏剂、洗剂或凝胶剂或者仅仅非水或水介质中的超氧化微泡制剂。一个重要的考虑是制剂中的微泡浓度和它增加上皮下氧分压的能力。
应用于皮肤的方法可以是浸泡、沉浸、喷射、摩擦或气雾剂。制剂可用于接触皮肤的敷料,如石膏和伤口包扎。在其它应用中,灌洗或灌肠可用于阴道或直肠给药。方法的选择取决于具体的病人需要、应用区域和可用设备类型。
超氧化组合物是发明的另一方面。组合物含有水基的氧微泡溶液,微泡直径从约0.6到约100微米且氧浓度在约45ppm和220ppm之间。较佳实施方案包括微泡直径范围从约0.6到约5微米的超氧化组合物和微泡直径从约5到约8微米的组合物。一种非常优选的超氧化组合物含有在1-2微米范围中的氧微泡。
尽管液体微泡超氧化组合物在大部分应用中优选,组合物可以固体或冷冻形式。在水基溶液中,这可低至-40℃但在冷冻气体中可低至-70℃,如二氧化碳或在液化气中如氮。这些低温对应用于活组织不可行;然而,某些细胞或生物材料的长期储存可受益于此种类型的环境。在任何情况中,有一些超氧化含水固体的应用提供例如缓慢释放氧环境或其中冰可接触为了移植目的而运输的切除器官。

设备也可包括温度控制器,控制器在超氧化溶液中控制氧水平有用。另一种效果也许是提高一些受试者通过皮肤的氧吸收,吸收提高是由于皮肤表面温度的增加。为了用于病人,也许会倾向调节温度在约37℃和约45℃之间。
在一具体应用中,方法和组合物可用于治疗厌氧感染。一般这包括将任何上述的组合物用于怀疑带有厌氧细菌的皮肤损伤。方法必须对通常发现于坏疽或溃烂组织的厌氧细菌特别有效。这种厌氧细菌也在伤口感染中发现。病人可能受益于伤口区域中增加的组织氧。烧伤的皮肤区域尤其易受感染影响,具体是二和三度烧伤中组织被破坏或严重损伤的地方。烧伤病人预期受益于这种可预防和治疗应用的治疗。其它受益于增加的组织氧的情况包括口腔中的软组织,具体是在治疗由厌氧细菌引起的齿龈疾病中。
为了方便,试剂盒可用于包装多种根据发明制备的超氧化组合物。具有用于局部增加组织氧化适当说明的范例试剂盒可在一种或更多所述变化中包含密封的可渗透的韧性容器和容器包装的超氧化组合物。试剂盒可另外包括涡流产生装置、和/或用于调节容器内温度的恒温/加热装置。
如所讨论的,所示方法应用超氧化组合物于表面,应用时间足够使上皮下组织氧分压(pO2)在基线pO2水平上从约30%增加到约120%。方法具体用于患如组织坏疽、褥疮、溃疡、烧伤或厌氧感染这种情况的人。

本方法不仅有益于防止一些皮肤疾病,也在化妆品和药物应用上有用。许多青少年甚至成人尤其感兴趣的是有助于健康皮肤的制剂,制剂同时也促进普通痤疮、一定程度损伤外貌的皮肤情况的复原。
超氧化组合物也可有益于忍受吸入烟和吸入热空气的损伤的受害者。在这些情况中,为了提高受损细胞的氧浓度,所示超氧化组合物直接给药于肺。这种治疗也可用于从肺中洗出吸入的微粒并可与抗生素和抗炎症药物溶液一起在所示处给药。超氧化液体可用作喷雾或插管作为浸泡液以提供与肺内表面更多受控的接触。

超氧化组合物通常是直径从约0.1到约10微米的氧微泡的水溶液,较佳的是约1到约8微米,更佳的是至少约0.6到约8微米且氧浓度从约45ppm到约220ppm。在大部分应用中溶液中有各种大小的微泡,包括小于0.6微米直到1、2、3、4、5、6、7、8、9和10微米,随着微泡融合可包含更大的微泡大小,这取决于温度。当然氧浓度取决于液体温度,通常氧浓度在2℃至约220ppm,或在34℃至约118ppm。这些浓度可变化,变化依赖于待治疗组织表面的情况、组织类型和组织表面的位置。

氧微泡可在水或药物上可接受载体中制备。生理盐水、多种缓冲液或增加湿度和孔隙度的化合物式组合物变化的例子。在一些情况中,为了加速复原或更有效治疗某些细菌感染,可希望加入抗生素、抗炎症化合物或其它药物到组合物中。
在一些应用中,以霜剂、洗剂、凝胶剂或固体的形式施用超氧化组合物是理想的。这种制剂很好地被识别且本领域技术人员可得到。超氧化组合物也可维持冷冻状态,例如用于储存或用于治疗,治疗中冰可方便地用于组织表面从而可一贯维持更高水平的氧。在一具体的重要应用中,冷冻或冷藏超氧化组合物可用于储存和运输旨在移植的器官。这可避免或改善由于缺乏来自正常血液供应的器官而产生的缺氧条件。冷冻或冷藏超氧化组合物对这些组织尤其有利,这既因为酶过程在较低温度受阻碍,也因为在低温时高水平氧可结合入超氧化组合物,因此退化被抑制。
超氧化组合物可以一些方式给药,如通过与含超氧化溶液的软质袋相连的管,或在一些应用中通过组织浸入含氧化溶液的浴。为治疗齿龈疾病的牙齿应用,通过类似于水pic(water pic)的装置给药是局部给药适当超氧化溶液的有效方法。一些应用受益于混合或搅拌过程,因此新鲜溶液一直洗组织;例如,灌洗过程或受影响肢浸入的涡流浴。
在一些实施方案中,提供组织氧化环境给需要增加组织氧化的哺乳动物的设备也在本发明的范围内。这种设备包括产生氧微泡的机器,机器可仅仅是以90-110psi范围的压力与加压容器相连的氧圆筒,且引导氧气进入保存液体的容器,液体如水或其它适当以水为基础的液体。充氧器也可使用,产生约50psi。管或其它容器出口提供氧化溶液给靶组织。溶液中氧水平可通过搅拌或超声波处理容器来提高。超声波设备外部流量吸收和控制容器内扩散模式的适应或浴也可使用。
溶液温度影响总的氧浓度,因此另外的实施方案中设备可结合任意一些熟知的控制温度的装置如恒温浴。所以在开放空气如涡流浴中应用是整肢或身体应用的地方,氧浓度不是经常超过约55ppm。为治疗内部上皮层,如在口部粘膜感染中,较冷的温度和相应更高的氧浓度可耐受。氧浓度变化取决于应用方法是否通过浸泡、沉浸、喷射、摩擦或气雾剂;然而,任何情况下,接触受影响组织的组合物有显著增加的氧浓度,浓度范围至少约45ppm。
尽管大部分应用使用水溶液,发明者不想过度受限制,因为高氧浓度可在非水或水/有机溶剂中获得。这类溶剂应该无毒且药理学上可接受用于人使用。全氟碳化合物是也许有用的非水溶剂的具体例子。其它溶剂包括那些水-易混合的如酒精和乙二醇。在一些应用中,使用胶制剂如从藻酸盐或角叉胶制备的亲水胶也许是方便的。
其它实施方案包括方便地提供一些形式的上述设备的试剂盒并对局部提高组织氧化有用。范例试剂盒可包括含超氧化组合物的密封的可渗透的韧性容器和应用组合物于组织表面或需要增加氧化的皮肤的说明。选择试剂盒组分包括调节容器内温度的恒温/加热装置和与加压容器相连的用于混合、搅拌或超声波处理氧化液体的氧供应。

4.0例举性实施方案的详细描述
对皮肤疾病如坏疽、皮肤溃疡、褥疮、烧伤和其它皮肤病问题需要改进的治疗。本发明在治疗一些皮肤疾病中使用高氧化产物。所示高氧化产物对治疗与皮肤组织退化相关皮肤疾病有用,退化由于氧缺乏如溃疡、烧伤和皮肤伤口。
4.1氧释放到细胞
如图1所示,氧从空气运输到身体。空气含约20%氧,随着吸入进入支气管并最终进入肺泡。在气泡中,氧扩散穿过很薄的毛细血管壁进入血流,在血流中结合红细胞中的血红蛋白以形成氧合血红蛋白。随着血液循环通过身体,氧从氧合血红蛋白释放并扩散入组织和身体细胞,包括皮肤。
气体通常以压力的形式测定。空气是混合气体并以绝对和分压形式测量。例如,在海平面空气有760mm汞柱(Hg)的大气压,意味它可在1mm直径管中支持760mm高的汞柱。氧组成空气中20%的气体;因此氧分压(pO2)是760的20%,或152mmHg。在更高的高度,空气的pO2减少。在肺中,氧分压是100mmHg。
氧扩散入细胞和组织取决于氧分压、氧在体液中的溶解度和组织的健康。氧在大气压时不穿过皮肤,但仅与外部表面反应。因此在正常情况下,皮肤不从空气中的氧获得营养,而从扩散O2中获得,从呼吸扩散到上皮的深、活层(living layer)并从真皮毛细血管到下面的真皮(图2A)。损伤或疾病危及皮肤血液供应,因此严重影响受损皮肤获得足够氧供应的能力。
高压氧治疗是增加组织氧化的全身治疗,包括在高于大气压的压力下给予氧。这要求使用特殊室来包含高压(通常在2和3倍大气压间),需要室来推动另外的氧溶解于血浆,血浆依次推动氧进入组织。迄今为止,缺少氧产生的大部分皮肤情况用全身高压方法和非氧化局部应用治疗。例如,建立高压氧室作为治疗医学疾病如Clostridial Myonecrosis(气性坏疽)中的主要治疗。平均起来,治疗通常在充分压力下持续1到2个小时,这可能有问题因为在这些压力下延长暴露于高压处理产生高毒性风险。
高压氧治疗也用于治疗褥疮。皮肤有充足的传输氧到所有皮肤层的血液供应。如果血液供应断绝超过两或三个小时,皮肤开始死亡,死亡起始于外层,上皮。流入皮肤的血液减少的一个普便原因是压力。正常移动改变压力并使血液供应连续运转。一旦人移动受限或卧床不起,他们有产生褥疮的高风险。褥疮可进一步发展成褥疮溃疡(decubitus ulcers)。这些溃疡会使皮肤打开露出骨头,引起大量疼痛并可导致威胁生命的病症。
在一些情况中,“局部高压”治疗褥疮包括暴露身体单独部分于氧气,声称在感染早期有效。此方法要关心的是缺少局部应用氧气的渗透,这主要由于皮肤表面下压力和大气的区别(FDA Advisory Meeting,1998年11月17日)。局部氧传输连续应用于皮肤后也在皮肤表面引起烧伤效果。
在本发明中,用超氧化化合物的组织氧化方法被发展来治疗皮肤病问题,问题通过引起更迅速的复原来治疗。通过使用局部应用高度氧化水或其它结合显微氧泡的液体提供氧给组织。例如当应用于皮肤时,氧从皮肤表面穿过更深的层向内运输(图2B),从而提供氧给上皮和下面真皮的细胞。高度氧化的溶液会提高皮下和上皮下组织中的氧水平并通过提供氧给氧-缺乏细胞来促进复原。
5.0实施例
下列实施例被包括以证明发明的较佳实施方案。本领域技术人员应掌握实施例中所示的技术,实施例接着表现发明者发现的技术以在发明的实践中作用良好,因此可考虑为其实践建立优选模式。然而,本领域技术人员应该根据本说明,明白在具体实施方案中可作出许多改变,这些所示实施方案仍获得相似或类似的结果而没有偏离发明的精髓和范围。
材料和方法
根据增补氧给溶液的过程制备超氧化溶液,通过引导液体进入氧富集容器来增补氧,这类似于美国专利号5,006,352的说明,本文纳入参考文献。简要地,氧化过程在氧富集设备中进行,如美国专利号5,766,490和5,814,222所示,本文将它们的全文纳入参考文献。
发明者所用过程引导液体进入密封空间或加压容器,在湍流混合其中混合液体和氧并重新获得氧含量至少40mg/l氧的氧-富集液体。用蒸馏水制备氧浓度为180-217的超氧化液体。如用阻流装置(flow impedence device)所测,此溶液含平均约1微米直径的氧微泡,直径通常在0.6-8微米的范围内。为测量泡直径,氧化的水样品通过在20ml试管中混合各等量样品和Isoton II来分散。分析通过30μm孔径的管用时间模式进行30秒。表1概括了结果,显示氧泡平均大小在1μm的范围内。用激光衍射仪器测定颗粒大小的尝试是不成功的。

5.1实施例1-猪皮肤中皮下氧的增加
此实施例证明含超氧化微泡的溶液局部应用于猪皮肤会增加下面皮下组织中的氧水平。
皮肤用酒精、过氧化氢洗,然后水洗。能同时测量二氧化碳压力(pCO2)、温度和pH的动脉血气体监控装置(“传感器”)被插入身体左右两边皮肤表面下1-4mm。用于测试溶液的容器,如一端有管的软质袋,粘贴到围绕传感器的皮肤上。容器提供在试验中将皮肤浸入测试柱或对照溶液的方法。填充容器前,建立对照使粘合剂的选择(Fixodent或Stromahesive paste)对基线pO2读数没有任何作用。
允许传感器平衡30分钟并在干扰开始前都稳定。对照和测试溶液加热至34℃,等体积加入容器以确保测量中皮肤位置上的压力相同。对照溶液是蒸馏水(约7-9ppm O2)且测试溶液是O2含量为180-217ppm的超氧化水。
如图4所示,在试验起始,位于左边测试位置的传感器中基线pO2是55mmHg。对照溶液在容器中放置6分钟,其间没有观察到任何pO2的增加(图4)。取出对照溶液,测试溶液在t1时置于容器中并在4分钟后再次放置(第二条虚线)。加入测试溶液导致pO2从54增加到117mmHg。此组织水平比95mmHg的动脉值高23%(图4)。一旦测试溶液通过抽吸取出,pO2就回到基线。
然后重复循环。对照溶液30分钟后(12∶30)置于容器中且加入更多的以保持覆盖皮肤表面。没有观察到pO2的增加。随后,取出对照溶液并通过几次加入测试溶液代替(虚线)。随着测试溶液加入,pO2水平出现峰(图4)。
已知传感器反应受温度影响。确定试验位置pO2增加和皮肤温度间的关系是重要的。如图5所见,对照和测试溶液都改变皮肤温度1-2℃。因为pO2仅随着超氧化测试溶液增加,可推断pO2增加不是由于温度变化而实际上由于应用测试溶液于皮肤表面。
第二次试验使用植入动物右侧的传感器,显示电极位置的重要性。用对照和测试溶液记录的pO2水平间没有观察到不同。然而,位置上基线pO2是75mmHg,非常接近90mmHg的血液pO2水平(如与左侧位置55mmHg相比)。这可能由于传感器紧密接近毛细血管。也许从血液交换的气体引起流动血液和测试溶液间的区别太小而无法观察到。另外,传感器可能放置地太深而无法检测到O2扩散穿过皮肤。
5.2实施例2:人受试者中皮下氧化的增加
用人受试者过程的结果证明所述超氧化溶液中的氧可穿过健康人皮肤传输给皮下组织以增加基线水平上皮下pO2。
得到十名人受试者的同意后,测量基线血压和心跳。将脉搏血氧计探针置于受试者手指上以在研究中连续监控心跳和氧饱和度。左小腿外部皮肤用betadine和从皮肤表面清处的betadine消毒。如手术般插入导管(22 Ga,1-3/8”导管,产品号04122,Arrow)于皮肤表面下并随后拔出,从而暴露针头和导管且几乎整个长度的导管在皮下空间。导管尽量放置接近皮肤表面而不推动针过早穿过皮肤,这是为了使导管置于真皮中或真皮边缘和皮下组织。
放置导管后,取出针且传感器顶端与导管头在一个方向上。传感器用传感器调节装置通过导管调节直至它可从导管另一端看见。取出导管并缩回传感器直到传感器顶端正好在皮肤下消失。感应元件2.5cm长且完全包含在皮肤下。导管产生的洞用防水敷料(Duodermthin,产品号NDC 0003-1879-55,Conva Tec)覆盖,传感器机架在适当位置录音以保持机架的重量和防止电缆拉出传感器。
用屏障刷(barrier wipe)(Allkare,Conva Tec)准备传感器周围的皮肤。将气孔糊剂(stoma paste)(Stomahesive,产品号NDC 0003-1839-10,Conva Tec)置于皮肤上,这是软质袋(ActiveLife,NDC 0003-0254-33,Conva Tec)和2.5”洞要放置的地方。随后袋放在所用糊剂上。袋和皮肤间任何间隙,具体是传感器机架周围,用气孔糊剂填充。袋也用传感器机架周围的Hytape与皮肤密封。
试验分两个阶段进行,如实施例1,第一个阶段包括测试对对照和氧化溶液的反应,溶液通过粘贴袋的方式应用于传感器位置。第二个阶段,在相同病人第一个阶段后进行,包括在适当位置维持传感器时取出袋以随后在受试者的腿浸入涡流浴中的测试溶液时测试反应。涡流浴是标准不锈钢浴室,约30”长×18”宽×30”高的通常用于物理治疗和其它临床设施的种类。
在阶段一,有两种不同浓度的氧化水和对照溶液在袋中。用于此试验的氧化水如前面所述制备。插入后允许传感器平衡30分钟。传感器同时测量和记录温度、氧分压(pO2)二氧化碳分压(pCO2)和pH。在整个研究中数据在笔记本电脑上每10秒收集一次。各个含这些溶液的瓶加热至32℃(除了一些受试者;见表2)。这些溶液的次序随机选择。500ml的所选溶液置于软质袋中,袋随后用夹子密封。溶液保持与皮肤接触15分钟。各溶液间维持一个15分钟的稳定阶段。不是所有的受试者都接受3种溶液(见表2)。此阶段完成时袋从粘贴框上切下以使皮肤-覆盖的传感器暴露于空气和最终涡流中的水。
对于第二个(涡流)阶段,产生氧化溶液的过程如下修饰。已知氧在开放容器中迅速从溶液消散。为了如开放浴室中循环的溶液一样持续维持溶液中高的氧水平,沐浴的流出物与氧化机器进口相连,这允许35gal/min速度的再循环,所有的水每2分钟回到浴室。对照和氧化溶液在机器中接受90psi的压力,在试验中连续循环。活化机器中的输入氧后迅速获得循环水的氧化,试验中O2水平通过浴室中溶解氧计监控。水中氧的存在也通过其出现在乳白色溶液中可明显检测。
取出覆盖传感器的带子并将袋粘合剂超后拉出。传感器用传感器收回装置从皮肤缩回。在研究结束时测量受试者的血压和心跳。在三个受试者中,真皮厚度和传感器深度用来自GWB International的20MHz超声波系统测量。
在这阶段的起始时,传感器在组织中至少1小时。当腿插入加热的涡流时,温度迅速(<1分钟)上升到浴室的水平。在温度正好稳定到浴室水平时记录温度、pO2、pH和pCO2。在各受试者阶段结束时读取对照读数和皮下氧化读数。统计分析在整个涡流阶段收集的数据上进行。温度和皮下pO2在基线、对照和氧化阶段间的变化用重复-测量Anova试验来比较,接着通过Tukey HSD试验来说明时间阶段间的不同。
五个男性和五个女性受试者在此研究中被测试。表2详述了各受试者和操作过程的具体特征。所有的受试者被认为是超重(BMI 25)。受试者在涡流研究前接受稍微不同的治疗。然而每个受试者在涡流研究开始前至少有1小时稳定时间且各受试者花约30分钟在氧化的水种。所有统计分析受限于涡流中的时间段。

表3详述了传感器深度的定性评估,定量测量用超声波研究的3个受试者。以超声波测量为基础,如果传感器可感觉为穿过皮肤的膨胀,可能传感器在真皮和下面皮下组织间的界面。在一个受试者中(HY03),传感器因为无法锁定位置而不能工作。此受试者在组织的相同区域重插入传感器。另一个受试者(HY09)当传感器插入时出血且皮肤下观察到一条沿着传感器的蓝色血线,说明此传感器可能位于血液中。在第三个受试者中(HY10),当传感器取出时出血,说明此受试者通道中可能也有血。

对于试验的涡流阶段,浴室加热到32℃(除了一些受试者,见表4)。对于头5个受试者,腿浸入对照溶液(4ppm O2的蒸馏水)的浴中15分钟。对于最后5个受试者,对照阶段持续30分钟。对照阶段后,打开氧化机器以氧化水。3-4分钟内到达充分氧化(~55ppm)且腿浸入氧化水30分钟。随后受试者的腿从浴中取出并接着进行15分钟。
所选受试者在整个操作过程中温度和皮下pO2的图显示于图6和图7。

所有受试者涡流数据的概括示于表4。10个受试者pO2的平均值起始于31+/-13mmHg,但在对照阶段结束时(29+/-14mmHg)没有显著不同。浸入氧化的水后,皮下pO2显著增加到44+/-22mmHg(与基线相比p0.026且与对照末端相比p0.016)。各受试者皮下PO2在对照和氧化阶段增加的百分比(或减少)图示于图8。
从图8可看出pO2增加百分比在受试者间相当不同,10个中的6个显示皮下pO2增加至少30%。受试者HY07、HY01、HY05和HY02分别观察到更显著的增加(141%、130%、90%、70%)。从表3可看出在研究的对照和氧化阶段中组织温度都上升。氧化阶段末期的温度与基线(p0.0006)和对照阶段末期(p0.015)显著不同。这些温度变化伴随着组织pCO2增加和组织pH减少。表5显示各测量参数基线值的平均增加百分比。在此分析中pH转化成氢离子浓度[H+]。

数据显示当受试者腿浸入氧化的水时,皮下氧张力有显著增加,或与基线或与对照值比较时涡流包含均匀氧化(~4ppm)水。与皮下pO2增加一致的是温度上升。温度上升似乎不影响传感器反应(它校正温度变化)但有一些物理和生理效果。随着温度增加,氧在血液中的溶解度降低,提高其释放给组织。温度增加也导致氧从氧合血红蛋白解离提高。最终温度增加导致要带更多氧给组织的血管舒张。然而,对受试者HY01、HY05、HY07、HY08和HY10,pO2增加似乎是由于温度上升引起外的来源,而对所有受试者除了HY01,温度上升小于1℃。但可能观察到的pO2增加也许由于受试者HY02、HY04和HY06的温度,其中pO2和温度间的平方相关系数(R2)高于0.8℃。
在循环水被氧化的阶段中pCO2和[H+]水平增加。两者增加相同的量,考虑到它们在重碳酸盐平衡中的依存这不令人惊奇。这些变化的来源没有确定。
一个受试者(HY03)显示pO2减少,减少是氧化的结果。在此受试者中,传感器再引入第二次。组织损伤可能导致对氧的反应受损。第二个受试者(HY09)当传感器插入时明显出血且传感器也许通过血液与组织液隔离,抑制对增加氧的反应。当从受试者HY10上收回传感器时观察到一些尽管较少的血液,这也可解释此受试者的小反应。当传感器插入组织时,必须有充足的时间使暂时的组织损伤减小。最初30分钟被认为是可接受的,但此阶段后连续下降表明不是所有的损伤都被解决。涡流阶段开始时的基线读数允许各受试者至少60分钟的稳定。此时间与瑞典用Paratrend传感器直接在猪皮下组织中进行的研究一致(Mellstrom等,1999)。那些研究中的基线值与此研究所测的区别不大:pO2:58+/-16;pCO2:42+/-5,和pH:7.46+/-0.06。在手术病人的研究中使用极谱氧电极,发现基线皮下pO2是43+/-10(Hopf等,1997)。因为那些传感器比现在可能接近真皮边缘的传感器适当放置更深,那些报道的值可与这些实验中的稍微不同。
结果显示至少一半受试者中有显著的组织pO2增加,增加与涡流浴中引入氧微泡有关。涡流比仍氧化的水(试验的阶段1)在产生皮下pO2增加上更有效,尽管软质袋中使用的水有更高浓度的氧(101-105ppm O2对涡流中55ppm)。两个在涡流中有很好反应的受试者中(HY01和HY07),袋中也有对氧化的水可测量的反应。这是在研究的袋部分中唯一有反应的两个受试者。这些结果表明各受试者氧穿过皮肤到传感器的扩散速度可能不同。
传感器深度控制在皮肤表面下1-3mm。然而在三个用超声波检测的受试者中,皮肤机构和上皮厚度变化相当大。如果氧扩散受限,也许仅到达一定深度。不同受试者与水化合的能力或他们的皮肤气体渗透性可能不同。然而,研究结果显示氧化水中的高氧分压允许皮肤外足够高浓度的氧来协助氧扩散穿过大部分健康受试者的皮肤。
5.3实施例3:应用组织超氧化的方法于伤口复原
在糖尿病病人中进行初步的研究并与在动物和正常人试验中进行的研究比较来确定当超氧化微泡给药于糖尿病病人非-复原伤口时对复原速度的作用。病人保持在紧密控制的环境条件下。此外,分析伤口区域并根据斯克兰顿大学分子生物学和医学院(Institute of Molecular Biology and Medicine at University ofScranton)进行的研究鉴定厌氧细菌。根据此研究,约10-20%糖尿病足部伤口最初的抗生素治疗失败。一般认为一些细菌种类可存在于这些类型的伤口中。因为一些这种生物体不能方便地培养,适当鉴定是问题,因此不能应用适当治疗形式。报告检测了存在于慢性糖尿病足部伤口的细菌菌群,伤口使抗生素治疗失败。组织样品从伤口底部收集并用于标准微生物培养。来自样品的DNA用于扩增细菌16SrDNA基因序列并制备文库,文库的克隆被测序。以培养为基础的方法鉴定一种单独的厌氧种类,脆弱拟杆菌(Bacteroides fragilis),而用rDNA测序的方法鉴定脆弱拟杆菌为一种优势生物且臭位假单胞菌(Pseudomonas mephitica)(紫色杆菌属)为次要组分。结果表明rDNA测序方法在鉴定来自伤口的细菌中可以是重要的工具(Redkar等,2000)。
以控制的随机方式通过给药组合物于伤口区域进行实验,给药以不同的浓度和形式,随后分析存在于未治疗和用超氧化水治疗的伤口中的细菌。
5.4实施例4:治疗腿溃疡中的组织超氧化
进行临床研究以调查和比较感染和非感染腿溃疡的需氧和厌氧微生物。腿溃疡,在临床征兆的基础上定义为感染,擦拭取样并用严格分离和鉴定技术测试需氧和厌氧微生物(Bowler等,1999)。
在此研究中,培养了220种来自44种感染腿溃疡的分离物和110种来自30种非感染腿溃疡的分离物。统计分析表明与非感染溃疡组相比,每种感染溃疡的厌氧细菌的平均数目显著增大(尤其是消化球菌属和普雷沃氏菌属)(分别为2.5对1.3)(P<1.5)。同样,厌氧性生物代表49%感染腿溃疡中的总微生物组成,与之相比在非感染腿溃疡中为36%(Bowler等,1999)。
以这些研究为基础,超氧化微泡组合物可用于测试氧化治疗对需氧和厌氧微生物系统分布的效果,微生物系统存在于腿溃疡中。组合物抗击腿溃疡和其它伤口感染有效性的指示通过注意厌氧和需氧生物分布的相对变化来确定。
5.5实施例5:超氧化冰
下列实施例证明高度超氧化冰保持高水平氧和在高水平释放氧的能力。结果表明冰可用高度氧化水制造且冰和融化的液体与作为对照的自来水和冰相比包含高浓度氧。
两瓶超氧化(SO)液体在-15℃贮存。第三瓶在8℃冷冻。对照样品是来自自来水的冰和冷冻到8℃的自来水。超氧化液体在盖紧的瓶中保存约6个月。氧水平在贮存时间中范围从107到123ppm。从贮藏库中取出瓶并用修饰高范围Oxygard HandyMKII仪以每百万测量部分的标准单位来测量氧水平。仪器在冰开始融化的冰表面测量氧(冰读数)。也测量融化的冰(融化读数)。对照自来水和自来水冰进行同样的测量。样品在8℃注入开放容器并直接测量氧水平。结果显示于表6。

Claims (25)
1.超氧化组合物在提高哺乳动物组织氧化中的应用,其特征在于,所述应用包括将超氧化组合物用于组织表面一段足够长的时间以使上皮下氧分压在基线pO2上从30%增加到120%。
2.如权利要求1所述的应用,其特征在于,所述哺乳动物是人。
3.如权利要求1所述的应用,其特征在于,所述组织是皮肤。
4.如权利要求3所述的应用,其特征在于,所述组织受症状或疾病如褥疮、伤口、烧伤、溃疡、细菌感染如厌氧病原菌感染的影响。
5.如权利要求1所述的应用,其特征在于,所述应用用于口部或阴道粘膜表面、肺组织表面或移植器官。
6.如权利要求1所述的应用,其特征在于,所述超氧化组合物含有45ppm氧到220ppm氧或55ppm氧且组合物的温度是0℃到34℃。
7.如权利要求1所述的应用,其特征在于,所述超氧化组合物含有药学上可接受的载体。
8.如权利要求1所述的应用,其特征在于,所述超氧化组合物含有水和氧微泡,其中氧微泡的直径为2μ到10μ或0.6μ到5μ。
9.如权利要求8所述的应用,其特征在于,所述超氧化组合物在搅拌下通过浸泡、沉浸、喷射、摩擦或气雾剂在霜剂、洗剂或凝胶剂中应用。
10.如权利要求9所述的应用,其特征在于,所述搅拌在涡流浴(whirlpool bath)中提供。
11.一种超氧化组合物,其特征在于,所述超氧化组合物含有氧微泡的水溶液,微泡直径从0.6微米到100微米,其中氧浓度在55ppm和220ppm之间。
12.如权利要求11所述的超氧化组合物,其特征在于,所述微泡直径范围从0.6到8微米或5到8微米。
13.如权利要求11所述的组合物,其特征在于,所述组合物呈固体形式。
14.如权利要求13所述的组合物,其特征在于,所述组合物的温度在-70℃和0℃之间。
15.一种为需要增加组织氧化的哺乳动物提供组织氧化环境的设备,其特征在于,所述设备包含用来保存至少55ppm超氧化水溶液的容器,水溶液产自与所述容器相连的氧生成机器。
16.如权利要求15所述的设备,其特征在于,所述设备还包括搅拌所述水溶液的方法,如通过超声波或涡流。
17.如权利要求15所述的设备,其特征在于,所述设备还包括温度控制器。
18.如权利要求17所述的设备,其特征在于,温度被控制在37℃和45℃之间。
19.一种增加厌氧细菌暴露于氧的方法,其特征在于,所述方法包括将权利要求11所述的超氧化组合物用于怀疑带有厌氧细菌的皮肤损伤。
20.权利要求11所述的超氧化组合物在增加厌氧细菌暴露于氧中的应用,其特征在于,所述应用包括将超氧化组合物用于怀疑带有厌氧细菌的皮肤损伤。
21.如权利要求20所述的方法,其特征在于,厌氧细菌是病原性的。
22.如权利要求21所述的方法,其特征在于,厌氧细菌含在坏疽、溃疡组织或伤口如烧伤伤口中。
23.一种局部使用以提高组织氧化的试剂盒,其特征在于,所述试剂盒包括密封的可渗透的韧性容器,容器中含权利要求11的组合物;和
将所述组合物用于需要提高组织氧化的受试者皮肤的说明。
24.如权利要求23所述的试剂盒,其特征在于,所述试剂盒还包括一个容器,容器装有涡流产生装置和任选的用于调节容器内温度的恒温/加热装置。
25.如权利要求1所述的组合物在减少疤痕组织或预防性减少与伤口、烧伤或手术过程相关的疤痕的治疗中的应用。
