Abstract

在优选实施例中,撑隔器包括在相对端之间的一通路,端帽的内端延伸到该通路内。端帽的侧壁从外端向内端汇聚,以将端帽楔形地配合在撑隔器内。此外,端帽包括至少一个凸起,该凸起连接于侧壁,其形状适合于与撑隔器接合。凸起阻挡住端帽在撑隔器的通路中的滑动。外端还包括一促进骨骼向内生长的外表面,例如一个多孔涂层或一锯齿状表面。优选的是,端帽侧壁和凸起与撑隔器12的侧壁的接合提供了端帽与撑隔器之间的唯一的机械连接。
一种替换形式是,端帽的侧壁包括一个在内外端之间延伸的槽。该槽使端帽的侧壁在插入撑隔器的通路时能够受到挤压。一旦定位在该通路内,该侧壁就向撑隔器的侧壁膨胀,以便以摩擦的方式将端帽锁定在撑隔器的通路中。从可压缩的端帽的侧壁上至少伸出一个凸起,该凸起用于与撑隔器接合,以阻挡端帽在通路中的滑动。
进而,撑隔器的侧壁包括一外表面,一个替换形式的端帽成形为适合于在撑隔器的相对端上延伸,并且可以连接于外表面。该端帽包括一个外端、一个内端和一个在内外端之间延伸的侧壁。此外,侧壁包括一个在内外端之间延伸的槽。该槽使得端帽放在撑隔器的一端上时端帽的侧壁能够膨胀。只要一定位在撑隔器的外表面上,端帽的侧壁就向撑隔器的侧壁收缩,以便通过摩擦将端帽锁定在撑隔器的侧壁上。从可压缩的端帽的侧壁上至少伸出一个凸起,用于与撑隔器接合,从而阻挡端帽在通路中的滑动。

附图的详细描述
按照本发明,提供端帽10,用来与在锥间盘替换或脊椎体替换手术时用于一脊柱14内的脊椎体撑隔器12连用,以形成一撑隔器组件100。如图1所示,端帽10连接于一撑隔器12,并适合于置入脊柱14的一前部18。可以进行这种置入来替换一个内锥间盘20或一脊椎体22或其各种形式。
撑隔器12例如为一钛网构成的外科网管。撑隔器12罩住骨头(未示出),使得撑隔器12在其与脊柱14之间大致上没有运动的地方与脊柱14融合,以减轻病人的疼痛。撑隔器10包括相对的两端24、26和一在该两端24、26之间延伸的圆柱形侧壁30,该侧壁限定一具有预定内径28的通路32。相对的两端24和26各包括峰部34和谷部36,这些峰部和谷部构成爪38。此外,侧壁30具有一内表面40、一外表面42和在该两表面40、42之间延伸的孔44。如图2所示,孔44大致为宝石形,其定位成使彼此之间呈一宝石状图案。然而,可以理解的是,孔44可以具有各种形状和尺寸,并以各种方式定位,以便在治愈过程中促使骨头向内生长。除了钛网之外,端帽10还适合于与其它形式的用于脊柱14的前部18的撑隔器一起使用。具体地说,端帽10使用于各种尺寸和由各种金属、组合物、组织或骨头制造的网式撑隔器,或者其它类型的设计成作为一脊柱撑隔器放置在脊柱14内的网式撑隔器。
参见图1,脊柱14包括前部18和一后部46。此外,脊柱14包括脊椎体48,其中七个是颈椎体,十二个是胸椎体,五个是腰锥体。本发明的端帽连接于分配给脊柱14的前部18或后部46,无论是颈部的、胸部的还是腰部的脊柱均可以。撑隔器12可以设计成或者替换一脊面20,如图1所示,或者替换一脊椎体48。
如图2所示,端帽10连接于撑隔器12的两端24和26,以用于内体熔接手术。端帽10保持在撑隔器12的两端24和26上,不会向下滑入通路32。如图2-5所示,端帽10包括一外端50、一内端52和一锥形的侧壁54,该侧壁从外端50向内端52收缩。参见图4,侧壁54包括一大致为圆柱形的内表面56和一外表面62,该内表面具有一不变的直径58,并限定一通路60。虽然在图4和5中示出的端帽10具有大致圆柱形的外和内端50、52,但是应该理解,外和内端50、52可以成形为各种形状,以便与各种撑隔器12的相对端24和26相配合,并且可以大致平行或者相互彼此呈一角度。还可以理解,端帽10的尺寸可以改变,以便与各种撑隔器12配合。
侧壁54的外表面62与侧壁30的内表面40配合,在端帽10与撑隔器12之间形成一个摩擦的锁定,消除了将端帽10与撑隔器12连接在一起的附加部件的必要性。现在参见图4,外表面62具有与外端50邻接的第一外径64和与内端52相邻的第二外径66。第一外经64大于第二外经66,因此,侧壁54的外表面62收缩大约8度的锥度,如图3所示。应该理解,按照本公开,侧壁54可以形成各种角度,只要端帽10与撑隔器12之间形成摩擦锁定即可。此外,与端帽10的外端50邻接的侧壁54的第一外径64大致等于撑隔器14的直径,因此,外端50与撑隔器12的端部24相邻。
如图3所示,外表面62还形成为包括凸起或交错齿78,这些齿彼此间隔开,并与端帽10的外端50相邻。参见图6和7,齿78定位在外表面62上,因此,当端帽10楔入撑隔器12内时,齿78保持抵靠相对端24、26的谷部36。由于各齿78处在爪38内,抑制了端帽10滑入撑隔器12的通路32内。侧壁54和齿78与撑隔器12的侧壁30的接合提供了端帽10与撑隔器12之间的唯一的机械连接。虽然图示的端帽10有七个齿78,但应该理解,按照本发明,可以将多于或少于七个齿连接于外表面62。
参见图6,端帽10的外端50包括一形状形成促进骨头向内生长的外侧68。例如,一种类型的促进骨头向内生长的外侧68是一种多孔并涂覆的成品69,如图11和12所示。另一种适合于骨头生长的外侧68为一带齿的外表面70。见图1-7。端帽10的带齿的表面70包括峰部74和谷部76。峰部74和谷部76提供了粗糙的表面,以便减小相对于脊椎体48的移动。虽然描述了多孔和带齿的外精加工件70,但是应该理解,按照本发明,可以采用各种促进骨头生长的抗摩擦的表面。
当替换锥间盘20或脊椎体48时,医生首先要去除损坏的锥间盘20或锥体48,以便形成一空间80。例如图1中的空间80。只要空间80一形成,医生就选择适当尺寸的撑隔器12和至少一个相应的端帽10。虽然后面只讨论一种端帽10和撑隔器12的端部24,但应理解,说明书和权利要求书适合于不止一种端帽10和端26。现在参见图2,端帽10的内端52与撑隔器12的端部24对正。内端52插入通路32,如图7所示,直到外表面62与撑隔器的侧壁30楔入接舍,以形成一种撑隔器组件100。这时,齿78定位在爪38内,以防止端帽10进一步向撑隔器12的通路32内移动。端帽10连接于撑隔器12后,就采用各种外科技术可以将撑隔器组件100插入空间80。
在本发明的一替换实施例中,其设置成端帽110具有一椭圆的形状。见图8-10,端帽110成形为与一椭圆形撑隔器(未示出)配合,除了椭圆形的侧壁30以外,该撑隔器形成与撑隔器12的形状一致。端帽110的形状类似于端帽10,采用类似的标记表示类似的部件。如图8所示,端帽110具有一与外端50相邻的第一环宽164和一与内端152相邻的第二环宽166。第一环宽164大于第二环宽166,因此,侧壁54的外表面62从外端50向内端152收敛,并形成大约为8度的锥度,如图9所示。应该理解,锥形角可以不同,只要端帽10可以楔入撑隔器12即可。此外,端帽包括十个连接于外表面62的齿78。然而应该理解,也可以在外表面62上连接多于或少于十个齿78。
在本发明的一替换实施例中,提供一端帽210。该端帽210形成为类似于端帽10,并用类似的标记表示类似的部件。如图11和12所示,端帽210包括一单独的凸起或外唇282,该外唇从侧壁54上延伸出,与外端50相邻。外唇282围绕着外端50的周边延伸,它包括一内表面284,该内表面分别座放在相对端24、26的峰部34上,以防止端帽210滑入撑隔器12的通路32内。侧壁54和齿唇282与撑隔器12的侧壁30的接合提供了端帽210与撑隔器12之间的仅有的机械连接。
在本发明的另一实施例中提供一种端帽310,如图13和14所示,端帽310是一实心板358,如图所示,其具有椭圆形状,且特别适用于骨质疏松的骨头。端帽310包括一外端350、一内端352和一从外端350向内端352收缩的锥形侧壁354。现在参见图14,侧壁354包括一外表面362。侧壁354的外表面362呈类似于外表面62的锥形,以便与撑隔器12形成一种摩擦的锁定。侧壁354与撑隔器12的侧壁30的接合提供了端帽310与撑隔器12之间的唯一的机械连接。此外,孔360在外与内端350、352之间延伸。虽然图13和14所示的端帽10具有大致椭圆的外端350,但是可以理解,端帽310可以是类似于端帽10或其它的形状,以便与各种撑隔器12配合和/或形成为不带孔360。还可以理解,端帽10可以具有不同的尺寸,以便与各种撑隔器12配合。而且还可以理解,端帽310可以包括外齿54和/或外唇56。
本发明的另一实施例示于图15-17。端帽410包括一外端414、一内端416以及在外货内端414、416之间延伸的第一和第二侧壁420、429。第二侧壁429以斜面形成,易于将端帽410插入撑隔器12,并且是类似于侧壁54,从第一侧壁部分420向内端416呈锥形。第二侧壁429示于图15。第一和第二侧壁420、429与内表面418共同限定通路422。第一侧壁部分420还包括外表面421,而第二侧壁429包括外表面423。六个齿454沿着第一侧壁部分420的周边延伸,并以与齿78相同的方式连接于外表面421。齿454防止端帽410过多地凹入撑隔器12。第一侧壁部分420和齿454与撑隔器12的侧壁30的接舍提供了端帽410与撑隔器12之间的仅有的机械连接。可以理解,按照在此公开的内容,齿454的数量可以大于或小于六个。还可以理解,外和内端414、416可以定位成大致彼此平行,或者可以彼此呈一角度。
如图15和16所示,端帽410还包括内横截端424、426,因此,端帽410具有一为开口的环形。横截端424和426限定一槽或切开部分428。槽428使端帽410能够在其插入撑隔器12的通路32时在方向430上受到挤压。在图15和16中示出端帽410处于完全敞开的位置,因此,一个下部外径(图17)大于通路32的直径28。当端帽410沿着方向430挤压在一起时,其尺寸适合于插入撑隔器12的通路32中。一旦插入撑隔器12,端帽410就扩张到其原始的形状和尺寸,从而压靠在撑隔器12的内表面40上,以保证一种牢固的紧密配合。
图18示出本发明的另一替换实施例。端帽510与端帽410几乎在所有方面都一样,类似的标记表示类似的部件。端帽510包括连接于外表面414并离开侧壁420延伸的齿554。外端414的结构提供进一步有利于骨头向内生长的固定物。
如图19-22所示,提供端帽610。端帽610的形状类似于端帽410,类似的标记表示类似的部件。端帽610包括一在外和内端414、416之间延伸的通路622。通路622的形状是不规则的,这是为了使第一和第二侧壁420、429的内表面632的表面积最大。如图19和20所示,内表面632限定相互彼此隔开的囊634。虽然图示的端帽610具有不规则的形状的通路622并带有曲线的囊634,但是可以理解,按照本公开文件,端帽610可以形成包括具有几种形状和尺寸的通路。
现在参见图23-26,根据本发明设置端帽710。端帽710的形状类似于端帽410,类似的标记表示类似的部件。端帽710包括一在内和外端714、716之间延伸的侧壁720。侧壁720形成为不带斜面。此外,如图25和26所示,内和外端714、716相互之间以一预定的角度731定位,并形成包括不同的表面730和732,例如多孔的表面69和带齿面70。如图23和24所示,端帽710包括孔734,这些孔用于接收一器械(未示出),该器械用于沿方向430挤压插入撑隔器12的通路32中的端帽。由于侧壁720不是锥形的,所以,医生可以将内或外端714、716插入通路32,使所需的面背离通路32,以便与脊椎体48接合。可以理解,按照本公开文件,内和外端相互之间可以呈各种角度,也可以平行定位。
图27-30示出按照本发明的另一实施例的一端帽810。端帽810装配在撑隔器12的外表面42上,而不是像以前描述的端帽10、110、210、310、410、510、610和710那样装入通路32内。端帽810包括一外端814、一内端816以及一在外和内端814、816之间延伸的新月形侧壁820。可以理解,按照本公开文件,端帽可以成形为圆形、椭圆形或任何复杂的多种形状。
新月形侧壁820包括一限定一通路822的内表面818和外表面812。一凸起或唇854围绕着侧壁820的与外端814相邻的周边延伸到通路822内。唇854防止端帽810过多地凹入撑隔器12。可以理解,按照本公开文件,唇854的数量可以大于一个。还可以理解,外和内端814、816可以彼此平行放置,也可以互相呈一个角度放置。
如图27和28所示,端帽810还包括内横截端824、826,它们限定了一个槽或切口部分828。槽828使端帽810能够沿方向830膨胀,从而使内表面818的尺寸适合于围绕撑隔器12的侧壁30的外表面42延伸。撑隔器12的端部24定位好后,端帽810向图27所示的其原始的形状和尺寸挤压,从而压靠在撑隔器12的外表面42上,以保证牢固的紧配合。唇854与撑隔器12的端部24接触,以阻止轴向负荷。此外,唇提供了与齿78所提供的相同的支撑,因此,端帽810不会围绕着撑隔器12的侧壁30向端部26凹入。侧壁820和唇854与撑隔器12的侧壁30的接合提供了端帽810与撑隔器12之间的唯一的机械连接。
因此,端帽10、11、210、310、410、510、610、710和810的形状适合于与撑隔器配合,以形成用于脊柱外科的撑隔器组件。端帽10、11、210、310、410、510、610、710和810加强了用于脊柱和创伤外科的撑隔器,以提高稳定性和对剪切力的阻力。本发明的端帽配合在撑隔器内、撑隔器外、或者撑隔器上,并通过锥形或为开口的环的力连接于撑隔器。端帽10、11、210、310、410、510、610、710和810优选自锁在撑隔器12内或上。此外,本发明的端帽可以具有各种不同的多种形状,以配合各种撑隔器的形状,并具有各种表面涂层或结构,以促进骨头生长或形成防滑特征,以防止向骨头移动,提高稳定性。至少一个凸起从端帽向撑隔器延伸,以抑止端帽滑入撑隔器体的通路内。
虽然前面已经参考一优选实施例详细描述了本发明,但是,在权利要求书所描述和限定的本发明的范围和精神内可以有变化和改进。
Claims (34)
1.一种用于脊柱外科的撑隔器组件,该组件包括:
一撑隔器,其形成为包括相对的端和在相对的端之间延伸的侧壁;和
至少一个连接于所述相对端中的至少一个上的端帽,至少一个端帽中的每个包括一面向撑隔器的内端、一外端和一在该内和外端之间延伸的侧壁,该侧壁与撑隔器的侧壁接合,以提供在至少一个端帽与撑隔器之间的机械连接。
2.如权利要求1所述的撑隔器组件,其中,撑隔器包括在相对端之间延伸的通路,端帽的内端伸入该通路。
