本发明涉及治疗人类卵巢癌的方法和组合物。更具体地说,本发明涉及使用紫杉烷(taxane)组合物治疗卵巢癌的方法。
卵巢癌是女性癌症死亡中第四大最频繁的原因,在美国每年估计有大约13,000人死亡。而且,在发达国家发病率逐渐上升。尽管已有与紊乱的内分泌功能相关的流行病证据,但是肿瘤转移的病因仍不清楚。在未经产的妇女和早绝经的妇女中卵巢癌的发病率较高。
对晚期卵巢癌患者来说化疗仍然是主要疗法。在33-65%的患者中烷化剂都诱发反应。这类烷化剂包括但不限定为苯丁酸氮芥、塞替派、环磷酰胺。目前还已知其他试剂也是有效的。其中,顺铂和咔铂被认为是最具活性的单一的细胞毒药物并且反应率似乎与剂量有关。在一项近期包括八十年代对卵巢患者的大多数化疗临床试验的分析后证实以铂为主的治疗剂是最有效的单一试剂。也发现当把以铂为主的疗法与其他试剂结合比单用更有效。顺铂也是首次被证实作为第二线治疗有用的细胞毒毒物,第二线治疗指的是在用烷化剂治疗的第一线治疗后对肿瘤复发患者的治疗。
在八十年代后期,人们进行了一些临床试验并导致在卵巢患者中使用了紫杉酚。现在在许多国家已批准把紫杉酚用作第二线治疗并且正估计把紫杉酚作为与顺铂结合作为一种前线治疗。
紫杉酚是稳定微管的新抗肿瘤剂,最初是从太平洋短叶紫杉即西方的紫杉树的茎皮中分离出来的。紫杉酚是通过促进异常稳定的微管的形成并且抑制需要有丝分裂和细胞增生的微管网的正常动态组织再生来起作用的。(参见Schiff,P.B.,等(1979)自然277,665;Schiff,P.B.,等(1981)生物化学20,3247)。在紫杉酚存在时,需要聚合的微管蛋白浓度显著下降。微管组配是在无GTP和低温度下出现的,并且所形成的微管对于稀释、钙、冷和抑制药物引起的解聚更稳定。紫杉酚能够可逆性地与聚合的微管蛋白结合,而在紫杉酚存在时其他结合微管蛋白的药物将与微管蛋白结合。
紫杉酚能够与多种类型器官的微管系统发生反应。例如,在哺乳动物的细胞中50nM的紫杉酚浓度通常就可引起微管数目的显著增加,并伴随着细胞形状的变化和活性分裂细胞中有丝分裂的中止。(Parness,J.,等,(1982)生物化学生物生理学研究通报105,1082)。由紫杉酚所引起的微管功能紊乱对细胞具有一定影响,这是由于在细胞能动性、分泌和细胞分裂中微管起一定作用。
对紫杉酚抗肿瘤生长作用已在使用各种给药计划的几个临床试验中进行了研究。在把紫杉酚给药后已观察到了严重的过敏反应。尽管如此,已证实过敏反应的发病率和严重程度是受紫杉酚输液速度所影响的(Weiss,R B.,等,(1990)临床肿瘤学杂志8,1263)。
在Taxol(Bristol Meyers Squibb的紫杉酚产品的商标)的研究中,把400多名卵巢癌患者随机分成两类,(1)在135到175mg/m2之间和(2)在3小时和24小时之间的输液。在该试验中,看到最小程度的脸红者占42%而看到严重过敏反应者仅占1-2%,在频率和程度上不受剂量或顺序的影响。相对于低剂量出现被描述为高热异感症的感觉神经症状(36%),在175mg/m2剂量更频繁(52%)。在给药后1-2天和持续几天后发作关节痛症状是普遍的(55-65%)并且在高剂量时更频繁。
伴随24小时的输液,骨髓抑制(主要是粒细胞缺乏症)频繁出现但不是最普遍的(71%)。只有18%的通过3小时输液接受药物的患者发展成4级粒细胞缺乏症。文件证明仅伴随24小时的输液出现热性中性白细胞减少症。口炎出现率为22-30%并且严重的少于2%。心血管作用几乎很少。仅在1%的循环(2354循环中的26)观察到了症状性的心动过缓(脉搏<50次/分钟)并且仅在2354次循环中的13次中观察到了低血压。9名患者因为非血液性毒性中断了Taxol的治疗。这些患者中,4名有神经毒性,3名有高过敏性,1名有粘膜炎和1名有自发性肺水肿。
在紫杉酚治疗中使用从3小时直到24小时的输液时间降低了毒性和副作用包括对药物过敏反应的发病率。来自这些试验的数据表明可逆性的骨髓抑制是受剂量限制的毒性,伴有在275mg/m2或更高剂量所观察到的显著外周神经病理。其他毒性包括肌痛、粘膜炎和脱发。
在1992年12月FDA批准了紫杉酚作为在第一线或随后的化疗失败后卵巢癌的治疗。这次通过是以原始的II期研究和国家癌症研究会同意的请求计划为基础的,其中是紫杉酚在24小时期间给药。更具体地说,对相同的病症在许多国家中已批准了更方便的3小时方案。
现已报导了紫杉酚在乳腺癌治疗中的研究。在单一药剂的试验中,现已发现反应率与剂量相关,相对于伴有G-CSF的250mg/m2的剂量有36-71%的反应率,在135到175g/m2的剂量有13-37%的反应率。现已报导在600名患者的试验中使用紫杉酚(135mg/m2)结合顺铂(75mg/m2)有最高的反应率。
在一次欧洲-加拿大多重研究试验的卵巢癌患者中,把407名患者随机分成两个剂量水平(135mg/m2或175mg/m2)的一组,每三周给药,采用3小时或者24小时方案。参见Eisenhauer,E.A.等“紫杉酚在复发卵巢癌中的欧洲-加拿大随机试验:高剂量对低剂量和长期对短期输液”临床肿瘤学杂志1994.12:2654-66。反应率在175mg/m2剂量(20%)比在135mg/m2剂量(15%)时高,但差异在统计学上不显著。在接受高剂量的患者中肿瘤发展的时间显著延长(19周对14周)。在不同小组中患者总存活占死亡大约25%是相似的。这样试验得出结论为较短(3小时)的输液与24小时的输液几乎是一样有效的,但更安全并更方便。
象其他化疗剂一样紫杉酚已在肿瘤细胞中出现抗药性。肿瘤细胞的抗药性是对化疗剂的共同反应。在体外已经区分出对紫杉酚抗药性的两种机理。在一个细胞类型中,抗药性是由于药物的流出引起的,药物流出导致引起药物流出增加的膜P-糖蛋白含量的升高。见(Gupta,R.S.(1985)癌症治疗报导69,515)。这些细胞也对长春花属的生物碱、阿霉素和其他天然产物有抗药性,并且抗药性用钙通道阻断剂如维拉帕米可逆转(Racker,E.,等(1986)癌症治疗报导70,275)。在其他紫杉酚抗药性细胞中发现的抗药机理包括α-或β-微管蛋白亚单位中的突变(Schiber,M.J.,等(1986)细胞生物杂志102,1522)。
试验证据表明紫杉酚的细胞毒性是依赖于给药方案并且高度取决于暴露的时间。这样估计随着输液的延长抗肿瘤作用和骨髓抑制会增强。延长一些天然药剂包括紫杉酚药物的暴露时间也可以部分性地克服与mdr-1基因相关的对多种药物的抗药性。紫杉酚24小时连续暴露比3小时暴露的MCF7细胞抗药性低4.4倍。参见Lai,G.M.等“在人结肠癌细胞系中P-糖蛋白表达和对阿霉素方案依赖性”癌症国际杂志(1991)49:696-703。以体外数据为基础,在抗阿霉素或米托蒽醌乳腺癌患者中完成了96小时方案的I/II期临床试验。参见Wilson,W.H.等“紫杉酚在抗阿霉素或抗米托蒽醌乳腺癌中:96小时输液的I/II期试验”临床肿瘤杂志(1994)12:1621-1629。在试验的II期部分,乳腺癌患者在96小时期间内接受了140mg/m2的紫杉酚(BMS,BristolMeyers Squibb)。在33名患者中的16名(48%)观察到了部分反应而5名(15%)观察到了较小的反应。乳腺癌治疗的其他报导表明96小时输液紫杉酚的方案(BMS)对乳腺癌是有效的。参见Hochhauser,D.等,“在短期紫杉烷输液失败后延长紫杉酚输液的功效:在迁移性乳腺癌中的II期和药理学试验”Br.Ca.Res.and Trt.(1994)32:34。这些研究者以140mg/m2的总剂量通过96小时输液把紫杉酚(BMS)给予25名患有可测得的迁移性乳腺癌患者,其中每个患者中的肿瘤在前面疗程中已经恶化,前面疗程是3小时输液紫杉酚(BMS)或者是1小时输液紫杉酚类化合物(taxotere)。尽管前面临床证明紫杉烷的抗药性,仍有28%的患者达到了客观的肿瘤反应。
现在需要的是治疗卵巢癌患者的方法和组合物。特别是需要治疗用紫杉烷短期输液治疗后肿瘤恶化的患者的方法和组合物。而且没有人能够证实一种治疗卵巢癌的有效方案,该方案能够克服多种药物(mdr)的抗药性问题。
这样,需要一种能够治疗对短期紫杉烷如紫杉酚类化合物或紫杉酚输液方案抗药的卵巢癌的方法和组合物。而且,一种能够对多种药物抗药的卵巢癌的有效输液治疗方案应当是有益的。另外,容易给药的方法和组合物也是必要的。除了输液方法外,一种简单而有效的治疗方法应当是通过口服途径。
还需要的治疗是治疗在用已知化疗剂如以铂为主的化疗法治疗后肿瘤已经恶化的卵巢癌患者的方法和组合物。特别需要的是治疗在用已知化疗剂如以铂为主的化疗法全身治疗后已经肿瘤恶化的和对短期输液(1到24小时)紫杉烷治疗抗药的卵巢癌患者的方法和组合物。
这样,需要能够治疗对已知化疗剂和短期紫杉酚输液方案抗药的卵巢癌的方法和组合物。而且,在治疗多药物拮抗卵巢癌中有效的输液治疗方案应当是有益的。另外,容易给药的方法和组合物也是必要的。一种简单而有效的治疗方法是通过口服途径,特别是,如果口服给药能够提供相似于96小时输液紫杉酚的药物动力学的药物动力学益处。
按照本发明,提供了在治疗卵巢癌上有效的组合物和方法。这些组合物容易通过至少72小时的长期输液计划给药并且能够以安全的剂量给药并提供易控制的副作用。本发明提供了针对这样一些患者治疗卵巢癌的方法和组合物,这些患者在紫杉烷的短期治疗期间肿瘤恶化或没有反应,以前也经过已知的化疗剂治疗如用以铂为主的化疗的患者,以及对短期紫杉烷治疗抗药的患者。令人惊奇的是,本发明显示出了长期输液期间如96小时比短期输液更低的神经病理和一些其他副作用。这样,本发明所预期的较长的输液期避免了对驱前给药的需要,这种驱前给药在其他紫杉酚疗法中是需要的。
本发明包括用长期暴露给紫杉酚来治疗卵巢癌的方法和组合物。这类计划包括至少72小时更优选至少96小时的输液时间。这种长期输液计划可以提高药物如通过P-糖蛋白运送的紫杉酚的活性。这样本发明优选的实施方案是对卵巢癌患者按96小时的输液疗法把紫杉酚给药从而有效地治疗卵巢癌并减少出现mdr(多种药物抗药性)紫杉酚抗药性的机率。
本发明也包括含紫杉酚的卵巢癌治疗组合物。这些卵巢癌治疗组合物是以70mg/m2到200mg/m2的剂量对卵巢癌患者给药的,其中剂量较优选100mg/m2到175mg/m2,最优选140mg/m2。剂量大小取决于紫杉酚对患者的毒性。
所以,本发明的一个目的是提供治疗卵巢癌的方法和组合物。
本发明的另一个目的是提供包括长期输液计划的治疗卵巢癌的方法和组合物。
本发明的又一目的是提供对这样的卵巢癌患者的治疗方法,其中该患者在用化疗方法治疗后肿瘤已经恶化。
本发明的其他目的是提供含有紫杉烷类的治疗卵巢癌的组合物。
本发明又一其他目的是提供治疗这样的卵巢癌患者的疗法,其中该患者抗拒紫杉烷类短期输液疗法的治疗。
本发明的另一目的是提供治疗这样的卵巢癌患者的方法和组合物,其中该患者已经经过至少一种前期化疗方案,包括但不限定为以铂为主的疗法或紫杉烷疗法并且该患者已经肿瘤恶化。
本发明的另外一个目的是提供紫杉酚治疗的方法和组合物,该方法和组合物降低或消除了多种药物对紫杉酚抗药性的发生。
本发明另外一个目的是提供了紫杉酚治疗的方法和组合物,该方法和组合物降低或消除了患者对前驱给药的需要。
在阅读了下列公开实施方案的详细描述和所附的权利要求后,对本发明的这些和其他目的、特征和优点将会更清楚。
本发明包括治疗卵巢癌的组合物和方法。本发明的一个实施方案是通过长期输液计划使用紫杉烷类如紫杉酚或紫杉酚类化合物来治疗顽固性卵巢癌。用本发明治疗患者比短期输液能引起实际较少的神经病变和一些其他的副作用。这样,本发明中所预期的较长期的输液期间避免了短期输液紫杉酚治疗所需要的前驱给药。
本发明还含有对晚期卵巢癌患者的疗法,其中该患者已经用较短期如1到24小时的输液紫杉烷疗法失败过。这里所使用的紫杉烷疗法包括用紫杉酚、紫杉醇(BMS)和紫杉酚类化合物和其他相关化合物的治疗。
本发明优选的实施方案包括以70mg/m2到200mg/m2紫杉烷剂量治疗卵巢癌患者(晚期卵巢癌),剂量优选为100mg/m2至175mg/m2,最佳为140mg/m2,剂量取决于紫杉酚对患者的毒性。用于这类方法的输液计划包括在至少72小时更优选96小时输液。本发明的组合物含有紫杉烷类,优选紫杉酚或紫杉酚类化合物,最优选紫杉酚。在最优选的实施方案中,本发明包括用每21天140mg/m2的紫杉酚剂量以96小时的输液率治疗卵巢癌患者的疗法。
如本文所述,紫杉酚(USAN的通用名称)是5β,20-环氧-1,2α4,7β10β13α-六羟基紫杉-11-烯-9-酮4,10-二乙酸酯(盐)2-苯甲酸酯(盐)12-酯,且带有(2R,3S)-N苯甲酰-3-苯基异丝氨酸。
用本发明治疗的患者包括卵巢癌患者。在本发明的一个实施方案中,这种方法和组合物可用于这样的患者,其中患者以前经过1-24小时输液率的紫杉烷治疗。由本发明也可以预想到已经进行过全身化疗如包括咔铂和顺铂的以铂为主的疗法的卵巢癌患者应当可以用本发明的方法和组合物治疗。而且,也可以预想到本发明具有治疗这种卵巢癌患者的用途,即该患者已经进行过全身化疗和短期紫杉烷输液并且肿瘤已经恶化。
例如,以下描述教导把含有紫杉酚的组合物给药。使用其他紫杉烷类来替代紫杉酚的例子被认为是本发明的一部分。由本发明也可以使用其他医疗设施如容器和输液设备。
以5安瓿(30mg/安瓿)6mg/mL的浓缩无菌溶液来提供紫杉酚BNP(Baker Norton药品)。每mL的无菌溶液包括527mg聚氧乙基化的蓖麻油(CremophorEL)和49.7%(w/v)无水乙醇BP。在临床使用前必须稀释安瓿的内容物。任何打开的安瓿未用部分应当按照OSHA批准的要求存放。
小瓶应当在室温(大约25℃)或者冰箱(2-8℃)下存放。每种准备输液的紫杉酚溶液(紫杉酚输液)应当在配制后24小时内给药。紫杉酚输液可有一种轻微的烟雾,这与药物的浓度和配制后时间的推移成正比。当配制时,紫杉酚输液(0.3-1.2mg/mL紫杉酚)在室温(大约25℃)和正常光照条件下直到48小时是稳定的。在配制紫杉酚输液后已经观察到了在紫杉酚输液中少量纤维的制剂(在由USP颗粒物质试验对LVP所建立的可接受的范围内)。尽管颗粒的形成不表明药物功效的丧失,但应当不使用显示过多颗粒物形成的溶液。过滤是必要的并且通过在输液泵远端的液体通道中加入孔径不大于0.22微米的疏水微孔滤器(IVEX-HP在管线内的滤器Set-SL,15”,Abbott型号#4525或相当)完成。
由于紫杉酚可以从聚氯乙烯(PVC)袋和静脉管中滤去二乙基己基邻苯二甲酸酯(DEHP)增塑剂,所以紫杉酚必须在非塑化溶液容器(如玻璃、聚烯烃或聚丙烯)中制备。紫杉酚不必通过PVC静脉装置给药。所以,聚烯烃-或聚乙烯-线状装置如IV硝化甘油装置(或等同物)应当用来连接紫杉酚输液容器到IV泵,然后把0.22微米的滤器接到IV泵上,再直接安装到患者中心进入装置上。如果需要的话,可以使用聚烯烃线材的延伸装置(PolyfinTM-延伸装置,MiniMed技术,型号#126)来提供IV泵和患者中心进入装置之间的其他距离。
为了实施本发明,在玻璃、聚烯烃或聚丙烯容器中,用250或500mL的5%右旋糖注射液USP或0.996氯化钠注射液USP稀释每日紫杉酚的总剂量(即24小时所提供的)来制备最终的紫杉酚输液。每份紫杉酚输液将在24小时内通过一台输液控制装置输液。一共四份紫杉酚输液需要96小时输液。每份紫杉酚输液应当在使用前马上制备这样从制备时间直到每袋/瓶输液完将不超过25小时。聚烯烃-或聚乙烯-线材装置应当用来连接袋/瓶与IV泵,接着把在线滤器直接安装到患者的中心进入装置上。
本发明的一个实施方案包括以96小时连续静脉输液把紫杉酚输液给药。该紫杉酚输液通过每21天重复循环的恒定中心静脉导管输送。优选患者有恒定或暂时的中心静脉进入装置以较容易地给药。另一方面,相似于96小时输液治疗提供的药效益处的紫杉烷类的口服给药可不需要患者有一台中心静脉进入装置而给药。
本发明的一个优选实施方案是卵巢癌患者的治疗方法,该患者在以前的治疗后疾病恶化。这类以前的治疗包括但不限定为紫杉烷的短期输液、以铂为主的治疗如咔铂或顺铂或者其他全身化疗法。本发明最优选的实施方案是卵巢癌患者的治疗方法,该患者在用紫杉酚短期(1-24小时)输液治疗后疾病恶化。
本发明将通过下列实施例作进一步的详细说明,这些实施例不是以任何方式对本发明范围的限定。相反,应当清楚地理解为它是对各种其他实施方案、改良和等同替换的必要帮助,在阅读完说明书后,可以对本领域普通技术人员暗示它们没有超出本发明的构思和/或所附权利要求书的范围。实施例1
在试验中选用可诊断出患有卵巢癌的妇女,并且该患者肿瘤已经恶化或者在早期所进行的短期(1-24小时)输液紫杉烷治疗没有反应。如果患者完成至少两个紫杉酚BNP循环,那么该患者是可以评估的。把抗紫杉烷卵巢癌患者定义为具有(1)当接受紫杉烷治疗时疾病恶化;(2)在12个月的紫杉烷治疗期间恶化;(3)在六个紫杉烷治疗程后保持稳定的疾病;或者(4)保持稳定的疾病伴随至少CA-125增加的测量(到数值≥100)。
在96小时输液治疗计划中,不需要抗过敏的驱前给药。令人惊奇的是本发明已经显示出较长的输液周期如96小时比较短期的输液实际较低的神经病变和一些其他副作用。这样,本发明中所预期的较长输液期避免了其他紫杉酚疗法所需要的驱前给药的要求。没有肝功能异常(正常的碱性磷酸酶、葡糖苷酸转移酶、SGOT)或者没有肝转移瘤的患者接受35mg/m2/天×4天(在96小时输液总共140mg/m2的剂量)剂量的紫杉酚BNP。没有肝转移瘤并且具有轻度肝功能异常(SGOT和碱性磷酸酶低于正常上限1.5倍)和正常葡糖苷酸转移酶的患者接受140mg/m2的总剂量。具有中度和严重肝功能异常(SGOT和碱性磷酸酶高于正常上限1.5倍)的患者接受26.25mg/m2/天(在96小时输液总共105mg/m2的剂量)剂量的紫杉酚BNP。
每隔21天重复紫杉酚BNP的循环。如果血液回复和其他所有的毒性(除秃发外)已经返回到1级,接着进行这种治疗计划。
在5mL小瓶(30mg/瓶)可以得到6mg/mL的紫杉酚BNP浓缩溶液。每mL无菌溶液含有527mg聚氧乙基化的蓖麻油(CremaphorEL)和49.7%(w/v)无水乙醇。然后把该紫杉酚用500mL5%的右旋糖稀释成在24小时期间内足以在每平方米(以患者体表面积为准)提供35mg紫杉酚的浓度。在每24小时输液开始前30到60分钟内制备紫杉酚输液。在体积为500mL的聚丙烯线性半固体的容器中配制紫杉酚输液。
通过聚乙烯导管把含紫杉酚输液的容器连接到IV泵上。然后通过聚乙烯线性导管把孔径为0.22微米的IVEX-HP管线滤器装置SL,15”,Abbott型号#4525安装在IV泵上。接着把管线滤器连接到目标物中心进入装置上。
在24小时内输液该紫杉酚溶液并通过输液装置控制。把该步骤重复三次以上从而达到总共96小时的连续输液。最终的剂量为140mg/m2/96小时。
使用下列标准测定上述紫杉酚疗法的反应:
完全反应(CR):经过下一个评估期6周后所存在的疾病临床症状全部消失。所评估的损伤必须完全消失。
部分反应(PR):经过下一个评估期6周后,所存在的全部可诊断的损伤的总产物直径至少降低50%。另外,伴发的可评估的损伤必须没有出现疾病恶化或新损伤的迹象。
稳定的疾病(SD):反应既不合格而疾病又没有恶化的患者。
进行性疾病(PD):在一个化疗循环或一直有转移瘤疾病的任何新损伤的出现内,任何可诊断的损伤直径的总产物恶化25%或更多。
14名以前曾进行过短期紫杉烷治疗但疾病仍在恶化的卵巢癌患者,经过了上述96小时紫杉酚输液治疗。14名中,3名有部分反应,占21%的反应率。一名患者病情稳定。这样,总反应率为29%。
当然,应当理解为上文只与本发明优选实施方案相关并且在不脱离本发明构思和超出所附的权利要求书前提下可以作许多改进或变化。
Claims (7)
1.治疗卵巢癌患者的方法,它包括在96小时期间以连续给药方式给卵巢癌患者静脉输入含有紫杉烷的组合物。
2.根据权利要求1的方法,其中的紫杉烷为紫杉酚。
3.根据权利要求1的方法,其中连续给药剂量包括每96小时100到140mg/m2。
4.根据权利要求1的方法,其中卵巢癌患者已经过了1-24小时输液的紫杉烷治疗。
5.根据权利要求1的方法,其中卵巢癌患者已经过了以铂为主的化疗。
6.根据权利要求1的方法,其中卵巢癌患者已经过了以铂为主的化疗和1-24小时输液的紫杉烷治疗。
7.根据权利要求1的方法,其中每隔21天重复该输液步骤。
