背景技术

发明内容

本公开涉及包含黄腐酚、异黄腐酚、6-异戊烯基柚皮素、8-异戊烯基柚皮素的组合物以及其在治疗癌症中的用途。在一些方面,所述组合物可用于治疗白血病和实体瘤。在其它方面,所述组合物可用于治疗耐药性癌症。

文献中描述的用于从植物材料中提取和纯化黄腐酚、异黄腐酚、6-异戊烯基柚皮素和8-异戊烯基柚皮素的方法具有许多缺点。例如,需要大量有机溶剂并且特别地是高毒性溶剂(二氯甲烷、己烷、氯仿)来进行有效萃取。目前的方法还采用大量昂贵的材料,例如硅胶。所述方法使黄腐酚不可逆地分解为副产物(主要是同分异构异黄腐酚)和聚合氧化降解产物(用强碱(例如,NaOH、KOH)的水溶液萃取)。逆流色谱技术具有低效率和工艺可扩展性。最重要的是,几乎不可能获得具有高药物纯度(最低95wt%)的黄腐酚。然而,本公开提供了一种用于制备HPLC纯度>95%的黄腐酚的方法。
通过化学合成获得黄腐酚和其它物质的已知方法是低效的,并且不是非常可扩展的。此外,废弃物排放(littering)的最后阶段不允许经济可行地制备具有高药物纯度的大量黄腐酚。
在一些方面,本公开提供了一种用于由植物性药材制备黄腐酚的方法。在一些方面,所述植物性药材是啤酒花。一种或多种啤酒花(蛇麻草(Humulus lupulus))是一种属于大麻科(family Cannabaceae)荨麻目(order Urticacales)葎草属(genus Humulus)的攀藤。更早的分类学包括桑科(mulberry family/Moraceae)葎草属。
啤酒花是原产于北半球的雌雄异株多年生植物。啤酒花在灌木林中和在森林的边缘处(能接触足够的水)被发现,并且其高度可达到至多7m至8m(23英尺至26英尺)。许多雌花形成称为球果的花序,其由附着于之字形毛轴(hairy axis)的膜状托叶和苞片组成。啤酒花的苞片和托叶含有多酚;药物的气味和味道主要是由于蛇麻素腺中含有的非常复杂的分泌物产生的。
在收获之后,出于稳定性原因,将花序立即干燥至约10%的含水量。此外,根据环境条件,啤酒花在从收获到最终产品的一些或所有步骤期间保持在恒定的冷藏下。已知苦味素在储存期间迅速分解,并且除非冷藏,否则其浓度仅在6个月内降低50%至70%。
25%的采集的啤酒花球果用乙醇或超临界二氧化碳提取以获得尽可能多的α酸。由于乙醇和二氧化碳在酿造过程期间是天然存在的,因此这些溶剂的使用是无关紧要的。
从巨大的生物质生产开始,花序(球果)是所使用的啤酒花植物的唯一部分。除了一些在沙拉中食用的幼芽的用途之外,对于茎、叶、根茎和根还没有人类用途。地上(空中)部分被制成堆肥,并且用于田地施肥。然而,使用本发明的方法,黄腐酚和其它分子以生物学活性和安全的比率从啤酒花、从其它过程如啤酒生产(啤酒花残渣)产生的啤酒花废物或任何含啤酒花的产品中纯化。
在某些方面,本公开的组合物被配制成以治疗有效量施用。术语“有效量”、“药学有效量”和“治疗有效量”是指可以有效引起期望的生物学或医学反应的量,包括当施用于受试者以治疗疾病时足以影响所述疾病的此类治疗的化合物的量。有效量将根据化合物、疾病和其严重程度以及要治疗的受试者的年龄、体重等而变化。有效量可以包括一定范围的量。进一步地,有效量包括当与其它药剂组合时有效的药剂的量。

使用方法
异戊烯基化查耳酮和类黄酮不仅在营养方面而且在疾病预防方面都受到越来越多的关注,这是因为其在人类体内具有生物学和分子活性。在本公开的一方面,本文所述的组合物可用于治疗/预防多种疾病和病状,包括癌症,特别是白血病,如TKI耐药性慢性髓系白血病(CML)。
CML是与分子改变相关的骨髓增生性肿瘤,所述分子改变即融合基因BCR-ABL1,其编码酪氨酸激酶癌蛋白BCR-ABL1。这导致TKI的发展,其中伊马替尼是第一个被批准用于治疗用途的TKI。尽管绝大多数CML患者对伊马替尼有响应,但其对该靶向疗法的耐药性导致治疗失败和复发。
对靶向疗法的耐药性是一个复杂且多因素的过程,所述过程在选择具有逃避治疗能力的癌症克隆时达到顶点。在CML中,TKI耐药性机制通常被细分为BCR-ABL1依赖性和非依赖性机制(Morozova E.V.等人《生物标志物观察(Biomark.Insights.)》2015;10:43–47)。然而,在治疗指南中,仅考虑BCR-ABL1相关机制用于剂量调整或TKI切换(BaccaraniM.等人2013.《血液(Blood.)》2013;122:872–884)。基于BCR-ABL1蛋白抑制的白血病干细胞(LSC)和LSC样表型的持续存在也已报道为主要TKI耐药性机制(Baykal-S.等人《公共科学图书馆·综合(PLoS ONE.)》2020;15:e0229104)。
虽然不束缚于机制,但是在一些方面,本公开的组合物可用于激动法尼醇X受体。所述法尼醇X受体(FXR)属于称为核激素受体的受体家族。FXR是广泛涉及肿瘤发生的胆汁酸激活的核受体。

在一些方面,本公开的组合物可用于抑制细胞中的NFκB活性。核因子κB(NF-κB)是一种古老的蛋白质转录因子(Salminen,A.,Huuskonen等人(2008).《衰老研究评论(AgeingRes.Rev.)》7,83–105)并且被认为是先天免疫的调节剂(Baltimore,D.(2009).《冷泉港生物学观点(Cold Spring Harb.Perspect.Biol.)》1:a000026.)。NFκB也是涉及癌症的发病机理和治疗的重要信号传导通路(Xia,L.等人《肿瘤靶向疗法(Onco Targets Ther.)》(2018)11:2063-2073)。
在一些方面,本公开的组合物可用于调节核因子-红细胞2(NF-E2)相关因子2(Nrf2)的表达/活性。Nrf2/Keap1通路是负责抵抗由外源性化学品诱导的氧化损伤的重要信号传导级联。其维持氧化还原稳态,通过调节其多个下游细胞保护性基因来发挥抗炎和抗癌活性,由此在细胞存活中发挥至关重要的作用。有趣的是,近年来,积累的证据表明Nrf2在癌症中具有矛盾的作用。Nrf2的异常激活与预后不良相关。Nrf2在各种癌症中的组成型激活诱导促存活基因,并且通过代谢性重编程、抑制癌细胞凋亡和增强癌症干细胞的自我更新能力来促进癌细胞增殖。更重要的是,Nrf2被证明有助于癌细胞的化疗耐药性和辐射耐药性以及炎症诱导的致癌作用。
在一些方面,将本公开的组合物与其它治疗剂组合施用。在一方面,将本公开的组合物与一种或多种酪氨酸激酶抑制剂组合施用。酪氨酸激酶抑制剂(TKI)是一组通过若干种抑制模式破坏蛋白激酶的信号转导通路的药理学药剂。蛋白激酶的突变、失调和过表达涉及许多疾病过程。在每40个人类基因中就有约1个人类基因编码蛋白激酶,并且那些基因中的几乎一半定位于疾病基因座或癌症扩增子。对蛋白激酶抑制剂的兴趣始于FDA在2001年批准酪氨酸激酶抑制剂(TKI)伊马替尼。伊马替尼是被设计成靶向BCR-Abl杂合蛋白(一种在费城染色体阳性慢性髓系白血病患者体内产生的酪氨酸激酶信号传导蛋白)的口服化疗药物。
总之,酪氨酸激酶磷酸化底物酶上的特定氨基酸,其随后改变信号转导,从而导致细胞生物学发生下游变化。由TK引发的下游信号转导可以改变细胞生长、迁移、分化、细胞凋亡和死亡。通过突变或其它方式引起的组成型激活或抑制可以导致信号级联失调,从而潜在地导致恶性肿瘤和其它病理。因此,通过TKI阻断这些初始信号可以防止突变的或功能失调的TK的异常作用。
激酶抑制剂是不可逆的或可逆的。不可逆激酶抑制剂倾向于共价结合并阻断ATP位点,从而导致不可逆抑制。基于结合口袋以及DFG基序的确认,可逆激酶抑制剂可以进一步被细分成四种主要亚型。

限制TKI疗法的有效性的不可避免的障碍是耐药性问题——当今对长期疾病控制的普遍挑战。癌症本质上是进化的缩影。其存活由受TKI疗法的选择压力影响的遗传多样性和突变的纵向积累驱动。这些基本但复杂的原理构成TKI耐药性的难治性质的基础,所述TKI耐药性传统上被分类为原发性(固有)或继发性(获得性)。在原发性耐药性中,患者对靶向疗法没有任何治疗反应。在继发性耐药性中,患者最初实现一些临床益处,随后是疾病进展。随着每种致癌驱动因子和靶向抑制剂的发现,数量和多样性不断增长的抗性机制正在被定义。
在本公开的一方面,将本文所述的组合物单独地或与TKI组合地施用于患有原发性或继发性TKI耐药性的受试者。在一方面,将本公开的组合物作为一线疗法施用,以避免继发性TKI耐药性的发展。在一些方面,将本公开的组合物与一种或多种TKI同时或以任何顺序组合施用。

“合成的大麻素”是具有大麻素或大麻素样结构的化合物,并且使用化学方式而非植物制造。
植物大麻素可以以中性(脱羧形式)或羧酸形式获得,这取决于用于提取大麻素的方法。例如,已知加热羧酸形式将使大部分羧酸形式脱羧成中性形式。
实施例
生物活性原料药的纯化
输入材料
黄腐酚含量为0.3wt.%至1.0wt.%的绿灰色粉尘和颗粒(生产废物,“剧增(leaps)”)。原料含有大量的非极性和极性杂质,所述非极性和极性杂质在经典的提取方法(水提取/水不混溶的有机溶剂)中难以去除。
萃取程序
将植物材料悬浮于正庚烷中并在20℃至30℃下进行连续萃取过程,以去除非极性杂质。将萃取温度增加至50℃至60℃(在该温度下植物原料的一些组分熔化),并且重复该过程。使庚烷蒸发,并且将黄色残余物通过棉质滤器在压力下过滤。将干燥残余物转移到萃取反应器中,添加乙酸乙酯,并且类似于正庚烷萃取重复该过程,不同的是现在在旋转蒸发器上将半成品收集在烧瓶中。在该过程结束时,微黄色沉淀物中的黄腐酚含量为约10重量%至12重量%。
将沉淀物溶解于适当量的有机溶剂和液态烷烃混合物中,以获得浓度为约0.05M的黄腐酚。用0.2M有机酸溶液、0.05M ZnCl2、5% NaHCO3和盐水使所得深色溶液经受萃取过程。在用无水Na2SO4干燥有机层并使有机溶剂蒸发之后,获得黄腐酚含量为20%至25%的固体混合物。然后,将混合物溶解于足够的乙酸乙酯中以获得浓度为0.05M至0.1M的黄腐酚,然后使溶液经受过滤并且使用液-液分离器以流动方式进行碱-酸萃取。简言之,黄腐酚的乙酸乙酯溶液用0.05M至0.1M NaOH溶液萃取,丢弃有机层,将水层中的黄腐酚(以钠盐形式)添加至0.05M-0.1M有机酸溶液中,这引起Xn沉淀,将所述Xn沉淀萃取到乙酸乙酯中并用无水Na2SO4干燥。将乙酸乙酯和盐水添加至残余物中。在过滤掉无机盐之后,蒸发掉大部分乙酸乙酯,并且添加计算量的正庚烷以引发黄腐酚从溶剂混合物中沉淀出来。继续蒸馏过程,直到所有黄腐酚沉淀。添加醇(以去除痕量乙酸乙酯),并且继续蒸发过程。将沉淀过滤掉,用正庚烷洗涤并真空干燥,以得到呈浅黄色粉末的最终产物(HPLC纯度>95.0%)。
原料药表征
使用以上过程制备并分析含有黄腐酚、异黄腐酚、6-异戊烯基柚皮素、8-异戊烯基柚皮素的三批原料药(XN-54、XN-63-10和XN-87)。
粒径分布(PSD)
通过激光衍射使用马尔文分析仪(Malvern analyzer)的Mastersizer 2000进行PSD测试。表1呈现了三个测试批次的平均值(以μm计):XN-54(样品在分析之前在研钵中研磨)、XN-63-10和XN-87。表2总结了批次XN-63和其微粉化衍生物的结果。示出了XN-54(图2A)、XN-63-10(图2B)和XN-87(图2C)的粒径分布。

差示扫描量热法(DSC)
在TA仪器差示扫描量热法(DSC)(DSC Q20 V24.10型122结构(Build))上进行差示扫描量热法分析。两个DS批次的DSC分析显示两种不同的熔点结果。DS批次XN-54的熔点为153.22℃(图3A)并且DS批次XN-87的熔点为171.86℃(图3B)。
热重分析(TGA)
在TA仪器热重分析仪(TGA)(Q50 V20.13型39结构)上进行热重分析。图4A中示出了原料药批次XN-54的TGA,并且图4B中示出了原料药批次XN-87的TGA。
傅里叶变换红外分析(FTIR)
在以下FTIR分光光度计上进行傅里叶变换红外分析:岛津(Shimadzu),IRTracer-100型(DS,批次XN-54)和JASCO FT/IR-6200型(DS,批次XN-63-10)。图5A和5B分别呈现了DS批次XN-54和XN-87的FTIR分析的结果。
X射线粉末衍射(XRPD)
X射线粉末衍射分析表明原料药的结晶性质并且对于两个不同批次显示出两种不同的结晶形式。图6A中示出了XN-54的衍射图,并且图6B中示出了XN-87的衍射图。
原料药溶解度
在5种介质中进行原料药溶解度测试:水、0.1N盐酸、乙酸盐缓冲液pH=4.5、磷酸盐缓冲液pH=6.8以及另外在人工唾液中。还研究了添加各种表面活性剂对材料溶解度的影响。在室温下使用磁力搅拌器(不包括初步研究,其中使用定轨振荡器)对每种介质中的每种材料的2个样品进行测试。根据欧洲药典(European Pharmacopeia)10.5,5.17.1制备pH=4.5和pH=6.8的缓冲液。根据以下出版物制备人工唾液:人工唾液及其在生物学实验(Artificial saliva and its use in biological experiments)J.Pytko-Polonczyk1,A.Jakubik,A.Przeklasa-Bierowiec,B.Muszynska,《生理学与药理学杂志(Journal ofPhysiology and Pharmacology)》2017,68,6,807-813中的用途。通过HPLC分析溶液中原料药的含量。

使用方法
使用K562细胞系模型测试原料药对慢性髓系白血病(CML)细胞的影响。使用了三种细胞系:K562-IR(伊马替尼耐药性)和K562-IS(伊马替尼敏感)购自美国典型培养物保藏中心(American Type Culture Collection,ATCC);并且K562-DR1000 nM(伊马替尼和达沙替尼耐药性)是通过用TKI连续治疗和克隆选择来开发的。通过α微管蛋白/β微管蛋白和GAPDH的蛋白质印迹法来证实基础蛋白表达(数据未显示)。K562-DR1000 nM中的pCrkl水平降低,而达沙替尼处理的细胞之间的BCR-ABL水平类似(分别为图7A和7B),这表明双重耐药性是独立于BCR-ABL的。与未处理的对照相比,用1000nM伊马替尼处理K562-IS细胞没有显著改变pCrkl水平(数据未显示)。
为了测量XN-54(化合物1)对K562细胞系的影响,根据制造商的说明书,使用CellTiter-Glo 2.0系统(普洛麦格公司(Promega))进行活力实验。简言之,将培养基中含有K562细胞的不透明壁多孔板与XN-54在室温下温育所需的时间。将与细胞培养基的体积相等体积的CellTiter-Glo试剂添加到每个孔中。在稳定发光信号之后测量亮度。
在96孔板形式中,在200μl专用于每个细胞系的培养基中进行细胞培养。将细胞以以下密度铺板:5x105/ml、2.50x105/ml、1.25x105/ml,持续24、48或72小时。图8中示出了响应于增加浓度的XN-54或达沙替尼(对照)在48小时和72小时时间点两者的活力。如图8所示,XN-54(化合物1)在K562伊马替尼敏感细胞和K562伊马替尼耐药性细胞两者中均显示出显著活性。为了确定XN-54影响活力的机制,使用早期标志物(膜联蛋白V)和晚期标志物(膜联蛋白V/碘化丙啶)(BD Pharmingen公司(BD Pharmingen),目录号556547)两者分析细胞的细胞凋亡。如图9和10所示,XN-54使用早期细胞凋亡(图9)和晚期细胞凋亡(图10)标志物两者在所有细胞系中诱导细胞凋亡。
组合疗法
还分析了XN-54与TKI的组合对K562细胞的影响。在K562-IR细胞中,响应于XN-54和帕纳替尼的剂量-反应组合测量百分比活力。如图11所示,基于log 10(M)XN-54(图11A)或帕纳替尼(图11B),XN-54和帕纳替尼的组合对细胞活力具有显著影响。在使用XN-54与尼罗替尼的组合的K562-IS细胞中观察到类似的结果(图12)。
使用上述细胞活力方法测定XN-54(化合物1)和大麻二醇(化合物2)的作用。如图13所示,XN-54和CBD的组合显著地影响K562-DR(图13A)、K562-IS(图13B)和K562-IR细胞(图13C)中的细胞活力。
Claims (41)
1.一种药物组合物,所述组合物包含治疗有效量的黄腐酚和异黄腐酚以及至少一种药学上可接受的载体。
2.根据权利要求1所述的组合物,所述组合物进一步包含6-异戊烯基柚皮素和8-异戊烯基柚皮素。
3.根据权利要求1或2所述的组合物,其中所述黄腐酚以介于至少50%w/w至99%w/w之间的量存在,所述异黄腐酚以介于1%w/w至15%w/w之间的量存在,6-异戊烯基柚皮素以介于0.0%至5%之间的量存在,并且8-异戊烯基柚皮素以介于0.0%至5%之间的量存在。
4.根据权利要求1至3中任一项所述的组合物,其中所述黄腐酚、所述异黄腐酚、所述6-异戊烯基柚皮素或所述8-异戊烯基柚皮素中的至少一种是从植物性药材中提取的。
5.根据权利要求4所述的组合物,其中所述植物性药材是啤酒花、啤酒花残渣或任何含啤酒花的产品。
6.根据权利要求1至3中任一项所述的组合物,其中黄腐酚、异黄腐酚、6-异戊烯基柚皮素、8-异戊烯基柚皮素中的至少一种是化学合成的。
7.根据权利要求1至6中任一项所述的组合物,其中所述药学上可接受的载体是粘合剂、崩解剂、表面活性剂或润滑剂中的至少一种。
8.根据权利要求7所述的组合物,其中所述粘合剂是淀粉1500、聚乙烯吡咯烷酮和微晶纤维素中的一种或多种。
9.根据权利要求8所述的组合物,其中所述粘合剂以介于10%w/w至30%w/w之间的量存在。
10.根据权利要求7所述的组合物,其中所述崩解剂是交联羧甲基纤维素钠。
11.根据权利要求10所述的组合物,其中所述崩解剂以介于1%w/w至5%w/w之间的量存在。
12.根据权利要求7所述的组合物,其中所述表面活性剂是十二烷基硫酸钠。
13.根据权利要求12所述的组合物,其中所述表面活性剂以介于1%w/w至5%w/w之间的量存在。
14.根据权利要求7所述的组合物,其中所述润滑剂是硬脂酸镁。
15.根据权利要求14所述的组合物,其中所述润滑剂以介于0.5%w/w至3%w/w之间的量存在。
16.一种由啤酒花制备包含黄腐酚和异黄腐酚的组合物的方法,所述方法包括:
-将啤酒花植物材料悬浮于正庚烷中以去除非极性杂质;
-使庚烷蒸发并过滤残余物;
-用乙酸乙酯萃取残余物并在旋转蒸发器中收集含黄腐酚的级分;
-用含有0.05M ZnCl2、5%NaHCO3和盐水的有机酸溶液萃取所述黄腐酚;
-用无水Na2SO4干燥有机层;以及
-从含有乙酸乙酯的混合物中沉淀出所述黄腐酚;以及
-干燥所沉淀的黄腐酚和异黄腐酚。
17.一种组合物,所述组合物包含通过根据权利要求16所述的方法获得的所述黄腐酚和所述异黄腐酚。
18.一种激动有需要的受试者体内的法尼醇X受体活性的方法,所述方法包括向所述受试者施用治疗有效量的根据权利要求1至15和17中任一项所述的组合物。
19.根据权利要求18所述的方法,其中激动法尼醇X受体活性使癌症得以治疗。
20.根据权利要求19所述的方法,其中所述癌症是酪氨酸激酶抑制剂耐药性的。
21.一种抑制有需要的受试者的NFκB活性的方法,所述方法包括向所述受试者施用治疗有效量的根据权利要求1至15和17中任一项所述的组合物。
22.根据权利要求21所述的方法,其中抑制NFκB活性使得癌症得以治疗。
23.根据权利要求22所述的方法,其中所述癌症是酪氨酸激酶抑制剂耐药性的。
24.一种增加有需要的受试者的核因子-红细胞2相关因子2(NRF2)的表达和/或激活的方法,所述方法包括向所述受试者施用治疗有效量的根据权利要求1至15和17中任一项所述的组合物。
25.根据权利要求24所述的方法,其中增加NRF2的表达和/或激活使得癌症得以治疗。

28.根据权利要求27所述的方法,其中所述细胞是白血病细胞。
29.根据权利要求27或28所述的方法,其中所述细胞含有bcr-abl基因突变。
30.根据权利要求27至29中任一项所述的方法,其中所述细胞位于患有慢性髓系白血病(CML)、急性淋巴细胞性白血病(ALL)或急性髓系白血病(AML)的患者体内。
31.一种治疗受试者的慢性髓系白血病(CML)的方法,所述方法包括向所述受试者施用治疗有效量的根据权利要求1至15和17中任一项所述的组合物。
32.根据权利要求31所述的方法,其中所述CML对使用酪氨酸激酶抑制剂(TKI)的疗法具有耐药性。
33.根据权利要求32所述的方法,其中所述TKI是伊马替尼(imatinib)、达沙替尼(dasatinib)或帕纳替尼(ponatinib)。

36.根据权利要求34所述的方法,其中所述BCR-ABL非依赖性耐药性是由CRKL和STAT5磷酸化的抑制引起的,或者翻译调节子核糖体蛋白S6(RPS6)的持续磷酸化所指示的mTOR复合体1(mTORC1)的激活。
37.根据权利要求18至36中任一项所述的方法,所述方法进一步包括向所述受试者施用TKI。
38.根据权利要求37所述的方法,其中所述TKI是在所述组合物之后施用的。
39.根据权利要求37或38所述的方法,其中所述TKI是伊马替尼、达沙替尼、帕纳替尼或尼罗替尼(nilotinib)。
