技术领域
本发明属于生物检测技术领域,具体涉及DPP4作为NK/T细胞淋巴瘤肿瘤标志物的应用。
技术背景
NK/T细胞淋巴瘤是一类发病率较低但恶性进展迅猛的非霍奇金淋巴瘤,其发病与EB病毒的感染呈强相关,且绝大部分患者病灶表现为侵犯鼻腔或鼻咽。临床上,NK/T细胞淋巴瘤的肿瘤细胞中较高的P-糖蛋白的表达会导致其对传统的包含蒽环类药物的淋巴瘤化疗方案(如CHOP方案)具有抵抗特性,因而需采用纳入了非蒽环类药物(左旋门冬酶或培门冬酶)的多药联合和同步/序贯放疗的治疗方案,这些发现与改进使得NK/T细胞淋巴瘤的临床治疗疗效有了较大提升,早期患者的5年总生存率能达到70%甚至更高。尽管如此,晚期、复发和难治性的NK/T细胞淋巴瘤患者占总比约为30%,他们的预后依旧较差,治疗方案更加具有局限性和挑战性。近年来随着癌症治疗技术水平的不断提高,新型的靶向和免疫治疗方案(例如免疫检查点抑制剂和组蛋白去乙酰化酶抑制剂)也在NK/T细胞淋巴瘤患者的治疗中得以尝试并取得了一定的成效。然而新型治疗方案的疗效并不稳定,治疗响应的程度在患者间存在了较大差异。因而,这提示了NK/T细胞淋巴瘤患者所具有的高度异质性,并且亟需针对该异质性而寻找新的治疗靶点和设计新的精准化治疗策略。
NK/T细胞淋巴瘤的异质性主要包含了以下层面:首先,从肿瘤细胞的来源类型而言,NK/T细胞淋巴瘤分为NK细胞起源的和T细胞起源的不同类别,T细胞来源的患者只占很小的一部分,而从正常淋巴细胞向NK/T细胞淋巴瘤肿瘤细胞的转化过程中会形成从小到大的不同细胞形态;在遗传背景的层面上,我们之前的研究发现了人群中存在位于HLA-II和IL18RAP等基因处的遗传位点序列的改变,这些遗传位点的改变引起了人的对NK/T细胞淋巴瘤的易感性,因而可从遗传背景上将人群分为较易感群体和非易感群体;在分子层面,国内外不同团队通过高通量测序的手段,对DNA和RNA的测序数据进行分析,发现了NK/T细胞淋巴瘤所具有的特异的体细胞突变和转录失调,进而可依据这些信息对患者进行分子分型、并设计相应的治疗策略。上述研究从不同的层面揭示了NK/T细胞淋巴瘤患者中存在的高度异质性,然而这些发现仍不足以有效解释NK/T细胞淋巴瘤的致病机制和肿瘤进展机理。
除了肿瘤细胞之外,肿瘤组织中还存在有大量浸润的免疫细胞,它们与肿瘤细胞一同构成了异质性特征的肿瘤微环境结构。近年来,随着单细胞转录组测序技术的普及,大量研究探索了多种癌症的肿瘤微环境中的异质的免疫细胞和基质细胞组分,并结合其他手段揭示了这些免疫微环境组分与肿瘤发生发展及患者治疗响应之间的联系。以往的研究曾报道了NK/T细胞淋巴瘤组织中也存在有大量免疫细胞的浸润,然而这些免疫微环境组分的具体构成以及它们与NK/T细胞淋巴瘤发生发展的联系仍然是未知的。因此,亟需从肿瘤微环境的角度出发,找到与肿瘤发生发展密切相关的重要分子、关键细胞群和细胞相互作用对,从而能够以此作为NK/T细胞淋巴瘤的新型有效的肿瘤标志物以及治疗靶点,并最终提高患者的生存率。
发明内容
针对现有技术的缺陷,本发明的目的是提供一种新的NK/T细胞淋巴瘤的肿瘤标志物及其应用,以解决上述背景技术中提出的问题。
本发明提供了DPP4(二肽基肽酶4)作为NK/T细胞淋巴瘤肿瘤标志物的应用。
另一方面,本发明还提供了DPP4检测试剂在制备用于筛查NK/T细胞淋巴瘤的试剂盒中的应用。
优选地,所述试剂盒包括如SEQ ID NO:1和SEQ ID NO:2所示的核苷酸序列。
另一方面,本发明还提供了DPP4抑制剂在制备用于治疗NK/T细胞淋巴瘤的药物中的应用。
优选地,所述DPP4抑制剂包括利格列汀、阿格列汀、安奈格列汀、贝戈洛单抗、吉格列汀、奥格列汀、沙格列汀、西格列汀、曲格列汀、维格列汀中的一种或多种。
对DPP4在NK/T细胞淋巴瘤患者外周血及肿瘤组织中各细胞类型中的表达水平采用单细胞转录组测序的方法进行检测,发现DPP4在肿瘤细胞中显著高表达。对DPP4在NK/T细胞淋巴瘤肿瘤细胞中的表达可通过对肿瘤组织石蜡包埋蜡块切片采用免疫荧光共染的方法进行检测,发现DPP4在肿瘤细胞中广泛且大量的表达。本发明还发现,NK/T细胞淋巴瘤肿瘤细胞表达的DPP4可有效降解CXCL2、CXCL9和CXCL10等趋化因子,且肿瘤组织中的DPP4的表达水平与CXCL2、CXCL9和CXCL10趋化因子的蛋白含量呈负相关,进一步功能实验证实了肿瘤细胞表达的DPP4阻碍了正常NK细胞的趋化,可用于临床上对NK/T细胞淋巴瘤患者的精确分型和精准治疗。此外,DPP4对正常NK细胞趋化的阻碍效果可受到DPP4抑制剂的抑制,因此DPP4可以作为NK/T细胞淋巴瘤治疗的重要靶点,可应用DPP4抑制剂来治疗NK/T细胞淋巴瘤患者。DPP4作为NK/T细胞淋巴瘤的肿瘤标志物,可推广并应用于临床上NK/T细胞淋巴瘤患者的筛查(初筛)。

图1示出了NK/T细胞淋巴瘤单细胞转录组测序数据中DPP4在各类细胞中的表达水平和表达比例。

图6示出了由NK/T细胞淋巴瘤肿瘤细胞来源的细胞上清液与正常NK细胞的体外趋化实验所得的不同处理组中的NK细胞的迁移率。A:空白对照、趋化因子、DPP4蛋白、DPP4抑制剂、趋化因子+DPP4蛋白、趋化因子+DPP4蛋白+DPP4抑制剂;B:细胞上清液(YT细胞)、细胞上清液(YT细胞)+趋化因子、细胞上清液(YT细胞)+趋化因子+DPP4抑制剂、细胞上清液(NK-92细胞)、细胞上清液(NK-92细胞)+趋化因子、细胞上清液(NK-92细胞)+趋化因子+DPP4抑制剂。
图7示出了多色免疫荧光共染实验结果。A:DPP4高表达;B:DPP4低表达。
具体实施方式
为了能够更清楚地理解本发明的上述目的、特征和优点,下面结合附图和具体实施例对本发明进行进一步的详细描述。本领域技术人员应该明了,所述实施例仅仅是帮助理解本发明,不应视为对本发明的具体限制;如无特殊说明,下述实施例中所使用的测序和分析方法均为常规方法。
实施例1:DPP4在NK/T细胞淋巴瘤患者中高表达
一共收集了来自中山大学肿瘤防治中心的10例NK/T细胞淋巴瘤患者的肿瘤组织样本进行单细胞转录组测序。简而言之,首先对NK/T细胞淋巴瘤肿瘤组织样本进行酶解消化得到单细胞悬液,随后使用商业化的单细胞转录组建库试剂盒(10x Genomics)对活细胞进行上机和建库,最后将得到的cDNA文库进行高通量测序得到序列文件。所获取的单细胞转录组测序数据经分析得到了NK/T细胞淋巴瘤患者的肿瘤组织中所涵盖的全部细胞类型,包括了正常NK细胞、肿瘤NK细胞、T细胞、B细胞、髓系细胞和其他细胞。图1示出了NK/T细胞淋巴瘤单细胞转录组测序数据中,DPP4在各类细胞中的表达水平和表达比例。其中,DPP4主要在肿瘤NK细胞中表达,而在正常NK细胞或其他细胞类型中基本不表达,表明DPP4的表达是NK/T细胞淋巴瘤肿瘤细胞所特有的特征。
另一方面,分别通过对NK/T细胞淋巴瘤肿瘤组织的石蜡包埋蜡块切片采用免疫荧光共染的方法,以及对NK/T细胞淋巴瘤的细胞系进行实时荧光定量PCR和蛋白质印迹法检测。在免疫荧光共染实验中使用了CD56的特异性抗体(Zsbio;货号Cat#ZM-0057)来标定肿瘤组织蜡块切片中的肿瘤细胞,并使用DPP4的特异性抗体(CST;货号Cat#67138)来标定肿瘤组织中的DPP4蛋白,通过检测相应的荧光信号来明确NK/T细胞淋巴瘤的肿瘤细胞中DPP4的表达。对NK/T细胞淋巴瘤细胞系的实时荧光定量PCR实验中所用到的引物序列(SEQ IDNO:1和SEQ ID NO:2)如以下表1所示,以检测DPP4转录水平的表达量。

上述引物的寡核苷酸链序列均由广州派真生物技术有限公司合成。
蛋白质印迹法同样使用了上述DPP4特异性抗体,分别检测了NK/T细胞淋巴瘤细胞系的细胞裂解液标本及分离所得的上清液,以明确DPP4在肿瘤细胞中的表达及分泌。
通过这上述方法进一步证实了NK/T细胞淋巴瘤的肿瘤细胞中DPP4的表达,并且肿瘤细胞上清液中存在细胞分泌的DPP4分子(如图2和图3所示)。图2示出了免疫荧光共染实验检测到的NK/T细胞淋巴瘤肿瘤组织的石蜡包埋蜡块切片中的肿瘤细胞的DPP4的表达(CD56和DPP4双阳性细胞)。图3示出了蛋白质印迹法证实NK/T细胞淋巴瘤的肿瘤细胞系(YT细胞和NK-92细胞)的细胞裂解液和细胞上清液中分别存在DPP4蛋白的表达和分泌。
实施例2:NK/T细胞淋巴瘤中高表达的DPP4可阻碍正常NK细胞的趋化募集
使用CellPhoneDB软件对上述NK/T细胞淋巴瘤患者的单细胞转录组测序数据进行细胞通讯分析,得到了肿瘤细胞亚群和免疫细胞亚群间显著的细胞-细胞相互作用。图4示出了肿瘤NK细胞亚群和髓系细胞亚群间的CXCL2/CXCL9/CXCL10/CXCL11-DPP4相互作用对的作用强度。如图4所示,发现所有的肿瘤NK细胞亚群(NK_C5_ISG15、NK_C6_ITM2A、NK_C7_CCL4、NK_C8_MT1E、NK_C9_CXCL13和NK_C10_STMN1)均可通过上述相互作用发挥DPP4降解趋化因子的功能。
进一步地,通过将NK/T细胞淋巴瘤肿瘤细胞来源的细胞上清液与正常NK细胞进行体外趋化实验,观察潜在包含肿瘤细胞分泌的DPP4的细胞上清液是否对正常NK细胞的趋化有阻碍作用,并且该作用是否能够进一步受到DPP4抑制剂的抑制。图5是NK/T细胞淋巴瘤肿瘤细胞来源的细胞上清液与正常NK细胞的体外趋化实验的示意图,将经DPP4的小分子抑制剂利格列汀(Linagliptin;CAS号:668270-12-0;Selleck;货号Cat#S3031)或DSMO对照物(Sigma-Aldrich;货号Cat#D4540)预处理的来自NK/T细胞淋巴瘤肿瘤细胞系的细胞上清液用于处理正常NK细胞,从而证实NK/T细胞淋巴瘤肿瘤细胞的上清液中存在分泌的DPP4,并可通过其对趋化因子(CXCL2、CXCL9、CXCL10)的降解作用阻碍正常NK细胞的趋化。最终以迁移穿过小孔进入下室的NK细胞的比率来表示其趋化强度,迁移率越高则说明该处理组下的正常NK细胞的趋化能力越强。图6示出了由NK/T细胞淋巴瘤肿瘤细胞来源的细胞上清液与正常NK细胞的体外趋化实验所得的不同处理组中的NK细胞的迁移率(A:空白对照、趋化因子、DPP4蛋白、DPP4抑制剂、趋化因子+DPP4蛋白、趋化因子+DPP4蛋白+DPP4抑制剂;B:细胞上清液(YT细胞)、细胞上清液(YT细胞)+趋化因子、细胞上清液(YT细胞)+趋化因子+DPP4抑制剂、细胞上清液(NK-92细胞)、细胞上清液(NK-92细胞)+趋化因子、细胞上清液(NK-92细胞)+趋化因子+DPP4抑制剂),由趋化因子(CXCL2、CXCL9、CXCL10)引起的正常NK细胞的趋化受到了DPP4的阻碍,而该阻碍作用受到了DPP4抑制剂的抑制。
DPP4抑制剂利格列汀能特异性靶向DPP4。这里以利格列汀为例说明本发明的具体实施方案,但基于本发明的基本原理,本发明的DPP4抑制剂包括但不限于利格列汀,其他DPP4抑制剂也可应用于本发明,并落入本发明的保护范围,例如阿格列汀(Alogliptin,SYR-322;CAS号:850649-61-5)、安奈格列汀(Anagliptin;CAS号:739366-20-2)、贝戈洛单抗(Begelomab,SAND-26;CAS号:1403744-56-8)、吉格列汀(Gemigliptin,LC15-0444;CAS号:911637-19-9)、奥格列汀(Omarigliptin,MK-3102;CAS号:1226781-44-7)、沙格列汀(Saxagliptin,BMS-477118;CAS号:361442-04-8)、西格列汀(Sitagliptin,MK-0431;CAS号:486460-32-6)、曲格列汀(Trelagliptin,SYR-472;CAS号:865759-25-7)、维格列汀(Vildagliptin,LAF-237;CAS号:274901-16-5)。
进一步地,对NK/T细胞淋巴瘤肿瘤组织的石蜡包埋蜡块切片采用多色免疫荧光共染的方法,以检测肿瘤组织中DPP4与趋化因子(CXCL2、CXCL9、CXCL10)间的蛋白表达上的相关性。在免疫荧光共染实验中除了再次用到上面所提的CD56和DPP4特异性抗体来分别标定肿瘤细胞和DPP4蛋白表达,还用到趋化因子的特异性抗体——CXCL2(Proteintech;货号Cat#26791-1-AP)、CXCL9(Proteintech;货号Cat#22355-1-AP)和CXCL10(Proteintech;货号Cat#10937-1-AP),以明确这些趋化因子在NK/T细胞淋巴瘤组织中的表达。
图7示出了高表达DPP4的NK/T细胞淋巴瘤患者中少量的趋化因子(CXCL2、CXCL9、CXCL10)的表达(A),与之相对的是低表达DPP4的患者中的大量趋化因子的表达(B)。该结果进一步表明,NK/T细胞淋巴瘤的肿瘤细胞可通过表达DPP4来降解趋化因子,从而能够阻碍正常NK细胞向肿瘤组织中的趋化募集。
由以上研究结果可见,肿瘤细胞表达的DPP4阻碍了正常NK细胞的趋化,可用于临床上对NK/T细胞淋巴瘤患者的精确分型和精准治疗。DPP4对正常NK细胞趋化的阻碍效果可受到DPP4抑制剂的抑制,因此DPP4可以作为NK/T细胞淋巴瘤治疗的重要靶点,可应用DPP4抑制剂来治疗NK/T细胞淋巴瘤患者。DPP4作为NK/T细胞淋巴瘤的肿瘤标志物,可推广并应用于临床上NK/T细胞淋巴瘤患者的筛查(初筛)。

1.DPP4作为NK/T细胞淋巴瘤肿瘤标志物的应用。
2.DPP4检测试剂在制备用于筛查NK/T细胞淋巴瘤的试剂盒中的应用。
3.根据权利要求2所述的应用,其特征在于,所述试剂盒包括如SEQ ID NO:1和SEQ IDNO:2所示的核苷酸序列。
4.DPP4抑制剂在制备用于治疗NK/T细胞淋巴瘤的药物中的应用。
5.根据权利要求4所述的应用,其特征在于,所述DPP4抑制剂包括利格列汀、阿格列汀、安奈格列汀、贝戈洛单抗、吉格列汀、奥格列汀、沙格列汀、西格列汀、曲格列汀、维格列汀中的一种或多种。
