Blood routine is still higher than normal after 1 week of taking antibiotics, and for further clear diagnosis, rotary cut biopsy of breast tumor under ultrasound guidance is performed.
Western diagnosis: chronic mastitis; the diagnosis is based on the following:
(1) Clinical manifestations: the patient was 35 years old and had a unilateral disease in the right breast during non-lactation. With the depression of the nipple at the right side, the onset of the disease is rapid, and the initial lump is red, swollen and painful. The mass in the lower quadrant of the right breast was 11 x 9cm, the skin was reddish, the tongue was dark red, the coating was yellow and greasy, and the pulse was slippery.
(2) Laboratory examination: blood convention: WBC:10.66*10 9 /L↑,NEUT%:77.5%↑,NEUT:8.26*10 9 L ≡, LYM%15.7 ≡CRP:24.76ug/L ≡, inflammatory cells and C-reactive protein (CRP) were seen to be elevated.
(3) Imaging examination: the patient has breast B ultrasonic: the right milk has a disturbance of the structure of the lower outer quadrant, in which multiple hypoechoias are visible, the larger size being about 8 x 6cm, and inflammatory changes are likely. Breast ultrasound contrast cues: the right lower external quadrant of the milk and the glands around the nipple are disordered, suggesting that the possibility of inflammatory lesions is high.
(4) Pathology examination: right mammary gland diseases, and massive acute and chronic inflammatory cell infiltration of interstitium.
Diagnosis of traditional Chinese medicine: acne-like acute mastitis and liver meridian stagnation heat syndrome.
The initial diagnosis fashion of 20X1 year 1 month 27 belongs to the main stage of lump formation without breaking, but a small amount of micro-abscess is found during puncture, and the traditional Chinese medicine comprehensive treatment is adopted. Orally taken, modified five-flavor disinfection decoction of traditional Chinese medicines for soothing liver and activating blood, clearing heat and detoxicating, and one dose of the decoction is taken in the morning and evening; the external application medicine bag provided by the invention is externally applied;
re-diagnosis is performed for 2 months and 3 days in 20X1 years, the pain of the right breast mass of the patient is improved, the local mass is not red, the oral administration of the five-ingredient disinfection drink is continued, and the medicine bag is externally applied;
three diagnoses of 20X1 years, 2 months and 5 days, and auxiliary treatment of fire needle acupuncture at the local micro abscess by external treatment;
four diagnoses are carried out on the right breast lump of the patient in 20X1, 2 months and 9 days, the pain is relieved, the tongue is red, the tongue fur is yellow and greasy, the pulse is deep and thin, and the mammary gland B ultrasonic display is carried out: right mastitis was accompanied by local abscess formation, right axillary lymph nodes were visible, and reactive hyperplasia was considered. The traditional Chinese medicine treatment method is used for clearing heat and detoxicating, expelling pus and going out, an oral administration proved recipe of 'Jiangru No. 2' traditional Chinese medicine formula is externally applied to a medicine bag, and a fire needle is used for assisting in treatment, so that local medicine changing is realized;
after the clinic treatment is continued for 7 months, the right breast lump disappears, the crumple heals, a small amount of local pigmentation is formed, the blood convention and the mammary gland B ultrasonic are checked again, the patients are basically healed, and the patients do not relapse after follow-up half a year.
Cases 2, liu Moumou, 40 years old for women, initial diagnosis time: the patient has left breast lump with red swelling and pain for more than 3 months at 20X0, 8 months and 11 days. The drainage operation is cut before hospital delivery, and 20 mg/day of prednisone is taken together with traditional Chinese medicines for oral administration, which is not obviously improved.
Western diagnosis: chronic mastitis; the diagnosis is based on the following:
(1) Clinical manifestations: the patient was 40 years old, and had a single side disease in the non-lactation period, with a lump in the left breast with red swelling and pain. The left nipple is concave, the upper quadrant of the left breast is a lump with the size of about 10 x 5cm, and a crumple is visible in the middle of the lump. The depth of the detected sinus is about 2cm, and the local Pi Fuse is red; the lower quadrant lump in left milk, about 6 x 4cm in size, soft and red; right milk examination was free of abnormalities.
