Abstract

本发明属于辅助生殖技术领域。本发明所述的含人成熟卵泡液的培养液用于人未成熟卵的体外培养技术,其成分由Ham′sF-10或TCM199,人代血清SSS,rFSH,HCG,17β雌二醇,以及人成熟卵泡液组成。利用未成熟卵体外成熟技术(IVM),将卵子成熟障碍的不孕症妇女的未成熟卵取出,采用本发明技术制备的培养液将其培养成熟,并在体外受精和进行胚胎移植,帮助其妊娠。获得了65%的卵子成熟率和33.3%的妊娠率。本发明技术提供了一种高效、价廉、方便的人卵子体外成熟的培养液,并有可能将其制成商品化的制剂,用于人未成熟卵的体外成熟技术。

Description

人成熟卵泡液在未成熟卵体外成熟技术中的应用
一、技术领域
本发明属于辅助生殖技术领域。
二、背景技术
未成熟卵体外成熟(in-vitro maturation,IVM)是生殖医学领域中辅助生殖技术的一项前沿技术。自1978年世界上第一例“试管婴儿”诞生以来,辅助生殖技术及其相关的一系列衍生技术发展迅速,帮助许多不孕夫妇获得生育。但是其中一些妇女因各种原因不能获得正常的成熟卵子,常规的辅助生殖技术对此也无能为力。未成熟卵体外成熟(in vitro maturation,IVM)技术就是将妇女卵巢中核泡期(GV stage)的未成熟卵取出,采用特殊的培养液模拟体内的卵泡微环境,在体外短时间培养成熟后,再进行显微授精,发育成胚胎后植入母体子宫腔内。
1935年Pincus和Enzmamn首次证实哺乳动物卵巢中未成熟卵母细胞脱离卵泡内环境,在体外合适环境中培养能自发恢复减数分裂而成熟。目前IVM技术广泛应用于畜牧业的实际生产并逐步开始商业化和产业化。直到1991年Cha等利用IVM技术才在世界上获得了人类首例妊娠。
我国不孕症患者中大约有25%是因为排卵障碍,其中相当一部分患者的卵子成熟有障碍,即使常规“试管婴儿”技术也因此无能为力。IVM技术能够将未成熟的卵母细胞在体外特殊的培养液中培养成熟,继而完成体外受精-胚胎移植的过程,帮助这类患者妊娠。目前在世界范围内,仅有加拿大、南韩、日本、丹麦等为数不多的生殖中心为病人提供该技术治疗,全世界仅约200例左右的IVM“试管婴儿”诞生。
人未成熟卵母细胞的体外成熟技术现已在不育症治疗中逐渐显示出了优势。但与其它哺乳类相比较,人未成熟卵母细胞的体外成熟率、受精率以及种植后的妊娠率均显著低。而卵母细胞的成熟培养体系,培养液的成份和培养条件能影响甚至调节卵母细胞的减数分裂规律,是该技术的核心部分。目前有各种体外成熟的培养液报道,但目前还没有较为理想稳定的商品化未成熟卵体外成熟培养液问世。
该技术的临床应用不仅能够帮助一些顽固性多囊卵巢综合征、有卵巢过度刺激病史、及某些卵子成熟障碍的不育患者获得妊娠,大大节约促超排卵药物的昂贵费用,以及药物对卵巢刺激带来的诸多副作用,而且能为人类卵子发生发育机制的基础研究提供重要的理论依据。特别是对未成熟卵的冷冻保存、复苏、成熟和受精的临床价值更为重要,使“卵子库”的建立成为可能。
三、发明内容
本发明目的是建立一种高效、价廉、方便实用的未成熟卵体外成熟系统,即未成熟卵体外成熟培养液,以期获得较高的临床妊娠率。
本发明的技术方案为首次将人成熟卵泡液作为重要成分,加入培养液中。培养液成分包括Ham′s F-10或TCM199基础培养液(Sigma,USA),10%SSS(人体代血清,Irven,USA),0.05-0.1IU/ml rFSH(Serono,Switzerland),0.2-0.7IU/ml HCG(Serono,Switzerland),0.2-1.5ug/ml 17β-E2(Sigma,USA),以及10-15%人成熟卵泡液。供者的成熟卵泡液经过56℃灭活30分钟,过滤灭菌,-20℃冻存。
南京医科大学第一附属医院/江苏省人民医院妇产科临床生殖医学中心,江苏省生殖医学重点实验室的研究人员在动物实验研究的基础上,自2001年以来,采用本发明技术配制的含人成熟卵泡液的培养系统,进行了IVM技术的临床实验研究,经过对13例患者15个IVM周期的治疗,共获得5例临床妊娠(第一例于2001年11月9日(39+周)上午因婴儿臀位行剖腹产,9时35分顺利出生一健康女婴,体重2900g,Apgar评分满分),临床妊娠率为33.3%(5/15),已接近并达到国际先进水平。
该发明技术制备的卵母细胞体外成熟培养液高效、价廉、方便、以及来源容易,取得良好的临床效果。该培养液可制成商品化的制剂,用于未成熟卵体外成熟技术。
四、具体实施方式
1.患者进行轻微的促排卵准备和子宫内膜准备,B超监测卵泡直径平均在6-12mm时,在B超下经阴道穿刺获取未成熟卵子。
2.将未成熟卵取出,在本发明配制的含成熟卵泡液的培养液中,于37℃,5%CO2的环境中培养36-48小时,将卵丘颗粒细胞除去,对卵子进行成熟度评分,未成熟卵的成熟率达68.4%。
3.取得丈夫精子处理后,将呈现第一极体成熟标志的成熟卵,按单精子卵浆内注射方法进行体外受精,受精率达75%,体外培养2-3天,将卵裂至4-8细胞的胚胎种植到母亲子宫内。
4.母亲的子宫内膜按卵子成熟和受精的时相,进行同步化准备,接受胚胎着床,并进行健黄体和胚胎维持,妊娠率33.3%。
