Abstract
本发明属于生物医药技术领域,具体涉及联合调控细胞周期和Atoh1表达在治疗前庭功能障碍中的应用。本发明提供了一种试剂在制备治疗和/或预防前庭功能障碍的产品中应用,所述的试剂包括:1)促进已分化的感觉上皮细胞重新进入细胞周期或再生的试剂I;和,2)表达Atoh1的试剂II。该试剂能够使感觉上皮细胞重新进入细胞周期进行有丝分裂,增加前庭支持细胞和/或前庭毛细胞的数量,用于治疗前庭功能障碍(特别是眩晕或平衡障碍等)。

技术领域
本发明属于生物医药技术领域,具体涉及一种联合调控细胞周期和Atoh1表达的试剂在治疗前庭功能障碍中的应用。
背景技术
内耳前庭毛细胞承担着感受头部运动并通过机-电转换参与维持机体平衡的重要作用。在临床上,前庭毛细胞容易受到耳毒性药物如氨基糖苷类抗生素或顺铂,以及年龄增长或自身免疫等因素损伤从而产生眩晕、平衡障碍等症状,严重影响患者的生活质量。哺乳动物前庭毛细胞损伤之后自发性再生能力有限,再生过程中缺乏有丝分裂,主要以支持细胞转化为主,感觉上皮各种细胞比例明显失调,且再生的毛细胞数量远低于正常水平。由于前庭系统尚缺乏有效的人工替代装置,目前前庭功能的恢复只能依靠有限的中枢代偿作用。因此,找到行之有效的方法促进前庭毛细胞充分再生对于平衡障碍的患者来说至关重要。
目前现有技术中促进前庭支持细胞向前庭毛细胞发生转化是前庭毛细胞再生的主要手段。但是,通过此方法促进前庭毛细胞再生将带来一个明显的问题:由于成年哺乳动物支持细胞缺乏有丝分裂,前庭支持细胞向前庭毛细胞发生转化后其自身数量明显减少。而前庭支持细胞对于维持内耳感觉上皮的正常功能具有不可或缺的意义。通过单一的前庭支持细胞转化实现前庭毛细胞再生,造成前庭毛细胞与前庭支持细胞比例失衡,可能是目前研究中哺乳动物前庭功能难以恢复的一个重要原因。能否通过增加哺乳动物前庭毛细胞再生过程中的有丝分裂程度,对于前庭毛细胞充分再生、维持前庭毛细胞与前庭支持细胞比例平衡,从而实现前庭功能恢复变得十分重要。
细胞周期依赖性激酶抑制剂(cyclin-dependent kinase inhibitor, CDKI)在内耳胚胎发育的过程中介导祖细胞脱离细胞周期,结束有丝分裂。非专利文献:p27(Kip1) isrequired to maintain proliferative quiescence in the adult cochlea andpituitary(Oesterle Elizabeth C,Chien Wei-Ming,Campbell Sean et al., CellCycle, 2011, 10: 1237-48.)公开了抑制新生小鼠内耳感觉上皮的CDKI如p19Ink4d、p21Cip1、p27Kip1或Rb等因子表达,能够促进已分化的感觉上皮细胞重新进入细胞周期,产生有丝分裂细胞,但随着年龄的增长,内耳感觉上皮对于CDKI调控的反应迅速减少,成年小鼠内耳感觉上皮有丝分裂数量极少。由于人类与小鼠内耳发育并不同步,小鼠出生时内耳发育尚不成熟,只相当于人类胚胎约23周水平,而人类内耳出生时已经发育成熟,针对新生小鼠进行的有丝分裂研究对于临床实践的参考价值有限。且在临床中,前庭感觉上皮遭受药物、免疫、感染、老龄等因素损伤从而产生眩晕症状的情况更常见于成年患者。
非专利文献:Life after deaf for hair cells(Ruth Taylor AF, Science2005; 307 (5712):1056-1058.)和非专利文献:Proliferation of functional haircells in vivo in the absence of the retinoblastoma protein(Sage C, Huang M,Karimi K, et al., Science 2005; 307 (5712):1114-1118.)公开了p27Kip1基因持续表达于分化成熟的支持细胞,维持其细胞周期的稳定性。非专利文献:Progressive hearingloss in mice lacking the cyclin-dependent kinase inhibitor Ink4d(Chen P,Zindy F, Abdala C, et al., Nature Cell Biology 2003; 5 (5):422-426.)公开了虽然抑制CDKI能够诱导有丝分裂,但分裂的细胞无法继续向成熟的感觉上皮细胞分化,很快启动凋亡程序而不能长期存活。并且在专利文献(CN112359018A)也公开了通过例如阻断Rb1和p27Kip1的途径检测了胚胎小鼠或新生小鼠中的毛细胞重新进入细胞周期的情况,但毛细胞一般随后死亡。然而,在成年小鼠内耳中的类似处理未曾诱导使其重新进入细胞周期。因此,如何克服成年哺乳动物有丝分裂稀缺的瓶颈,对于恢复前庭感觉上皮的正常结构,从而应用到在临床上防治眩晕至关重要。
另外,由于前庭感觉上皮的损伤最突出的表现为毛细胞受损,如何促进增殖后的前庭支持细胞转化为前庭毛细胞,从而恢复感觉上皮的正常结构更为重要。
专利文献(CN111655228A)公开Atoh1作为一种转录因子,可以将新生儿和少年哺乳动物耳蜗中的支持细胞转换为毛细胞,但Atoh1诱导的毛细胞转化效率低(<17%)、不完全(缺乏成熟的毛细胞标记物)和年龄依赖性(成年耳蜗无反应)。
发明内容
为解决再生毛细胞数量有限、且支持细胞数量减少造成两种细胞比例失调的矛盾。本申请提供了一种通过同时促进已分化的感觉上皮细胞重新进入细胞周期并过表达Atoh1的方法,不仅可以促进前庭感觉上皮(如前庭支持细胞和/或前庭毛细胞)细胞重新进行有丝分裂,还可以促进前庭支持细胞向前庭毛细胞的转化,有助于修复内耳感觉上皮的正常结构,能够用于治疗和/或预防前庭功能障碍(特别是眩晕或平衡障碍等)。
本发明的第一方面,提供了一种试剂在制备治疗和/或预防前庭功能障碍的产品中应用,所述的试剂包括:
1)促进已分化的感觉上皮细胞重新进入细胞周期或感觉上皮细胞再生的试剂I;和,2)表达Atoh1的试剂II。
优选的,所述的感觉上皮是指含有感觉上皮细胞(初生的、次生的),具有刺激的感受机能的上皮组织。
优选的,所述的感觉上皮细胞包括前庭支持细胞和/或前庭毛细胞。
优选的,所述的试剂I可以是现有技术中任一可以促进已分化的感觉上皮细胞重新进入细胞周期或再生的材料。
优选的,所述的试剂I抑制p19Ink4d、p21Cip1、p27Kip1或Rb中的一种或两种以上。进一步优选抑制前庭感觉上皮细胞或组织或前庭组织中的p19Ink4d、p21Cip1、p27Kip1或Rb。
其中,p19Ink4d为细胞周期蛋白依赖性激酶抑制因子2D,编码CDKN2D蛋白;
p21Cip1为周期素依赖性激酶抑制因子1A,编码CDKN1A蛋白;
p27Kip1为细胞周期蛋白依赖性激酶抑制剂1B,编码CDKN1B蛋白;
Rb为视网膜母细胞瘤抑制蛋白,编码RB蛋白。
在本发明的一个具体实施方式中,所述的试剂I抑制p27Kip1。
进一步优选的,所述的试剂I包括但不限于干扰RNA、反义寡核苷酸、抗体、拮抗剂、阻断剂、micro RNA或基因编辑所需要的材料中的一种或两种以上。
所述的试剂I为组织特异性敲除所需材料。优选包括Cre/loxp重组酶系统所需的材料,例如使用病毒或非病毒载体将Cre和/或loxp导入的试剂。
进一步优选的,所述的基因编辑所需要的材料包括但不限于CRISPR/Cas系统所需的材料、Flp/FRT重组酶系统所需的材料或Cre/loxp重组酶系统所需的材料中的一种或两种以上。
优选的,所述的干扰RNA包括但不限于siRNA、dsRNA、shRNA、aiRNA或miRNA中一种或两种以上。

所述的Cre/loxp重组酶系统是一种特定位点的重组酶技术,可在DNA的特定位点上执行删除、插入、易位及倒位,用该系统可以针对特定的细胞类型或采用特定的外部刺激,对细胞中DNA进行修改。Cre重组酶识别loxP位点两端的反向重复序列并结合形成二聚体,然后此二聚体与另一个loxP位点上的二聚体结合形成一个四聚体。LoxP位点是有方向性的,四聚体连接的两个位点在方向上是平行的。然后两个loxP位点间的DNA序列被Cre重组酶切断。接着,DNA连接酶快速高效地将这些链连接起来。如果两个loxP位点位于同一条DNA链上且方向相同,Cre重组酶介导loxP间的序列切除;如果两个loxP位点位于同一条DNA链上且方向相反,Cre重组酶介导loxP间的序列反转;如果两个loxP位点位于不同的DNA链或染色体上,Cre重组酶介导两条DNA链发生交换或染色体易位。
在本发明的一个具体实施方式中,所述的试剂I包括Cre/loxp重组酶系统所需的材料,所述的Cre/loxp重组酶系统中两个loxp位点设置在目标基因两端,同向排列(flox/flox),然后再导入Cre重组酶,介导loxP间的序列切除。
优选的,所述的试剂II可以是现有技术中任一可以表达Atoh1的材料。
优选的,所述的试剂II包括但不限于载体、促进Atoh1基因转录的因子或Atoh1蛋白激活剂中的一种或两种以上,所述的载体包含编码Atoh1蛋白的核酸。
所述试剂II可以上调感觉上皮细胞或组织,或者前庭中的Atoh1(mRNA或蛋白)表达量。
优选的,所述的试剂包括:
1)敲除或敲低p19Ink4d、p21Cip1、p27Kip1或Rb的载体;和2)表达Atoh1的载体。
优选的,敲除或敲低p19Ink4d、p21Cip1、p27Kip1或Rb的载体包含Cre重组酶编码序列和/或loxp序列。
进一步优选的,所述的试剂包括:
1)敲除或敲低前庭感觉上皮细胞或组织或前庭器官中的p19Ink4d、p21Cip1、p27Kip1或Rb的载体;和2)在前庭感觉上皮细胞或组织或前庭器官中表达Atoh1的载体。
在本发明的一个具体实施方式中,所述的试剂包括:
1)敲除或敲低前庭感觉上皮细胞或组织或前庭器官中的p27Kip1的载体;和2)在前庭感觉上皮细胞或组织或前庭器官中表达Atoh1的载体。
优选的,所述的载体可以是病毒载体或非病毒载体。
进一步优选的,所述的非病毒载体包括但不限于能与基因编辑片段形成复合物并促进细胞内基因的人工合成或天然化合物,载体材料可选自例如脂类、聚合物、蛋白质和多肽。
进一步优选的,所述的病毒载体包括但不限于慢病毒载体、假病毒载体、腺病毒载体、腺相关病毒载体、单纯疱疹病毒载体或巨细胞病毒载体。
所述的病毒载体由CMV早期增强子/鸡β-肌动蛋白启动子驱动。

在本发明的一个具体实施方式中,所述的给药方式为局部给药,优选为耳部给药,进一步优选为前庭器官给药,更优选通过前庭器官中半规管给药。

优选的,所述的方法可以用于治疗前庭功能障碍。进一步优选的,对前庭功能障碍的相关限定同本发明的第一方面。
本发明所述的“治疗”表示在疾病已开始发展后减缓、中断、阻止、控制、停止、减轻、或逆转一种体征、症状、失调、病症、或疾病的进展或严重性,但不一定涉及所有疾病相关体征、症状、病症、或失调的完全消除。
本发明所述的“药学上可接受的”是指既不显著刺激生物体也不抑制所施用的产品的活性物质的生物学活性及特性。
本发明所述的“包含”或“包括”为开放式写法,含有所描述的指定成分或步骤,以及不会实质上影响的其他指定成分或步骤。

本发明的有益效果:
1. 本申请在成年哺乳动物前庭共同调控p27Kip1与Atoh1表达,评估同时调控两条通路对前庭感觉上皮增殖和前庭毛细胞再生的作用,观察到二者强烈的协同作用,实现了成年哺乳动物前庭感觉上皮大量有丝分裂及在此基础上发生的前庭毛细胞再生。有助于未来治疗前庭功能障碍(特别是眩晕或平衡障碍等)。
2.目前的技术手段尚无法在成年哺乳动物内耳实现大量有丝分裂,这是目前毛细胞再生领域的一个重要瓶颈。而由于人类与实验小鼠内耳发育的不同步性,成年小鼠的实验结果对于临床疾病的治疗才更有参考价值。本申请在成年小鼠内耳观察到大量有丝分裂基础上实现的前庭毛细胞再生,是外周性眩晕治疗相关研究领域的一个重要突破。
3. 目前研究中多采用转基因鼠结合他莫昔芬注射的方式实现基因调控,但由于p27Kip1与Atoh1都表达在全身多个重要器官,无法避免全身副作用,动物无法长期存活,对于临床应用的实施和参考价值有限。本申请,利用内耳具有骨性包囊这样一个解剖优势,创新性采用转基因鼠结合内耳局部注射的方式,将病毒载体携带的Cre元件精准注射到小鼠内耳前庭,实现组织特异性的基因调控,从而避免了全身副作用,动物能够长期存活。

具体实施方式
下面将结合本发明实施例中的附图,对本发明实施例中的技术方案进行清楚、完整地描述,显然,所描述的实施例仅是本发明的部分实施例,而不是全部。基于本发明中的实施例,本领域普通技术人员在没有做出创造性劳动前提下所获得的所有其他实施例,都属于本发明保护的范围。
本申请中应用的材料来源与方法:
1.腺相关病毒载体:
使用CMV早期增强子/鸡β-肌动蛋白(CAG)启动子驱动的纯化的AAVie病毒载体。使用具有增强绿色荧光蛋白(EGFP)报告基因的AAVie进行实验。AAVie载体购自PackGeneBiotech有限公司(货号:ssAAV-control-200),滴度为1×1013vg/ml。载体通过三重质粒转染到HEK293T细胞中产生,经分装后于温度为-80°C的冰箱中储存。
2.p27Kip1[flox/flox]转基因小鼠:购自:Jackson Lab,货号为#027328;
3.部分试剂来源:
链霉素溶液:购自:Sigma-Aldrich,货号S1277-50G;
Triton X-100:购自:Sigma-Aldrich,货号V900502;
EdU溶液:购自:Thermo,货号A10044;
EdU 594 cocktail:购自:Thermo,货号C10354;
myosin VII抗体:购自:Proteus BioSciences,货号25-6790;
Alexa Fluor 488 or 594 or 647:购自:Invitrogen,货号A32733;
鬼笔环肽:购自:Invitrogen,货号A12379;
DAPI:购自:AppliChem,货号A1001。
4.动物手术(见图2):
所有实验动物均取左耳为术耳,右耳作为自身对照。其中,
造模(前庭毛细胞损伤的动物模型)手术操作:培育p27Loxp/Loxp小鼠,在动物5周龄时采用左侧水平半规管注射的方式(参见非专利文献:Guo Jing-Ying,He Lu,Qu Teng-Feiet al. Canalostomy As a Surgical Approach to Local Drug Delivery into theInner Ears of Adult and Neonatal Mice.[J] .J Vis Exp, 2018, undefined:undefined.),具体的:小鼠麻醉后,行耳后切口,分离皮下组织和肌肉,暴露后半规管和水平半规管。用26G针头在半规管上钻孔,将150µg链霉素溶液(浓度为150mg/ml,1µl)通过微导管由水平半规管的钻孔处注入小鼠内耳。微量注射泵控制注射速度为0.5µl/min,建立前庭感觉上皮中度损伤的动物模型(前庭毛细胞大部分死亡而前庭支持细胞存活)。注射完毕后静置3分钟,用一小片肌肉封闭钻孔处,缝合切口。
给药手术(二次手术)操作:在造模成功一周后,通过后半规管导入AAVie病毒载体,给药手术操作与造模手术操作大致相同,不同之处在于将1µl或2µl的AAVie病毒载体通过后半规管注射入小鼠内耳前庭。
5.EdU标记有丝分裂细胞
在二次术后第12-14天连续三天腹腔注射EdU溶液(1mg/ml,0.05mg/g)标记有丝分裂细胞。
6.评估前庭功能
首先观察并记录小鼠是否出现前庭功能受损的行为,如头部倾斜、步态不稳、转圈、提起尾巴时沿身体纵轴打转等。然后进行游泳试验,具体方法为:准备一个装有24-26℃温水、水深至少15cm的容器,将小鼠轻柔放入水中,观察并评估小鼠的游泳能力,时长1分钟,若小鼠出现溺水行为,应立即将其打捞出水面;将小鼠放入垫有电热毯的笼子内,待复温后再转移入鼠笼。评分标准:1)0分:正常姿势游泳。小鼠身体纵轴延长,尾巴似鞭毛运动状摆动。2)1分:异常游泳动作。如:垂直方向游泳,小范围内打圈游泳,贴容器壁游泳,尾巴翘起及拍打水面。3)2分:漂浮。小鼠刚开始时被动漂浮于水面,没有主动的游泳动作,不久失去平衡,沉至水下翻跟斗或呈慌张游泳状。4)3分:水下翻跟斗,小鼠无法保持平衡,接触水面后立即沉入水下翻跟斗。此时应立即将小鼠打捞。
7.免疫荧光染色
椭圆囊铺片免疫荧光染色:小鼠麻醉后,断头,分离内耳;解剖显微镜下用针尖挑开蜗尖、圆窗和卵圆窗,夹断水平半规管,于含4%多聚甲醛的PBS中固定,PBS冲洗后,分离椭圆囊,用含3%牛血清的PBS冲洗2次,每次5分钟,用含0.5% Triton X-100的PBS浸泡20分钟,用提前配置好的EdU 594 cocktail进行染色,时间为30分钟,之后用含有3%牛血清的PBS清洗2次,PBS清洗3次,每次均为5分钟。用含5%羊血清的PBS封闭1小时,置于一抗中4℃孵育过夜,一抗包括myosin VII抗体或GFP Alexa Fluor 488抗体,次日经PBS清洗后,加入荧光标记的二抗(Alexa Fluor 488 or 594 or 647),室温孵育1小时,若需要标记纤毛,则加入Alexa Fluor 594或者Alexa Fluor 647标记的鬼笔环肽额外孵育1小时,若需要标记细胞核,则加入DAPI抗体孵育5分钟,PBS清洗后,椭圆囊铺片,抗荧光淬灭封片剂封片,激光共聚焦扫描荧光显微镜下观察并拍照;行细胞计数分析。
8.椭圆囊毛细胞、支持细胞、有丝分裂细胞数量计数:
①毛细胞计数:myosin VIIa(后简称myo)抗体染色。a.正常椭圆囊:共聚焦显微镜63倍物镜+数字放大2倍镜头在毛细胞表皮板层面采集图片,于椭圆囊微纹区和微纹外区各自采集3个视野,计数每个视野myosin VIIa阳性细胞数量并取平均值;b.损伤后椭圆囊:共聚焦显微镜20倍物镜+数字缩放0.9倍镜头在毛细胞表皮板层面采图。
②支持细胞计数:DAPI抗体染色,共聚焦显微镜63倍物镜+数字放大2倍镜头在支持细胞核层面采集图片,于椭圆囊微纹区和微纹外区各自采集3个视野,计数每个视野DAPI阳性细胞数量并取平均值。
③有丝分裂细胞数量:Edu抗体染色,共聚焦显微镜63倍物镜+数字放大2倍镜头分别在毛细胞和支持细胞核层面采图,椭圆囊微纹区和微纹外区各自3个视野,计数EdU阳性细胞数量并取平均值。其中,具体视野选取如图1所示。
9.统计学分析:
数据以平均值±标准差表示,采用单因素方差分析(ANOVA, Prism),p<0.05表示差异具有统计学意义。
实施例
造模手术1周后通过后半规管导入不同组病毒载体,各组有效病毒成分是等量的,分组情况:①联合调控组:AAVie-Cre-EGFP-1μl+AAVie-Atoh1-1μl混合液,共2μl;②单独调控细胞周期组:AAVie-Cre-EGFP病毒液1μl(简称Cre组);③单独过表达Atoh1基因组:AAVie-Atoh1病毒液1μl(简称Atoh1组)。其中,Atoh1基因NCBI号:NM_007500.5。
在给药手术后12-14天行3次的EdU腹腔注射,术后2周和1月时行游泳实验评估前庭功能,第3次注射后的6h(图3)或2周(图4)取椭圆囊行免疫荧光染色(myosin VIIa、Phalloidin、DAPI),评价椭圆囊毛细胞、支持细胞数量的变化,行GFP与Edu染色,评价AAVie-Cre-EGFP在两种细胞的表达范围及诱导产生有丝分裂的数量,评估小鼠椭圆囊感觉上皮有丝分裂及毛细胞再生的程度。
结果表示,1)联合调控组支持细胞EdU+细胞数量(10.51±9.97个/高倍镜视野)明显大于Cre组(2.06±3.72个/高倍镜视野)和Atoh1组(0.44±1.42个/高倍镜视野);
2)联合调控组myo+细胞数量(47.17±18.6个/高倍镜视野)明显大于Cre组(3.53±4.6个/高倍镜视野)和Atoh1组(9.21±9.3个/高倍镜视野);
3)联合调控组支持细胞数量(145±38.2个/高倍镜视野)与Cre组(161.1±24.6个/高倍镜视野)和Atoh1组(161.8±60.8个/高倍镜视野)无统计学差异。说明联合调控组在促进支持细胞向毛细胞发生分化的基础上,维持了正常的支持细胞数量,实现了保持两种感觉上皮细胞比例平衡的基础。
4)联合调控组病毒导入后2周时观察到较多myo+EdU+细胞(1.3±1.16个/高倍镜视野),而Cre组和Atoh1组未观察到该类细胞,1月时细胞数量进一步增加(2.0±1.0个/高倍镜视野)。
综上所述,本申请采用联合调控p27Kip1基因和Atoh1基因的方法,通过抑制p27Kip1基因表达并上调Atoh1基因表达,感觉上皮细胞的有丝分裂细胞增加,毛细胞数量也明显增加,能够用于治疗前庭感觉上皮损伤相关疾病,尤其是外周性眩晕。
以上详细描述了本发明的优选实施方式,但是,本发明并不限于上述实施方式中的具体细节,在本发明的技术构思范围内,可以对本发明的技术方案进行多种简单变型,这些简单变型均属于本发明的保护范围。
另外需要说明的是,在上述具体实施方式中所描述的各个具体技术特征,在不矛盾的情况下,可以通过任何合适的方式进行组合,为了避免不必要的重复,本发明对各种可能的组合方式不再另行说明。
Claims (2)
1.一种试剂通过增加感觉上皮细胞的有丝分裂细胞和毛细胞数量在制备治疗眩晕的药物中应用,其特征在于,所述的试剂包括:
1)促进已分化的感觉上皮细胞重新进入细胞周期或再生的试剂I,所述的试剂I为敲除前庭感觉上皮细胞或组织或前庭器官中的p27Kip1的载体;所述的试剂I包括Cre/loxp重组酶系统所需的材料,所述的Cre/loxp重组酶系统所需的材料包括使用病毒载体将Cre导入的试剂;和,
2)过表达Atoh1的试剂II,所述的试剂II为在前庭感觉上皮细胞或组织或前庭器官中过表达Atoh1的病毒载体,所述的试剂II中包含编码Atoh1蛋白的核酸;
所述的病毒载体为AAVie载体;
所述的感觉上皮细胞包括前庭支持细胞和前庭毛细胞。
2.根据权利要求1所述的应用,其特征在于,所述的病毒载体由CMV早期增强子/鸡β-肌动蛋白启动子驱动。
