在过去的二十年中,使用化学能或热能的成像引导的肿瘤消融(IGTA)已经成为用于小恶性肝肿瘤的最有效的局部区域治疗方式之一。引入临床实践的第一种肿瘤消融技术是经皮乙醇注射(PEI),其是肝细胞癌(HCC)的化学消融。然而,在20世纪90年代早期,开发了使用射频(RF)的热消融,并证明了其在早期非手术HCC患者中在更好的存活率和疾病的局部控制方面优于PEI。此后,开发了其他类型的IGTA技术,例如微波消融(MWA)、冷冻消融、激光消融、不可逆电穿孔和高强度聚焦超声(US),并将其用于治疗恶性肝肿瘤。其中,射频消融(RFA)由于其安全性和有效性以及相当好的临床结果而成为用于小恶性肝肿瘤、特别是HCC和结肠直肠癌肝转移(CRLM)的IGTA最广泛使用的方法。目前,新技术是否将扩展图像引导消融的临床作用并改善RFA方面的长期患者结果是未知的。
除了常规的机械球囊血管成形术或药物洗脱支架或球囊化之外,最近已提出研究动脉血管(冠状动脉或外周动脉)中的狭窄动脉粥样硬化斑块的RFA治疗。
射频(RF)
关于用于管理HCC患者的RFA,先前的研究已经报道,早期HCC的RFA后的总体存活率与手术切除的总体存活率相似。
射频是治疗结肠直肠肝转移中研究最多的消融方式。
与切除相比,经皮方法报道的广泛的局部肿瘤进展(LTP)(12%-48%)限制了其用于具有小的、定位良好的肿瘤或肝切除术后肝脏复发的高度选择的患者。具有足够的放射学边缘和组织学证实的坏死的消融显著降低LTP。
RFA使用在200kHz至1200kHz之间振荡的交流电流[15]。射频电极用作阴极。邻近电极尖端的靶向肿瘤的离子响应于这些交流电而快速振动。该振动摩擦能量被转换成热量,而能量沉积远离尖端以指数方式下降。组织与由射频诱导的温度的相互作用类似于激光的相互作用。韩国甲状腺放射学会修订的2017年甲状腺RFA指南建议几种标准技术。对于良性甲状腺结节,建议甲状腺周围注射利多卡因来控制疼痛。经峡部入路和移动注射技术是甲状腺病变的重要治疗方法。该技术可用于最小化并发症和边缘结节再生长。最近,已报道一种称为“血管消融技术”的新技术。提出了两种不同的血管消融技术:动脉首次消融和边缘静脉消融(静脉染色)。这些技术具有增强治疗功效和降低再生长风险的潜力。对于复发性甲状腺癌,指南建议在消融之前仔细评估关键结构,水分离以降低周围关键结构热损伤的风险,以及移动注射技术。
在可能的并发症中,神经损伤是RFA期间最严重和最可怕的一种。特别地,喉返神经热损伤引起的嗓音改变是与神经损伤相关的最常见的并发症。为了降低损伤的风险,已提出一些技术措施。最近,引入冷5%葡萄糖溶液注射来治疗消融期间的神经损伤。如果在RFA期间发生神经损伤的症状,例如声音改变、心悸、霍纳综合症、肩部运动问题或感觉异常,则应立即停止消融,并将冷5%葡萄糖溶液直接注射到神经所在的空间中。在大多数患者中,冷流体可以有效地治疗神经的热损伤。
执行甲状腺RFA的大多数操作者使用具有短轴(7cm)和小直径(18-19号)的甲状腺专用直型内部冷却电极。在小复发性甲状腺癌或甲状旁腺病变中,指南推荐19号电极尖端(即,3.8mm或5mm有源尖端)。最近引入的甲状腺专用双极电极已经被引入用于孕妇和携带电气装置(即,起搏器)的患者。
射频发射已在体内、在动物模型中、在外周血管中诱导的动脉粥样硬化斑块上成功地测试。已研究了RFA对动脉粥样硬化斑块和血管结构的影响,并且可以说它不会对血管膜引起任何解剖学损伤。RFA对斑块的作用是明显的,其中去细胞化并且中膜和内膜中新血管显著减少。细胞去细胞化主要负责平滑肌细胞(SMC),平滑肌细胞是动脉粥样硬化斑块增殖和血管成形术后再狭窄的原因。该专利中报道的发明技术也基于单独应用RFA或与血管支架植入一起与MW或LS关联。这意味着能量发射将在血管支架植入或再狭窄期间关联。
微波(MW)
微波(MW)消融在肝脏肿瘤的消融中获得共识,希望利用其优于RF消融的潜在益处,这主要在动物研究中得到证明。这些包括不需要接地垫、对散热现象的敏感性较小、消融区较大、消融时间较短以及可能更好的局部肿瘤控制。先前的MW消融与RF消融的临床比较表明MW消融在CLM的局部肿瘤控制方面具有优势。
与RF消融相比,MW消融被证明对散热效应适应能力强,为血管周围肿瘤提供了良好的控制。治疗的少量CLM的LTP率在RF消融和MW消融之间是相似的。MW消融的使用可能优于RF消融,因为其治疗血管周围肿瘤的能力得到改善,以及其在显著减少的时间内实现消融的能力,并且不需要接地垫。
激光(LS)
激光(“通过辐射受激发射的光放大”的首字母缩写)是高度相干的、准直的和单色的能量,其可以从主光源或通过允许施加器的长度和形状的极大变化的光纤精确地递送到小目标中。常见的主光源是激光二极管或钕-钇铝石榴石,其产生820nm或1064nm波长的光。当激光照射目标时,发生温度的局部升高,导致永久损伤,例如凝固性坏死(46-100℃)和组织碳化/蒸发(100-110℃)。组织损伤的等级和快速性取决于许多因素,包括释放的能量的量、施加时间、血管化和组织的含水量。颈部中的激光消融通常在超声(US)引导下执行,这允许实时监测程序。
在消融之前,利用US或对比增强US(CEUS)来执行对要治疗的目标病变的综合评估。需要仔细评估结节的大小和形状以及与相邻器官的空间关系,以避免对周围器官的部分治疗或损伤。通常根据患者的焦虑和操作者的偏好,有或没有清醒镇静,来使用局部麻醉。根据结节的大小和形状,在实时US引导下将一个或多个21号针头插入每个结节的最深部分。可以同时插入多达四个针头。在插入的针头之间应理想地保持1cm的距离。然后,插入300mm直径的平面切割石英光纤并向上推进到导引器针头尖端。然后将导引器针头拉回以使每个纤维的尖端暴露至少5mm,与目标组织直接接触。常见方案基于3-5W的平均功率,每次照射的总能量递送为1200-1800J。在第一次照射之后,可以将(一个或多个)光纤收回1/1.5cm,并且可以执行随后的照射(拉回技术),直到整个目标被照射。在治疗期间由于气体形成而导致的高回声性扩大之后,可以实时监测组织消融。对于良性结节,治疗的目的是实现至少>50%的体积减小,并且通常在结节的周边保持可变量的活结节组织,也是为了降低可能并发症的风险。相反,对于恶性结节,需要实现完全组织破坏。因此,恶性结节的激光消融需要仔细评估周围结构和结节的唯一定位,理想地距离甲状腺囊5mm。可以执行水分离以使靠近肿瘤的其他结构移位。在消融之后,可以执行CEUS以更好地识别消融区域的实际范围,其通常会被消融期间形成的气体高估;在CEUS显示未治疗区域的情况下,可以在同一疗程期间执行进一步的消融。激光消融通常作为门诊手术进行,其在消融后疼痛的罕见情况下不需要除皮质类固醇或镇痛药之外的特定药物。
根据现有技术,最有希望的肿瘤治疗基于三种不同能量源的发射,即,射频(RF)、微波(MW)和激光(LS)。从涉及RF与MW或RF与LS的比较研究的临床数据中可以清楚地看出,每种能量源都具有局限性和优点。
欧洲专利申请EP3355821A1公开了几种装置,每种装置提供这三种能量源中的一种。
美国专利申请2019/374 276Al公开了一种装置,其可以提供MW和LS,但不同时或以组合方式提供。
因此,需要以更适宜的方式提供所有三种能量源。
发明内容
本发明涉及一种用于实质器官、中空解剖导管或动脉/静脉血管的内部治疗的组织消融系统;所述系统包括EM波发生器和具有有源远端的针头或导管。根据本发明的系统的特征在于,所述发生器包括至少两个EM波输出,并且适于通过所述输出提供三种类型的EM波,即RF、MW和LS;所述发生器还包括处理单元,所述处理单元尤其被编程为同时发射所有三种EM波并控制它们之间的相互作用。
在优选实施例中,根据本发明的系统在同一程序期间分别提供三种不同的EM波。
根据本发明的系统可以毫秒间隔以脉冲的交替开关方式递送RF和MW。该系统可以递送RF和LS或其他EM波组合。组合的EM波可以用能够在冷却和精确的局部温度控制的情况下产生可变消融场体积的针头来施加。
在优选实施例中,该系统包括导航导管和同轴地放置在其内部的有源快速交换消融针头或导管。
导航导管被设想为能够将快速交换应答器针头精确地带入待治疗的肿瘤病变中的中枢。有源消融针头/导管在导航导管内部前进以到达待治疗的病变。操作者可以通过导航导管监测同轴快速交换针头或导管的前进,从而控制组织消融体积的尺寸。
将导航导管放置在患者皮肤上的适当位置以获得正确的引入角度。将配备有应答器的针头通过导航导管引入,由专用系统(计算机断层摄影术或回波描记术)引导,以精确定位到肿瘤病变中。然后移除应答器针头并用不同的快速交换有源消融针头或导管替换,所述快速交换有源消融针头或导管具有简单或组合的能量特征(例如,RF+MW或RF+LS),这取决于待治疗的器官或肿瘤病变的尺寸。
针头或导管优选地是小的和锥形的,以在其进入组织期间减少组织的损伤。针头/导管的尺寸和形状取决于待治疗的器官、所递送的能量的类型和功率以及冷却系统的存在与否。例如,针头可以具有16G(号)或更小的直径,并且导管可以具有6Fr(French)或更小的直径。上述针头或导管可以配备有放置在其远端处的特定冲洗系统。
根据本发明的系统的附加可选特征是放置在尖端上或替换也用作电极的尖端的机械活检取样器。该特征使得可以在消融程序之后收获组织样本以用于组织学评估。
针头/导管的使用指示用于以下列出的器官,但它不排除在其他器官中存在可能的消融区:肝、甲状腺、肾、肺、膀胱、脑、垂体、胰腺等。
导管的使用具体指示用于中空器官中的所有组织消融,例如脑血液循环血管、心脏血管(冠状动脉)、动脉或静脉血管如肺静脉(心律失常治疗)、肾血管(血压控制)、呼吸树、肝胆管、胃肠道、膀胱、上或下输尿管导管或生殖器官腔。
同时发射至少两种相关的EM波类型。发生器及其处理单元设计成在程序期间控制消融组织的温度和阻抗,这取决于所选择的能量源,所述能量源作用在放置在发生器上的冷却泵上,所述冷却泵可以基于液态CO2、制冷空气或水。

具体实施方式
图1中描述的EM波发生器1配备有屏幕2,所述屏幕示出消融参数,并且通常示出温度上升斜坡或阻抗,以及一系列EM波输出,例如RF输出3、MW输出4、LS 5以及联合RF和MW输出6。
几个光标使发生器1具有调节不同消融功能的功能。
在图2a中,示出了中空解剖导管(血管、胆管等)中的RF消融程序。金属自扩张支架网8配合导管壁7并允许任何种类的体液穿过它,从而保持导管管腔13内的流动循环。球囊状可自扩张金属支架8在近侧锚固到带正电的外部轴11,并且在远侧锚固到带正电的外部轴12。外部阴极轴11、12和具有其尖端9的内部阳极轴10是同轴且可伸缩的。外部轴具有两个部分:一个阳极10和两个阴极11、12。外部轴11相对于内部轴10的相对移动确定能够消融导管壁7的组织的可变电场的产生。
在图2b和2c中,金属支架8塌缩并且外部轴11缩回。然后闭合导管的远侧部分,从而推进导引器套管15。该系统允许冲洗14导引器套管15与内部轴和外部轴10、11、12之间的间隙。
在图3a、3b、3c中,示出了与支架植入程序关联的导管/血管7消融的示例。带正电的金属自扩张支架16包含在带负电的中空轴18中。第二顺序自扩张支架17在带负电的柔性或刚性外部轴19中塌缩。当消融单元完全部署时(图3a),可以利用支架16和中空轴18之间以及支架17和外部轴19之间的能量场执行双消融。在图3b中,通过在支架16和外部轴19之间建立能量场来执行消融。在图3c中,导管闭合并准备取回。
在另一实施例中,可以执行单个支架组织消融(图4a和4b)。带正电的支架16在部署时与外部轴19的带负电的远侧部分18建立能量场。
可以执行消融程序,并且支架在结束时留在原位以向导管提供机械支撑(图5a至5f)。首先将支架16部署到导管中并通过机械和电连接部20保持栓系到外导管18。支架16通过连接部20用作阴极,而外部轴19的远侧部分18用作阳极。在阴极和阳极之间生成能量场。在图5c中,支架留在原位。在图5d中,外部轴19被重新引入导管中,尖端9穿过支架16,系绳20被暴露并连接到支架16(图5e)。可以执行第二消融程序,并且此后可以简单地取回支架16(图5f)。该解决方案允许治疗解剖导管中的肿瘤,从而给予机械支撑以保持导管通畅性,并且同时治疗RF肿瘤病变。具有栓系系统的优点是可以多次执行肿瘤消融,将支架留在原位。在附加的实施例中,在支架再狭窄的情况下,其中发生冠状动脉或外周血管中的动脉粥样硬化斑块或导管中的肿瘤增殖,可以执行RF消融。在图5g中,示出了植入血管/导管中的支架16,并且所述支架穿有导管19,所述导管承载塌缩在内部的支架(图5h)。当导管19定位在支架16内部时,带正电的有源RF支架部署在前一个支架上。然后导管19的轴带负电,可以实现支架部分的RF消融以治疗再狭窄(图5i)。有源RF支架可以在镍钛诺或类似合金中是可自扩张的,或者当需要更多的机械支撑时可以是可球囊扩张的。
如图6a中所示,伸缩式双极针头/导管发射两个消融能量场。内部导管轴21用作阴极,而外导管轴22用作阳极。两个导管21'、22相对于彼此移动,从而产生可变消融场。在本实施例中,外部导管轴21'、22在一定长度上是扇形的,从而暴露内部导管轴21的表面。该条件产生第二消融场。利用该解决方案,生成两个连续的消融场,从而允许延伸的消融表面。为该消融导管提供若干替代方案,包括扇形消融表面的数量、长度和形状。在图6b的实施例中,针头/导管是单极的,具有接地23。外部轴22具有扇形部,示出用作阴极的内轴21。

在图9中描述了LS组织消融应用。LS光纤34用作外部多腔导管32的内部轴。在该实施例中,LS消融导管可以被流体/气体冷却以减轻组织碳化的风险,组织碳化是激光消融程序的关键问题。
LS光纤34包含在内导管轴32'中;在外部存在外导管轴32。通过将聚合物球囊的近端密封在外导管轴32的轴上并将远端密封到尖端9来获得聚合物球囊31。球囊31用作冷却剂(优选为气体)的扩张室。事实上,当LS消融程序正在进行时,光纤34可以达到相当高的温度并且必须冷却下来。冷却剂被泵送到外部轴32和内轴32'之间的间隙33中。它扩张到球囊中,并且通过内轴32'中的孔,它流回到由内轴32'和光纤34产生的间隙35内部。气体冷却系统用CO2气体或其他气体/液体维持。
在另一实施例中,如上所述的基于导管的LS以更好地导航到解剖导管(例如胆管或血管)中的方式实现,包括丝上或快速交换解决方案。
LS消融针头/导管的替代实施例在图10a、10b、11、12a和12b中描述。
在图10a和10b中,在纵向截面中描述两个针头/导管。冷却系统类似于图9中描述的冷却系统。冷却剂被泵送通过间隙33并通过间隙35返回。扩张室处于尖端9的水平。图10b中的实施例配备有靠近LS光纤34的远端定位的热电偶36。这允许获得被消融组织的真实温度测量。在图11中,在横截面中示出了图10b中描述的相同实施例。
在图12a和12b中,上述LS消融针头/导管以混合RF+LS配置表示。这意味着该装置可以同时或顺序地递送两种能量。LS针头/导管38具有用作阴极的金属内轴部分,并且插入由绝缘层39分离的用作阳极的外部轴40中。该系统是可伸缩的,因此,轴38和40的长度的相对变化可以引起由LS或RF生成的消融能量场的尺寸变化。

Claims (15)
1.一种用于实质器官、中空解剖导管或血管(7)的内部治疗的组织消融系统;所述系统包括电磁(EM)波发生器(1)和具有有源远端的导管(8-14、16-40);其特征在于,所述发生器(1)包括至少两个EM波输出(3-6),并且适于通过所述输出(3-6)提供三种类型的EM波,即射频(RF)、微波(MW)和激光(LS);所述发生器(1)还包括处理单元,所述处理单元尤其被编程为同时发射所有三种EM波并控制所述三种EM波之间的相互作用。
2.根据权利要求1所述的系统,其中,一个输出(6)是提供两种类型的EM波的联合输出。
3.根据权利要求1或2所述的系统,包括四个输出(3-6)。
4.根据权利要求1至3中任一项所述的系统,其中,所述导管包括内部轴(10、21、21'、26、32'、34、38)和外部中空轴(11、12、19、22、32、40),两个轴是同轴的并且能相对于彼此移动。
5.根据权利要求4所述的系统,其中,所述导管是基于激光的导管,其中,所述内部轴是具有自由部分的光纤(34),并且其中,激光消融场尺寸的幅度通过所述外部轴(32)的相对移动而被伸缩地调节。
6.根据权利要求5所述的系统,其中,所述基于激光的导管还包括冷却剂出口(35),所述冷却剂出口适于在所述光纤(34)的自由部分周围提供冷却剂。
7.根据权利要求6所述的系统,包括固定到所述外部轴(32)的远端的可扩张球囊(31),所述球囊(31)用作冷却剂的扩张室。
8.根据权利要求4所述的系统,其中,所述导管包括阳极部分和阴极部分。
9.根据权利要求8所述的系统,其中,所述阳极部分位于所述内部轴(10)上,并且其中,所述阴极部分位于两个外部轴(11、12)上。
10.根据权利要求9所述的系统,包括球囊状金属网(8),所述球囊状金属网位于导管的远端周围,以便传导地连接所述两个外部轴(11、12)。
11.根据权利要求9所述的系统,包括连结到所述内部轴(26)的远端的活检钳(24)。
12.根据权利要求9所述的系统,包括位于所述内部轴(35)内的天线(42),并且其中,所述导管适于单独提供MW、单独提供RF或两者的组合。
13.根据前述权利要求中任一项所述的系统,包括用于导航导管的手柄(62)和用于有源导管(8-14、16-40)或针头的手柄(63)。
14.一种与用于中空器官或血管(7)的内部治疗的组织消融系统一起使用的导管,其中,所述导管是根据权利要求5所述的基于激光的导管。
15.一种与用于中空器官或血管(7)的内部治疗的组织消融系统一起使用的导管,其中,所述导管是根据权利要求7至13中任一项所述的RF和/或MW导管。
