Abstract

本发明公开了一种取栓支架的输送系统,所述的取栓支架的输送系统由内管、中间管、外鞘管、输送短鞘、远端拦网以及近端拦网;所述的输送短鞘具有弹性缩口和限位台阶(8);所述的近端拦网由平直网管和喇叭状密网两部分构成。通过本发明提供的外周动、静脉取栓支架的输送系统,能适配取栓支架,使其可抵达大多数外周血管解剖部位;大幅度简化介入器械治疗外周血管栓塞疾病的操作难度;大幅降低器械本身引入的风险,并且消除取栓过程中的血栓逃逸风险。

Description

一种取栓支架的输送系统
技术领域
本发明属于医疗器械技术领域,涉及一种输送取栓去除器械的输送系统,特别涉及一种外周动、静脉取栓支架的输送系统。
背景技术
急性肢体缺血是指肢体灌注迅速减少,通常引发新的或逐渐加重的症状或体征,常威胁到肢体的存活力。急性下肢缺血绝大多数与动脉闭塞相关,不过广泛静脉闭塞也可导致肢体缺血(即股青肿),但这种情况罕见。急性外周动脉闭塞导致急性下肢缺血的年发生率约为1.5例/10,000人。临床表现取决于病因和患者是否有基础性外周动脉疾病(peripheral artery disease, PAD)。多年来,主要治疗方法一直是外科或基于导管的取栓术和旁路移植术。后来,溶栓治疗和经皮腔内血管成形术(percutaneous transluminalangioplasty, PTA)成为某些患者的治疗选择。
静脉血栓栓塞症(venous embolism,VTE)是指血栓造成静脉系统血运阻塞的一类疾病,诱因包括静脉壁损伤(如静脉穿刺、直接机械损伤、手术损伤等)、静脉血流异常(如血流缓慢、术后长期卧床、术中制动,麻醉药物使用)、血液成分改变(如血液粘滞度增加(围术期禁食禁饮,血容量不足)、凝血、纤溶功能异常)。静脉血栓栓塞症状早期并不明显,肉眼无法直观辨别诊断,若是忽视筛查常会导致深静脉血栓甚至肺动脉栓塞。与外周动脉血栓疾病类似,静脉血栓的治疗方式也以溶栓与外科/经导管取栓为主。
虽然经导管取栓术已经在临床上广泛应用,基于如流体力学、支架刮取、负压抽吸等多种机理的介入取栓治疗也展现出很高的成功率,但相适配的输送系统仍然相对缺乏。由于静脉回血进入心脏后直接泵入肺动脉,栓子逃逸易造成肺栓塞的风险,因此静脉取栓装置的输送系统对于防止血栓逃逸要求更高,有必要设计一套针对性的输送系统以保证高的血栓抓取率。在此,我们提出一套适配外周取栓支架的脉管内输送系统。
发明内容
本发明的目的是克服现有技术中存在的不足,提供一种通用型外周血管内取栓支架输送系统(以下简称输送系统),适用于外周动、静脉腔内取栓治疗。其目的在于将取栓支架输送至大部分外周血管解剖位置,解决因部分血管迂曲、直径较细造成取栓支架无法抵达的问题。同时,其具有近、远端双保护伞设计,可兼顾动、静脉取栓需求,避免血栓逃逸造成肺栓塞、远端动脉堵塞等不良后果。并且,该器械操作简单,便于医生学习,能够降低操作不当带来的风险。
为了实现上述目的,本发明所述输送系统采用四层式导管组合,分别为内管、中间管、外鞘管和输送短鞘。其中内管远端与取栓支架远端固定,中间管远端与取栓支架近端固定,通过内管、中间管的相对滑移实现支架的压缩与释放。所述中间管具有灵活的调弯性能,适应大部分外周血管生理曲率。所述外鞘管用于容纳压缩状态的支架,并将支架推送至目标位置。所述输送短鞘用于扩张血管提供鞘管通路,并容纳近端保护伞使血栓在近端无法逃逸,同时提供在近端的抽吸功能,进一步将血栓清除干净。
进一步地,所述输送短鞘远端具有弹性缩口,所述弹性缩口可由硅胶、pebax等弹性材质制成,所述弹性缩口能够卡住保护伞平直段,限制保护伞滑移。
进一步地,所述输送短鞘具有限位台阶,该处与管身整体材质保持一致,可通过热熔压缩、两根不同内径管材熔接等不限定可行方式制作;该台阶与保护伞限位环配合,从而限定保护伞释放位置,避免保护伞滑出短鞘。
进一步地,所述输送短鞘材质不限,包括但不限于高分子单层管、编织管、coil管、薄壁金属管等可行方式,本例优选高分子单层管举例说明。
进一步地,所述近端保护伞具有远端闭口的喇叭形密网结构,其远端大开口直径一般大于入路处血管直径,以提供较大径向支撑力抵住血管内壁,保证内腔贯通。
进一步地,所述近端保护伞的远端闭口可采用多种方式实现,包括但不限于编织丝绕回的编织方式、丝头焊接等。
进一步地,所述的近端保护伞具有近端平直段,该平直段外径与短鞘远端缩口段内径紧密配合,保证内腔贯通。
进一步地,所述近端保护伞通常为编织支架,编织丝可为具有形状记忆功能的合金丝或高分子丝。
进一步地,所述近端保护伞的近端具有双层限位环。保护伞平直段夹于两层限位环之间,并焊接牢固。外层限位环远端端面能够与短鞘限位台阶配合限定保护伞推送位置。
进一步地,所述输送短鞘和近端保护伞具有适配的扩张管12,扩张管所用材质包括但不限于PE、PEBAX、PI等可行材料。
进一步地,所述扩张管12具有推送段与装载段,两段内径一致、内腔贯通容纳导丝,外径在相接处突变形成限位面,限位面与近端保护伞的限位环近端面配合限定保护伞装载时的位置。推送段外径与短鞘大内径配合,装载段外径与短鞘小内径配合,装载段负载压缩的近端保护伞后刚好可顺畅通过短鞘限位台阶。
进一步地,所述短鞘配有操作手柄,所述手柄具有旋转锁紧式和单片硅胶密封结构,通过可相对旋转锁紧的手柄部件可将弹性体密封块压紧,从而压缩内腔与所容纳鞘管/扩张管之间的空间实现密封,所述单片硅胶密封结构为完整薄片状弹性体,其中心具有小孔,小孔可在外力下(如鞘管通过)撑大,并在自身弹力下压紧所容纳的鞘管,实现密封。
进一步地,所述远端保护伞为取栓支架组成部件,连接于取栓支架远端,与近端保护伞为类似的编织密网结构,在保证血流通过的同时,在远端阻止栓子逃逸。工作时,近端与远端保护伞同时撑开,紧贴血管壁,使血栓被限制于两伞之间,保证血栓在抓取全过程中不会逃逸。
进一步地,所述中间管可选地为蛇骨管、弹簧管、薄壁高分子管等,其具有灵活的过弯性能,适应大部分外周血管生理曲率。
与现有技术相比,本发明具有的有益效果:通过本发明提供的外周动、静脉取栓支架的输送系统,能适配取栓支架,使其可抵达大多数外周血管解剖部位;大幅度简化介入器械治疗外周血管栓塞疾病的操作难度;大幅降低器械本身引入的风险,并且消除取栓过程中的血栓逃逸风险。

具体实施方式
为使本发明的上述目的、特征和优点能够更加明显易懂,下面结合附图对本发明的具体实施方式做详细的说明。在详述本发明实施例时,为了便于说明,示意图不依一般比例局部放大,不应以此作为对本发明的限定。
本发明专利中所涉及的“近端”是指距离该取栓支架的输送系统的操作者较近的一端,“远端”是指距离该取栓支架的输送系统的操作者较远的一端。
如图1所示,本发明的输送系统包含内管(1)、中间管(2)、外鞘管(3)、输送短鞘(4)、远端拦网(5)以及近端拦网(6);同时,如图2所示,与输送系统适配的一款取栓支架所述的取栓支架的输送系统由该支架具有远端保护伞,保护伞通过缠结、焊接等可行方式与切割支架相连形成取栓支架的整体。工作时,保护伞随切割支架一起膨大,其外径紧贴血管内壁,从而阻挡血栓飘向远端。
如图3和4所示:近端保护伞为喇叭形编织支架,其收口处为闭环,避免损伤血管。保护伞近端为一平直段,平直段的端面夹于两层限位环之间,并焊接牢固。外层限位环的远端端面能够与短鞘限位台阶配合限定保护伞推送位置。保护伞通常使用编织机或手工编织架编织而成,其网孔大小根据临床需求调整,确保血栓不会逃逸。
输送短鞘具有弹性缩口,自然状态下缩口头端直径小于短鞘的小内径,当近端保护伞被推出后,缩口的压缩力可以限制保护伞位移。同时,短鞘具有限位台阶,短鞘在台阶远端和近端具有不同内径(小内径和大内径),限位台阶与外层限位环的远端端面配合限制保护伞滑出短鞘。
扩张管具有限位面以配合输送短鞘和保护伞,扩张管在限位面远端与近端具有不同外径(小外径和大外径),分别为装载段和推送段。保护伞压缩于装载段送入短鞘并推送出管口释放。保护伞近端的限位环端面与限位面配合限定保护伞位置。
本发明的输送系统在合适的尺寸下可以适合于其它取栓支架的输送。本领域技术人员可以理解,以上描述只是示例性的。即凡依本发明申请专利范围的内容所作的等效变化与修饰,都应为本发明的技术范畴。

1.一种取栓支架的输送系统,其特征在于:所述的取栓支架的输送系统由内管(1)、中间管(2)、外鞘管(3)、输送短鞘(4)、远端拦网(5)以及近端拦网(6);所述的输送短鞘具有弹性缩口(7)和限位台阶(8);所述的近端拦网由平直网管(9)和喇叭状密网(10)两部分构成。
2.根据权利要求1所述的取栓支架的输送系统,其特征在于:所述的中间管具有360°转弯性能。
3.根据权利要求1所述的取栓支架的输送系统,其特征在于:所述的近端拦网具有双层限位环(11)。
4.根据权利要求1所述的取栓支架的输送系统,其特征在于:所述的弹性缩口由硅胶、聚醚嵌段酰胺中的一种构成。
5.根据权利要求1所述的取栓支架的输送系统,其特征在于:所述的输送短鞘为高分子管、编织管、线圈管以及金属管中的一种。
6.根据权利要求1所述的取栓支架的输送系统,其特征在于:所述的远端拦网与取栓支架的远端相连接。
