背景技术
胰腺癌(pancreatic cancer,PC)仍然是最致命的疾病之一,并且在2019年内在美国造成56,770人死亡,占所有癌症死亡率的7%。尽管最近加强了对免疫治疗和纳米药物治疗的研究,但PC患者的预后非常差,其中5年存活低于8%。
发明内容

具体实施方式
迄今为止,胰腺癌(PC)仍然是最致命的疾病之一,在2019年内在美国造成56,770人死亡,占所有癌症死亡率的7%。尽管最近加强了对免疫治疗和纳米药物治疗的研究,但PC患者的预后非常差,其中5年存活低于8%。这些不期望的结果主要是由于PC肿瘤的免疫抑制性肿瘤微环境(tumor microenvironment,TME),其特征在于结缔组织增生性基质和血管形成不良。这样的TME产生了阻止T细胞或纳米药物有效浸润肿瘤且与PC细胞直接相互作用的物理屏障,导致不利的效力。这强调了开发可更好地浸润PC肿瘤同时维持强效的肿瘤特异性效力的新靶向治疗的迫切需要。
已表明抗体药物缀合物(ADC)针对数种类型的癌症,包括对T细胞免疫治疗响应不佳的侵袭性实体瘤(如乳腺癌)有前景的临床效力。尽管已经利用常规PC靶标(例如EGFR、EpHA2和间皮素)开发了数种靶向PC的ADC,但仍然缺乏对建立的PC靶标和其他候选物在其细胞表面蛋白水平上的系统性和定量性比较。

本公开内容的一些方面提供了包含胞间黏附分子1(ICAMl)和药物的抗体-药物缀合物(ADC)、在胰腺癌治疗中使用所述抗体-药物缀合物(ADC)的方法、以及预测对用ICAM1抗体或ICAM1抗体-药物缀合物(ADC)治疗的响应性的方法。
抗体-药物缀合物(ADC)
I.ICAM1抗体
抗体-药物缀合物(ADC)是一类包含与药物缀合的抗体的免疫治疗剂。本公开内容的ADC可靶向表达ICAM1的细胞。ICAM1是细胞表面糖蛋白,其已显示出结合CD11a/CD18或CD11b/CD18类型的整合素,并且已参与介导细胞-细胞相互作用并促进白细胞内皮迁移。ICAM1也称为ICAM-1、BB2、分化簇54(CD54)和P3.58。

本文中所述的单克隆抗体涵盖“嵌合”抗体,其中重链和/或轻链的一部分与来源于特定物种或属于特定抗体类别或亚类的抗体中的对应序列相同或同源,而所述链的其余部分与来源于其他物种或属于其他抗体类别或亚类的抗体以及这样的抗体的片段中的对应序列相同或同源,只要它们表现出期望的生物活性即可(参见美国专利No.4,816,567;以及Morrison et al.,Proc.Natl.Acad.Sci.USA,81:6851-6855(1984))。本文中的目的嵌合抗体包括包含来源于非人灵长类(例如,旧大陆猴(Old World Monkey)、猿(Ape)等)的可变结构域抗原结合序列和人恒定区序列的“灵长类化”抗体。
在一些实施方案中,抗体是多克隆抗体。“多克隆抗体”是与抗原的多于一个免疫原性决定子反应的不同抗体分子的混合物。多克隆抗体可从哺乳动物的血液、分泌物或其他流体或者从卵中分离或纯化。多克隆抗体也可以是重组的。重组多克隆抗体是通过使用重组技术产生的多克隆抗体。重组产生的多克隆抗体通常包含高浓度的不同抗体分子,其全部或大部分(例如,多于80%、多于85%、多于90%、多于95%、多于99%或更多)都对由多于一个表位构成的抗原显示出所期望的结合活性。

Fv片段是包含完整的抗原识别和结合位点的最小抗体片段。该片段由紧密非共价缔合的一条重链和一条轻链可变区结构域中的二聚体组成。从这两个结构域的折叠中,产生为抗原结合贡献了氨基酸残基并对抗体赋予了抗原结合特异性的六个高变环(来自H和L链各3个环)。然而,即使单个可变结构域(或者仅包含对抗原具有特异性的三个CDR的Fv的一半)也具有识别和结合抗原的能力,尽管亲和力低于整个结合位点。

本公开内容的抗体包括抗体模拟物,包括亲和体分子。亲和体是包含三螺旋束的小蛋白质,其作为抗原结合分子(例如,抗体模拟物)发挥作用。通常,亲和体的长度为约58个氨基酸并且摩尔质量为约6kDa。具有独特结合特性的亲和体分子是通过随机化位于两个α螺旋中的13个氨基酸获得的,所述α螺旋涉及亲本蛋白结构域的结合活性。结合期望的靶蛋白的特异性亲和体分子可使用方法例如噬菌体展示从包含数十亿不同变体的库(文库)中分离。
在一些实施方案中,ICAMl抗体与存在于ICAM1的胞外部分中的表位结合。ICAM1的“胞外部分”是指ICAMl的在胞质外和在细胞表面上的部分(与胞质内的部分相反)。ICAM1的胞外部分通常包含

单克隆抗体通常通过细胞培养来制备,其涉及将骨髓瘤细胞与用期望的抗原免疫接种的小鼠脾细胞进行融合(即,杂交瘤技术)。将细胞混合物稀释,并且使克隆在微滴定孔上从单亲本细胞中培养。然后测定由不同克隆分泌的抗体其与抗原(用例如ELISA或抗原微阵列测定的测试)或免疫斑点印迹结合的能力。然后选择最高生产力且稳定的克隆用于将来使用。
II.药物
适用于ADC的药物包括针对胰腺癌有治疗活性的药剂。药物的一些非限制性实例包括化学治疗。在一些情况下,药物是小分子。在一些实施方案中,药物是细胞毒性小分子。在一些实施方案中,药物是细胞抑制小分子。
适用于ADC的药物的一些非限制性实例包括N2’-脱乙酰基-N2’-(3-巯基-1-氧代丙基)美登素(DM1)、单甲基奥瑞他汀E(MMAE)、单甲基奥瑞他汀F(MMAF)、和倍癌霉素、紫杉醇、依维莫司(everolimus)、氟尿嘧啶(5-FU)、吉西他滨、盐酸吉西他滨、丝裂霉素C、及其衍生物。在一些实施方案中,药物是美登素或其类似物。在一些实施方案中,药物是DM1。DM1是已显示出抑制微管蛋白聚合的细胞毒性美登素类似物。在一些实施方案中,美登素类似物是DM4。

III.接头
可使用本领域已知的技术将一种或更多种药物与ICAM1抗体缀合。在一些实施方案中,多种(例如如2、3、4、5、6、7、8、9、10或更多种)药物与ICAM1抗体缀合。ADC中ICAM1抗体与药物的比例可以是1∶1至1∶10(例如1∶1、1∶2、1∶3、1∶4、1∶5、1∶6、1∶7、1∶8、1∶9或1∶10)。在一些实施方案中,ADC中ICAM1抗体与药物的比例是1∶4。
ICAM1抗体可直接或通过接头与第二实体缀合。本文中使用的“缀合”或“连接”意指两个实体优选地以实现两个实体之间缔合的治疗或诊断益处的足够的亲和力缔合。在一些实施方案中,接头在ADC中将ICAM1抗体与药物缀合。ICAM1抗体的N端或C端可与药物缀合。在一些实施方案中,接头可用于将ICAM1抗体与成像剂缀合。ICAM1抗体的N端或C端可与成像剂缀合。
在一些实施方案中,接头是可切割接头。本文中使用的可切割接头能够响应于刺激释放缀合部分。在一些实施方案中,刺激是生理刺激。刺激的一些非限制性实例包括酶的存在、酸性条件、碱性条件或还原条件。例如,可切割接头包括肽接头、β-葡糖苷酸接头、谷胱甘肽敏感性接头(或二硫化物接头)和pH敏感性接头。在一些实施方案中,pH敏感性接头在5.0至6.5的pH或4.5至5.0的pH下被切割。在一些实施方案中,当pH为7至7.5时,pH敏感性接头不被切割。在一些实施方案中,当pH为7.3至7.5时,pH敏感性接头不被切割。在一些实施方案中,可切割接头是蛋白酶敏感性接头。
可切割接头的一些实例包括N-琥珀酰亚胺基4-(2-吡啶基二硫代)戊酸酯(SPP)、N-琥珀酰亚胺基3-(2-吡啶基二硫代)丁酸酯(SPDB)、磺基-SPDB、缬氨酸-瓜氨酸二肽(Val-cit)、乙酰丁酸酯和CL2A。在一些实施方案中,可切割接头是Val-cit。另参见,例如,Donaghy,MAbs.2016May-Jun;8(4):659-71。
在一些实施方案中,接头是不可切割的。在一些实施方案中,不可切割接头是在对象中的体循环内不切割的接头。在一些实施方案中,不可切割接头是对蛋白酶切割具有抗性的接头。不可切割接头包括N-琥珀酰亚胺基4-(N马来酰亚胺基甲基)环己烷-1-羧酸酯(SMCC)和马来酰亚胺基甲基环己烷-1-羧酸酯(MCC)。在一些实施方案中,不可切割接头是N-琥珀酰亚胺基4-(N马来酰亚胺基甲基)环己烷-1-羧酸酯(SMCC)接头。
可使用本领域已知的方法来合成任何抗体-药物缀合物。参见,例如,Yao et al.,Int J Mol Sci.2016 Feb 2;17(2).pii:E194。
包含与药物缀合的ICAM1抗体的ADC也有利于治疗性使用,部分是因为药物(例如化学治疗药物)具有毒性并引起严重的副作用。通过将药物(例如,DM1)与ICAM1抗体缀合,ADC的毒性与其游离形式的药物相比可降低至少20%、至少30%、至少40%、至少50%、至少60%、至少70%、至少80%、至少90%、至少99%。
其他ICAM1抗体缀合物

可使用合适的检测方法(例如,通过CT、PET、MRI、超声和/或内窥镜检测)使成像剂显现。
药物组合物及其用途
包含本文中公开的ADC或其他ICAM1抗体缀合物中任一种的组合物涵盖在本公开内容中。在一些实施方案中,所述组合物被配制成用于向对象施用的药物组合物。
对象可以是患有胰腺癌、怀疑患有胰腺癌或处于患胰腺癌的风险中。将胰腺癌根据启动肿瘤的细胞类型进行分类。最常见的胰腺癌类型是胰腺腺癌(pancreaticadenocarcinoma),其为胰腺外分泌的癌症。相比之下,胰腺神经内分泌肿瘤(pancreaticneuroendocrine tumor,NET)或胰岛细胞肿瘤开始于神经内分泌细胞。
也可将胰腺癌根据癌症是否已转移进行分层。胰腺癌可以是0期(原位癌)、I期、II期(例如IIA期或IIB期)、III期或(IV)期。一种非限制性的分期方法是TNM系统,它评价肿瘤的程度(T)、癌症向附近淋巴结的扩散(N)以及癌症是否已扩散到远处部位(M)。然后可使用多种T、N和M水平(例如,表1)来确定胰腺癌的分期(例如,表2)。表1至2示出了基于第八版AJCC/UICC TNM分期系统以及如由Cong et al.Sci Rep.2018 Jul 10;8(1):10383所述的胰腺肿瘤分类。

在一些实施方案中,施用本文中公开的ICAM1抗体和/或包含ICAM1抗体的ADC或其药物组合物不降低健康细胞的生存力。在一些实施方案中,施用本文中公开的ADC或包含ADC的药物组合物允许药物的有效量(例如浓度)低于药物未与ICAM1抗体缀合的情况。在一些实施方案中,与施用单独药物相比,药物的有效量降低了至少5%、至少10%、至少20%、至少30%、至少40%、至少50%、至少60%、至少70%、至少80%、至少90%、至少100%、至少200%、至少300%、至少400%、至少500%、至少600%、至少700%、至少800%、至少900%或至少1,000%。

药物组合物可方便地以单位剂量形式存在,并且可通过药学领域中公知的任何方法来制备。术语“单位剂量”当关于本公开内容的药物组合物使用时,是指适合作为用于对象的单一剂量的物理上离散的单元,每个单元包含与所需稀释剂(即,载体或载剂)缔合的经计算产生所期望治疗作用的预定量活性物质。
药物组合物的制剂可取决于施用的途径。适合于肠胃外施用或瘤内、瘤周围、病灶内或病灶周围施用的可注射制剂包括例如无菌可注射水性或油性混悬剂,并且可根据己知技术使用合适的分散剂或润湿剂和助悬剂来配制。无菌可注射制剂还可以是在无毒的肠胃外可接受的稀释剂或溶剂中的无菌可注射溶液剂、混悬剂或乳剂,例如如1,3-丙二醇或1,3-丁二醇中的溶液剂。在可用的载剂和溶剂中,可使用水、林格液、U.S.P.和等张氯化钠溶液。另外,无菌的固定油通常用作溶剂或助悬介质。出于这个目的,可使用任何温和的固定油,包括合成的甘油单酯或甘油二酯。另外,脂肪酸(例如油酸)可用于可注射剂的制剂中。可对可注射制剂进行灭菌,例如,通过细菌保留过滤器进行过滤,或者通过引入无菌固体组合物形式的灭菌剂,所述无菌固体组合物在使用之前可溶解或分散在无菌水或其他无菌可注射介质中。
适合于经口施用的组合物可以以各自包含预定量抗炎剂的离散单元(例如胶囊剂、片剂、锭剂)存在。其他组合物包括水性液体或非水性液体中的混悬剂,例如糖浆剂、酏剂或乳剂。
在一些实施方案中,用于治疗性施用的药物组合物必须是无菌的。无菌通过无菌过滤膜(例如,0.2微米膜)通过过滤来容易地实现。或者,可使用防腐剂以防止微生物的生长或作用。多种防腐剂是公知的,并且包括例如苯酚和抗坏血酸。如果药物组合物对热变性和氧化变性高度稳定,则通常将药物组合物以冻干形式或作为水性溶液储存。制剂的pH通常将为约6至8,尽管更高或更低的pH值在某些情况下也可适合。
本文中使用的“治疗有效量”或“有效量”是指赋予对象治疗效果所需的单独或与一种或更多种其他治疗剂组合的本公开内容的每种治疗剂(例如,用于治疗本文中所述的任何脑病的治疗剂)的量。如本领域技术人员所认识到的,有效量根据所治疗的具体病症、病症的严重程度、个体对象参数(包括年龄、身体状况、大小、性别和体重)、治疗持续时间、同时治疗的性质(如果有的话)、特定的施用途径等健康从业者的知识和专业知识中的因素而变化。这些因素是本领域普通技术人员所公知的,并且可仅通过常规实验来解决。通常优选使用最大剂量的单个组分或其组合,即根据合理的医学判断的最高安全剂量。然而,本领域普通技术人员将理解,对象可出于医学原因、心理原因或实际上任何其他原因坚持较低剂量或可耐受剂量。
经验考虑因素(例如半衰期)通常将有助于确定剂量。例如,与人免疫系统相容的治疗剂(例如包含来自人源化抗体或完全人抗体的区域的多肽)可用于延长多肽的半衰期并防止多肽被宿主的免疫系统攻击。施用频率可在治疗过程中确定和调整,并且通常但不是必须基于疾病的治疗和/或抑制和/或改善和/或延迟。或者,多肽的持续连续释放制剂可以是合适的。用于实现持续释放的多种制剂和装置是本领域中已知的。
在一些实施方案中,剂量是每天、每隔一天、每三天、每四天、每五天或每六天。在一些实施方案中,给药频率是每周一次、每2周一次、每4周一次、每5周一次、每6周一次、每7周一次、每8周一次、每9周一次或每10周一次;或每月一次、每2个月一次,或每3个月或更长时间一次。通过常规技术和测定可容易地监测该治疗的进展。给药方案(包括所使用的抗癌剂)可随时间变化。
在一些实施方案中,对于正常体重的成年对象,可施用约0.01至1000mg/kg的剂量。在一些实施方案中,剂量为1至200mg。具体的剂量方案(即,剂量、时机和重复)将取决于特定对象和对象的病史以及抗癌剂的特性(例如抗癌剂的半衰期,以及本领域中公知的其他考虑因素)。
出于本公开内容的目的,如本文中所述的治疗剂的合适剂量将取决于所用的具体药剂(或其组合物)、制剂和施用途径、疾病的类型和严重程度,无论抗癌剂是否施用用于预防或治疗目的,先前的治疗、对象的临床病史和对拮抗剂的响应,以及主治医师的判定如何。通常来说,临床医师将施用抗癌剂直至实现所期望结果的剂量。一种或更多种抗癌剂的施用可以是连续的或间歇的,这取决于例如接受者的生理状况,施用的目的是治疗性的还是预防性的,以及技术人员已知的其他因素。抗癌剂的施用可在预选的时间段内基本上连续,或者可以是一系列间隔剂量,例如在疾病发生之前、期间或之后。
本文中使用的术语“治疗”是指向有此需要的对象施加或施用抗癌剂。“有此需要的对象”是指患有疾病、具有疾病的症状或对疾病有倾向的个体,其目的是治疗、治愈、减轻、缓解、改变、补救、改善、改进或影响疾病、疾病的症状或对疾病的倾向。
考虑施用的“对象”是指人(即任何年龄组的男性或女性,例如,儿科对象(例如,婴儿、儿童或青少年)或成人对象(例如,年轻成年人,中年成人或老年成人))或非人动物。在一些实施方案中,非人动物是哺乳动物(例如,啮齿动物(例如,小鼠或大鼠)、灵长类动物(例如,食蟹猴或恒河猴),商业相关的哺乳动物(例如,牛、猪、马、绵羊、山羊、猫或犬)或鸟(例如,商业相关的鸟,例如鸡、鸭、鹅或火鸡)。非人动物可以是处于发育任何阶段的雄性或雌性。非人动物可以是转基因动物或遗传改造动物。
在一些实施方案中,对象是伴侣动物(宠物)。本文中使用的“伴侣动物”是指宠物和其他家畜。伴侣动物的一些非限制性实例包括犬和猫;牲畜,例如马、牛、猪、绵羊、山羊和鸡;以及其他动物,例如小鼠、大鼠、豚鼠和仓鼠。在一些实施方案中,对象是研究动物。研究动物的一些非限制性实例包括:啮齿动物(例如,大鼠、小鼠、豚鼠和仓鼠)、兔或非人灵长类动物。
减轻疾病(例如癌症)包括延迟疾病的发生或进展,或降低疾病严重程度。减轻疾病并不一定需要有疗效的结果。如其中使用的“延迟”疾病的发生意味着延迟、阻碍、减慢、延缓、稳定和/或推迟疾病的进展。这种延迟可以是不同的时间长度,这取决于病史和/或被治疗的个体。“延迟”或减轻疾病发生或延迟疾病发作的方法是这样的方法,其与不使用该方法相比,在给定时间框架内降低发生一种或更多种疾病症状的可能性和/或降低症状的程度。这样的比较通常基于使用足以给出统计学上显著结果的许多对象的临床研究。
疾病的“发生”或“进展”意指疾病的初始表现和/或随后的进展。使用本领域中公知的标准临床技术可检测和评估疾病的发生。然而,发生也指可无法检出的进展。对于本公开内容的目的,发生或进展是指症状的生物学过程。“发生”包括发生、复发和发作。本文中使用的疾病的“发作”或“发生”包括初始发作和/或复发。

实施例
介绍
迄今为止,胰腺癌(PC)仍然是最致命的疾病之一,在2019年内在美国造成56,770人死亡,占所有癌症死亡率的7%。尽管最近加强了对免疫治疗和纳米药物治疗的研究,但PC患者的预后非常差,其中5年存活低于8%。例如,免疫检查点阻断治疗包括细胞毒性T淋巴细胞相关抗原4(cytotoxic T-lymphocyte-associated antigen 4,CTLA4)抑制剂或程序性死亡配体1(Programmed death-ligand 1,PD-L1)抗体迄今尚未在治疗PC患者方面显示出足够的临床效力。创新的纳米药物制剂(例如,靶向EphA2的脂质体多西他塞(docetaxel))也未能为治疗晚期PC带来临床益处。这些不期望的结果主要是由于PC肿瘤的免疫抑制性肿瘤微环境(TME),其特征在于结缔组织增生性基质和血管形成不良。这样的TME产生了阻止T细胞或纳米药物有效浸润肿瘤并与PC细胞直接相互作用的物理屏障,导致不利的效力。这强调了开发可更好地浸润PC肿瘤同时维持强效的肿瘤特异性效力的新的靶向治疗的迫切需要。
抗体-药物缀合物(ADC)是一类快速增长的免疫治疗剂,其已显示出针对数种类型的癌症,包括对T细胞免疫治疗响应不佳的侵袭性实体瘤(如乳腺癌)有前景的临床效力。与常规的化学治疗不同,ADC利用化学接头将细胞毒性药物与肿瘤归巢抗体缀合,所述抗体能够选择性地归巢肿瘤同时通过识别肿瘤表面抗原保护正常组织,随后将细胞毒性药物内化并递送到靶向的肿瘤细胞中。与T细胞免疫治疗(例如,嵌合抗原受体T细胞(chimericantigen receptor-T cell,CAR-T)或免疫检查点阻断)或者纳米药物(例如,脂质体或外排体)相比,ADC由于其是T细胞尺寸约1,000倍小的超小尺寸(<10nm)而具有优异的肿瘤组织穿透性,这为提高药物递送到基质致密的PC肿瘤中创造了有吸引力的机会。
然而,开发靶向PC的ADC的主要障碍是鉴定有效区分PC与正常组织的合适的免疫治疗靶标。为了满足最佳ADC的安全性和效力标准,需要在正常组织中无法检出的水平的情况下将这样的靶标大量呈递在PC肿瘤的细胞表面,使其可评估为ADC的肿瘤归巢抗体。同时,还需要促进ADC上缀合的细胞毒性载荷的快速和稳健的细胞内化。尽管已经利用常规的PC靶标(例如EGFR、EpHA2和间皮素)开发了数种靶向PC的ADC,但仍然缺乏对建立的PC靶标和其他候选物在其细胞表面蛋白水平上的系统性和定量性比较。本公开内容显示,对细胞表面蛋白进行这样的无偏倚和定量筛选导致发现更优化的PC免疫治疗靶标并促进靶向PC的ADC的开发。
ICAM1,也称为CD54,是免疫球蛋白超家族的跨膜糖蛋白,其在多种类型的癌症(例如三阴性乳腺癌)中异常过表达,并且经常与侵袭性表型和较差的预后相关。在PC中,ICAM1通过KRASG12D突变直接在胰腺腺泡细胞上被诱导,该KRASG12D突变是70至95%的PC患者中最常见的致癌突变,并驱动胰腺赘生物病变的形成,导致PC肿瘤发生。本公开内容描述了基于无偏和定量筛选算法将ICAM1鉴定为和应用为潜在PC免疫治疗靶标。因此,开发了在体内诱导强效且持久的PC肿瘤消退的ICAM1 ADC。为了开发精准药物,设计了鉴定适合于ICAM1靶向免疫治疗的ICAM1表达肿瘤的非侵入性MRI方法。
结果和讨论
ICAM1是合理鉴定的人PC的细胞表面蛋白靶标。
为了鉴定将恶性PC肿瘤与正常组织区分开的合适的蛋白质靶标,开发了合理设计的细胞表面蛋白靶标发现算法(图1A)。首先,在四种已建立的人PC细胞系(PANC-1、BxPC-3、Capan-1和Capan-2)中,对72种癌症相关表面抗原组进行了无偏和定量筛选,将其与作为正常对照的两种正常人胰管上皮细胞(HPDE和HPNE)进行了比较(图1B)。在68种所筛选的靶标中,发现31种候选物在所有四种PC细胞系中普遍过表达,并被选择用于进一步评价。通过比较其在人PC细胞和正常胰腺细胞中的水平,ICAM1成为前10种候选物中最过表达的PC靶标,并且在非赘生性HPNE和HPDE细胞中几乎未表达(图1C)。在四种PC细胞系上,ICAM1的细胞表面密度为3×105至1×106个分子/细胞,显著高于已建立的PC靶标(例如,EGFR、MUC1或EphA2)的那些。另外,ICAM1在所有四种受试的人PC细胞系中普遍过表达,表明PC患者中有广泛的靶标群体。使用免疫荧光(IF)染色进一步确定了人PC细胞中ICAM1的过表达。ICAM1主要在四种PC细胞系(PANC-1、BxPC-3、Capan-1和Capan-2)的质膜上表达,但在正常HNPE和HPNE细胞中不存在(图1D)。ICAM1在人PC细胞上的这种强细胞表面表达使其易于评估ICAM1靶向免疫治疗(例如ADC或CAR-T细胞)。
为了研究高ICAM1表达是否是人PC中的临床相关发现,在80个人PC肿瘤组织和20个正常胰腺组织中进行了ICAM1的免疫组织化学(IHC)染色。在图1E和图1F中,ICAM1在来自不同疾病分期的肿瘤组织的PC细胞的质膜上和胞质中始终过表达,而ICAM1在正常人胰腺组织中完全不存在。ICAM1的染色程度和病理评分显示ICAM1水平与疾病TNM分期正相关(图1G)。还评价了ICAM1靶向免疫治疗在正常组织中的中靶、肿瘤外位点(on-target,off-tumor site)。通过查询人蛋白质图谱数据库(Human Protein Atlas database)(proteinatlas.org)在45个正常人器官的综合组群中检查了ICAM1的蛋白质水平。通过IHC分析发现大多数正常组织中不存在ICAM1表达,并且仅4%(2/45,肺和肾)的正常组织分别显示出ICAM1的高阳性染色。这表明肺和肾是ICAM1靶向免疫治疗的潜在中靶、肿瘤外位点。此外,先前的工作显示,ICAM1靶向T细胞免疫治疗在晚期甲状腺癌模型的雄性和雌性小鼠二者中均未诱导任何急性的或延迟毒性。
通过查询R2:基因组学分析和可视化平台数据库(hgserver1.amc.nl/,数据表:Mixed Pancreas Tumor-Zhang),研究了ICAM1过表达对PC患者临床结局的影响。具有高ICAM1表达的PC患者的总存活显著糟于具有低ICAM1表达的患者(图1H,P=0.021,对数秩检验),表明ICAM1可用作PC患者中不良预后的临床生物标志物。
ICAM1抗体在体内识别和靶向PC肿瘤。
为了评估ICAM1作为潜在免疫治疗靶标,首先在原位PC肿瘤模型中确定ICAM1抗体的体内肿瘤特异性(图2A)。将ICAM1单克隆抗体用红色荧光染料AF-647进行荧光标记,并静脉内注射到荷PANC-1瘤小鼠中。经AF-647标记的IgG(IgG-AF)用作非靶向对照。由于体内荧光信号受到原位PC肿瘤腹膜内位置和腹部皮肤吸收的干扰的事实,在注射24小时之后将动物安乐死并且切除PC肿瘤及其周围的胰腺组织。然后,进行离体成像以确定ICAM1-AF抗体的肿瘤累积。如图2B中观察到的,与非靶向IgG对照相比,ICAM1抗体以高亲和力选择性地识别和靶向原位PC肿瘤。与PC肿瘤相邻的正常胰腺组织未被ICAM1抗体靶向,这进一步确定了其PC肿瘤特异性。量化的荧光信号(图2C)确定了在一次单剂量尾静脉施用之后,ICAM1抗体的肿瘤累积是非靶向IgG-AF的肿瘤累积的约6倍高。这些体内发现强烈支持了基于ICAM1抗体的免疫治疗用于PC靶向治疗的开发。
鉴于细胞进入活性是ADC设计中的关键因素,因此使用成像流式细胞术测定研究了ICAM1抗体在人PC细胞中的细胞内化。如图2D中所示,藻红蛋白(PE)缀合的ICAM1抗体通过ICAM1抗原介导的内吞被PANC-1和BxPC-3细胞二者稳健地内化,然而由于ICAM1抗原表达的显著缺乏,几乎无PE-ICAM1抗体被正常HPNE细胞内化。被人PC细胞内化的PE-ICAM1抗体的量被量化为被HPNE细胞内化的PE-ICAM1抗体的量的约300倍高(图2E)。
接下来,在人PC细胞中阻断ICAM1信号传导级联的治疗结果使用其中和抗体进行研究。在图2F中,用ICAM1中和抗体(2μg/mL)的处理未明显改变PANC-1或BxPC-3细胞增殖。然而,与IgG对照相比,ICAM1中和抗体强效地抑制了PANC-1和BxPC-3细胞迁移,其分别使PANC-1和BxPC-3细胞的细胞迁移降低了39%和44%(图2G)。相关地,也报道了ICAM1中和抗体在体内强效抑制PC肿瘤的发生。这些发现表明ICAM1可用作PC肿瘤归巢靶标。其还表明靶向ICAM1信号传导级联也可阻碍疾病进展。
ICAM1抗体-药物缀合物的合理设计
为了将ICAM1靶标转化为PC治疗,将合理设计的ICAM1 ADC设计为靶向PC的治疗的免疫治疗剂(图3A)。鉴于化学接头和细胞毒性载荷基本上影响了ADC的效力,因此第一步使用无偏和定量筛选方法来选择用于PC治疗的最佳ADC制剂。使用四种经临床测试的ADC接头和细胞毒性载荷(SMCC-DM1、Vc-MMAE、Mc-MMAF、倍癌霉素)以为1的等同的药物与抗体比例(drug-to-antibody ratio,DAR)改造了一系列ICAM1 ADC,并且与非靶向IgG ADC对照相比比较了它们针对人PC细胞的细胞毒性。如图3B所示,ICAM1-SMCC-DM1在处理PANC-1细胞中的四种受试ADC制剂中显示出最低的IC50(38.1nM)(其他IC50:83.9至240.4nM)。ICAM1-SMCC-DM1的IC50是PDAC治疗的一线化学治疗剂Gem(89.1μM)的2,000多倍低。SMCC-DM1是由不可切割的化学接头和强效的微管抑制剂美登素(DM1)组成的经临床验证的ADC制剂。因此,选择SMCC-DM1作为优化的ADC制剂,并随后合成ICAM1-SMCC-DM1(ICAM1-DM1)作为用于靶向PC的治疗的优化的ICAM1 ADC。IgG-SMCC-DM1(IgG-DM1)也在与非靶向对照相同的实验条件下制备。ICAM1-DM1和IgG-DM1的药物与抗体比例(DAR)通过DM1和抗体的输入量来控制,并且对于ICAM1-DM1达到了3.4,对于IgG-DM1达到了3.2,如使用UV/VIS光谱测定确定的。
ICAM1-DM1在体外和在体内选择性地消融PC细胞。
在两种人PC细胞(PANC-1和BxPC-3)和正常HPNE细胞中确定ICAM1-DMI的PC特异性细胞毒性(图3C至3E)。一线化学药物GEM和非靶向IgG-DM1用作对照。如图3D和图3E中观察到的,ICAM1-DM1显示出针对PANC-1和BxPC-3细胞的强效细胞毒性。针对PANC-1确定的ICAM1-DM1的IC50为9.8nM并且针对BxPC-3确定的ICAM1-DM1的IC50为4.0nM,显著低于GEM和IgG-DM1的那些(30nM至88μm)。此外,ICAM1-DM1显示在正常HPNE细胞中无细胞毒性,由于其缺乏ICAM1的表达(图3E)。这些体外结果强烈支持了在PC模型的体内环境中评价ICAM1-DM1的抗肿瘤活性。
在体内抑制原位PC肿瘤生长中检查了ICAM1-DM1的抗肿瘤活性(图3F)。将ICAM1-DM1或IgG-DM1(非靶向对照)以15mg/kg每3周在荷PANC-1-Luc瘤小鼠中静脉内施用。相比之下,由于GEM循环半衰期短(0.28小时),GEM以5mg/kg的剂量每周静脉内施用。在两次ADC注射之后,与其他组相比,经ICAM1-DM1处理组表现出强效且持久的肿瘤消退(图3G至3H)。量化的肿瘤质量显示,与PBS(假)组相比,ICAM1-DM1使PC肿瘤生长显著降低了49%(图3I)。通过测量PC肿瘤组织中细胞增殖标志物Ki67的表达来进一步检查ICAM1-DM1诱导毒性的机制。如图3K和图3L中观察到的,与其他组相比,经ICAM1-DM1处理组中Ki67阳性细胞群显著降低,这有助于强效且持久的肿瘤抑制。ICAM1-DM1的这种强效抗肿瘤活性还有效地抑制了自发的PC向正常器官包括肺、肝和脾的转移(图3M和图5)。没有从经ICAM1-DM1处理组收集的正常重要器官的组织病理学损伤的证据。
通过MRI非侵入性地评价肿瘤ICAM1的表达。
为了构建精准药物,开发了基于MRI的分子成像方法以用于非侵入性地和快速地鉴定可受益于ICAM1靶向免疫治疗的PC患者。在临床实践中,通常在靶向治疗之前采用针刺活检来检查肿瘤组织中靶标表达的充分性,但由于瘤内的复杂性和异质性,这种方法被其侵入性和缺乏准确性(<50%)限制。为了克服这些障碍,开发了ICAM1靶向MRI探针,并将其用于使用MRI绘制原位PC模型中的肿瘤ICAM1表达(图4A)。ICAM1靶向MRI探针最初是通过将ICAM1抗体与临床使用的MRI造影剂DTPA-Gd共价缀合来改造的。制备IgG-Gd作为非靶向对照。然后以5mg/kg小鼠重量的剂量将ICAM1-Gd或IgG-Gd静脉内施用到荷ICAM1表达PANC-1瘤小鼠中。在MRI探针注射之前和MRI探针注射之后24小时使用MRI序列组,包括T1、T2加权自旋回波成像,对荷PC瘤小鼠进行体内MRI。在图4B中,分析高分辨率T2加权MRI图像以定位腹膜腔中的PC肿瘤(黄色圆圈)。一旦定位了PC肿瘤,就使用T1加权MRI图像来定量测量PC肿瘤区域(黄色圆圈)中的MRI信号变化作为来自所施用MRI探针的钆的瘤内累积的函数。在图4C中,ICAM1-Gd组中的肿瘤T1 MRI信号提高约50%,而非靶向IgG-Gd组中未观察到MRI信号变化(n=3/组)。这些MRI信号变化与所靶向肿瘤上的抗原表达水平正相关,这可用于鉴定可受益于ICAM1靶向免疫治疗的ICAM1阳性患者。
总之,本公开内容提供了ICAM1是适合用于人PC的ADC靶标的实验证据。该靶标的效用可扩展到开发其他免疫治疗剂,包括CAR T细胞或针对ICAM1的双特异性抗体。
材料和方法
材料。
纯化的抗人CD54抗体(克隆:HCD54)、藻红蛋白(PE)缀合的小鼠抗人ICAM-1抗体(PE-ICAM1)和PE缀合的小鼠IgG同种型(PE-IgG)均购自BioLegend(San Diego,CA,USA)。ADC预筛选G-DM1、G-MMAE、G-MMAF、G-Duoca均购自Levena Biopharma(San Diego,CA)。SMCC-DM1购自Medkoo(Morrisville,NC,USA)。ZebaTM Spin Desalting柱,(7K MWCO)、AlexaFluor 647NHS酯、Lab-Tek II Chamber Slide系统、ProLong Gold Antifade封固剂均获自Thermo Fisher Scientific。盐酸吉西他滨(GEM)、氯化钆(III)六水合物(GdCl3·6H2O)、二亚乙基三胺五乙酸二酐(DTPAA)、碳酸氢钠、柠檬酸三钠二水合物均购自Sigma-Aldrich(St.Louis,MO)。Dulbecco’s PBS、DAPI、Quant-iT RNA测定试剂盒、0.25%胰蛋白酶/2.6mMEDTA溶液、Gibco DMEM、Gibco DMEM/F12(1∶1)、Roswell Park Memorial Institute(RPMI)-1640培养基和McCoy’s 5A培养基均购自Invitrogen(Carlsbad,CA)。MEGM乳腺上皮细胞生长培养基购自Lonza(Basel,Switzerland)。Quantum Simply Cellular微珠购自Bangs Laboratory(Fisers,IN)。Dojindo细胞计数试剂盒CCK-8购自Dojindo MolecularTechnologies(Rockville,MD,USA)。人胰腺癌组织和正常组织阵列(PA1002a)购自USBiomax(Derwood,MD)。
细胞培养。
PANC-1、BxPC-3、Capan-1、Capan-2和HPNE细胞均购自ATCC(Manassas,VA)。HPDE细胞购自Kerafast(Boston,MA)。PANC-1,具有10%FBS的Dulbecco改良Eagle培养基,BxPC-3,具有10%FBS的RPMI-1640培养基;Capan-1,具有20%FBS的Iscove改良Dulbecco培养基,Capan-2,具有10%FBS的改良McCoy’s 5a培养基;HPNE,无葡萄糖具有另外的2mM L-谷氨酰胺和1.5g/L碳酸氢钠、25%培养基M3 Base(Medium M3 Base)、FBS 5%、10ng/ml人重组EGF、5.5mM D-葡萄糖(1g/L)、750ng/ml嘌呤霉素的75%DMEM;HPDE,角质形成细胞基础培养基+提供的补充剂(Lonza,Clonetics KBM,目录号CC-3111)。将所有细胞均维持在37℃下具有5%(vol/vol)CO2的湿润培养箱中。
ICAM-1表面表达的量化。
如前所述,通过BD FACSCalibur流式细胞仪(BD Biosciences)来评价胰腺癌细胞ICAM1表面蛋白表达。基于Quantum Simply Cellular微珠使用如由制造商提供的方案来确定细胞表面上ICAM-1密度的量化。收集106个细胞并通过悬浮-旋转循环冲洗两次。将细胞在冰浴中通过PBS中的1%BSA封闭30分钟。在BSA封闭之后,将细胞与藻红蛋白(PE)缀合的ICAM1抗体在室温下孵育1小时。将细胞用PBS中的1%BSA冲洗三次,重悬于PBS中,并通过流式细胞术进行评价。
免疫组织染色。
免疫组织化学研究在经石蜡包埋的人PDAC和正常组织微阵列(PA1002a,USBiomax)上进行。如前所述,评价了40例人PDAC组织和10例人正常组织微阵列样品的ICAM-1表达。微阵列中的单独组织核心由外科病理学家在样品身份未知的情况下进行评分。通过计算H评分对免疫染色进行评分,其中染色强(3+)、中等(2+)和弱(1+)的细胞百分比根据下式相乘:H评分=3×(染色3+的细胞百分比)+2×(染色2+的细胞百分比)+1×(染色1+的细胞百分比)。在Olympus BX41显微镜上通过使用Olympus Q-Color5数码相机(OlympusAmerica Inc.)来拍摄显微照片。
ICAM-1 Ab的体外结合和内化
使用PE-ICAM1抗体来评估ICAM1抗体与人胰腺癌细胞系的体外特异性结合。IgG用作对照。将细胞以5×103个细胞/孔的密度接种在8孔室载片中。在恢复24小时之后,用包含PE-ICAM1抗体和1%FBS的培养基替换完全培养基。将细胞与包含PE-ICAM1的培养基在37℃下孵育另外的4小时。然后用冷的磷酸缓冲盐水(phosphate-buffered saline,PBS)冲洗细胞单层,并将其用PBS溶液中的4%多聚甲醛固定。使用ProLong Gold Antifade封固剂用4’,6’二脒基-2-苯基吲哚盐酸盐(DAPI)来复染细胞核。使用Zeiss LSM8880共聚焦显微镜(Oberkochen,Germany)来获取和分析荧光图像。
在成像流式细胞术研究中,将细胞以1×106个细胞/孔的密度接种在6孔室载片中。在细胞恢复24小时之后,用包含PE-ICAM1抗体和1%FBS的培养基替换完全培养基。将细胞与包含PE-ICAM1的培养基在37℃下孵育1小时。然后收集细胞单层并将其用冷PBS冲洗两次,重悬并使用Amnis imagestreamX Mark II成像流式细胞术(Luminex,Austin,TX,USA)进行评价。
ICAM-1 Ab的治疗作用
使用定量相位成像来评估ICAM1抗体对人胰腺癌细胞系的体外治疗作用。IgG用作对照。将细胞以5×104个细胞/孔的密度接种在6孔板中。在恢复24小时之后,用包含2μg/mL剂量的ICAM1抗体或IgG的培养基替换完全培养基。将细胞与包含ICAM1或IgG的培养基在37℃下孵育24小时。之后,将板置于培养箱中放置的定量相位成像显微镜(Holomonitor M4,Phase Holographic Imaging Phi AB,Lund,Sweden)下,并以5分钟间隔成像另外的24小时。然后使用Hstutio4分析细胞运动、形态和增殖。
ADC Ab-Gd的制备和表征。
通过将IgG或ICAM1 Ab(2.4mg,16nmol)与SMCC-DM1(1.2mg,1.1umol)在磷酸缓冲液(pH 7.2)中混合,在室温下旋转1小时来制备IgG-DM1、ICAM1-DM1。通过Ultra-4离心过滤器(30K MWCO)去除游离的SMCC-DM1。将IgG-DM1、ICAM1-DM1用PBS(pH 7.4)洗涤数次并再分散在PBS中。
ADC通过UV/Vis光谱来表征,并且抗体药物比例根据以下等式来计算:

IgG-DTPA-Gd、ICAM1-DTPA-Gd如前所报道来制备。将DTPAA(0.4mg,1.1μmol)缓慢添加至NaHCO3缓冲液(pH 9.0,0.1M)中的IgG或ICAM1 Ab(0.2mg,1.3nmol),并将混合物在室温下旋转过夜。DTPA缀合的IgG或ICAM1 Ab通过Ultra-4离心过滤器(30K MWCO)来纯化并将其再分散在柠檬酸缓冲液(pH 6.5,0.1M)中。然后将0.1M柠檬酸缓冲液(pH 6.5)中的GdCl3(0.1mg,0.27μmol)与DTPA缀合的IgG或ICAM1 Ab在室温下旋转混合24小时。通过Ultra-4离心过滤器(30K MWCO)去除游离Gd3+,并将IgG-或ICAM1-DTPA-Gd再分散在PBS中用于后续使用。通过液相色谱电喷雾电离质谱LC-MS来表征Ab-Gd。

将人胰腺癌细胞系以5×103个细胞/孔的密度接种在96孔板中,并允许其黏附过夜。然后用包含不同药物浓度的游离GEM、IgG或ICAM1缀合的DM1、Duo、MMAE、MMAF的培养基替换培养培养基。在将细胞培养另外72小时之后,通过CCK-8测定按照供应商提供的方案来确定细胞毒性。在除去包含药物的培养基之后,用PBS仔细冲洗细胞,并且在此之后添加包含CCK-8的培养基溶液。然后将细胞与CCK-8培养基一起孵育4小时。使用微板阅读仪(Synergy2;BioTek,Winooski,VT,USA)在450nm的吸光度波长下读出板。通过比较与药物一起孵育的细胞的吸光度与在不存在药物的情况下孵育的对照细胞的吸光度来确定细胞生存力。
原位PDAC小鼠模型。
所有动物实验均按照由波士顿儿童医院(Boston Children’s Hospital)的机构动物护理和使用委员会(Institutional Animal Care and Use Committee,IACUC)批准的方案进行。用表达萤光素酶和GFP基因的质粒根据制造商说明(MGH,Boston)来转染PANC-1细胞。在感染之后72小时,通过以荧光显微术对GFP的显现来确定基因转移成功。针对GFP信号,使用FACSAria II流式细胞术(BD Biosciences)通过流式细胞术将稳定转染的细胞分选两次,并维持在DMEM-10%FBS中。使用已建立的手术方法,通过将PANC-1细胞注射到8周龄的雄性无胸腺裸鼠(Charles River;每组n=6)的胰腺中来制备原位胰腺癌模型。在手术期间通过异氟烷(5%含O2)将小鼠麻醉。在胰腺通常位于的腹部区域的左胁腹,脾中部的后面进行切口。然后用镊子轻轻拉出胰腺,并且将50μL 1×106个PANC-1细胞小心地注射到胰腺中。在注射之后,将胰腺放回在腹肌前面的腹腔中,并用4-0Polysorb缝合线和手术钉缝合皮肤。在恢复两周之后开始处理。将动物随机分为四组(n=6):PBS对照、用吉西他滨(GEM)处理、用非靶向IgG-DM1处理和用ICAM1-DM1处理。针对IgG-DM1和ICAM1-DM1每三周以12mg/kg小鼠重量的剂量,针对GEM每周两次以80mg/kg小鼠重量的剂量,通过静脉内尾静脉注射来处理小鼠,而对照组仅接受PBS注射。经ADC处理组和对照组在3周间隔下总共有2次注射,并且GEM在3天或4天间隔下有12次注射。每周两次测量体重,并且在小鼠接受i.p.注射D-萤光素之后使用IVIS谱成像系统(PerkinElmer)监测肿瘤生长。
体内MRI。
在分别用IgG-Gd和ICAM-Gd(剂量为5mg/kg小鼠重量)静脉内注射的两组荷瘤小鼠上进行体内MRI。在注射之前和注射24小时之后使用9.4 T Bruker Horizontal Bore MRI用针对T1和T2加权MRI的涡轮自旋回波序列获得图像。成像参数如下:用于T2加权成像的重复时间(repetition time,TR)为1,523毫秒、TE为33毫秒、340×220矩阵、40×28mm2视场、180°翻转角和0.6mm切片厚度;用于T1加权成像的TR为700毫秒并且TE为22毫秒。为了量化肿瘤的信号强度,在同一切片上用相同的成像深度围绕整个肿瘤绘制目的区域(regionsof interest,ROI)。通过ImageJ软件计算像素强度并将其相对于ROI区域归一化。
组织学。
在终点收集器官(肝、脾、肾、胰腺、心脏、肺和肌)和肿瘤样品。通过H&E染色、Ki67染色和ICAM1免疫组织染色来研究用ICAM-DM1、IgG-DM1、GEM和PBS处理的原位PANC-1肿瘤的病理状况。针对肿瘤切片的所有染色均按照标准方案进行。
统计学分析。
定量数据以平均值±SD表示。使用未配对t检验比较差异。使用Microsoft Excel软件进行统计分析。P值≤0.05被认为是统计学上显著的。

Claims (34)
1.治疗胰腺癌的方法,所述方法包括向有此需要的对象施用有效量的抗体药物缀合物(ADC),所述抗体药物缀合物(ADC)包含与药物缀合的胞间黏附分子1(ICAM1)抗体。
2.权利要求1所述的方法,其中所述药物选自:N2’-脱乙酰基-N2’-(3-巯基-1-氧代丙基)美登素(DM1)、单甲基奥瑞他汀E(MMAE)、单甲基奥瑞他汀F(MMAF)和倍癌霉素。
3.权利要求2所述的方法,其中所述药物是DM1。
4.权利要求1至3中任一项所述的方法,其中所述ICAM1抗体与所述药物通过接头缀合。
5.权利要求4所述的方法,其中所述接头是可切割接头。
6.权利要求5所述的方法,其中所述可切割接头选自:N-琥珀酰亚胺基4-(2-吡啶基二硫代)戊酸酯(SPP)、N-琥珀酰亚胺基3-(2-吡啶基二硫代)丁酸酯(SPDB)、磺基-SPDB、缬氨酸-瓜氨酸(Val-cit)、乙酰丁酸酯和CL2A。

17.治疗胰腺癌的方法,所述方法包括向有此需要的对象施用有效量的抗体药物缀合物(ADC),所述抗体药物缀合物(ADC)包含通过N-琥珀酰亚胺基4-(N马来酰亚胺基甲基)环己烷-1-羧酸酯(SMCC)接头与N2’-脱乙酰基-N2’-(3-巯基-1-氧代丙基)美登素(DM1)缀合的胞间黏附分子1(ICAM1)抗体。
18.在患有胰腺癌的对象中预测对用胞间黏附分子1(ICAM1)抗体或包含与药物缀合的ICAM1抗体的抗体药物缀合物(ADC)的治疗的响应性的方法,所述方法包括:
(i)向所述对象施用有效量的用成像剂标记的ICAM1抗体;以及
(ii)通过成像使肿瘤显现;
(iii)确定所述肿瘤上ICAM1的水平,
其中与具有较低的ICAM1水平的对象相比,较高的ICAM1水平表示所述对象对用所述ICAM1抗体或所述ADC的治疗更具响应性。
19.权利要求18所述的方法,其中(i)中的所述ICAM1抗体用DTPA-Gd标记。
20.权利要求18或权利要求19所述的方法,其中(ii)中的所述显现通过磁共振成像(MRI)进行。
21.权利要求18至20中任一项所述的方法,其还包括向被预测为响应于所述治疗的所述对象施用有效量的所述ICAM1抗体或所述ADC以治疗胰腺癌。
